Maculae and Urticaria Caused by Wind-Heat Invasion
"Facima" (maculae) refers to red patches on the skin resembling cloud patterns. "Yinzhen" (urticaria) refers to spots on the skin resembling flea bites. Both conditions are caused by invasion of Wind-Heat pathogenic factors. Treat with Xijiao Xiaodu Yin (Rhinoceros Horn Detoxifying Decoction). If accompanied by severe constipation, treat primarily with Fangfeng Tongsheng San (Saposhnikovia Unblocking the Interior Powder).
Xijiao Xiaodu Yin
Xijiao Xiaodu Yin consists of Rhinocerotis Cornu (Xijiao), Arctii Fructus (Niubangzi), Saposhnikoviae Radix (Fangfeng), Saposhnikoviae Radix with Ligusticum Chinense Rhizome and Flower Buds (Jingjie), and Glycyrrhizae Radix et Rhizoma (Gancao); it causes red maculae to clear rapidly.

Deafness Originates from Kidney Deficiency
The ears are the orifices of the Kidney. When Kidney Qi is substantial, hearing is acute; when Kidney Qi is deficient, deafness occurs. This is a general statement. In reality, the Hand-Shaoyang Sanjiao Meridian and Foot-Shaoyang Gallbladder Meridian pass through the ears. Therefore, there is deafness due to Qi reversal, deafness due to Wind pathogen, and deafness due to labor injury; treatment must be based on specific symptoms. For deafness due to Kidney deficiency, use Siwu Tang (Four Substances Decoction) added with Lycii Fructus (Gouqi), Cynomorii Herba (Congrong), Anemarrhenae Rhizoma (Zhimu), Phellodendri Cortex (Huangbai), Acori Tatarinowii Rhizoma (Changpu), and Bupleuri Radix (Chaihu). For deafness due to Qi stagnation, use Erchen Tang (Two-Chen Decoction) added with Cyperi Rhizoma (Xiangfu), Aucklandiae Radix (Muxiang), Scutellariae Radix (Huangqin), Gentianae Radix (Longdancao), Bupleuri Radix (Chaihu), and Acori Tatarinowii Rhizoma (Changpu). For deafness due to Wind, use Jiwei Qianghuo Tang (Nine-Ingredient Qianghuo Decoction) added with Bupleuri Radix (Chaihu) and Acori Tatarinowii Rhizoma (Changpu). For deafness due to labor injury, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Polygalae Radix (Yuanzhi) and Acori Tatarinowii Rhizoma (Changpu).

Eye Diseases Originate from Liver Fire
Zhang Zihe said: "The eyes are the external manifestation of the Liver." He also said: "Although sages state that the eyes can see only when nourished by blood, blood can be either excessive or insufficient. Excess leads to ocular congestion and pain; insufficiency leads to ocular depletion and blindness. Generally, young people suffer from excess, while the elderly (or frail) suffer from insufficiency." The general principle is to drain excess and tonify deficiency. Principles for treating eye diseases: To disperse Wind, use Saposhnikoviae Radix (Fangfeng), Saposhnikoviae Radix with Ligusticum Chinense Rhizome and Flower Buds (Jingjie), Notopterygii Rhizoma (Qianghuo), Angelicae Dahuricae Radix (Baizhi), Viticis Fructus (Manjingzi), Chrysanthemi Flos (Juhua), and Menthae Haplocalycis Herba (Bohe). To clear heat, use Scutellariae Radix (Huangqin), Coptidis Rhizoma (Huanglian), Gardeniae Fructus (Zhizi), Phellodendri Cortex (Huangbai), Forsythiae Fructus (Lianqiao), Anemarrhenae Rhizoma (Zhimu), and Gentianae Radix (Longdancao). To nourish blood, use Angelicae Sinensis Radix (Danggui), Chuanxiong Rhizoma (Chuanxiong), Paeoniae Radix Alba (Baishao), Rehmanniae Radix (Shengdi), Rehmanniae Radix Preparata (Shudi), Lycii Fructus (Gouqi), and Prunellae Spica (Xiakucao). To regulate Qi, use Cyperi Rhizoma (Xiangfu), Aurantii Fructus (Zhiqiao), Citri Reticulatae Pericarpium Viride (Qingpi), Arecae Semen (Binglang), Amomi Fructus Rotundus (Baidoukou), Atractylodis Rhizoma (Cangzhu), and Glycyrrhizae Radix et Rhizoma (Gancao). To tonify Qi, use Ginseng Radix (Renshen), Astragali Radix (Huangqi), and Atractylodis Macrocephalae Rhizoma (Baizhu). To remove nebula (corneal opacity), use Equiseti Herba (Muzei), Tribuli Fructus (Jili), Cicadae Periostracum (Chantui), and Serpentis Skin (Shetui). To brighten vision, use Dicentrae Bulbus (Mimenghua), Eyebright Herb (Gujingcao), Veronicaceae Seed (Qingxiangzi), Cassiae Semen (Caojueming), Sheep Liver (Yanggan), and Bupleuri Radix (Chaihu).
