If one desires to stay alone with doors and windows closed, and Yin fails to overcome Yang, the blood vessels will flow rapidly and turbidly, leading to Qi and Blood converging in the Yang aspect, resulting in mania. (See "Treatment of Deficiency and Excess" for the full text.)
Blood converges in the Yang, and Qi converges in the Yin, thus causing fright and mania. (See "Acupuncture for Deficiency" for the full text.)
[Suwen] The Yellow Emperor asked: There is a disease characterized by anger and mania; how does this disease arise? Qibo replied: It arises from Yang. The Yellow Emperor asked: How does Yang cause mania? Qibo said: The Yang Qi of the body, due to sudden frustration and difficulty in resolution, tends to become angry easily; this disease is named "Yang Jue" (Yang Repletion). The Yellow Emperor asked: How do we know this? Qibo said: The Yangming channel usually has a pulsation, while the Taiyang and Shaoyang channels do not usually pulse. If the non-pulsating areas show a large and rapid pulse, this is the sign. The Yellow Emperor asked: How to treat it? Qibo said: Depriving the patient of food will cure it. Because food enters the Yin and generates Qi in the Yang, depriving food cures it. One can also administer a drink made from raw iron filings (Sheng Tie Luo). Raw iron filings have the effect of descending rebellious Qi quickly. (From "Chapter on Disease Capabilities")
[Wu Ming Shi] Guo Fan Dan is effective in treating mania. (Formula see "Epilepsy Section")
Zhen Xin Dan treats palpitations with spontaneous sweating, vexation, shortness of breath, uncontrollable emotions such as joy, anger, sorrow, and aversion, complete lack of self-awareness, loss of soul and spirit, appearing possessed by spirits, as well as seminal emission in men and leukorrhea in women.
Cinnabar (ground) White Alum (calcined until juice is exhausted, equal parts)
Grind the above into powder, make water pills the size of a corn kernel. Take one pill per dose, decocted with Ginseng Decoction, taken after meals.
[Worldly Transmission] A woman in Zhejiang suffered from incessant epilepsy and mania. A doctor used half a liang of Genggen (Melon Stalk) ground into powder, taking one qian per dose, mixed with well water to fill a cup and administered. This induced severe vomiting, followed by deep sleep without disturbance, after which she recovered completely.
[Bamboo Hall Medical Cases] Laisu Gao treats long-standing or recent wind-phlegm heart diseases, wind-mania, stroke, tidal phlegm obstruction, locked jaw, and tetanus, all of which are treated effectively.
Gleditsia Sinensis Fruit (one liang, select plump ones without insect damage, remove skin and strings)
First, chop the Gleditsia fruit finely. Use one bowl of fermented rice water. Soak for three to four days in spring and autumn, seven days in winter, or one to two days in summer. Rub to extract the thoroughly soaked Gleditsia juice. Place in a silver vessel or clay pot. Simmer over gentle fire, stirring with new willow branches or locust branches. Boil until it resembles an ointment consistency, then take out. Spread on layered paper, dry in shade, and store. When encountering a patient, take a piece the size of a palm, dissolve in warm fermented rice water in a cup. Use a bamboo tube to hold the medicinal liquid. Assist the patient to sit upright, tilt the head slightly back, and blow the medicine into both nostrils. After a while, lift the patient; the discharge of phlegm indicates efficacy. To stop the discharge, let the patient drink one or two mouthfuls of warm salt water. Avoid chicken, fish, raw, hard, and damp dough foods.
[Zhang Zihe] A man fell from a horse and developed mania. Upon rising, his eyes were wide open, he spoke wildly, did not recognize relatives or strangers, removed his clothes and ran, shouted curses continuously, and had double the strength, unable to be restrained by three or four strong men. He burned talismans, performed rituals, consulted ghosts, and danced for spirits, disregarding everything else. He took Cinnabar, Realgar, Rhinoceros Horn, Pearl, Borneol, and Musk, depleting his wealth, leaving his home desolate. Traveling two hundred li to seek Dai Ren's treatment, Dai Ren buried a cart axle in the ground, about two zhang high, mounted a medium-sized wheel on top, carved a hole in the wheel like a basin, tied the patient on it, and made him lie prone, cushioned with soft material. Another person sat below operating a mechanism, turning it with a stick for thousands of rotations. The patient vomited one or two dou of green and yellow phlegm, circling the wheel several times. The patient said, "I cannot endure this, untie me." They untied him as requested. He asked for cool water, drank several sheng of ice water, and the mania ceased.
