Deficiency consumption (Xu Lao) refers to the conditions of Five Labors (Wu Lao), Six Extremes (Liu Ji), and Seven Injuries (Qi Shang). The Five Labors are: first, Labor of Will (Zhi Lao); second, Labor of Thought (Si Lao); third, Labor of Heart (Xin Lao); fourth, Labor of Worry (You Lao); fifth, Labor of Fear/Suffering (Wei Ku).
The Six Extremes are: first, Extreme of Qi (Qi Ji), causing internal deficiency, insufficiency of Qi in the five Zang organs, excess of pathogenic factors, scarcity of righteous Qi, and reluctance to speak; second, Extreme of Blood (Xue Ji), causing lack of luster in complexion, falling out of eyebrows and hair, mental confusion, and propensity to forgetfulness; third, Extreme of Sinews (Jin Ji), causing frequent muscle spasms (Zhuan Jin), pain in all ten fingers and nails, fatigue preventing prolonged standing; fourth, Extreme of Bones (Gu Ji), causing soreness and wasting of bones, painful teeth, vexatious pain in hands and feet, and reluctance to move; fifth, Extreme of Flesh/Muscles (Ji Ji), causing emaciation, lack of moisture and luster in skin, and inability of food intake to generate muscle tissue; sixth, Extreme of Essence (Jing Ji), causing shortness of breath, a sense of internal emptiness, insufficiency of Qi in the five Zang organs, falling out of beard, hair, and body hair, sadness, and propensity to forgetfulness.
The Seven Injuries are: first, Yin Cold; second, Impotence (Yin Wei); third, Internal Urgency/Constraint (Li Ji); fourth, Continuous Spermatorrhea (Jing Lian Lian); fifth, Scanty Essence with Dampness in the Yin region; sixth, Clear Essence; seventh, Painful Frequent Urination (Xiao Bian Ku Shu).
Additionally: First, overeating injures the Spleen; Spleen injury leads to belching, desire to lie down, and yellowish complexion. Second, severe anger causing Qi rebellion injures the Liver; Liver injury leads to blood deficiency and dim vision. Third, strenuous lifting or sitting long on damp ground injures the Kidney; Kidney injury leads to scanty essence, lower back and waist pain, cold limbs, and coldness in the lower body. Fourth, exposure to cold or drinking cold water injures the Lung; Lung injury leads to shortness of breath, coughing, and nasal congestion. Fifth, worry and excessive thinking injure the Heart; Heart injury leads to fright, emotional instability, and forgetfulness. Sixth, wind, rain, and cold injuring the Form (Xing); Form injury leads to withered and premature aging of the skin. Seventh, excessive fear without moderation injuring the Will (Zhi); Will injury leads to mental confusion and unhappiness.
In men, a large pulse indicates extreme labor, and a deficient pulse also indicates labor. For men suffering from labor disease, the pulse is floating and large, hands and feet are vexatious, symptoms worsen in spring and summer, and improve in autumn and winter; internal cold causes spontaneous emission of essence, accompanied by soreness. At the Cun Kou position, a pulse that is floating and slow indicates floating as deficiency and slow as labor. Deficiency implies insufficiency of Defensive Qi (Wei Qi); floating indicates labor; Qi exhaustion with a straight upward-rebellious pulse indicates deficiency. A choppy pulse without Yang signs indicates scanty Kidney Qi. Choppy pulses at the Cun and Guan positions without blood, combined with rebellious Qi and cold, indicate great deficiency. Floating, slightly rapid pulses all indicate deficiency; slow and large pulses indicate labor. Interacting micro and soggy pulses indicate the Five Labors, Six Extremes, and Seven Injuries. Interacting weak and slight pulses indicate deficiency damage disease.