Toothache is Due to Stomach Heat and Qi Deficiency
Teeth are the exterior markers of the Kidney and the remnants of bone. When Kidney Qi is substantial, teeth are firm; when Kidney Qi is deficient, teeth become loose. Toothache is not a Kidney issue but rather heat in the Yangming Meridian. Treat with Qingwei San (Clear the Stomach Powder). For pain due to Qi deficiency, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Rehmanniae Radix Preparata (Shudi), Moutan Cortex (Danpi), Poria (Fuling), and Paeoniae Radix Alba (Baishao). For dental caries cavities, use Zanthoxyli Fructus (Chuanjiao) and burnt lime ground into powder, formed into pills with honey, and inserted into the cavity for cure. For periodontal disease (gangrene), burn Myrobalanus Fructus (Wubeizi) to ash, add a small amount of Os Draconis (Longgu) powder, and apply externally for miraculous effect.
Qingwei San
Qingwei San uses Cimicifugae Rhizoma (Shengma) and Coptidis Rhizoma (Huanglian); Angelicae Sinensis Radix (Danggui), Rehmanniae Radix (Shengdi), and Moutan Cortex (Danpi) are complete. Adding Gypsum Fibrosum (Shigao)平和s Stomach heat; it treats mouth ulcers, hematemesis, epistaxis, and gingival recession.
Throat Bi is Due to Fire Stirring and Phlegm Rising
Throat Bi refers to blockage and obstruction of the throat and pharynx. Called Ru'e (tonsillar hypertrophy) or Chanhou Feng (winding throat wind), although names differ, the cause is Fire and Phlegm. Pulse floating and faint indicates untreatable cases. Medication principles: Clear heat with Coptidis Rhizoma (Huanglian), Scrophulariae Radix (Yuanshen), Sophorae Tonkinensis Radix (Shandougen), and Juncus Thallus (Dengxin). Detoxify with Belamcandae Rhizoma (Shegan), Arctii Fructus (Niubangzi), and Glycyrrhizae Radix et Rhizoma (Gancao). Eliminate phlegm with Fritillariae Bulbus (Beimu), Trichosanthis Radix (Huafen), Poria (Fuling), Platycodonis Radix (Jiegeng), and Aurantii Fructus (Zhiqiao). Nourish Yin with Paeoniae Radix Alba (Baishao), Rehmanniae Radix (Shengdi), Phellodendri Cortex (Huangbai), Anemarrhenae Rhizoma (Zhimu), and Bamboo Juice (Zhuli).
A youth returned from a long journey during extreme heat, suddenly experiencing throat blockage, facial heat, and rolling tears. I stated that sudden illness belongs to Fire, and strange illness belongs to Phlegm. I used Chen Sha (Cinnabar) 5 fen and Bai Fan (Alumen) 2 qian ground into powder, taken with cold water; he recovered instantly. I also treated a patient mute due to Phlegm who spoke immediately after taking this formula.
Nasal Congestion is Due to Lung Qi Obstruction
The nose is the orifice of the Lung. There are two types of nasal congestion: inability to smell fragrances or odors, or congestion mainly in cold months, or congestion upon slight exposure to Wind-Cold; this is due to latent Fire pathogen in the Lung Meridian. Excessive Fire prefers warmth and dislikes cold, so it congests in winter and flares up with Wind. If nasal congestion and heavy voice occur suddenly due to Wind-Cold exposure, treat as Wind-Cold. Generally, except for Wind-Cold nasal congestion, nasal diseases are caused by Fire-Heat and should be treated with heat-clearing herbs.
Xinyi San (Magnolia Powder)
Xinyi San contains Litseae Fructus (Gaoben), Saposhnikoviae Radix (Fangfeng), Angelicae Dahuricae Radix (Baizhi), Cimicifugae Rhizoma (Shengma), and Akebiae Caulis (Mutong), plus Chuanxiong Rhizoma (Chuanxiong), Asari Radix et Rhizoma (Xixin), and Glycyrrhizae Radix et Rhizoma (Gancao), taken with tea; it attacks nasal polyps.
Canger San (Xanthium Powder)
Canger San uses Menthae Haplocalycis Herba (Bohe), combined with Magnolia Flower (Xinyi) and Angelicae Dahuricae Radix (Baizhi); taken with scallion and tea, it disperses Liver and Lung Qi, clears the upper and descends turbidity, curing Biyuan (sinusitis).
