Discussion states: It is rumored that "Zhang" (miasma) refers to the fierce and toxic qi of mountains and rivers. It is also said that the mist and fog of rivers and mountains often contain miasma, generally because the qi is stagnant and steaming. Various schools have different methods for treating Zhang disease in their prescriptions and theories. Some believe its symptoms are similar to Shanghan (Cold Damage), with exterior syndromes treatable by sweating, interior syndromes treatable by purging, and upper diaphragm syndromes treatable by vomiting. Others use methods for treating Malaria (Nue). Here, we trace the causes of miasma generation in the mountainous terrain of Guangnan, the time when miasma arises during the transition between spring and summer, and the methods for rescuing and treating people infected with miasma. All these are recorded in this chapter for reference.
Yang qi is born at Zi hour (midnight) and peaks at Si hour (9-11 AM); Yin qi is born at Wu hour (noon) and peaks at Hai hour (9-11 PM). If Yang does not reach its extreme, Yin will not sprout; if Yin does not reach its extreme, Yang will not bud. Guangnan is located in the Si-Wu direction, so it can be understood that the Qi of Yin and Yang accumulates here. Heaven is deficient in the northwest, and earth is incomplete in the southeast. The northwest belongs to Yin, with high and thick land; the southeast belongs to Yang, with low and shallow land. Since Guangnan belongs to the southeast, the lowliness of the land can be known. Due to the low land and the accumulation of Yin and Yang Qi, the surrounding mountains are high and winding, and all hundred rivers flow towards them. In autumn, grass and trees do not wither; in winter, hibernating insects do not hide. The toxicity of cold and heat accumulates and does not dissipate, and the qi of fog and dew easily harms people. This is exactly what Qibo said: "The south has low ground, weak water and soil, and is where fog and dew gather." Therefore, miasma is particularly abundant in Guangnan.
However, there are two names for arising miasma: In Mengxia (early summer), the miasma is called Fangcao (Fragrant Grass) miasma, ending in autumn. In Mengdong (early winter), the miasma is called Huangmang (Yellow芒) miasma, ending in spring. It can harm people in all four seasons, but between July and August, the toxic qi of mountain mist, smoke, snakes, and scorpions is especially severe. Thus, at this time, Zhang diseases break out on a large scale. Regardless of age, whether due to overeating or fasting, weakness of Ying and Wei (Nutritive and Defensive Qi), exposure to smoke and mist, or wading through streams, breathing this air leads to Zhang disease. Symptoms include headache, body pain, chest and diaphragm fullness and distress, alternating chills and fever, cough with excessive phlegm, complete loss of appetite, thirst with desire to drink, yellowing of the body, swelling, and falling out of eyebrows and beard. These are all caused by the stagnation and steaming of toxic pestilence qi. People living in hilly areas are accustomed to it, and locals should have no worries. Yet they eat Betel Nut daily, likely because Betel Nut breaks hard toxins and removes accumulation. The treatment method should first use Magnetite (Cishi) to descend and collect Yin Qi, then use Cinnabar (Dansha) to ascend and rectify Yang Qi. Once Yin and Yang Qi are rectified, middle jiao fullness naturally disappears, and then Areca Nut (Binglang) is used to resolve remaining accumulation.
