Metrorrhagia and Metrostaxis
Duan XX, female, 45 years old, farmer. July 1955.
First visit: Having given birth eleven times, she suffered a strain on her waist and abdomen due to heavy labor during menstruation last year, resulting in continuous bleeding. Although treated with herbal medicine, the flow decreased but remained dripping and unclean, with episodes of severe metrorrhagia (profuse vaginal bleeding) occurring every one or two months. Symptoms included sallow complexion, emaciation, dizziness, mental fatigue, reduced appetite, and a bearing-down sensation in the waist and abdomen accompanied by dull stabbing pain. The pulse was Kōu (cun Kou pulse, floating and large in the middle, empty like a scallion stalk) at the cun position and Se (rough, choppy) at the chi position; the tongue body was pale with a thin, greasy coating. This indicates injury from overexertion leading to blood stasis obstruction, damage to the Chong and Ren vessels, prolonged metrorrhagia and metrostaxis, and deficiency of both Qi and Blood, constituting a pattern of deficiency mixed with excess. Treatment should focus on invigorating blood and resolving stasis, while simultaneously consolidating Qi and Blood. Prescription:
Ejiao Pearl (Prepared Donkey-hide Gelatin) 18g, Mugwort Leaf Charcoal (Stir-fried) 9g, Angelicae Sinensis Radix (Danggui) 24g, Ligusticum Chuanxiong Rhizoma (Chuanxiong) 9g, Paeoniae Radix Alba (Stir-fried Hangshao) 12g, Codonopsis Radix (Shangdangshen) 15g, Corydalis Rhizoma (Yanhusuo) 6g, Sepiae Endoconcha (Wuzeigu) 12g, Dipsaci Radix (Stir-fried Xuduan) 9g, Licorice Root (Zhi Gancao) 3g, Pollen Typhae (Raw and Stir-fried) 4.5g each, Zingiberis Rhizoma Preparatum (Paojiang) 9g, Panax Notoginseng Powder (Raw) 3g (mixed in portions).
Second visit: After taking two doses of the above prescription, dark black blood clots were passed smoothly, and immediately the waist and abdominal pain subsided significantly. However, dizziness, mental fatigue, poor appetite, nighttime irritability, spontaneous sweating, cold limbs, and aversion to cold persisted. The pulse became fine, weak, and powerless; the tongue was light red with a thin, moist coating. This indicates that the stasis has been expelled, leaving the Sea of Blood empty, and Yuan Yang (Primordial Yang) is insufficient. Urgent warming and tonification are required. Prescription:
Testudinis Carapax et Plastrum (Guiban) and Cervi Cornu Degelatinatum (Lujiaojiao) 9g each (melted and mixed), Atractylodis Macrocephalae Rhizoma (Stir-fried Baizhu) 12g, Aconiti Lateralis Radix Praeparata (Fupian) 18g (simmered thoroughly in boiling water first), Angelicae Sinensis Radix (Danggui) 15g, Astragali Membranacei Radix (Huangqi) 18g, Paeoniae Radix Alba (Stir-fried Hangshao) 12g, Draconis Os (Calcined Longgu) 12g, Mugwort Leaf Charcoal (Stir-fried) 9g, Poria (Fushen) 15g, Licorice Root (Zhi Gancao) 3g, Zingiberis Rhizoma Preparatum (Paojiang) 9g, Jujubae Fructus (Dazao) 5 pieces.
Third visit: After taking three doses of the above prescription, metrorrhagia and metrostaxis stopped completely, hands and feet turned warm, spontaneous sweating ceased, and appetite returned. The complexion was bright white, but dizziness, mental fatigue, and weakness in the legs persisted. The pulse was soft, slow, weak, and fine; the tongue was pale red. This indicates deficiency and damage to the Chong and Ren vessels, and insufficiency of the Liver, Spleen, and Kidney meridians. Another prescription is formulated below for regular consumption to regulate the body.