[Suwen] The Yellow Emperor asked: In severe Yangming disease, patients discard their clothes and run, climb high and sing, may go without food for several days, climb walls and roofs, going to places they could never reach before, yet the disease enables them to do so. Why is this? Qibo replied: The four limbs are the root of all Yang meridians. When Yang is excessive, the four limbs are solid and full, enabling climbing high. The Yellow Emperor asked: Why discard clothes and run? Qibo said: Heat is intense in the body, so they discard clothes and wish to run. The Yellow Emperor asked: Why speak wildly, curse, ignore kinship, and sing? Qibo said: Yang excess causes wild speech, cursing, ignoring kinship, and lack of desire to eat. Lack of desire to eat leads to wandering. (From "Yangming Pulse Resolution Chapter" What is meant by Yangming disease manifesting as wanting to climb high, sing, discard clothes, and run is that Yin and Yang struggle, and Yang merges outwardly into Yang, causing these symptoms.)
[Li Dongyuan] Xu suffered from Yang Jue, manic rage, cursing kin and strangers, sometimes crying, sometimes singing. All six pulses were weak upon lifting and pressing. The body surface felt cold like water and stone. Onset was accompanied by loud shouting. Ji Gu stated: Restricting food cures it. Therefore, no food was given. I used Da Cheng Qi Tang to purge him, expelling several sheng of fecal matter and filth from the viscera. The mania subsided slightly. It relapsed after a few days, and purgation was repeated. This happened five or seven times, expelling several dou of stool, and the disease was cured. The body became warm, pulses returned, and he fully recovered. This is Yi Lao's (Zhang Yuansu) method of restricting food.
[Baoming Ji] Dang Gui Cheng Qi Tang
Angelica Sinensis Rhizome (Dang Gui) Rhubarb Root (Da Huang) (each one liang) Licorice Root (Gan Cao) (half liang) Mirabilite (Mang Xiao) (seven qian)
Chop the above ingredients to the size of hemp seeds. Take two liang per dose, add one bowl of water, five slices of ginger, and ten jujubes. Decoct until half remains, strain, and take warm. If there is Yang mania with running and cursing, unaware of kinship, this indicates Yang excess and Yin deficiency. Da Huang and Mang Xiao clear excess heat in the stomach. Dang Gui nourishes blood and benefits Yin. Gan Cao moderates the middle. Ginger and jujube are added because the stomach belongs to Earth, guiding the medicine to the stomach. As the classics say, mild cases are treated oppositely, severe cases similarly. This is what is meant. Aim for significant purgation. If mildly relieved, use Gua Di San with Fang Feng powder and Li Lu powder to induce vomiting, and the disease will settle immediately. Afterwards, regulate with Tiao Xin San, Xi Xin San, Liang Ge San, Jie Du Tang, etc.
[Zhang Zihe] A manic patient had Yin failing to overcome Yang, causing blood vessels to flow rapidly and turbulently, with Yang merging causing mania. Nan Jing states: Extreme Yang leads to mania, extreme Yin leads to depression. Yang refers to Fu organs, Yin to Zang organs. It is not that Yang is hot and Yin is cold, but rather heat merging into Yang causes mania, and mania generates cold; merging into Yin causes depression, and depression leads to death. Nei Jing states: Foot Yangming excess leads to mania. Thus, climbing high and singing, discarding clothes and running, doing anything, represents extreme heat. Use Diao Wei Cheng Qi Tang in large doses to induce dozens of bowel movements. Every three to five days, induce vomiting of one or two sheng. Every three to five days, purge again. Over fifty to sixty days, more than a hundred purgations and seven or eight vomitings occurred. During vomiting, keep the room warm and place fire to promote sweating. Only after multiple sweatings did it calm down.
An elderly man of sixty, suffering from heavy corvée labor and disturbance, suddenly developed mania. He felt insects crawling on his mouth and nose, scratching with both hands for years without relief. Dai Ren diagnosed his pulses as both large and forceful like a drum. He judged: Liver governs planning, Gallbladder governs decision-making. Under urgent corvée pressure and financial inability to cope, the Liver planned repeatedly while the Gallbladder failed to decide. Grievance had no outlet, anger had no release. Heart Fire surged, attacking Yangming Metal. Although the Stomach belongs to Earth, Liver belongs to Wood, and Gallbladder is Ministerial Fire, Fire follows Wood Qi into the Stomach, causing sudden mania. He ordered the patient to be placed in a warm room, inducing vomiting and sweating, repeating this three times. Nei Jing states: For Wood stagnation, disperse it; for Fire stagnation, vent it. This is exactly what is meant. Then use half a jin of Diao Wei Cheng Qi Tang, decocted in five sheng of water until half boiling, divided into three doses. Profuse purgation occurred twenty times, expelling several sheng of bloody water mixed with stasis. He recovered the next day. Regulate afterwards with Tong Sheng San.
[Sun Simiao] Monk Chong of Xiangguo Temple suddenly suffered from epilepsy. After half a year, various famous doctors' medicines were ineffective. His lay brother Pan, whose family was wealthy, summoned Sun Simiao. Sun said: He will sleep tonight and recover tomorrow or the day after. Pan said: Please tell me the prescription first, so I may remember the kindness. Sun said: Give him salty food. Wait until he is thirsty, then come find me. That night, the monk indeed became thirsty. Sun arrived, requested a cup of warm wine, mixed one dose of medicine, and gave it to him. After a while, he asked for more wine, and Sun gave him half a cup. The monk then slept for two days and nights before waking, with normal consciousness. Pan thanked Sun and asked about the treatment. Sun said: Others can calm the spirit but cannot induce spiritual unconsciousness for sleep. This is the Zhu Sha, Suan Zao Ren, and Ru Xiang Powder from Ling Yuan Fang, which people fail to use correctly.
