[Li Dongyuan] In the *Da Lun* (Great Treatise) of the *Liu Yuan Zheng Ji*, it is stated: "When the Taiyang qi arrives, it manifests as lower back pain." It further states: "Ju Yang, which is Taiyang, when deficient, causes pain in the head, nape, lower back, and spine." The Foot-Taiyang Bladder Meridian passes through these areas; it emerges from the head, descends to the nape, runs along the medial aspect of the shoulder blades, flanks the spine, and reaches the middle of the waist. Therefore, when diseased, symptoms include a sensation of the neck being pulled out, pain flanking the spine, a sensation that the waist is about to break, and the inability to flex the thighs. This occurs because when the meridian qi is deficient, pathogenic factors invade, generating pain. Pathogenic factors refer to Wind, Heat, Dampness, Dryness, and Cold, all of which can cause disease; generally, Cold-Dampness is more common than Wind-Heat. However, there are cases of lower back pain due to Kidney deficiency caused by sexual exhaustion. This is because Yang Qi is weak and unable to move and transform. The Classics state: "The waist is the mansion of the Kidney; if turning and shaking become difficult, the Kidney Qi is failing." Suitable formulas include Shenqi Wan (Kidney Qi Pill) and Huixiang Wan (Fennel Pill) to supplement insufficient Yang Qi.
For those who consume rich and greasy foods, long-term use of decoctions, and engaging in sexual activity while drunk damages True Qi, leading to Heat in the Kidney Qi. When Kidney Qi is hot, the lower back and spine ache and cannot be lifted. Over time, Marrow diminishes and bones wither, developing into Gu Wei (Bone Withering). Suitable formulas include Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia), Zishen Wan (Nourishing Kidney Pill), and Fengsui Dan (Sealing Marrow Elixir) to supplement insufficient Yin fluids. The *Ling Shu* states: "For lower back pain with cold above, select points on the Foot-Taiyin Spleen and Foot-Yangming Stomach meridians; for heat above, select points on the Foot-Jueyin Liver meridian; for inability to bend or extend, select points on the Foot-Shaoyang Gallbladder meridian." Generally, the three Foot-Yang meridians run from head to foot, and the three Foot-Yin meridians run from foot to abdomen. Pain can occur wherever the meridians pass. Treatment requires carefully determining which meridian's domain is affected, needling the corresponding acupoints, and prescribing herbs based on Cold or Heat patterns. For example, lower back pain caused by the Foot-Taiyang meridian, radiating to the nape, spine, sacrum, and back with a heavy sensation, should be treated by bleeding the Weizhong point and the Taiyang meridian. Other cases follow this pattern. Those who insist on using a single formula for all types of lower back pain are indeed inflexible.
[Zhu Danxi] For chronic lower back pain, Guigui (Cinnamon Twig/Cassia Bark) must be used to open the obstruction for the pain to cease; the same applies to abdominal and hypochondriac pain.

In the 8th month of the Wuxu year, heavy floods occurred in Huainan, inundating the city walls for months. I suddenly experienced internal organ dysfunction, with roaring water sounds in my abdomen for several days. After treatment, I recovered. Since then, I suffered from lower back pain, unable to flex or extend, even affecting washing my face. Various medicines were ineffective for three months. I later reasoned that this was likely due to excessive Yin Water Qi invading the Kidney Meridian. I moxa'd the Shenshu acupoint 37 moxibustions and took this medicine, resulting in recovery.
