Jaundice, if it persists for a long time, transforms into Black Jaundice (Hei Dan), characterized by yellowing of the body and blackening of the forehead, or yellowing of both the body and face. If the body is not yellow but only the mouth and face are black, treatment should focus on the Zhangmen point (LR13). (The *Zhenjiu Jing* states: Shenmen (HT7) also treats jaundice.)
[Zhang Zhongjing] When Wine Jaundice (Jiu Dan) is treated with purgatives, it may transform into Black Jaundice after a long period. Symptoms include bluish-black discoloration around the eyes and face, a sensation in the heart region as if garlic had been consumed, pure black stool, and skin that feels numb to scratching. The pulse is floating and weak. Although the complexion is black, a faint yellow hue remains, which allows for diagnosis. Patients with Jaundice typically experience fever at dusk but paradoxically feel aversion to cold; this is caused by Female Labor Jaundice (Nv Lao De Zhi - jaundice due to sexual exhaustion). There is urgency in the bladder, fullness in the lower abdomen, yellowing of the entire body, blackening of the forehead, and heat in the soles of the feet, leading to Black Jaundice. If abdominal distension resembles water accumulation (ascites), with black stool and occasional loose stools, this is a disease of Female Labor Jaundice, not water retention. Abdominal fullness indicates a difficult-to-treat condition. It is treated with Xiaoshi San (Niter Powder).
Xiaoshi (Niter), Fanshi (Alum) (calcined, equal parts)
Grind into a fine powder and mix evenly. Take one fangcun bi (a square-inch measure) mixed with barley porridge, three times daily. As the disease resolves, pathogens will exit via urine and stool: yellow fluid in urine and black matter in stool indicate efficacy.
*[Shengji Zonglu]* Black Jaundice often has a poor prognosis and requires urgent treatment. Use one jin of Tugua Gen (Trichosanthes kirilowii root), crush it, extract six he of juice, and take it in one dose. Yellowish water should appear in the urine; continue administration.
Facial edema, back pain preventing upright posture, dark complexion, excessive sweating, and aversion to wind belong to Wind pathogen patterns. (Treatment see Chapter on Various Winds.)
[Zhu Danxi] Abdominal pain can be caused by Cold, Accumulated Heat, Dead Blood, Food Stagnation, or Damp-Phlegm.
[Li Dongyuan] Heart and stomach pain, as well as various pains in the abdomen, arise from excessive labor, irregular diet, deficiency of Middle Qi, allowing Cold Evil to invade when defenses are low, causing sudden severe pain. The Classics state that pain ceases upon receiving heat ("Jiong" means heat). Treating cold with heat is the correct approach. However, abdominal pain has specific locations, and organs have high and low positions; thus, treatment must be differentiated. For example, Jue Xin Tong (Jue-type Heart Pain) is caused by Cold Evil lodging in the Pericardium Meridian. Ancients used Liangjiang (Galangal Rhizome) and Changpu (Acorus Rhizome), both large acrid-hot herbs, ground into powder and taken with wine, stopping pain immediately; this directly subdues the pathogen. Zhen Xin Tong (True Heart Pain) occurs when Cold Evil injures the Heart Monarch; hands and feet turn cyan up to the joints, and death may occur within hours (morning onset leads to evening death, evening onset leads to morning death). Pain in the Zhongwan region belongs to the Taiyin (Spleen); treat with Lizhong Tang (Regulate the Middle Decoction), Jianzhong Tang (Establish the Middle Decoction), or Caodoukou Wan (Cardamom Pill). Pain around the umbilicus belongs to Shaoyin (Kidney); treat with Sini Tang (Reverse the Yin Decoction) or Jiangfu Yuhan Tang (Ginger and Aconite Cold-Repellent Decoction). Pain in the Lower Abdomen belongs to Jueyin (Liver); treat with Zhengyang San (Upright Yang Powder), Huiyang Dan (Return Yang Pill), or Danggui Sini Tang (Angelica Sinensis Reverse the Yin Decoction). Pain from miscellaneous diseases is treated with Koushen Tang (Melia Decoction), Jiuzhu Danggui Wan (Wine-Boiled Angelica Pill), or Dingxiang Lianshi Wan (Clove Melia Seed Pill). This follows the principle of treating based on location. Additionally, moxibustion and acupuncture should be applied to the Shu points corresponding to each organ. For Jue Xin Tong, where pain feels like needle insertion into the heart, involving severe Spleen pain, select Ran Gu (KI2) and Taixi (KI3); other organs follow similar principles. For disharmony in the abdomen causing pain, use Gancao Shaoyao Tang (Licorice and Peony Decoction). If Taiyang Exogenous Disease is mistakenly purged, transmitting to Taiyin causing abdominal fullness and pain, use Guizhi Jia Shaoyao Tang (Cinnamon Twig Plus Peony Decoction). For severe pain, use Guizhi Jia Dahuang Tang (Cinnamon Twig Plus Rhubarb Decoction). In summer, if there is muscle heat, aversion to heat, rapid and surging pulse with pain, use Huangqin Shaoyao Tang (Scutellaria and Peony Decoction). For pain caused by parasites, such as mental restlessness and abdominal pain moving up and down, with intervals of relief, abdominal heat, thirst, salivation, alternating blue/white/red facial color, and vomiting clear fluid, this is Ascaris biting. Press firmly and hold still to prevent movement, insert a needle, keep it in until the parasite stops moving, then remove. Alternatively, use Huachong Wan (Parasite-Dissolving Pill) from the *Ju Fang* or other anti-parasitic drugs, adjusted for deficiency or excess; do not treat uniformly.
