Food Injury: The pulse at the right Cun position is tight and vigorous, accompanied by chest and epigastric fullness and distension, tastelessness in the mouth, insufficient body fluids, inability to ingest food, and regular breathing.
Phlegm Syndrome: Vomiting and regurgitation, headache, floating and slippery pulse, accompanied by chest and epigastric fullness and distension.
Deficiency Heat Restlessness: No aversion to cold, no headache, body pain.
Hongxiangjiao (Gouty Arthritis/Foot Qi): Symptoms originate solely from the feet.
If there are neither exterior nor interior syndromes, and the disease persists for thirteen days, Da Chai Hu Tang (Greater Bupleurum Decoction) is the primary prescription. A rapid pulse with constipation indicates blood stasis; Di Dang Tang (Resist the Accumulation Decoction) is the primary prescription. If the pathogen remains unresolved after passing through the meridians, Cheng Qi Tang (Order the Qi Decoction) is the primary prescription.
Exterior and Interior Syndromes Present Simultaneously:
A floating and large pulse indicates an exterior syndrome; accompanied by vexation, thirst, red urine, and epigastric fullness, treatment should employ Da Chai Hu Tang (Greater Bupleurum Decoction), Gui Zhi Tang (Cinnamon Twig Decoction), or Wu Ling San (Five-Ingredient Powder with Poria).
A floating and tight pulse with dry throat, bitter taste, abdominal fullness, wheezing, fever, sweating, aversion to heat rather than cold, treatment should employ Zhi Zi Chi Tang (Gardenia and Fermented Soybean Decoction).
A slow, wiry, and fine pulse indicates an interior syndrome; accompanied by interior signs, treatment should employ Xiao Jian Zhong Tang (Minor Construct the Middle Decoction) or Xiao Chai Hu Tang (Minor Bupleurum Decoction).
If the exterior syndrome remains unresolved after erroneous purgation, leading to unceasing diarrhea with heat, Gui Zhi Ren Shen Tang (Cinnamon Twig and Ginseng Decoction) is indicated. Abdominal pain, wheezing, and thirst should be referred to their respective sections.
Zhu Danxi’s Note: From the ninth chapter of Shang Han Lun up to this clause, all belong to complicated syndromes of Cold Damage. Later mentions of Shang Han refer to its inclusion in the discussion of miscellaneous diseases. In treating diseases, if one clearly understands Shang Han, then miscellaneous diseases become clear. Shang Han specifically discusses the Foot Six Meridians: Foot Taiyang Bladder Meridian, Foot Shaoyang Gallbladder Meridian, Foot Yangming Stomach Meridian, Foot Taiyin Spleen Meridian, Foot Shaoyin Kidney Meridian, and Foot Jueyin Liver Meridian. Miscellaneous diseases involve both Hand and Foot Six Meridians: Hand Taiyang Small Intestine Meridian, Hand Shaoyang Sanjiao Meridian, Hand Yangming Large Intestine Meridian, Hand Taiyin Lung Meridian, Hand Shaoyin Heart Meridian, and Hand Jueyin Pericardium Meridian. This is called the pairing of the Twelve Meridians with the Zang-Fu organs, hence they are discussed separately for the reader's convenience.
Li’s Discussion:
Taiyang Syndrome: Floating and tight pulse without sweat is named Shang Han (Cold Damage), treated with Ma Huang Tang (Ephedra Decoction). Floating and slow pulse with spontaneous sweating is named Shang Feng (Wind Strike), treated with Gui Zhi Tang (Cinnamon Twig Decoction).
Yangming Syndrome: No aversion to wind or cold, spontaneous sweating, long pulse, treated with Bai Hu Tang (White Tiger Decoction); if the pulse is floating, sinking, and forceful upon pressure, treated with Da Cheng Qi Tang (Greater Order the Qi Decoction).
Shaoyang Syndrome: Wiry pulse, treated with Chai Hu Tang (Bupleurum Decoction).