------------------------------------------
① Xi: xi1 (sound 'xi'). Xi Rou: Nasal polyp.
Mouth Ulcers are Due to Wandering Spleen Fire
The mouth is the external manifestation of the Spleen. When Spleen Fire rises, ulcers form in the mouth; treat with Xiehuang San (Drain the Yellow Powder). Also, apply powder of Coptidis Rhizoma (Huanglian) and Zingiberis Rhizoma (Ganjiang). For deficiency Fire flaming upward unresponsive to cooling herbs, use Lizhong Tang (Regulate the Middle Decoction) for counter-treatment. Heart heat causes bitter taste; treat with Xiexin Tang (Drain the Heart Decoction). Spleen heat causes sweet taste; treat with Xiehuang San. Lung heat causes spicy/pungent taste; treat with Xiebai San. Kidney heat causes salty taste; treat with Zishen Wan (Nourish the Kidney Pill). Liver-Gallbladder excess heat causes sour and bitter taste; use Bupleuri Radix (Chaihu), Gentianae Radix (Longdancao), Citri Reticulatae Pericarpium Viride (Qingpi), and Scutellariae Radix (Huangqin). Stomach deficiency heat causes bland taste; use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). Dry, cracked lips with ulcers indicate Spleen Blood insufficiency; use Guipi Tang (Restore the Spleen Decoction).
Xiehuang San
Xiehuang San consists of Glycyrrhizae Radix et Rhizoma (Gancao), Saposhnikoviae Radix (Fangfeng), Gypsum Fibrosum (Shigao), Gardeniae Fructus (Zhizi), and Agastaches Herba (Huoxiang); fried until aromatic, mixed with honey and wine, it works for Stomach Fire mouth ulcers.
Xiebai San
Xiebai San (Decoction) uses Morus Alba Cortex (Sangpigu) and Daturae Radix (Gugupi), suitable with Glycyrrhizae Radix et Rhizoma (Gancao) and Oryzae Fructus (Jingsmi). Ginseng Radix (Renshen), Poria (Fuling), Anemarrhenae Rhizoma (Zhimu), and Scutellariae Radix (Huangqin) can also be added; it is applied for Lung Heat wheezing and cough.

Women's Irregular Menstruation is All Due to Qi Reversal
Danxi discussed the essence of menstruation: Menstrual blood is Yin Blood. Blood is paired with Qi and moves with Qi. Menstrual blood forming clumps indicates Qi coagulation. Pain before menstruation indicates Qi stagnation. Pain after menstruation indicates Qi and Blood deficiency. Pale color indicates Phlegm and deficiency. Erratic flow indicates Qi chaos. Purple color indicates Qi heat. Black color indicates severe heat. For all irregular menstruation, Siwu Tang (Four Substances Decoction) is the main formula.
Medication principles: Tonify Qi with Ginseng Radix (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), Astragali Radix (Huangqi), and Glycyrrhizae Radix et Rhizoma (Gancao). Nourish Blood with Angelicae Sinensis Radix (Danggui), Chuanxiong Rhizoma (Chuanxiong), Paeoniae Radix Alba (Baishao), Rehmanniae Radix Preparata (Shudi), Artemisiae Argyi Folium (Aiye), Colla Corii Asini (Ejiao), and Polygoni Multiflori Caulis Carbonized (Puhuang Tan). Resolve Blood stasis with Carthami Flos (Honghua), Persicae Semen (Taoren), Angelicae Sinensis Radix Tail (Guiwei), Moutan Cortex (Danpi), and Achyranthis Bidentatae Radix (Niuxi). Clear heat with Bupleuri Radix (Chaihu), Scutellariae Radix (Huangqin), Anemarrhenae Rhizoma (Zhimu), Phellodendri Cortex (Huangbai), Coptidis Rhizoma (Huanglian), and Rehmanniae Radix (Shengdi). Warm meridians with Zingiberis Rhizoma (Ganjiang), Aconiti Lateralis Radix Preparata (Fuzi), and Cinnamomi Cortex (Rougui). Stop pain with Amomi Fructus (Sharen) and Corydalis Rhizoma (Yuanhu). Remove Phlegm with Pinelliae Rhizoma (Nanxing) and Pinelliae Rhizoma Preparatum (Banxia). Astringe with Halloysitum Rubrum (Chishizhi) and Longtan Earth (Fulonggan).
See Siwu Tang on page 6.