Codonopsis Radix (Dangshen) 18g, Astragali Membranacei Radix (Huangqi) 18g, Angelicae Sinensis Radix (Danggui) 15g, Atractylodis Macrocephalae Rhizoma (Baizhu) 12g, Lycii Fructus (Gouqi) 12g, Paeoniae Radix Alba (Stir-fried Hangshao) 9g, Rehmanniae Radix Preparata (Shudi) 15g, Dioscoreae Rhizoma (Huaiyao) 12g, Poria (Fushen) 15g, Taxilli Herba (Sangjisheng) 12g, Amomi Fructus Rotundus (Sharen) 6g, Licorice Root (Zhi Gancao) 3g, Nucleus Jujubae (Longyanrou) 12 branches (shelled), Nelumbinis Semen (Quixin) 15g (heart removed).
[Commentary] Metrorrhagia and metrostaxis present complex symptoms. Ancient physicians proposed "clearing the source," "stopping bleeding," and "restoring health." It is crucial to distinguish clearly. This case involves deficiency mixed with excess. If only hemostasis is emphasized without clearing the source, it will cause significant delay. This case offers valuable clinical reference.
Heat Entering the Blood Chamber
Zhang XX, female, 30 years old, residing in the suburbs of Kunming City. 1941.
First visit: The patient had been feverish for six or seven days. Menstruation arrived at this time, with dark, scanty flow, which suddenly stopped the next day. She felt distending and painful swelling in the waist, abdomen, and bilateral hypochondrium. Dry mouth and throat, desire for drinking, dizziness, mental confusion, delirious speech and irritability at night. Constipation for five days, dark urine. This indicates exterior pathogenic factors entering the interior and transforming into heat, Yangming dryness and accumulation, and heat entering the Blood Chamber (referring to the uterus or related meridians). Treatment should harmonize and clear heat, move blood and unblock the bowels. Prescription:
Bupleuri Radix (Stir-fried Chaihu) 9g, Scutellariae Radix (Stir-fried Huangqin) 6g, Persicam Semen (Tao Ren Paste) 9g, Aurantii Fructus Immaturus (Zhishi, stir-fried), 6g, Magnoliae Officinalis Cortex (Stir-fried Houpo) 9g, Rheum Palmatum Radix et Rhizoma (Sheng Dahuang) 6g, Ligusticum Chuanxiong Rhizoma (Chuanxiong) 6g, Angelicae Sinensis Radix Cauda (Dangguiwei) 9g, Cyperi Rhizoma (Zhi Xiangfu) 6g, Tetsu Herba (Tongcao) 6g, Allii Fistulosi Bulbus (Congbai) 1 piece, Child's Urine (Xiaotongbian, one small cup as an adjuvant).
Follow-up visit: After one dose of the above prescription, menstruation resumed, with dark color and clots. Bowels moved freely. Fever subsided, mental clarity returned, delirium stopped, with slight remaining irritability. Able to eat. Waist, abdominal, and hypochondriac pain gradually decreased. Pulse became fine, wiry, and slightly rapid; tongue red with thin yellow coating. This indicates Yangming fu-organ qi is unblocked, interior heat is resolving, and Chong and Ren vessels are relaxed. Continue to soothe the Liver, regulate Qi, invigorate blood, and clear heat. Prescription:
Bupleuri Radix (Stir-fried Chaihu) 9g, Scutellariae Radix (Stir-fried Huangqin) 6g, Angelicae Sinensis Radix (Danggui) 9g, Ligusticum Chuanxiong Rhizoma (Chuanxiong) 6g, Paeoniae Radix Alba (Stir-fried Hangshao) 9g, Moutan Cortex (Danpi) 6g, Gardeniae Fructus (Jiao Zhizi) 3g, Cyperi Rhizoma (Zhi Xiangfu) 6g, Dipsaci Radix (Stir-fried Xuduan) 9g, Nelumbinis Neocaulis Axillis (Oujie) 3 pieces.