Taiyin Syndrome: Same as above, sinking and fine pulse, treated with Si Ni Tang (Four Reversal Decoction); if floating, treated with Gui Zhi Tang (Cinnamon Twig Decoction).
Shaoyin Syndrome: Sinking and solid pulse, treated with Da Cheng Qi Tang (Greater Order the Qi Decoction); if fine, sinking, and slow, treated with Si Ni Tang (Four Reversal Decoction). Cool body, sinking, fine, and deficient pulse, treated with Xie Xin Tang (Drain the Epigastrium Decoction). Fever with restlessness, normal urination and defecation, floating and洪 (large) pulse without force, imperceptible upon deep pressure, treated with Fu Zi Xie Xin Tang (Aconite Drain the Epigastrium Decoction). Vomiting and diarrhea without thirst, weak and faint pulse, treated with Li Zhong Tang (Regulate the Middle Decoction). Thirst with a sinking, forceful, and rapid pulse, treated with Wu Ling San (Five-Ingredient Powder with Poria). Sinking pulse with fever requiring diaphoresis, treated with Ma Huang Xi Xin Fu Zi Tang (Ephedra, Asarum, and Aconite Decoction). Green diarrhea with dry mouth requires purgation. Without thirst, apply warming methods.
Jueyin Syndrome: Pulses are all slightly sinking and solid, forceful upon pressure, require purgation. If not forceful, apply warming methods.
Liu’s Discussion:
Exterior syndromes require Ma Huang Tang (Ephedra Decoction) to induce sweating. For interior syndromes with exterior manifestations, use Ma Huang Xi Xin Fu Zi Tang (Ephedra, Asarum, and Aconite Decoction) to soak the body to induce sweat. For interior syndromes, add Rhubarb according to the formula to purge.
Kidney Exterior Syndrome: Floating pulse, add Ginger and Aconite to the preceding formula. Kidney Interior Syndrome with diarrhea, add the same to the subsequent formula.
Liver Exterior Syndrome: Blue complexion, wiry pulse, add Notopterygium Root and Saposhnikovia Root to the preceding formula. Liver Interior Syndrome with constipation and dysuria, sinking and wiry pulse, add the same to the subsequent formula.
Heart Exterior Syndrome: Red complexion, floating and large pulse, add Gypsum and Scutellaria Root to the preceding formula. Heart Interior Syndrome with vexation, heart pain, and hiccups, sinking pulse, add the same to the subsequent formula.
Lung Exterior Syndrome: Pale complexion, sneezing, sadness, floating and choppy pulse, add Cinnamon Twig and Dried Ginger to the preceding formula. Lung Interior Syndrome with wheezing and cough, sinking pulse, add the same to the subsequent formula.
Spleen Exterior Syndrome: Yellow complexion, frequent belching, floating and slow pulse, add Atractylodes Macrocephala Root and Premna Root to the preceding formula. Spleen Interior Syndrome with abdominal fullness, sinking pulse, add the same to the subsequent formula.
Qiang Huo Tang (Notopterygium Decoction): Treats all types of Shang Han and Liang Gan (Dual Invasion). Source: Liu.
Ingredients: Qiang Huo, Fang Feng, Chuan Xiong, Gan Cao, Di Huang, Huang Qin (each 1 liang), Bai Zhu (2 liang), Xi Xin (2 qian 5 fen).
Add Shi Gao (Gypsum) 4 qian for fever; add Shao Yao (Peony Root) 3 qian for abdominal fullness; add Chai Hu (Bupleurum Root) 1 liang and Ban Xia (Pinellia Tuber) 5 qian for alternating chills and fever; add Zhi Shi (Immature Bitter Orange) 1 qian for epigastric fullness; add Da Huang (Rhubarb Root) 3 qian for interior syndromes. Stop when pathogens are exhausted.
Da Qiang Huo Tang (Greater Notopterygium Decoction): Treats the same as above. Source: Li.
