Li Zuo Initially suffered from abdominal pain, and mistakenly consumed ginger and vinegar. The excessive pungent and hot nature stirred up the fire of the Heart and Liver, causing it to flare upward. Damage to the Yang collaterals led to blood overflowing upward, resulting in bleeding from the tongue like a gush, coarse breathing with panting, dry mouth without desire to drink, and defecation accompanying the urge to urinate. The pulse was wiry, rapid, and hurried. The tongue was dry and devoid of fluid. The source of transformation of Metal (Lung) was exhausted, and the Fire of Dragon and Lei (Ministerial Fire) flew upward and surged. There was considerable concern regarding the danger of panting and collapse. Urgently, Shengmai Tang (Pulse-Rescuing Decoction) was prescribed to rescue the source of transformation, and Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) to clear blood heat.
Radix Panacis Quinquefolii (Xi Yang Shen, 2 qian), Fresh Radix Rehmanniae (Xian Sheng Di, 3 qian), Raw Radix Paeoniae Alba (Sheng Bai Shao, 2 qian), Fresh Caulis Bambusae in Taeniam (Xian Zhu Ru, 1.5 qian), Radix Ophiopogonis (Da Mai Dong, 2 qian), Cornu Rhinocerotis (Xi Jiao Jian, 4 fen), Cortex Moutan (Fen Dan Pi, 1.5 qian), Juice of Fresh Rhizoma Nelumbinis (Xian Ou Zhi, 1 cup, taken by pouring over), Fresh Dendrobii Nobiles Caulis (Xian Tie Shi Hu, 3 qian), Fructus Fritillariae Cirrhosae (Chuan Bei Mu, 2 qian), Radix Achyranthis Bidentatae (Huai Niu Xi, 2 qian).
Guo You Hair is the surplus of blood; blood deficiency leads to hair loss. Blood deficiency generates internal heat, which combats the Ying (Nutrient) level. Upward, this manifests as epistaxis (nosebleed); downward, as hematochezia (blood in stool). Treatment should focus on nourishing blood and clearing the Ying level.
Radix Rehmanniae Preparata (Xi Sheng Di, 4 qian), Radix Ophiopogonis (Tian Mai Dong, 2 qian each), Semen Sophorae Flavae Carbonized (Huai Hua Tan, 2 qian), Herba Prunellae Spicae (Xia Ku Cao, 1.5 qian), Radix Glycyrrhizae Uralensis (Sheng Gan Cao, 5 fen), Cortex Moutan (Fen Dan Pi, 1.5 qian), Folium Platycladi Carbonized (Ce Bai Tan, 1.5 qian), Cortex Anemarrhenae Asphodeloidis (Fei Zhi Mu, 1.5 qian), Folium Mori (Dong Sang Ye, 3 qian), Dendrobii Officinalis Caulis (Chuan Shi Hu, 3 qian), Fresh Rhizoma Nelumbinis (Xian Ou, 2 liang, sliced for decoction).
Shen You Qi rebellion causes vomiting, occurring irregularly. There is dry mouth with internal heat, delayed menstruation with scant flow, and restless sleep at night. The tongue body is red with thin yellow coating. The pulse is wiry on the left and choppy on the right. Wiry indicates Liver excess; choppy indicates blood deficiency. This is likely due to inner depression and obstruction, causing Liver Qi stagnation and failure to diffuse. Extreme stagnation transforms into fire. Fire has a flaming-upward nature; rushing upward causes vomiting. As the Classics state: "All rebellious qi rushing upward belongs to fire." The Liver and Gallbladder share the same palace. If Liver Qi is stagnant, how can the Clear Palace (Gallbladder) remain unaffected? Carrying Gallbladder fire upward, qi rebels and causes vomiting, which is a inevitable sign. Dry mouth and internal heat can be inferred similarly. In treating Liver disease, one must know that Liver pathology transmits to the Spleen. Liver Qi rebels horizontally and cannot be discharged, smoothly invading the Central Earth (Spleen/Stomach), constraining the Spleen and Stomach. The Stomach is Yangming. The Classics say: "Diseases of Yangming originate in the Heart and Spleen; there are hidden difficulties, and women do not menstruate." Because