After taking two consecutive doses of the above prescription, menstruation flowed smoothly, fever completely subsided, and waist, abdominal, and hypochondriac pain ceased. Appetite increased, bowel and bladder functions normalized, and she could sleep quietly at night. Pulse was weak and slow; tongue red and moist with thin white coating. This indicates Qi and Blood deficiency after illness. Use a regulating prescription:
Angelicae Sinensis Radix (Danggui) 15g, Paeoniae Radix Alba (Stir-fried Hangshao) 9g, Rehmanniae Radix (Gan Dihuang) 12g, Moutan Cortex (Danpi) 6g, Taxilli Herba (Sangjisheng) 15g, Dipsaci Radix (Stir-fried Xuduan) 9g, Ligusticum Chuanxiong Rhizoma (Chuanxiong) 4.5g, Poria (Fuling) 12g, Cyperi Rhizoma (Zhi Xiangfu) 6g, Nelumbinis Neocaulis Axillis (Oujie) 3 pieces, Bamboo Shavings (Zhuru) 4.5g, Jujubae Fructus (Xiaozao) 7 pieces, Glycyrrhizae Radix et Rhizoma (Gancao) 3g.
[Commentary] Excess pathogens of Shaoyang and Yangming entered due to deficiency, battling with Qi and Blood, knotting in the Blood Chamber. At that time, Tao Ren Cheng Qi Tang plus Chai Hu and Huang Qin should have been decisively administered to treat both exterior and interior, allowing turbid matter in the fu-organs to descend, resuming menstruation, followed by nourishing Blood and benefiting Yin.
Pediatrics
Measles
Zhao XX's daughter, 5 years old, from Kunming City. Spring 1940. During a severe epidemic of measles in Kunming, the child contracted the measles toxin, presenting with high fever, cough, hoarseness, runny nose, and tearing. The rash emerged diffusely red, covering the whole body. Subsequently, the condition changed suddenly: the rash color darkened, cough was obstructed, panting and phlegm rales occurred. Nasal alae flapping, unconsciousness, cyanotic face, dry lips, convulsions of hands and feet, constipation, and dark urine. Pulse was wiry, tight, slippery, and rapid; tongue crimson red with dry yellow coating; fingerprint cyanotic purple, penetrating the three gates (Wind, Qi, Life). This indicates measles toxin sinking inward, turbid phlegm blocking, latent heat in Liver and Lung generating wind. The disease progression is critical.
Immediately administer one tube of Snake Gallbladder and Tangerine Peel Powder mixed with boiled water, then formulate the following prescription:
Ephedrae Herba (Sheng Mahuang) 3g, Pruni Armeniacae Semen (Guang Xingren) 6g, Gypsum Fibrosum (Sheng Shigao, crushed) 12g, Mori Folium (Dongsangye) 4.5g, Platycodonis Radix (Bai Jiegeng) 3g, Crataegi Fructus (Jiao Shanzha) 6g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 3g, Bamboo Shavings (Jing Zhuru) 4.5g, Calculus Bovis (Niuhuang) 0.15g (powdered and mixed with snake gallbladder), Tangerine Peel Powder (Chenpi Mo) one tube (mixed into medicine in portions).
Second visit: After one dose of the above prescription, the next day, convulsions gradually stopped, mental state cleared somewhat, face and rash color turned red. Cough became unobstructed, phlegm rales and panting decreased, able to cry out.
However, fever did not subside, bowels not open, urine short and dark. Pulse wiry and rapid, tongue red with yellow coating, fingerprint retreated to the Life Gate. Continue to diffuse Lung, clear heat, and resolve wind-phlegm. Prescription:
Ephedrae Herba (Sheng Mahuang) 3g, Gypsum Fibrosum (Sheng Shigao) 12g, Pruni Armeniacae Semen (Guang Xingren) 6g, Crataegi Fructus (Jiao Shanzha) 6g, Mori Folium (Dongsangye) 6g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 3g, Gallus Gallus Domesticus Vermis (Shao Jijin) 1 piece, Phragmitis Rhizoma (Lubigen) 9g, Bamboo Shavings (Jing Zhuru) 4.5g, Coeli Fructus (Chao Guya) and Hordeum Vulgare Fructus (Chao Maiya) 6g each, Snake Gallbladder and Tangerine Peel Powder one tube (mixed in portions).