Zhu Sha San treats wind-phlegm various epilepsies, wild speech and walking, mental confusion, chaotic thoughts, alternating singing and crying, abnormal eating and drinking, sudden collapse during attacks, foaming at the mouth and staring eyes, loss of soul and spirit, and failure of medical treatments.
Cinnabar (one liang, must be bright and block-like) Sour Jujube Seed (Suan Zao Ren) (half liang, lightly stir-fried) Frankincense (Ru Xiang) (half liang, translucent)
Measure the patient's alcohol tolerance. First, let him drink freely until drunk, but do not allow vomiting. In a quiet room, mix all the above medicines into one dose, blend with warm wine to make one cup, and instruct to drink it all at once. For those who rarely drink alcohol, intoxicate according to tolerance. After taking the medicine, place pillows and bed for rest. For mild cases, half a day to one day; for deep cases, two to three days. Have family members observe secretly. Ensure regular breathing. Do not wake him, nor startle him awake. Wait for natural awakening, then the soul and spirit will settle. If startled awake, it cannot be treated again. Official Wu Zhengsu suffered from heart disease in his youth. Taking one dose of this formula, he woke after five days and recovered.
Yi Zui Gao treats heart ailments.
Two bowls of pure wine (Wu Hui Jiu), four liang of sesame oil. Mix evenly. Stir each of the twenty willow branches one or two hundred times. Wait until the oil and wine merge into a paste. Decoct to eight-tenths concentration and administer. Deep sleep ensues. Awakening, vomiting, or purgation brings relief.
Mania manifests as little sleep. Little sleep causes Wei Qi to travel only in Yang, not entering Yin. Thus, Yang is excessive and Yin is deficient, causing spiritual confusion. Sleep allows Wei Qi to enter Yin, and Yin is filled by Wei Qi without deficiency. Yang, without Wei Qi support, is not excessive. Thus, Yin and Yang balance, and recovery occurs.
[Bencao] Ning Zhi Gao
Ginseng Root (Ren Shen) Sour Jujube Seed (Suan Zao Ren) (each one liang) Cinnabar (Zhu Sha) (five qian) Frankincense (Ru Xiang) (one fen)
Grind into fine powder. Mix with refined honey and form pills the size of a bullet. Take one pill per dose, delivered with Mint Decoction. My clan brother lost his mind due to war fires. I prepared this formula for him. After taking twenty pills, he recovered. Relatives and friends widely transmitted it, and all found it effective.
[Li Dongyuan] Mania, evil, and epilepsy involve unwillingness to sleep, arrogance, self-righteousness, and ceaseless妄行. This formula calms the five Zang organs and descends Heart Qi. Cook one white rooster thoroughly. Season with five flavors and eat as soup or porridge. Classics state: Sadness moving internally injures Hun. Hun injury leads to madness and lack of essence. Lack of essence leads to disorder. This is sadness injuring Hun causing mania. Warm herbs should be used to tonify Hun Yang. Zhang Zhongjing's Di Huang Tang and Ben Shi Fang's Jing Qi Wan are examples. Classics state: Unrestrained joy injures Po. Po injury leads to mania. Maniacs disregard others. This is joy injuring Po causing mania. Cool herbs should be used to tonify Po Yin. Cinnabar, Curcuma, and Alum are examples.
[Worldly Transmission] Treat loss of mind.
Curcuma Longa Rhizome (Yu Jin) (ten liang, Sichuan Cicada Belly variety is authentic) Alum (Ming Fan) (three liang)
Grind into fine powder. Make pills with thin paste, size of tung tree seeds. Take 50-60 pills per dose, with water or broth. Formerly, a woman suffered from terrifying mania for years without cure. Later, she met a master who taught her this formula. Initially, she felt something leave her chest and mind, feeling refreshed. After a second dose, she revived instantly. The master said: This disease arises from worry and fright. Phlegm-saliva envelopes the Heart orifice. This medicine removes stagnant phlegm.
[Zhang Zhongjing] Fang Ji Di Huang Tang treats conditions resembling mania, wild behavior, continuous solitary talking, without chills or fever, with floating pulses.
Stephania Tetrandra Root (Fang Ji) (one qian) Cinnamon Twig (Gui Zhi) Saposhnikovia Root (Fang Feng) (each three qian) Licorice Root (Gan Cao) (two qian)
Take the above four ingredients. Soak in one cup of wine overnight. Strain and extract juice. Take two jin of fresh Rehmannia Root (Sheng Di Huang).