Ingredients: Fang Feng, Qiang Huo, Chuan Xiong, Gan Cao, Huang Qin, Xi Xin, Du Huo, Cang Zhu, Fang Ji, Bai Zhu, Huang Lian (each 1 qian), Zhi Mu, Di Huang (each 3 qian), Bai Zhi (add for Yangming syndrome).
Shuang Jie San (Double Release Powder): Mixed treatment. Source: Liu Hejian and Zhang Yuansu.
Do not take Ma Huang Tang in Spring and Summer; do not take Gui Zhi Tang in Autumn and Winter.
Do not take Da Qing Long Tang (Great Azure Dragon Decoction) in Summer; do not take Bai Hu Tang (White Tiger Decoction) in Winter.
Gui Zhi Tang (Cinnamon Twig Decoction): Releases the muscle layer and harmonizes the Defensive Qi, treating Taiyang Wind Strike with spontaneous sweating and floating pulse.
Gui Zhi (Cinnamon Twig) [Chief]: Wind invading internally is balanced with Acridity.
Shao Yao (Peony Root)
Gan Cao (Licorice Root) [Deputy]: Sour astringency and Sweet relaxation.
Jiang (Fresh Ginger) and Zao (Jujube) [Envoy]: Acrid dispersion and Sweet relaxation. Each 3 qian.
This formula can be used routinely in the Northwest, and only in Jianghuai regions during Winter and Spring. From late Spring to before Summer Solstice, add Huang Qin (Scutellaria Root) to call it Yang Dan Tang (Sunrise Dawn Decoction); after Summer Solstice, add Huang Qin 2.5 qian, Zhi Mu (Anemarrhena Rhizome) 0.5 liang, and Shi Gao (Gypsum) 1 liang. If the patient has a constitutionally deficient and cold state, no modifications are needed.
Add Shao Yao (Peony Root) 1 liang to treat abdominal pain with a floating pulse after purgation.
Add Da Huang (Rhubarb Root) to treat severe abdominal pain due to excess.
Add one Fu Zi (Aconite Lateral Root) to treat body pain from Wind-Dampness, and also for unceasing sweating leakage.
Add Gan Jiang (Dried Ginger) to treat cases where sweating and purgation have already occurred, and for alternating chills and fever.
Add Gua Lou (Trichosanthes Fruit) and Ge Gen (Kudzu Root) to treat sweating with soft rigidity.
Add Ma Huang (Ephedra Stem) 2 qian and Xing Ren (Apricot Seed) 12 pieces to treat alternating chills and fever, named Gui Ma Ge Ban Tang (Guizhi and Mahuang Half-Half Decoction).
Add Ma Huang (Ephedra Stem) 2 fen and Shi Gao (Gypsum) 3 qian to treat alternating chills and fever with a weak pulse, where sweating is contraindicated, named Gui Zhi San Yue Bi Yi Tang (Guizhi Three Parts, Yuebi One Part Decoction).
Add Hou Po (Magnolia Bark) and Xing Ren (Apricot Seed) to treat wheezing, aversion to wind, and absence of sweat, indicating an exterior syndrome.
Remove Shao Yao (Peony Root) to treat urgency of the pulse and chest fullness after purgation.
Ma Huang Tang (Ephedra Decoction): Treats Cold Pathogens.
Ma Huang (Ephedra Stem) [Chief]: 3 qian to disperse cold.
Gui Zhi (Cinnamon Twig) [Deputy]: 2 qian to release the muscle layer.
Gan Cao (Licorice Root) [Assistant]: 1 qian. Cold injures the Ying (Nutritive) level; Ying governs the Liver. The Liver dislikes tension, so use Sweetness to relax it.
Xing Ren (Apricot Seed) [Envoy]: Promotes Qi movement, 20 pieces.
Add Zhi Mu (Anemarrhena Rhizome) 1.5 qian and Shi Gao (Gypsum) 3 qian to treat heat diseases without sweat around the Summer Solstice.
Add Xing Ren (Apricot Seed) 50 pieces, Ma Huang (Ephedra Stem) 0.5 liang, Gan Cao (Licorice Root) 2 qian, and Shi Gao (Gypsum) 8.5 qian to treat wheezing.