Third visit: After two doses of the above prescription, consciousness cleared, measles gradually faded, fever receded, convulsions stopped. Cough reduced, phlegm resolved, poor appetite, bowels open but not smooth, yellow urine. Fingerprint retreated to the Qi Gate, pulse slightly wiry and rapid, tongue red and moist, yellow coating. This indicates residual toxins not cleared, Liver hyperactivity, Lung heat, and Stomach stagnation. Continue with the following prescription to clear:
Gypsum Fibrosum (Sheng Shigao) 9g, Pruni Armeniacae Semen (Guang Xingren) 6g, Paeoniae Radix Rubra (Chishaoyao) 4.5g, Crataegi Fructus Carbonized (Jiao Shanzha Tan) 6g, Forsythiae Fructus (Guang Lianqiao) 4.5g, Mori Folium (Dongsangye) 4.5g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 3g, Coeli Fructus (Chao Guya) and Hordeum Vulgare Fructus (Chao Maiya) 6g each, Gallus Gallus Domesticus Vermis (Shao Jijin) 1 piece, Passiflorae Fructus (Tongdahai) 1 piece.
Fourth visit: Measles fully faded, bowels smooth, fever gone, cough stopped, able to eat.
Pulse fine and slippery, tongue coating red and moist. Continue to clear and transform for regulation.

[Commentary] In mid-spring 1940, measles was rampant in Kunming. This pediatric case involved righteous Qi deficiency and pathogenic excess, a reverse syndrome of measles. Due to Wind-Cold closing the Lung, internal heat-toxin flourishing, and fire reaching extreme levels, Snake Gallbladder and Tangerine Peel Powder was used first to calm fright and resolve wind-phlegm. Then Ma Xing Shi Gan Tang plus real Xi Huang (Calculus Bovis) was administered to vent and diffuse again. Once the rash erupted and panting calmed, the Spleen and Stomach were regulated, advice was given to avoid Wind-Cold and adjust diet, leading to recovery.
Slow Convulsive Disease
Jin XX, male, 3 years old, from Kunming City. June 1938.
First visit: Diarrhea for over ten days, yellow-green color, thin consistency, seven to eight times daily. Vomiting, inability to eat, spontaneous sweating. Self-administered Shen Ling Bai Zhu San etc., ineffective. Subsequently, lips and mouth turned cyanotic, limbs cold. Administered Fuzi Lizhong Tang, but no improvement, approaching critical condition. Family brought him urgently for treatment. Symptoms: pale face and lips with cyanosis, coma, limb coldness, intermittent convulsions. White foam from mouth, continuous diarrhea. Tongue pale with white coating, pulse deep and minute, fingerprint hidden. This is due to prolonged vomiting and diarrhea, Spleen Yang about to perish, extreme deficiency-cold, reversal generating wind. Urgent use of Modified Zhu Han Dang Jing Tang is needed to save. Prescription:
Cinnamomi Cortex (Shang Rougui) 4.5g (mixed, taken with boiled water), Syzygos Aromatica Fructus (Gong Dingxiang) 3g, Piperis Longi Fructus (Bai Hujiao) 1.5g (mixed), Zingiberis Rhizoma (Chuan Ganjiang) 6g, Aconiti Lateralis Radix Praeparata (Chuan Fupian) 6g (simmered thoroughly in boiling water first), Picrasmae Fructus (Bichengqie) 3g, Evodiae Fructus (Zhi Wuyu) 3g, Caulis Ziziphus Spinosae (Zaoxintu, one piece, heated red and quenched in boiling water to decoct).
Second visit: After two doses of the above prescription, convulsions gradually calmed, vomiting decreased. Diarrhea continued, face pale, limbs not warm, coma and somnolence. Pulse slightly rose. This indicates interior cold is resolving, but prolonged diarrhea caused Middle Qi deficiency and True Yang insufficiency. Modify previous prescription:
Cinnamomi Cortex (Shang Rougui) 4.5g (taken with boiled water), Syzygos Aromatica Fructus (Gong Dingxiang) 2.4g, Aconiti Lateralis Radix Praeparata (Chuan Fupian) 9g (simmered thoroughly in boiling water first), Zingiberis Rhizoma (Chuan Ganjiang) 6g, Amomi Fructus Rotundus (Xi Sharen) 3g (mixed), Stir-fried Old Rice (Chao Laomi) 9g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 1.5g, Piperis Longi Fructus (Bai Hujiao) 1.5g (mixed), Roasted Jujube (Shao Dazao) 2 pieces.