the Heart generates blood, the Spleen controls blood, and the Liver stores blood, carefully tracing the source of Ying blood transformation reveals it actually originates from Yangming. The Classics say: "Food enters the stomach, its essence floats and overflows, and is transported upward to the Spleen." It also says: "The Middle Jiao receives Qi, extracts juices, transforms them into red, and this is called blood." It further says: "Ying Qi emerges from the Middle Jiao." Wood overcoming Earth leads to Earth deficiency; the Middle Jiao loses its transforming function, generating less blood daily. Upward, it cannot nourish the Heart and Spleen; downward, it cannot moisten the Chong and Ren vessels. Menstruation becomes delayed and scant, showing signs of amenorrhea. With transmission upon transmission, changes akin to wind dissipating and swelling accumulating occur. An ant hole breaches the dike; feathers break the axle. How can there be no deficiency? Ancients said: "The Liver is a rigid organ, requiring softening and nourishing to be controlled; the Stomach is Yang Earth, requiring clearing and unblocking to harmonize." The proposed method is to nourish blood and soften the Liver, harmonize the Stomach and unblock menstruation. Treating the Liver and Stomach rather than directly the Heart and Spleen is a strategy of tracing the source and returning to the root. However, this condition stems from Seven Emotions. Humans are not detached sages; one must cultivate joy and harmony so that medicine reaches the disease site and achieves swift results, preventing lingering illness.
Raw Radix Paeoniae Alba (Sheng Bai Shao, 2 qian), Polyporus cum Radice Pini (Zhu Fu Shen, 3 qian), Pinelliae Tuber Preparatum (Xian Ban Xia, 1.5 qian), Dendrobii Officinalis Caulis (Chuan Shi Hu, 2 qian), Semen Ziziphus Jujubae Stir-fried (Chao Zao Ren, 3 qian), Haematitum (Dai Zhe Shi, 2 qian), Flos Inulae (Xuan Fu Hua, wrapped, 1.5 qian), Radix Bupleuri (Yin Chai Hu, 1 qian), Os Draconis (Qing Long Chi, 3 qian), Pericarpium Citri Reticulatae (Guang Ju Bai, 1 qian), Fructus Viticis Trifoliae (Chong Wei Zi, 3 qian), Radix Salviae Miltiorrhizae (Dan Shen, 2 qian), Fresh Caulis Bambusae in Taeniam (Xian Zhu Ru, 1.5 qian), Raw and Stir-fried Hordei Fructus Germinatus (Sheng Shu Gu Ya, 3 qian each), Zuo Jin Wan (wrapped, 7 fen).
Second Visit: Qi rebellion and vomiting have ceased, but sleep remains restless. Menstruation flows but is scant. Coating is gray-yellow. Pulse is wiry, fine, and choppy. All result from Ying blood depletion, Liver failure to smooth flow, Spleen failure to transport, and Stomach failure to descend and harmonize. Yesterday's prescription of nourishing blood, softening Liver, harmonizing Stomach, descending rebellion, and assisting menstruation showed efficacy. Still prescribing Xiaoyao San combined with Fu Zhe Er Chen Tang modified. Once Wood and Earth do not contend, all ailments will heal.
Radix Angelicae Sinensis (Bai Gui Shen, 2 qian), Polyporus cum Radice Pini (Zhu Fu Shen, 3 qian), Semen Ziziphus Jujubae Stir-fried (Chao Zao Ren, 3 qian), Caulis Bambusae in Taeniam Stir-fried (Chao Zhu Ru, 1.5 qian), Raw Radix Paeoniae Alba (Sheng Bai Shao, 2 qian), Pinelliae Tuber Preparatum (Xian Ban Xia, 1.5 qian), Os Draconis (Qing Long Chi, 3 qian), Pericarpium Citri Reticulatae (Guang Ju Bai, 1.5 qian), Radix Bupleuri (Yin Chai Hu, 8 fen), Sorghi Fructus (Bei Shu Mi, wrapped, 3 qian), Haematitum (Dai Zhe Shi, 3 qian), Fructus Viticis Trifoliae (Chong Wei Zi, 3 qian), Dendrobii Officinalis Caulis (Chuan Shi Hu, 3 qian), Flos Inulae (Xuan Fu Hua, wrapped, 1.5 qian), Folium Citri Reticulatae Viride (Qing Ju Ye, 1.5 qian).
