"Collected Proven Experiences on Carbuncles and Ulcers"
The Yellow Emperor asked: You spoke of carbuncles (Yong) and ulcers (Ju), how should they be distinguished?
Qibo replied: When Ying Qi (Nutritive Qi) and Wei Qi (Defensive Qi) stagnate within the meridians, blood flow becomes sluggish and unsmooth. This causes righteous Qi to remain and accumulate, thereby forming pus and blood.
The Yellow Emperor said: Well said.
The classics state that dysfunction of the Five Zang Organs leads to ulcers (Ju), while disharmony of the Six Fu Organs generates carbuncles (Yong). The first type is called Acute Ju; if acute onset, death may occur within two or three days; if slow onset, within ten-plus days. The second type is called Acute Yong-Ju; if acute onset, death may occur within ten-plus days; if slow onset, within one month. The third type is called Chronic Ju; if acute onset, death may occur within a year; if slow onset, within several years. The fourth type is called Water Ju. Its onset often occurs in... [text missing], one is Brain Corpse, two is Tongue Root, three is Xuan Yong, four is Throat Joint. Five is Hu Mai, six is the Shu points of the Five Zang Organs, seven is the connections of the Five Zang Organs. Eight is both breasts, nine is the area of Xinjiuweil (CV15), ten is the Yuji areas of both hands. Eleven is between the curves of the intestines, twelve is behind the small pathways, thirteen is the Nine Orifices. Fourteen is both... [text missing] intestines, fifteen is the residence of the Spirit Master. (Another version calls it the residence of Host and Guest). These fifteen locations must not be injured, let alone develop carbuncles. If a carbuncle develops in these locations, encountering a good physician who can prevent large amounts of pus from forming allows for rescue; once large pus forms, harm ensues. (Fan Wang's view is the same.)
Observe the signs of Thief Wind Syndrome: pain at night extends to the bones, cannot be pressed or suppressed, the body cannot turn, the painful area is not hot, the body does not alternate between chills and fever, but feels cold and desires warmth. Applying heat compresses to the painful area brings slight relief, sometimes with sweating. This is Thief Wind Syndrome. One should immediately undergo acupuncture and moxibustion and take warming wind-dispelling medicines. (The formulas are in the general methods for treating Wind Syndromes; one should know these two diagnostic approaches as described.)
Furthermore, upon initially contracting Fugu Ju (Bone-Attached Ulcer), immediately administer Loulu Tang (Thistle Decoction) to purge, and apply a thin paste of Adzuki Beans externally to resolve it.
Furthermore, if diarrhea has ceased but swelling remains unresolved, remove Rhubarb, use fresh Rehmannia root and dried Rehmannia root, adjusting according to the condition. If heat gradually subsides but residual wind persists, Wuxiang Liaoqiao Tang (Five-Spice Forsythia Decoction) may be taken, removing Rhubarb. If residual heat remains, applying Shengma Gao (Cimicifuga Ointment) is excellent. If the timing is missed and it fails to resolve, turning into pus, treat with fire needles, ointments, and powders according to the methods for treating carbuncles. ... [text missing] ankle, red like woven mats, caused by cooling dampness obstructing wind condensation, heat temporarily ceasing, Qi not dispersing. Treatment should involve taking Loulu Tang to induce purging, externally using KT needles to puncture and drain blood and Qi, needling the Jie Mai (Knotting Vessels) at the upper writing location, and applying an Adzuki Bean thin paste to resolve it. All can follow ... [text missing].
Furthermore, one may also use a thin paste of Banana Root to cure.
Furthermore, if a carbuncle presents with high swelling, the disease source is shallow; if low swelling, the disease source is deep. Those with intense heat are easy to treat; those with mild heat are difficult. Initial severe pain damaging muscle indicates late-onset severe pain damaging bone. Uniform hardness indicates no pus yet; half-hard, half-soft indicates pus present. Uniform softness in swelling indicates a Blood Tumor, not a carbuncle. Swelling must be understood gradually. Prolonged duration without significant heat, with occasional pulling pain, indicates a tumor, not a carbuncle. In Wu dialect: various Qi knots can cause swelling; if unresolved for a long time, it becomes a carbuncle. Treatment should disperse Qi. Once Qi is dispersed... If initial swelling has floating Qi, the elderly will all develop carbuncles. Treatment should be undertaken when young and vigorous, dissipating heat and cold, avoiding this worry.