Women's Irritability and Tidal Fever are Mostly Due to Depression
Women often suffer from depression symptoms: alternating chills and fever, reduced appetite, and emaciation. Use Yueju Wan (Resolve Six Pills) to open depression, and Xiaoyao San (Free and Easy Wanderer Powder) to regulate menstruation.
See Yueju Wan on page 24.
Xiaoyao San
Xiaoyao San uses Angelicae Sinensis Radix (Danggui) and Paeoniae Radix (Baishao), plus Bupleuri Radix (Chaihu), Poria (Fuling), Atractylodis Macrocephalae Rhizoma (Baizhu), and Glycyrrhizae Radix et Rhizoma (Gancao), adding Menthae Haplocalycis Herba (Bohe); its merit in dispersing depression and regulating menstruation is swift. Combining with Moutan Cortex (Danpi) and Gardeniae Fructus (Zhizi) creates Tiaojing Dan (Regulate Menstruation Pill).
Leukorrhea and Gravel Lin are Due to Damp-Heat
Leukorrhea resembles thick, sticky mucus; treatment should focus on clearing the Heart and tonifying/nourishing. Gravel Lin resembles thin water; treatment should focus on clearing heat and drying dampness.
In healthy women, pure white discharge indicates Damp-Heat sinking downward. In chronically ill women, mixed red and white discharge indicates Qi deficiency sinking downward; treat with Guipi Tang (Restore the Spleen Decoction) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
A fifty-year-old woman suffered from white gravel Lin with chest oppression. I prescribed one dose of Yueju Wan (Resolve Six Pills), and both symptoms were completely cured.

Metrorrhagia and Metrostaxis (Beng Lou) are Due to Damage to Chong and Ren Mai
Beng Lou disease results from damage to the Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel). The Chong Mai is the Sea of Blood for the twelve meridians; the Ren Mai is the source of generative Qi. Damage to these two vessels causes blood to flow recklessly. Initially, it is excess-heat, requiring heat-clearing. After a short time, it becomes deficiency-heat, requiring blood-nourishing and fire-clearing. After a long time, it becomes deficiency-cold, requiring warming of meridians and blood-tonification.
Clear Heat: Scutellariae Radix (Huangqin), Coptidis Rhizoma (Huanglian), Phellodendri Cortex (Huangbai), Anemarrhenae Rhizoma (Zhimu), Rehmanniae Radix (Shengdi), Ophiopogonis Tuber (Maidong).
Nourish Blood: Angelicae Sinensis Radix (Danggui), Chuanxiong Rhizoma (Chuanxiong), Paeoniae Radix Alba (Baishao), Rehmanniae Radix Preparata (Shudi), Artemisiae Argyi Folium (Aiye), Colla Corii Asini (Ejiao), Polygoni Multiflori Caulis Carbonized (Puhuang Tan).
Tonify Qi: Ginseng Radix (Renshen), Astragali Radix (Huangqi), Glycyrrhizae Radix et Rhizoma (Gancao), Atractylodis Macrocephalae Rhizoma (Baizhu).
Regulate Qi: Citri Reticulatae Pericarpium (Chenpi), Cyperi Rhizoma (Xiangfu), Amomi Fructus (Sharen).
Raise Yang: Notopterygii Rhizoma (Qianghuo), Saussureae Radix (Duohuo), Saposhnikoviae Radix (Fangfeng), Cimicifugae Rhizoma (Shengma), Bupleuri Radix (Chaihu).
Astringe: Halloysitum Rubrum (Chishizhi), Longtan Earth (Fulonggan).

Seven Causes of Postpartum Fever
There are seven causes of postpartum fever: fever due to excessive blood loss, fever due to lochia retention, fever due to Wind-Cold invasion, fever due to dietary injury, fever due to lactation (milk steaming), fever due to breast engorgement, and fever due to early postpartum labor. One must inquire about the cause and palpate the pulse. For excessive blood loss, the six pulses will be deficient; use Yiqi Yangying Tang (Tonify Qi and Nourish Ying Decoction). For lochia retention, there will be abdominal pain; use Heishen San (Black Divine Powder). For Wind-Cold invasion, there will be headache; use Wuji San (Five Accumulations Powder). For dietary injury, there will be chest oppression; use Xiaoshi Yin (Digest Food Drink). For lactation fever, milk ducts are blocked; use Tongru Tang (Unblock Milk Decoction). For breast engorgement fever, no one suckles; use 5 qian of stir-fried Hordeum Vulgare Fructus (Maiya) ground into powder, taken with rice water. For early postpartum labor, there will be lower back/hip pain; use Zhushen Yin (Pig Kidney Drink). Generally, postpartum medication must be warm to promote lochia flow, focusing on major tonification of Qi and Blood; other symptoms are treated secondarily.
