There are cases where the patient experiences chills and aversion to cold, followed by fever. This occurs because when the Yin pulses are deficient, Yang Qi tends to follow into deficiency; when the Yang pulses are deficient, Yin pathogens tend to invade. What is meant by Yang Qi deficiency?
In treating alternating chills and fever: Use Chai Hu (Bupleuri Radix) to raise Yang Qi, preventing it from sinking into the Yin level, thus avoiding internal heat; use Huang Qin (Scutellariae Radix) to descend Yin Qi, preventing it from ascending into the Yang level, thus avoiding... Headache, eye pain, redness of the ears, swelling and pain of the cheeks... and all disorders of the Upper Jiao, apart from standard treatment with cooling and dispersing methods, people often find themselves helpless, not knowing... [text cuts off]
If the stool is constipated and the pulse is deep and solid, administer three qian of wine-processed Da Huang (Rhei Radix et Rhizoma) to induce mild purgation; this is called the method of "drawing firewood from under the cauldron" (removing the root cause).
If there is diarrhea, a deep pulse, and cold feet, Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used with added Niu Xi (Achyranthis Bidentatae Radix), Che Qian Zi (Plantaginis Semen), and Rou Gui (Cinnamomi Cortex); if foot coldness is severe, add Shu Di Huang (Rehmanniae Radix Preparata) and Fu Zi (Aconiti Lateralis Radix Preparata)... If the stool is normal but the pulse is weak, use Niu Xi (Achyranthis Bidentatae Radix) and Che Qian Zi (Plantaginis Semen) to guide the fire downward; this is called the method of "guiding fire back to its source."
When treating Lower Jiao diseases, if remedies targeting the primary disease are ineffective, one must treat from the Upper Jiao: for example, in cases of painful and swollen feet with weakness and softness, or red and swollen knee ulcers, use Mu Gua (Chaenomelis Fructus)... If the feet are weak, unable to walk but capable of eating, it is termed Wei Syndrome (Flaccidity Syndrome). It is appropriate to clear Lung Heat.
When treating diarrhea, if spleen-fortifying and water-draining formulas are ineffective, lifting methods should also be employed; use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) removing Dang Gui (Angelicae Sinensis Radix), adding Pao Jiang (Zingiberis Rhizoma Preparatum) and Cang Zhu (Atractylodis Rhizoma); if the pulse is slow... If there is hematuria, and blood-cooling and diuretic drugs are ineffective, Qing Xin Lian Zi Yin (Clear the Heart and Lotus Seed Decoction) is appropriate. If clearing Heart Fire does not stop the bleeding, add Sheng Ma (Cimicifugae Rhizoma) and Chai Hu (Bupleuri Radix).
If treating hematochezia, and astringent and hemostatic drugs are ineffective, or if accompanied by diarrhea, the pulse must be examined: if the Right Guan position pulse is faint and fine, or rapid and large but weak, this indicates Spleen Qi deficiency failing to control blood... [text cuts off] If there is Excess Heat in the Lung, it is appropriate to clear the Lung, using Mai Dong (Ophiopogonis Radix), Hua Fen (Trichosanthis Radix), Yuan Shen (Scrophulariae Radix), Ku Huang Qin (Scutellariae Radix), Jie Geng (Platycodonis Radix), Wu Wei Zi (Schisandrae Chinensis Fructus), Zhi Ke (Aurantii Fructus Immaturus), and other herbs.
If the exterior of the body is icy cold while the interior is blazing hot, presenting with chills, fever, and incessant shivering, this is a true heat condition manifesting as false cold to deceive. The natural approach is to use San Huang Tang (Three-Yellow Decoction) plus Shi Gao (Gypsum Fibrosum) and Sheng Jiang (Zingiberis Rhizoma Recens), drinking it while hot. Sprinkling well water on the heart region twenty to thirty times will naturally stop the internal heat, and the external shivering will cease. Subsequently, administer a decoction of Yuan Shen (Scrophulariae Radix), Mai Dong (Ophiopogonis Radix), and Bai Shao (Paeoniae Radix Alba), each two liang, allowing the patient to drink freely; thereafter, the condition will not worsen.