Add Ma Huang (Ephedra Stem) and Yi Yi Ren (Coix Seed) 2 qian, Gan Cao (Licorice Root) 1 qian, and Xing Ren (Apricot Seed) 10 pieces to treat body pain from Wind-Dampness interaction.
Add Ma Huang (Ephedra Stem) and Xi Xin (Asarum) 2 qian, and Fu Zi (Aconite Lateral Root) 10 pieces to treat Shaoyang syndrome with a sinking pulse.
Add Shao Yao (Peony Root), Ge Gen (Kudzu Root), Jiang (Fresh Ginger), and Zao (Jujube) to treat rigid rigidity without sweat, named Ge Gen Ma Huang Tang (Kudzu and Ephedra Decoction).
Jie Ji Tang (Release Muscle Decoction): Treats Spring Warmth and Epidemic Diseases.
Ge Gen (Kudzu Root) 2 qian, Ma Huang (Ephedra Stem) 3 qian, Gui Zhi (Cinnamon Twig), Gan Cao (Licorice Root) 1 qian, Shao Yao (Peony Root), Huang Qin (Scutellaria Root) 2 qian, decoct with Jujube.
Sheng Ma Ge Gen Tang (Cimicifuga and Kudzu Decoction): Treats seasonal epidemics in Spring and Winter.
Sheng Ma (Cimicifuga Rhizome), Ge Gen (Kudzu Root), Gan Cao (Licorice Root), Shao Yao (Peony Root) in equal parts.
Also treats Taiyang-Yangming Combined Syndrome with spontaneous diarrhea: Ge Gen (Kudzu Root) 1 liang, Huang Qin (Scutellaria Root), Huang Lian (Coptis Rhizome), Gan Cao (Licorice Root) (each 2 qian) decocted. Treats wheezing and sweating indicating interior syndrome. Also treats unceasing diarrhea with heat after erroneous purgation.
Yang Du Sheng Ma Tang (Yin-Toxin Cimicifuga Decoction):
Sheng Ma (Cimicifuga Rhizome) 2 qian, Xi Jiao (Rhinoceros Horn), She Gan (Belamcanda Rhizome), Huang Qin (Scutellaria Root), Ren Shen (Ginseng Root), Gan Cao (Licorice Root) (each 1 qian).
Da Qing Long Tang (Great Azure Dragon Decoction): Treats dual injury of Wind and Cold. Cold pulse is floating and tight with Wind Strike syndrome; Wind pulse is floating and slow with Cold Damage syndrome.
Ma Huang (Ephedra Stem) [Chief]: 6 qian to disperse cold.
Gui Zhi (Cinnamon Twig) [Deputy]: 2 qian to dispel wind.
Gan Cao (Licorice Root) 2 qian.
Xing Ren (Apricot Seed) 40 pieces: Acrid and Bitter assist Ma Huang.
Jiang (Fresh Ginger) and Zao (Jujube): Acrid and Sweet combine to assist Gui Zhi.
Shi Gao (Gypsum) [Envoy]: Harmonizes Ying and Wei Qi and specifically reaches the muscle surface.
Stop after one dose. If taken again, excessive sweating leads to loss of Yang. If the pulse is weak, there is sweating and aversion to wind, do not take it. Taking it causes reversal cold, twitching of sinews, and fasciculation of muscles. Da Qing Long Tang must not be taken erroneously; erroneous use causes reversal cold.
Xiao Qing Long Tang (Minor Azure Dragon Decoction): Treats dual injury of Wind and Cold, plus fluid retention in the epigastrium, thereby eliminating exterior and interior pathogens.
Ma Huang (Ephedra Stem) [Chief]: Disperses Wind-Cold from the exterior.
Shao Yao (Peony Root), Wu Wei Zi (Schisandra Berry) [Assistant]: Cold-fluid injuring the Lung causes cough, regurgitation, and wheezing; Sourness astringes Lung rebellious Qi.