Li You Tian Gui (Menstrual Essence) has just arrived, with scant flow. Abdominal pain is dull. Nosebleed is severe. Qi stagnation and blood stasis, with Liver fire carrying blood, fail to flow smoothly into the Chong and Ren vessels, instead rushing chaotically upward, overflowing into the clear orifices, presenting signs of retrograde menstruation (Ni Jing). For reversal, use smoothing; for agitation, use calming. Thus, regulating Qi, dispelling stasis, clearing Liver heat, and descending fire is an immutable method.
Radix Salviae Miltiorrhizae (Zi Dan Shen, 2 qian), Radix Achyranthis Bidentatae (Huai Niu Xi, 2 qian), Radix Angelicae Sinensis (Quan Dang Gui, 2 qian), Cortex Moutan (Fen Dan Pi, 1.5 qian), Fresh Caulis Bambusae in Taeniam (Xian Zhu Ru, 3 qian), Fructus Viticis Trifoliae (Chong Wei Zi, 3 qian), Rhizoma Cyperi Preparated (Zhi Xiang Fu, 1.5 qian), Flos Imperatae Cylindricae (Bai Mao Hua, wrapped, 1 qian), Schizonepetae Herba Stir-fried (Chao Jing Jie, 8 fen), Retinervus Citri Reticulatae (Fu Ju Luo, 1 qian), Pericarpium Amomi Rotundum (Chun Sha Ke, 8 fen).
Wu You Menstruation is delayed. Abdominal pain occurs before onset. Cold in the Blood Chamber, with Liver and Spleen Qi stagnation. Blood is the basis for Qi attachment; Qi is the pioneer of blood. Qi moves, blood moves; Qi stops, blood stops. To regulate menstruation, first regulate Qi. This is the principle of the Classics. Yan Shi Yi Qi San is prescribed, combined with warming and unblocking agents.
Rhizoma Cyperi Preparated (Zhi Xiang Fu, 1.5 qian), Polyporus (Yun Fu Ling, 3 qian), Argysti Folium (Guang Ai Rong, 8 fen), Rhizoma Corydalis Yanhusuo (Yan Hu Suo, 1 qian), Rosa Chinensis (Yue Ji Hua, 8 fen), Radix Angelicae Sinensis (Quan Dang Gui, 2 qian), Fructus Viticis Trifoliae (Chong Wei Zi, 3 qian), Fructus Meliae Toosendan (Jin Ling Zi, 2 qian), Amomi Fructus Rotundus (Da Sha Ren, ground, 8 fen), Radix Salviae Miltiorrhizae (Zi Dan Shen, 2 qian), Lardizablae Radix (Tai Wu Yao, 8 fen), Radix Achyranthis Bidentatae (Huai Niu Xi, 2 qian), Exocarpium Citri Reticulatae (Chen Guang Pi, 1 qian).
Zheng You Deficiency of Zheng Qi with latent pathogenic factors, disrupting the normal cycle of Ying and Wei. Long-standing aversion to cold, poor appetite, and mental fatigue. Menstruation has ceased for three months, gradually developing into consumption and weakness. Temporarily providing methods to support Zheng Qi and expel pathogens, harmonize Ying, and unblock menstruation.
Codonopsis Radix Stir-fried (Chao Lu Dang, 2 qian), Polyporus cum Radice Pini (Bao Fu Shen, 3 qian), Fructus Viticis Trifoliae (Chong Wei Zi, 3 qian), Radix Bupleuri (Yin Chai Hu, 8 fen), Radix Glycyrrhizae Uralensis Honey-roasted (Qing Zhi Gan Cao, 5 fen), Radix Salviae Miltiorrhizae (Zi Dan Shen, 2 qian), Rosa Chinensis (Yue Ji Hua, 5 fen), Scutellariae Radix Wine-stir-fried (Jiu Chao Huang Qin, 1.5 qian), Exocarpium Citri Reticulatae (Chen Guang Pi, 1.5 qian), Pinelliae Tuber Preparatum (Xian Ban Xia, 2 qian), Xiaoyao San (wrapped, 3 qian).
Second Visit: Aversion to cold and fever have stopped. Poor appetite and mental fatigue persist. Menstruation has ceased for three months. Pulse is wiry and rapid. External pathogens have retreated, but Zheng Qi has not recovered. Chong and Ren vessels are deficient, leading to menstrual obstruction. Adhering to the previous method.