Yu's Method: For carbuncles and ulcers, a洪粗 (Surging and Coarse) pulse is difficult to treat; a微涩 (Fine and Choppy) pulse is easy to treat. For all Floating-Rapid pulses, if fever is expected but chills are present instead, it is a carbuncle, possibly Fugu Ju with pus.
Zhao stated: Without consumptive disease or abdominal disorders, or if developing a Blood Tumor Sore, where the sore protrudes like a mound, black-headed and upright, moxibustion should not be used. Burning/warming causes charring and rotting. Scraping off the scab causes uncontrollable bleeding, leading to certain death. Carbuncles arising at joints, treated by incompetent physicians who fail to resolve them quickly, leading to large pus formation—can plaster revive the dead?
Furthermore, a carbuncle hard as stone, wandering in the skin without roots, is Luoli (Scrofula). If unresolved for a long time, due to other heat diseases, it may develop into a carbuncle.
Furthermore, a carbuncle extremely hard with roots is named Stone Carbuncle. The treatment method involves drinking wine; without wine, the medicinal power does not disseminate. Drink slightly until Qi and Blood harmonize and disperse, then stop. For all carbuncles/swellings, obese individuals should use Trichosanthes Root plasters; those with balanced constitutions should use Adzuki Bean plasters.
Take five he of Adzuki Beans, cook in vinegar until done, grind into powder, mix with vinegar, apply on paper to the swelling, sticking from both ends downwards. (Fan Wang agrees.)
Furthermore, discussing children often having thirst; after age forty, carbuncles and ulcers are common. Those with phlegm in the diaphragm and thirst will likely develop jaundice in youth, and carbuncles/ulcers in old age. (Fan Wang agrees.)
Furthermore, the Yellow Emperor said: I wish to hear about the forms and prognosis of carbuncles and ulcers. Qibo said: Briefly, there are eighteen extreme types of carbuncles and ulcers.
Furthermore, a carbuncle/ulcer developing in the throat is called Meng Ju (Fierce Ulcer). If untreated, blood turns to pus. If pus is not drained, blocking the throat causes death in half a day. After draining pus, hold Pig Fat in the mouth; do not eat cold food. Recovery in three days; fast for one day.
Furthermore, development in the thigh is called Gu Tuo Ju. Symptoms show little change, but the carbuncle swelling binds to the bone. Untreated, death occurs in thirty days. (Inner thigh is Gu, outer is Bi, bone above knee below thigh is Gu Ting.)
Furthermore, development in the hypochondrium is called Gai Zi, a disease of women. Over time, it becomes a large carbuncle with pus, containing raw flesh the size of a red bean. Formula: Cleave Forsythia herb and root each one sheng, boil in sixteen dou of water until dry, take three sheng, drink strongly, sit on a pot wrapped in thick clothes. Induce sweating down to the feet.
Furthermore, development in the sacrum is called Rui Ju. Symptoms are red, hard, and large. Treat urgently; untreated, death in thirty days.
Furthermore, development in the shin is called Tu Nie. Symptoms are red to the bone. Treat urgently; untreated, harmful. Furthermore, development above and below the foot is called Si Yin... [text missing] Moxibustion on the upper foot with one hundred burns; stone fragments should exit. If not, increase burns. (Consistent with Fan Wang regarding height of onset.)
Furthermore, Stone Carbuncle begins with skin and nodules adhering, not red-headed, not very hard, slight pain and heat. Heat gradually subsides, becoming hard as stone. Hence called Stone Carbuncle... [text missing] Cun to Chi... [text missing] Carbuncle.
For all carbuncles and ulcers, before pus appears, they are easy to treat. Apply three hundred moxa burns on top, two hundred burns at intervals around the sides. Excess patterns can be purged; deficiency patterns can be tonified... [text missing] Pus. Formulas. Pus should be ruptured; without pus, it is only Qi swelling. If blood is present, carefully avoid rupture/acupuncture. Pressing four sides hard and center soft indicates deep pus. Softness on one side also indicates pus. Uniform hardness indicates Zu He (Pickled Vegetable Nodule) or only Qi. Uniform softness indicates blood, a Blood Tumor. Examining hardness/softness and excess/deficiency is key. If a hard ulcer accumulates over time and becomes "ripe," with localized softness, do not rupture; keep the ulcer warm and covered. If moxa/puncture rupture is attempted, it will become violent and incurable, leading to tendon knotting or rat-milk tumors. Do not treat. Also take Neisai San (Internal Blocking Powder); it does not conflict with other treatments. Take ten-plus times day and night... [text missing].
