节无力。不可枝梧者。谓之瘫。其四肢虽能举动。而肢节缓弱。凭物方能运用者。谓之痪。
或以左为瘫。右为痪。则非也。但以左得之病在左。右得之病在右耳。推其所自。皆由气血

[Discussion] On the differentiation of paralysis (Tan Huan). "Tan" refers to flaccidity and weakness, inability to control or gather oneself. "Huan" refers to laxity and relaxation, inability to manipulate objects. Therefore, when symptoms manifest as the inability to lift the limbs, weakness in tendons, vessels, joints, and complete inability to support the body, it is called "Tan". When the limbs can move but are relaxed and weak, requiring external support for function, it is called "Huan". Some claim that left-sided onset is Tan and right-sided is Huan; this is incorrect. It simply means that if the disease affects the left side, the pathology is on the left; if it affects the right, the pathology is on the right. Investigating its origin, all cases arise from internal consumption of Qi and Blood, deficiency of the Liver and Kidney meridians, and imbalance or abandonment of Yin and Yang. Alternatively, it may begin with other diseases where excessive use of emetic or purgative medicines causes internal disturbance of True Qi and loss of defense by Ying (Nutritive) and Wei (Defensive) Qi.
**Shuang Wan Zi Formula (Double Pill)** for treating all types of wind diseases, cold damage, etc., associated with paralysis:
*Ophiopogon japonicus* (Wushe, Snake), soaked in wine, skin and bones removed, roasted; *Gastrodia elata* (Tianma); Scorpion tail (Xieshao, whole); *Arisaema erubescens* (Tiannanxing, processed); Silkworm pupa (Yuancan'e, fried); Rhinoceros horn (Xijiao, shaved); Antelope horn (Lingyangjiao, shaved); Calculus bovis (Niuhuang, separately ground); Moschus (Shexiang, separately ground); Calomel (Nifen, made into ash by burning claws, intestines, and stomach in a porcelain jar).
The above seventeen ingredients, except those to be separately ground, are crushed and sieved into powder, mixed evenly with the separately ground powders. Knead with refined honey for 300-500 times. Store in a porcelain vessel. Take two pills per dose, washed down with Bohe Tang (Mint Decoction). Adult pills should be the size of white beans; children's pills, the size of mung beans. For acute onset, take three doses consecutively. For paralysis due to wind-stroke, add a small amount of Nifen, dissolve in water with the medicine. For pediatric convulsions, wash down with Jin Yin Bohe Tang (Honeysuckle and Mint Decoction). For women's blood-wind, wind-stroke before or after childbirth, with spasticity of hands and feet, wash down with Danggui Honghua Jiu (Angelica and Safflower Wine). For cold damage, wash down with Doulun Jiu (Soybean淋 Wine).
**Ru Sheng Jian Wan Formula (Like-Sage Decoction Pills)** for treating paralysis and wind diseases:
*Ephedrae Herba* (Mahuang, remove roots and nodes, cut into pieces, 4 liang); *Morus Radicis Cortex* (Sanggenbaipi, 0.5 jin, cut); Peach heart (Taoxin); Willow heart (Liuxin, each 4 liang, cut); Black soybean (Heidou, 2 sheng); Child's urine (Tongbian, taken after preparing other herbs); *Arisaema cum Bile* (Tiannanxing, raw, 2 liang); *Aconitum kusnezoffii* root lateral bud (Tianxiong, remove skin and navel, raw, 2 liang); Dried scorpion (Ganxie, remove soil, fried, 2.5 liang); *Gastrodia elata* (Tianma, 2 liang); *Polygoni multiflori Caulis* (Tianliaohua, 1 liang); Moschus (Shexiang, 3 fen); Calomel (Nifen, 1 qian); *Boswellia carterii* Resin (Ruxiang, 0.5 jin); *Momordica cochinchinensis* Seed (Mubiezi, remove shell, 1 liang).
The above nine ingredients are crushed and ground into powder. Add juice of fresh *Rehmanniae Radix* (Shengdihuang) to the water containing the previous herbs, and simmer over gentle heat until a paste forms. Seal tightly in an airtight container.
*Cinnabaris* (Dansha, 1 liang); *Lamina Micae* (Yunmu mo, 1.5 liang); Gold leaf (Jinbo); Silver leaf (Yinbo, each 10 pieces).
The above twenty ingredients: Grind the last four ingredients into powder and store in a porcelain box. Seal with *Leonurus japonicus* (Yimucao) paste. Place in a pit in the ground and burn with 5 jin of fire until red hot. Cover with wet soil, wait for cooling, then retrieve. If it turns pinkish-red, it is effective. To take: First, take 3 sheng of clear wine, weigh 1 liang of the paste. If the patient is deficient, grind Dansha and weigh 0.5 liang. Put together in a silver or earthenware vessel, boil wine until reduced to 1 sheng. Divide into five doses. Take one dose at noon on the first day, warm. Take one dose in the evening. Take subsequent doses later. The sign of efficacy is the discharge of foul matter after walking about 20 li. Adjust dosage according to deficiency or excess. Deficient patients take less; excess patients take slightly more. If sweating continues excessively after taking the medicine, drink Mahuang Genjie Tang (Decoction of Ephedra Roots and Nodes). Rest in a sealed room for 20 days to recover completely.
