[Pathogenesis] The term "Xuan Yin" (Hanging Fluid Retention) refers to a condition where excessive water intake leads to fluid accumulation in the hypochondriac region, causing hanging-like pain in the intercostal spaces. Coughing and expectoration pull on the hypochondrium, inducing pain; hence it is called Xuan Yin. (From Volume 20)
Fan Wang's Da Gansui Wan (Great Kansui Pill), a formula for treating long-standing retained fluid.
Daphne Genkwa Flower (Yuanhua, processed), Kansui Root (Gansui), Lepidium Seed (Tinglizi, processed), Rhubarb (Dahuang), Sophora Root (Kushen), Euphorbia Root (Daji), Mirabilite (Mangxiao), Fritillaria Bulb (Beimu), Cinnamon Twig (Guixin) (each 1 liang), Apricot Seed (Xingren) (30 seeds), Croton Seed (Badou) (30 seeds, remove heart and skin, process), Aconitum Lateralis Radicae Preparata (Wuhui) (3 fen, process until brittle).
The above twelve herbs are crushed and sifted. The Badou and Xingren are pounded into a paste, then mixed with honey to form pills the size of soybeans. Take two pills per dose, three times daily. If ineffective, slightly increase the dosage according to judgment and adjust accordingly for optimal results. This treats severe fluid retention diseases. Avoid eating Asparagus cochinchinensis (Lusun), pork, raw green onions, etc. (From Volume 16)
Qianjin Fang's Shizao Tang (Ten Jujube Decoction) for treating Xuan Yin.
Daphne Genkwa Flower (Yuanhua), Kansui Root (Gansui), Euphorbia Root (Daji).
The above three herbs are equal parts, crushed and sifted. Boil ten large jujubes (Dazao) in 1 sheng 5 he of water, reduce to 8 he, strain to remove dregs, add the herbal powder. Strong individuals take one qian bi (a spoonful measure), weak individuals take half a qian bi. Take as a single dose in the morning. If no bowel movement occurs by dawn, increase the dose by half a qian bi. After evacuation, nourish oneself with millet porridge. (This formula originally comes from Zhang Zhongjing's Shanghan Lun, found in Volume 18)
[Pathogenesis] The term "Yi Yin" (Overflowing Fluid Retention) refers to a condition caused by sudden excessive drinking due to extreme thirst. The water qi overflows beyond the stomach and intestines, remaining between the skin and muscles. Hence it is called Yi Yin. Its symptoms include heavy body pain and profuse sweating. (From Volume 20)
Fan Wang states that Yi Yin should be treated by inducing sweating, primarily using Da Qinglong Tang (Major Blue-Green Dragon Decoction).
Ephedra Stem (Mahuang, 6 liang, remove joints), Cinnamon Twig (Guixin, 2 liang), Licorice Root (Gancao, roasted, 2 liang), Fresh Ginger (Shengjiang, 3 liang), Gypsum (Shigao, size of one chicken egg), Apricot Seed (Xingren, 40 seeds, remove tips and skins), Large Jujube (Dazao, 10 pieces).
The above seven herbs are cut. Boil in 9 sheng of water. First boil Mahuang to reduce by 2 sheng, then add the other herbs. Boil to obtain 3 sheng, strain to remove dregs. Drink 1 sheng when warm. Cover with blankets to induce sweating. If sweating is excessive, apply warm powder to the body. If sweating occurs after one dose, do not take another, as excessive sweating may lead to Yang collapse, resulting in reversal, deficiency, aversion to wind, restlessness, insomnia, weak pulse, and aversion to wind with sweating. In such cases, do not administer this formula; otherwise, it will cause cold limbs and muscle twitching, which constitutes a therapeutic error. Avoid consuming Seaweed (Haizao), Chinese Cabbage (Songcai), raw green onions. (From Volume 16)
Qianjin Fang states that Yi Yin should be treated by inducing sweating, preferably using Qinglong Tang (Blue-Green Dragon Decoction).
Ephedra Stem (Mahuang, remove joints), Peony Root (Shaoyao), Asarum Root (Xixin), Cinnamon Twig (Guixin), Dried Ginger (Ganjiang), Licorice Root (Gancao, roasted, each 3 liang), Schisandra Berry (Wuweizi, 0.5 sheng), Pinellia Tuber (Banxia, 0.5 sheng).
The above eight herbs are cut. Boil in 1 dou of water. First boil Mahuang to reduce by 2 sheng, then add the remaining herbs. Boil to obtain 3 sheng, strain to remove dregs. Drink 1 sheng warm. Avoid consuming Seaweed (Haizao), Chinese Cabbage (Songcai), mutton, malt sugar (Yitang), raw vegetables, raw green onions. (This is Zhang Zhongjing's Xiao Qinglong Tang from Shanghan Lun, found in Volume 18)
Qianjin Yifang's Da Wuyin Wan (Great Five Fluids Pill) treats five types of fluid retention: 1. Liu Yin (Retained Fluid): water stagnates below the heart; 2. Fu Yin (Hidden Fluid): water stagnates in the hypochondrium; 3. Tan Yin (Phlegm-Fluid): water stagnates in the stomach; 4. Yi Yin (Overflowing Fluid): water overflows between the diaphragm and the five zang organs; 5. Liu Yin (Flowing Fluid): water is in the intestines, making noise when moved. These five fluids arise from...
Polygala Root (Yuanzhi, remove heart), Sophora Root (Kushen), Cuttlefish Bone (Wuzheiyu), Veratrum Rhizome (Lilu), Atractylodes Rhizome (Baizhu), Kansui Root (Gansui), Schisandra Berry (Wuweizi), Rhubarb (Dahuang), Gypsum (Shigao), Platycodon Root (Jiegeng), Pinellia Tuber (Banxia, washed), Aster Root (Ziwan), Peucedanum Root (Qianhu), Mirabilite (Mangxiao), Trichosanthes Fruit (Gualou), Cinnamon Twig (Guixin), Cistanche Stem (Rongsheng), Fritillaria Bulb (Beimu), Daphne Genkwa Flower (Yuanhua), Angelica Root (Danggui), Ginseng Root (Renshen), Poria Sclerotium (Fuling), Peony Root (Shaoyao), Euphorbia Root (Daji), Lepidium Seed (Tinglizi), Scutellaria Root (Huangqin) (each 1 liang), Dichroa Root (Changshan), Licorice Root (Gancao, roasted), Yam Rhizome (Shanyao), Magnolia Bark (Houpo), Asarum Root (Xixin, each 3 fen), Prepared Aconite Root (Fuzi, 3 fen, processed), Croton Seed (Badou, 3...) The above thirty-three herbs are crushed and sifted, mixed with honey to form pills the size of wetapod fruit seeds. Take three pills with wine, three times daily, increasing dosage slightly if necessary. Avoid eating fox meat, plums, peaches, sparrow meat, pork, mutton, malt sugar, raw green onions, vinegar foods, raw vegetables, wild boar meat, asparagus, etc. (Hu Qia's version is identical, from Volume 19)