Furthermore, development in the armpit, hard and red, is called Mi Ju (Rice Carbuncle). Treat with Bian Shi (Stone Lancet), fine and long, open sparsely, apply Pig Fat. Recover in six days; do not bandage. If the carbuncle is hard and unruptured, it is Ma Dao Jia Ying (Horse Knife Bearing Tassel). Treat urgently. (Tai Su Jing says neck front bearing tassel.)
Furthermore, development in the inner thigh is called Chi Chi. Untreated, death in six days. Located inside both thighs, untreatable; some say death in sixty days.
Furthermore, development in the knee is called Ci Ju. Symptoms: large carbuncle, color unchanged, chills/fever, hard as stone. Do not lance with stone; lancing causes death. Wait for softness/color change before lancing ensures survival. (Cold stone compress softens, then rupture follows standard example.)
Furthermore, carbuncles/swellings developing at joints with corresponding symptoms are untreatable. (Tai Su Jing says Diaphragm Eight Joint Gates are thus untreatable.)
Furthermore, development in Yang regions causes death in one hundred days.
Furthermore, development in Yin regions causes death in thirty days. (Male genitalia is Yang, female genitalia is Yin.)
Furthermore, development in the ankle is called Zou Huan. Symptoms: flesh color unchanged. Frequent stone needling at Shu points stops it; chills/fever do not cause death.
Furthermore, development on the side of the foot is called Li Ju. Symptoms: not large, starts from little toe. Treat urgently. Remove black areas; if not resolved, it increases. Untreated, death in one hundred days. (Says side of foot.)
Furthermore, development in the chest is called Bei Ju. Shape like soybean. If not treated early after three-four days, it enters the abdomen. Entering abdomen untreated causes death in ten days. (Tai Su Jing says chills/fever persisting ten days leads to early death.)
Furthermore, development in the toe is called Tuo Ju. Symptoms: red-black. Untreated death; if not red-black, treatable. If treatment fails to reduce it, amputate urgently to survive; otherwise, death.
Furthermore, development in the skin is called She Ju. Shape like grain fruit or Trichosanthes. Suffers chills/fever. Treat urgently to remove chills/fever; untreated, death in ten years, pus excretes after death.
Furthermore, development in the neck is called Yao Ju. Shape large, red-black. Untreated, heat qi descends to Yuan Ye, damages Ren Mai anteriorly, steams Liver and Lung internally. Death in ten-plus days. (Tai Su Jing says Neck is Front is Jin.) Some say Fa Tou. Previous (Eighteen types together).
"Qianjin Lun" states: Initially, people do not consider carbuncles/ulcers urgent, but this is a strange disaster, requiring rapid treatment. If not treated rapidly, the disease becomes incurable. Thus... [text missing] wash away Cold Lao (Cold Consumption).
When carbuncles/ulcers first appear, they may seem minor, or cause great pain, slight pain, or appear like rice grains/big white pus seeds. These are subtle signs; observe well. Seeing any difference requires alarm and urgent treatment. Stop tasting flavors, quickly take various decoctions to descend heat toxins. If no medicine, moxa when... [text missing] must recover.
Regarding medication/plaster methods, one must open a hole in the center of the sore to vent heat Qi. Also, insert a large needle four cun deep at the head for recovery.
Sudden pain in the body resembling a blow is called Qi Pain. Unbearable pain, wandering without rest, occurring at times. During pain, slight heat; after pain, cold. This is caused by warm wind in winter, broken by sudden cold wind in spring, failing to become Warm Disease, instead becoming Qi Pain. Should first take Wuxiang Liaoqiao Tang, Mo Dan Li Tang.
"Sù Wèn" states: Cold Qi invading meridians causes blood coagulation, seepage, and obstruction, gathering into carbuncles/ulcers. It does not speak of heat production. Later, Yang Qi gathers, Cold transforms into Heat. Intense Heat rots flesh into pus. Apply Vinegar mixed with Bee Shell Ash; change when dry, stop when recovered.
Swelling root width less than one Cun is called Jie (Boil); more than one Cun is Xiao Yong (Small Carbuncle). Size of a bean is called ... [text missing] Zi. Upon onset, urgently take Wuxiang Liaoqiao Tang to purge. Several doses until recovery. (All from Volume 23).