Previous formula adds Radix Achyranthis Bidentatae (Huai Niu Xi, 2 qian) and Safflower (Zang Hong Hua, 8 fen).
Weng You Menstruation has been absent for nine months. Appetite is poor. The Classics state that irregular menstruation is attributed to the Chong and Ren vessels. The Chong vessel is the Sea of Blood, subordinate to Yangming. Yangming is the Stomach. Food enters the Stomach, transforms into essence and blood, and Ying Qi emerges from the Middle Jiao. Yangming deficiency fails to generate essence and blood to flow downward into the Chong and Ren. The Tai Chong vessel is not full; where does menstruation come from? Treatment should follow the principle of diseases originating from Yangming. Jin Gui Wen Jing Tang with modifications is prescribed.
Radix Angelicae Sinensis (Quan Dang Gui, 2 qian), Colla Corii Asini Bead (E Jiao Zhu, 2 qian), Radix Salviae Miltiorrhizae (Zi Dan Shen, 2 qian), Radix Paeoniae Rubra and Radix Paeoniae Alba (Chi Bai Shao, 1.5 qian each), Ramulus Cinnamomi (Chuan Gui Zhi, 4 fen), Fructus Evodiae Routei (Wu Zhu Yu, 4 fen), Pinelliae Tuber Preparatum (Xian Ban Xia, 2 qian), Radix Glycyrrhizae Uralensis Honey-roasted (Zhi Gan Cao, 5 fen), Fructus Viticis Trifoliae (Chong Wei Zi, 3 qian), Rhizoma Chuanxiong (Da Chuan Xiong, 8 fen), Cortex Moutan (Fen Dan Pi, 1.5 qian), Rhizoma Zingiberis Recens (Sheng Jiang, 2 slices), Fructus Jujubae (Hong Zao, 2 pieces).
Xu You The Classics say: Acute pain belongs to Cold; chronic pain belongs to Heat. Acute pain is in the Jing (Vessels); chronic pain is in the Luo (Collaterals). Paroxysmal lower abdominal pain, severe pain with sweating, lasting for a month. Aversion to cold, poor appetite, cough with nausea, chest tightness and discomfort, dry mouth with desire to drink. Liver heat with blood stasis, Qi stagnation and immobility. Yin injury with insufficient fluids failing to ascend upward. Lung deficiency with lingering Phlegm-Heat. Tongue is crimson red. Pulse is fine as a thread. Extreme emaciation and weakness; difficult to expect complete recovery.
Fructus Meliae Toosendan (Jin Ling Zi, 2 qian), Flos Inulae (Xuan Fu Hua, wrapped, 1.5 qian), Polyporus cum Radice Pini (Zhu Fu Shen, 3 qian), Radix Paeoniae Rubra and Radix Paeoniae Alba (Chi Bai Shao, 1.5 qian each), Fructus Trichosanthis (Quan Gua Lou, cut, 4 qian), Semen Armeniacae Amarum (Guang Xing Ren, 3 qian), Xin Jiang (Zhen Xin Jiang, 8 fen), Fritillariae Thunbergii Bulbus (Chuan Xiang Bei, 2 qian each), Charred Crataegi Fructus (Jiao Zha Tan, 3 qian), Radix Bupleuri (Yin Chai Hu, 8 fen), Shixiao San (wrapped, 3 qian), Folium Citri Reticulatae Viride (Qing Ju Ye, 1.5 qian), Gardeniae Fructus Stir-fried (Chao Shan Zhi, 1.5 qian).
Second Visit: Lower abdominal pain has eased, nausea has gradually stopped. Sweating is profuse. Extremities are cold. Emaciated to the bone. Thirst with desire to drink. Liver stagnation transforming into heat; deep heat leads to deep cold limbs (Jue). Yin injury with insufficient fluids failing to ascend. Lung deficiency with lingering Phlegm-Heat. Tongue is peeled. Pulse remains fine. Considerable concern regarding Yin failing to contain Yang, and Yang failing to store Yin, leading to Jue (cold limbs) and collapse. This is due to extreme emaciation, unable to tolerate attacking or tonifying treatments.
