Pulse
The pulse of malaria (Malaria) naturally presents as a string-like pulse (Xian Mai, String-taut Pulse). A string-like and rapid pulse indicates heat syndrome, suitable for diaphoresis (Diaphoresis); a string-like and slow pulse indicates cold syndrome, suitable for warming therapy (Warming Therapy); a string-like and tight pulse is suitable for purgation (Purgation); a floating and large pulse is suitable for emesis (Emesis); a string-like and short pulse suggests food stagnation (Food Stagnation); a string-like and slippery pulse suggests phlegm (Phlegm); a deficient, minute, and forceless pulse indicates chronic illness; a flooding, rapid, and forceless pulse indicates deficiency; the appearance of a intermittent pulse (Dai Mai, Intermittent Pulse) or a scattered pulse (San Mai, Scattered Pulse) indicates a fatal prognosis.
Symptoms
Malaria is classified into wind malaria (Wind Malaria), summerheat malaria (Summerheat Malaria), dampness malaria (Dampness Malaria), food malaria (Food Malaria), phlegm malaria (Phlegm Malaria), and malaria mass (Malaria Mass). The differences among various types of malaria are merely caused by external contraction of wind, cold, summerheat, and dampness, and internal injury from the seven emotions (Seven Emotions) and improper diet (Five Flavors). However, when internal and external defenses fail, true qi and pathogenic factors mix indistinguishably, yin and yang become imbalanced with excess, and cold and heat attack each other, resulting in malaria. It may occur once daily, every other day, every three days, or after an interval of one day followed by two consecutive days of发作, indicating that both qi and blood are affected. It may occur at night and day, before noon, or after noon. At the onset, there are yawning and stretching, aversion to cold with shivering, headache, thirst, or alternating chills and fever (Chills and Fever), or chills preceding fever, or fever preceding chills, or only chills without fever, or only fever without chills, or less chills with more fever, or more chills with less fever.
Treatment Principles
The treatment method should first focus on dispersing external pathogenic factors. Patients may have sweating or no sweating. For those without sweating, inducing sweating to disperse pathogens is primary; for those with sweating, stopping sweating and supporting healthy qi (Zheng Qi, Healthy Qi) is primary. However, if the disease does not recede after dispersing pathogens and supporting healthy qi, it is necessary to differentiate yin and yang, using Chai Ling Tang (Bupleurum and Poria Decoction) which is most effective. For severe cases, antimalarial drugs can be used to eliminate the disease, such as Bu Er Yin (Two-Not Decoction) and Sheng Jin Wan (Win Gold Pill). If still unhealed, it indicates severe deficiency of qi and blood, so supporting stomach qi (Stomach Qi) becomes fundamental, using Lu Jiang Yang Wei Tang (Dew Ginger Stomach-Nourishing Decoction) and Yang Wei Dan (Stomach-Nourishing Pill). For lingering malaria lasting over a year, after excessive use of sweating, vomiting, and purging methods, leading to depletion of Ying (Nutritive) and Wei (Defensive) qi, with pathogenic factors hiding between the hypochondrium and forming masses called malaria mass (Malaria Mass), use Nie Yin (Malaria Drink) and Huang Jia Wan (Yellow Shell Pill). Generally, malaria has new/old and shallow/deep distinctions, and treatment has urgency and sequence. One must combine pulse and symptom analysis, assess deficiency and excess, and treat accordingly. *Ji Yao* states: Taiyang channel involvement causes cold malaria, treated mostly with diaphoresis; Yangming channel involvement causes heat malaria, treated mostly with purgation; Shaoyang channel involvement causes wind malaria, treated mostly with harmonization. These three yang channels involved are called acute malaria (Acute Malaria). Onset after Summer Solstice and before Limit of Heat indicates shallow infection. Involvement of the three yang channels is generally termed cold malaria. Onset after Limit of Heat and before Winter Solstice indicates heavy infection. Malaria of the three yin channels occurring on Zi, Wu, Mao, You days is Shaoyin malaria; on Yin, Shen, Si, Hai days is Jueyin malaria; on Chen, Xu, Chou, Wei days is Taiyin malaria.
Precautions I: When malaria is about to发作, do not administer medicine during the active phase. Medication should be taken two hours before the expected发作. Otherwise, medicine and pathogen will contend, causing deeper harm. This must be avoided.
Precautions II: For those constitutionally weak, compounded by labor-induced internal injury and external pathogenic contraction leading to malaria, with alternating chills and fever, limb fatigue, weakness, and shortness of breath, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) added with Huang Qin (Scutellaria Root), Shao Yao (Peony Root), and Ban Xia (Pinellia Tuber). If sweating and severe aversion to cold, add Gui Zhi (Cinnamon Twig) and double Huang Qi (Astragalus Root); if severe heat, add Chai Hu (Bupleurum Root) and Huang Qin; if thirsty, add Mai Men Dong (Ophiopogon Tuber) and Tian Hua Fen (Trichosanthes Root).
Precautions III: Chronic malaria belongs to yuan qi (Primal Qi) deficiency. Qi deficiency generates cold, blood deficiency generates heat, stomach deficiency causes aversion to cold, spleen deficiency causes fever. Yin fire flowing downward causes alternating chills and fever, or spitting saliva and anorexia, diarrhea, abdominal pain, cold extremities, and chattering teeth. Misusing Qing Pi Yin (Clear Spleen Decoction) or Jie Nue Yin (Antimalarial Decoction) often leads to death.
Precautions IV: After malaria, with reduced appetite, limb weakness, sallow complexion, and body debility, use Si Jun Zi Tang (Four Gentlemen Decoction) combined with Er Chen Tang (Two-Cured Decoction), added with Ginger-fried Huang Lian (Coptis Rhizome) and Bran-fried Zhi Shi (Immature Bitter Orange), decocted and taken orally.
Precautions V: After malaria, profuse sweating indicates deficiency of Ying blood (Ying Blood), treated primarily with Ren Shen Yang Rong Tang (Ginseng Nourishing Ying Decoction).
