Scabies (Jie Chuang)
Dry papules causing itching and pain are called Scabies. They arise primarily from Blood Heat. Another type is Impetigo (Nong Ke Chuang), characterized by swelling and pain, resulting from Wind-Heat affecting the Lung. Treatment should focus on dispelling Wind and resolving toxicity. Externally, apply medicaments to kill parasites and relieve itching; use Yi Shang San (One-Up Powder) for treatment. For scabies, one may also use Xun Nian Zi (Fumigating Stick) for fumigation. However, fearing internal entrapment of toxins, it is advisable to first administer several doses of decoction before fumigating for optimal effect.
Saddle Sore (Zuo Ban Chuang)
This sore results from Damp-Heat and Damp-Toxin stagnating in the Spleen channel over a long period, manifesting on the buttocks. It is extremely painful and itchy. Furthermore, as this is a region of extreme Yin, blood circulation is sparse, making it difficult for medicinal efficacy to reach the area. Treatment requires internally purging Damp-Heat from the Spleen channel and externally applying a fire-wick oil-soaked cloth for better results. The prescription is listed later.
Leprosy-like Sores (Lai Feng Chuang)
Now commonly referred to as Da Feng (Hansen's Disease/Leprosy). Initially, pathogenic wind with destructive properties invades, and the Five Zang organs are affected due to deficiency. Over time, both Qi and Blood are compromised, sores appear all over the body, fingers and toes may fall off, eyesight is lost, the nose collapses, hair falls out, the body turns purple, becomes ulcerated, foul-smelling, and oozes pus. One should seek miraculous prescriptions for treatment. In the past, Sun Simiao treated four hundred patients but only cured one poor woman suffering from this condition.
Throat Closure (Hou Bi)
When unruptured, it is called Hou Bi (Throat Obstruction). It presents as either unilateral or bilateral. When ruptured, it is called Ru E (Tonsillar Abscess), which can also be unilateral or bilateral. The *Neijing* states: "When one Yin and one Yang knot together, it is called Hou Bi." This is a syndrome of constrained Fire. Whether ruptured or not, it can be punctured with a throat needle to drain toxicity. The urgency is akin to extinguishing a fire or rescuing someone from drowning. Internally, administer detoxifying agents; externally, apply insufflation powders. Delay may endanger life.
Snake Nest Sore (She Ke Chuang)
This sore occurs because clothing has been crawled over by a snake, or food/drink has been contaminated by snake venom, entering the skin and hair follicles, causing sores and pain. Treatment involves moxibustion directly on the sore head, or pricking the blood with a pine needle. Apply oil infused with centipede (Wu Gong) to the area, or mix Realgar (Xiong Huang) with wine and apply topically for relief.