[Wang Haogu] Autumn abdominal pain with cold muscles, aversion to cold, and deep-micro pulse involves Foot Taiyin and Foot Shaoyin meridians; treat with Guizhi Shaoyao Tang. Zhongwan pain belongs to Taiyin; treat with Lizhong Tang, Jianzhong Tang, or Huangqi Jianzhong Tang. Umbilical pain belongs to Shaoyin; treat with Sini Tang, Zhenwu Tang, or Fuzi Tang. Lower abdominal pain belongs to Jueyin; severe cases use Zhengyang San or Huiyang Dan, mild cases use Danggui Sini Tang. If Taiyin transmits to Shaoyin with severe pain, it should transform into diarrhea and stop.
[Zhang Zhongjing] For deficiency consumptive disorders with internal tension and abdominal pain, use Xiao Jianzhong Tang (Minor Establish the Middle Decoction). (Formula see Exogenous Disease chapter.)
[Luo Tianyi] Case of Liver Overacting on Spleen: Liu Zhongmei, Administrator of Zhending Road, over sixty years old, presented with Spleen-Stomach deficiency heat. In the leap eighth month of Xinsi year, during cold weather, due to official duties and labor, he drank cold water when thirsty and suffered two episodes of diarrhea at midnight. At dawn, I examined him: pulse was string-thin and micro, limbs cold, palms cold, lips and tongue cyan-brown, slight abdominal pain, shortness of breath, and anorexia. I recalled the *Neijing*: "Blue color indicates Liver; Liver belongs to Wood. Lips represent Spleen; Spleen belongs to Earth. Wood conquers Earth, hence blue appears on lips." The *Nanjing* also states: "Seeing Liver disease, know it will transmit to Spleen; therefore, first strengthen Spleen Qi to prevent Liver evil invasion." Master Jiegu said: "If organs act with their conquering qi and insult the unconquerable, heavily tonify the unconquerable and slightly drain the conquering." Treat with Huangqi Jianzhong Tang plus Shaoyao and Fuzi. Shaoyao is sour to drain Liver Wood slightly. Huangqi and Gancao are sweet-warm to tonify Spleen Earth heavily. Guizhi and Fuzi are acrid-hot to treat Cold Water, assist Yang, and retreat Yin. Yitang is sweet-warm to tonify Spleen deficiency. "Liver suffers from tension; eat sweet to relax." Ginger and Jujube are acrid-sweet and greatly warm to generate Spleen-Stomach Qi, ascend clear Yang, circulate Ying and Wei, and relieve tension. Take one liang per dose, decoct according to method, and cure after two doses. Huangqi Jianzhong Tang doubles Shaoyao; add Fuzi based on deficiency/excess. (See Treatment of Deficiency/Excess chapter.)
[Zhang Zhongjing] Lizhong Tang (Formula see Exogenous Disease Shaoyin chapter.)
[Li Dongyuan] Caodoukou Wan (Cardamom Pill) treats Spleen-Stomach deficiency with Heart Fire overacting, failing to nourish Upper Jiao Yuan Qi. In winter, when Kidney and Bladder Cold Water are prosperous, "Child makes Mother solid," causing Lung Metal and Large Intestine to assist in attacking Heart Fire and invading Spleen-Stomach, representing a major retaliatory Qi (Fu Qi). The Classics say: "Great victory leads to great retaliation." Thus, Yuan Qi is exhausted in skin, blood vessels, and muscles; combined with Great Cold and Great Dryness, symptoms include aversion to wind/cold, tinnitus, waist/back pulling chest pain, nasal congestion, loss of smell, cold forehead/headache, dizziness, desire to keep eyes closed, Cold Water counter-attacking in abdomen with phlegm/saliva, regurgitation, constant abdominal/stomach pain, hypochondriac tightness, inability to strain, frequent diarrhea/infrequent constipation, continuous flatulence/borborygmus. This is extreme Spleen-Stomach deficiency. Chest Qi chaos, irritability, precursor to Huoluan (Cholera-like disorder). Throat blockage, dysphagia, loud breathing, obstruction, relief in sun/warm rooms, recurrence with wind/cold, limb coldness, heavy body unable to turn, difficulty rising, frequent urination with restlessness. This medicine treats autumn/winter Cold Fu Qi.

Shensheng Fuqi Tang (Divine Holy Restoring Qi Decoction) treats Fu Qi invading during Winter when Foot Taiyang Cold and Foot Shaoyin Kidney Water are strong. Child makes Mother solid, Hand Taiyin Lung becomes excessive and insults Earth, injuring Fire/Wood. Symptoms: waist/back/chest oppression/pain, sneezing, salivation, tearing, turbid nasal discharge, polyps losing smell, cough with sputum, upper heat like fire, lower cold like water, paroxysmal headache, eyes flowing fire, blurred vision, tinnitus/deafness, head/nose/mouth aversion to wind/cold, preference for sun, restless sleep, feeling of phlegm obstruction, throat blockage, tastelessness, hypochondriac tightness/pain, loose teeth, cold waist/navel/sacrum/buttocks/knees/feet, genital sweating/cold, limping, difficulty moving, cold palms, wind-bi numbness, frequent urination (day more/night less), shortness of breath, sudden fecal incontinence.
