Xu Lao (Deficiency Consumption) refers to the conditions of Wu Lao (Five Exhaustions), Liu Ji (Six Extremes), and Qi Shang (Seven Injuries). The Five Exhaustions are: first, Zhi Lao (Exhaustion of Will); second, Si Lao (Exhaustion from Overthinking); third, Xin Lao (Heart Exhaustion); fourth, You Lao (Exhaustion from Worry); fifth, Bu Shou (Failure to Guard [the Spirit]). Symptoms include fearfulness preventing solitary sleep, and blurred vision. Xin Lao manifests as mental confusion and forgetfulness, difficult dry stools or occasional loose stools resembling duck manure, and oral ulcers. Pi Lao (Spleen Exhaustion) presents with a stiff tongue base and difficulty swallowing saliva. Shen Lao (Kidney Exhaustion) involves difficulty bending and extending the back, dysuria, reddish-yellow urine, post-void dribbling, pain within the penis, dampness and sores in the scrotum, and tightness and fullness in the lower abdomen.
The Six Extremes are: first, Qi Ji (Qi Extreme), causing internal deficiency, insufficiency of the five Zang organs, excess of pathogenic factors, scarcity of healthy Qi, and reluctance to speak; second, Xue Ji (Blood Extreme), leading to lack of luster in the complexion, loss of eyebrows and hair, mental confusion, and forgetfulness; third, Jin Ji (Tendon Extreme), causing frequent muscle spasms, pain in all ten fingers and nails, extreme fatigue making prolonged standing impossible; fourth, Gu Ji (Bone Extreme), resulting in sore and emaciated bones, painful teeth, vexing pain in hands and feet, inability to stand, and aversion to movement; fifth, Ji Ji (Muscle Extreme), causing emaciation and lack of moisture, where food intake fails to nourish muscles; sixth, Jing Ji (Essence Extreme), leading to shortness of breath, a buzzing sensation of internal emptiness, insufficiency of Qi in the five Zang organs, falling hair, sadness, and forgetfulness.
The Seven Injuries are: first, Yin Cold; second, Yang Impotence; third, Li Ji (Internal Urgency/Tightness); fourth, Jing Lou Yi Jing (Leakage and Spermatorrhea of Essence); fifth, Jing Shao with Yin Xia Shi (Scarcity of Essence with Dampness in the Genitals); sixth, Jing Ye Qing (Clear Essence); seventh, Xiao Bian Ku Shu with Lin Shi Bu Ju (Painful Frequent Urination and Failure to Erect during Intercourse). Additionally, there are seven types of damage: first, overeating damages the Spleen, causing belching, desire to lie down, and yellow complexion; second, excessive anger damages the Liver, reducing blood and causing dim vision; third, forceful lifting or sitting long on wet ground damages the Kidneys, reducing essence, causing lower back pain, cold limbs, and coldness in the lower jiao; fourth, external cold or cold drinks damage the Lungs, causing shortness of breath, cough, and nasal congestion; fifth, worry and overthinking damage the Heart, causing fright-like symptoms and alternating joy and anger; sixth, wind, rain, cold, and heat damage the Form, causing hair loss and withered skin; seventh, excessive fear without moderation damages the Zhi (Will), causing mental distraction and unhappiness.
In healthy men, a large pulse indicates Lao (Consumption); an extremely deficient pulse also indicates Lao. In men suffering from Lao disease, the pulse is floating and large. A sunken and fine pulse at the left Chi position indicates insufficiency of Wei Qi (Defensive Qi); a floating pulse at the right Cun position indicates exhaustion of Ying Qi (Nutritive Qi). A straight-upward pulse without gentleness indicates adverse deficiency. A choppy pulse lacking Yang characteristics indicates scanty Kidney Qi; a choppy pulse without blood at the Cun and Guan positions, accompanied by rebellious Qi and cold, indicates severe deficiency. Floating, minute, and slow pulses all indicate deficiency; a slow and large pulse indicates Lao disease. A combination of minute and soggy pulses indicates Wu Lao; a combination of weak and minute pulses indicates deficiency damage and Qi Shang.
Poetry states: When the pulse is floating, large, wiry, or rapid, one must recognize this Lao pattern early. Floating, large, deep, and slow pulses are still treatable if blood and Qi are not yet exhausted and the person is not weak. A double-wiry pulse shows Liver Wood invading the Spleen; if rapid, the condition is critical and medication should be approached with caution. Xu Lao generally generates alternating chills and fever due to disharmony of Ying and Wei Qi and exhaustion of body fluids. Fire flaming upward generates phlegm and cough; sweating, upper jiao Qi rebellion, and occasional hemoptysis occur. Water flowing downward leads to brittle hair, nocturnal emissions, spermatorrhea, and slippery discharge. Slippery and frequent urination with red or white turbidity occurs. Weakness and fatigue in the legs are present. There are thousands of variations in Xu Lao complications, but the general pathology is as described above. Treatment should first focus on nourishing the Stomach and regulating the Spleen, calming the Qi Sea according to seasonal rhythms. Subsequently, one should nourish the Kidney and nurture the Heart Spirit. Medical classics record the syndromes of Wu Lao and Liu Ji, which differ from Chuan Shi (Transmissible Corpse) and Gu Zheng (Bone Steaming Tuberculosis). These arise mostly from internal failure to preserve vitality, excessive use, violation of Yin-Yang, and injury to Ying and Wei Qi... (Text becomes fragmented here)... Si Cheng Hu Wu Ji...