Dendrobii Officinalis Caulis (Chuan Shi Hu, 3 qian), Polyporus cum Radice Pini (Zhu Fu Shen, 3 qian), Fritillariae Thunbergii Bulbus (Chuan Xiang Bei, 2 qian each), Longgu Flower (Hua Long Gu, 4 qian), Fructus Mume Carbonized (Wu Mei Tan, 8 fen), Gardeniae Fructus Stir-fried (Chao Shan Zhi, 1.5 qian), Radix Paeoniae Alba (Da Bai Shao, 2 qian), Tritici Fructus (Fu Xiao Mai, 4 qian), Atractylodis Macrocephalae Rhizoma (Sheng Bai Zhu, 1.5 qian), Radix Bupleuri (Yin Chai Hu, 8 fen), Radix Salviae Miltiorrhizae (Zi Dan Shen, 2 qian), Raw and Stir-fried Hordei Fructus Germinatus (Sheng Shu Gu Ya, 3 qian each), Eriobotryae Folium Honey-roasted (Qing Zhi Pi Pa Ye, de-haired, wrapped, 3 qian), Diospyros Frost (Shi Shuang, 8 fen).
Third Visit: Jue has reversed, sweating has stopped, appetite is gradually improving. Good omen. Emaciated to the bone. Pulse fine as a thread. Tongue red and crimson. Cough with nausea. Wood stagnation transforming into fire; Liver disease transmitting to Spleen. Yin injury with insufficient fluids failing to ascend. Lung deficiency with lingering Phlegm-Heat. Nan Jing states: "Coming from the element that overcomes you is called Thief Evil." Although signs of improvement appear, they are not yet reliable.
Previous formula removes Polyporus cum Radice Pini (Zhu Fu Shen), Radix Salviae Miltiorrhizae (Zi Dan Shen), and Diospyros Frost (Shi Shuang). Adds Radix Glycyrrhizae Uralensis (Sheng Gan Cao, 5 fen) and Chaenomelis Fructus (Chen Mu Gua, 2 qian).
Wang You Menstruation is approaching. Color is purple-black. Lower abdomen distends and hurts, refusing pressure. Severe pain with fainting sensation. Aversion to cold and fear of chill. Dry mouth with little desire to drink. Coating is yellow and greasy. Pulse is soggy and choppy.
New Cold binds externally; old stasis obstructs internally. The lower abdomen is the boundary of Jueyin. Jueyin is the organ of Cold and Heat. Liver fails to disperse and drain; Qi stagnates and blocks. Blockage causes pain. Qi is the commander of blood; Qi moves, blood moves. Moving blood requires regulating Qi first. This statement is profound!
Cortex Cinnamomi Cordatae (Rou Gui Xin, 5 fen), Fructus Meliae Toosendan (Jin Ling Zi, 2 qian), Pericarpium Amomi Rotundum (Chun Sha Ke, 2 qian), Folium Citri Reticulatae Viride (Qing Ju Ye, 1.5 qian), Fructus Foeniculi (Xiao Hui Xiang, 8 fen), Rhizoma Corydalis Yanhusuo (Yan Hu Suo, 1 qian), Shixiao San (wrapped, 3 qian), Pericarpium Citri Reticulatae Viride (Xi Qing Pi, 1 qian), Fructus Viticis Trifoliae (Chong Wei Zi, 3 qian), Charred Crataegi Fructus (Jiao Zha Tan, 3 qian), Rhizoma Cyperi Preparated (Zhi Xiang Fu, 1.5 qian), Radix Paeoniae Alba Wine-stir-fried (Jiu Chao Bai Shao, 2 qian), Acanthopanacis Senticosi Spina (Liang Tou Jian, wine-soaked, wrapped, 1.5 qian).
Separately: Sodium Chloride Powder (Yan Shi Mo, 2 liang), Rhizoma Cyperi Powder (Xiang Fu Mo, 4 liang). Stir-fry with wine and vinegar, apply warm compress to painful abdomen.
Wu You Women reach Tian Gui at age fourteen. Now sixteen, menstruation has not yet started. Complexion is pale white. Palpitations and jumping sensation. Mental fatigue and lack of strength. Ying blood depletion, failing to flow downward into Chong and Ren. Tongue coating is thin and greasy. Pulse is soggy, small, and weak. Temporarily providing methods to harmonize Ying and unblock menstruation.