**Da Sheng Hua She Niuhuang Wan Formula (Great Sage Flower Snake and Calculus Bovis Pills)** for treating paralysis with facial deviation, excessive saliva, slurred speech, weakness of sinews and bones, difficulty walking, and generalized pain:
*Bungarus multicinctus* (Baihuashe) and *Natrix natrix* (Wushe) (both soaked in wine, skin and bones removed, roasted); Magnetite (Cishi, quenched in vinegar); *Gastrodia elata* (Chijian/Tianma); *Pinellia ternata* (Banxia, soaked in ginger juice overnight, sliced, dried); *Cibotium barometz* (Weilingxian, remove seedlings and soil); *Saposhnikovia divaricata* (Fangfeng, remove forked branches); Pyrite (Ziran tong, quenched in vinegar seven times, each 4 liang); *Notopterygium incisum* (Qianghuo, remove rhizome); *Spatholobus suberectus* (Haitongpi, cut); Dried scorpion (Ganxie, remove soil, fried); *Bombyx Batryticatus* (Baijiangcan, fried); *Phellodendron amurense* bark (Baixianpi); *Vittaria flexuosa* fruit (Manjingzi, each 3 liang); *Angelica sinensis* (Danggui, slice and dry); *Ligusticum chuanxiong* (Chuanxiong); *Citrus reticulata* peel (Qingju pi, soak in water, remove white pith, dry); *Tribulus terrestris* fruit (Jilizi, fried, each 3 liang); *Schisandra chinensis* (Wuweizi); *Polygala tenuifolia* (Yuanzhi, remove heart); *Dioscorea hypoglauca* rhizome (Bixie); *Cinnamomum cassia* bark (Guizhi, remove rough bark); *Costus root* (Muxiang, each 1.5 liang); Fresh *Rehmanniae Radix* (Shengdihuang, half raw, half fried); *Schizonepeta tenuifolia* spike (Jingjisui); *Pulsatilla chinensis* (Baitouweng); *Cistanche deserticola* (Roucongrong, separately ground); *Calculus Bovis* (Niuhuang, separately ground, each 0.5 liang); *Ephedrae Herba* (Mahuang, remove roots and nodes,结 with 1 liang of Black Tin); *Borneolum Syntheticum* (Longnao, ground, 3 fen).
The above forty ingredients, except those to be separately ground, are crushed and sieved into fine powder, then mixed with the ground powders. Knead with refined honey into pills the size of a bullet. Take one pill per dose, chew thoroughly, and wash down with Doulin Bohe Wenjiu (Warm Wine with Soybean Infusion and Mint). Time is not restricted.
**Jiangcan Wan Formula (Silkworm Pills)** for treating wind-paralysis, inability to move hands and feet, and abnormal speech:
*Bombyx Batryticatus* (Baijiangcan, fried); *Aconitum carmichaelii* lateral root (Wutou, processed until cracked, remove skin and navel); *Myrrha* (Moyao, each 1 liang); *Scorpio* (Wugong, roasted, 0.5 liang).
The above four ingredients are crushed and sieved into powder. Make pills using flour paste boiled in wine, size of *Sophora japonica* seeds. Take ten pills per dose, washed down with Bohe Jiu (Mint Wine), three times daily.
**Shen Miao Huai Jiao Wan Formula (Divine Marrow Bean Gum Pills)** for treating wind-paralysis:
*Morus alba* gum (Huaijiao, 0.5 jin, finely crushed, soaked overnight in 5 sheng of good wine, then boiled in a pot, filtered to obtain 1 sheng of juice); *Sappan Wood* (Sufangmu, 4 liang, finely cut, boiled in 5 sheng of running water until... [text missing] ...each 1 fen). The above six ingredients: Grind the last four ingredients evenly, add to the juice of the first two ingredients, simmer slowly for a few boils, stop fire, then add the remaining herbs.
*Bungarus multicinctus* meat (Baihuashe, one whole strip, soaked in wine overnight, meat taken and roasted, 1.5 liang); *Gastrodia elata* (Tianma); *Arisaema cum Bile* (Tiannanxing, processed); *Typhonium giganteum* tuber (Baifuzi, processed); *Aconitum carmichaelii* lateral root (Fuzi, processed until cracked, remove skin and navel, each 1 liang); *Glycyrrhiza uralensis* root (Gancao, roasted, 2 liang).
The above twelve ingredients: Grind the last six ingredients into powder, mix with the previous herb paste, knead 500-700 times. Make pills the size of a bullet. Take half a pill per dose, chew with Ginger Wine, three to four times daily.
**Zijin Tianma Wan Formula (Purple Gold Gastrodia Pills)** for treating wind-stroke paralysis, traction of hands and feet:
