If the pus is mature but the ulcer does not rupture, the ulcer surface is not hot, appetite is poor, and stools are loose, it indicates Spleen Qi Deficiency (Spleen Qi Deficiency). The treatment should involve tonifying and supporting with warming and middle-jiao strengthening methods. If the pus is clear or excessive, the ulcer opening is scattered, and muscle regeneration fails to occur, it indicates severe internal deficiency with a tendency toward transformation into complicated symptoms; vigorous tonification is required. Although necrotic tissue has detached, new flesh grows slowly and appears frostbitten in color, indicating cold muscles and cold skin, vigorous tonification of Qi and Blood with warming properties is needed. After ulceration, if there is poor appetite, mental restlessness, insomnia, fever, and thirst, it indicates weak Spleen and Yin Deficiency; the method of Tonifying the Middle and Augmenting Qi (Tonifying the Middle and Augmenting Qi) should be used. If the body is cool, the pulse is fine, fatigue is present, speech is reluctant, and food tastes bland, it indicates Spleen weakness; awakening the Spleen and assisting the Stomach is indicated. If Wind-Cold invades the ulcer opening, causing the skin to turn white, split open, and fail to contract, external application combined with internal warming and tonification is required. If after ulceration the face is yellow, muscles are emaciated, pus is clear and thin, and the ulcer color is pale, aromatic and drying herbs to assist the Spleen should be used. If the pulse is large but forceless, fine and choppy, slightly deep, with spontaneous sweating and a cool body, it indicates dual deficiency of Qi and Blood; vigorous tonification is required. If there is excessive bleeding or pus, five-palm heat, extreme dryness, and inability to sleep, it indicates loss of Yang; emergency tonification is required. If the ulcer has healed but re-ruptures due to sexual exertion or acute stress, assisting Qi and Blood is still necessary.

After completing the three pastes, store each in porcelain bowls, immerse in water to cool, cover tightly to prevent air leakage. If the patient has short spirit, reluctance to move/speak, shortness of breath, and spontaneous sweating, use three spoons of Renshen Gao, two spoons of Baizhu Gao, one spoon of Dihuang Gao, dissolved in one cup of clean good wine heated thoroughly. If Spleen Qi is weak, appetite decreased, or food tastes bland, or undigested food, use three spoons of Baizhu Gao, two spoons of Renshen Gao, one spoon of Dihuang Gao, dissolved in hot wine. If the patient has soft waist/knees, weak legs/feet, rough/dry skin/hands/feet, use three spoons of Dihuang Gao, three spoons each of Renshen and Baizhu Gao, dissolved.