Third visit: After two doses of the above prescription, convulsions stopped, vomiting and diarrhea reduced, limbs turned warm, consciousness gradually cleared, able to cry out. Able to take small amounts of food, but face still pale.
Tongue light red, pulse stronger than before, fingerprint visible, light cyan color. This indicates True Yang gradually recovering, Spleen deficiency and Middle weakness. Continue with the following prescription for warming and consolidation.
Panax Ginseng Radix (Hong Renshen) 4.5g (decocted separately, mixed in portions), Atractylodis Macrocephalae Rhizoma (Chao Baizhu) 6g, Cinnamomi Cortex (Chong Shang Rougui) 3g (taken with boiled water), Poria (Yun Fushen) 6g, Aconiti Lateralis Radix Praeparata (Chuan Fupian) 9g, Amomi Fructus Rotundus (Xi Sharen) 3g (mixed), Zingiberis Rhizoma (Chuan Ganjiang) 6g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 3g, Jujubae Fructus (Dazao) 2 pieces. Take two doses.
Fourth visit: Condition greatly improved, vomiting and diarrhea stopped, limbs warm. Appetite increased, face ruddy. Tongue pink, thin coating, pulse harmonious. Formulate another prescription for regulation.
Codonopsis Radix (Shang Dangshen) 6g, Atractylodis Macrocephalae Rhizoma (Chao Baizhu) 6g, Poria (Bai Fuling) 9g, Licorice Root (Zhi Gancao) 3g, Aconiti Lateralis Radix Praeparata (Chuan Fupian) 6g (simmered thoroughly in boiling water first), Citri Reticulatae Pericarpium (Guang Chenpi) 3g, Zingiberis Rhizoma (Chuan Ganjiang) 6g, Amomi Fructus Rotundus (Xi Sharen) 3g (mixed), Jujubae Fructus (Dazao) 2 pieces, Polygonati Odorati Rhizoma (Chao Yuzhu) and Stir-fried Old Rice (Chao Laomi) 6g each.
[Commentary] Extreme Yin-cold in Zang-Fu organs causes fright, convulsions, and reversal. Thus, Modified Zhu Han Dang Jing Tang was administered. This formula stacks potent warming herbs like Rougui, Dingxiang, Hujiao, Fupian, Ganjiang, and Wuyu to directly drive away the deep-seated cold and yin-turbidity of the Zang-Fu, dispelling fright and restoring Yang. After administration, Yin-cold gradually dispersed, Spleen Yang revived, fright settled, and wind calmed, embodying the principle of "treating cold with heat."
Critical Slow Convulsive Disease
He XX, male, 2.5 years old, from Kunming City. Late Autumn 1938.
First visit: Fever for over twenty days, cough, panting, vomiting. Diarrhea, short dark urine, spontaneous sweating. Sallow complexion, edema of hands and dorsal feet. Decreasing appetite, no improvement despite treatment. Six pulses fine, slippery, and rapid; tongue coating thin yellow and slightly greasy; fingerprint cyanotic and dark.
Pattern: Combined disease of Shaoyang and Yangming, unclear exterior and interior. Method: Harmonize and digest food. Prescription:
Codonopsis Radix (Su Tiaoshen) 9g, Pinelliae Rhizoma Preparatum (Fa Banxia) 6g, Citri Reticulatae Pericarpium (Guang Chenpi) 3g, Scutellariae Radix (Chao Huangqin) 2.4g, Magnoliae Officinalis Cortex (Chao Houpo) 6g, Coeli Fructus (Chao Guya) 6g, Hordeum Vulgare Fructus (Chao Maiya) 6g, Amomi Fructus Rotundus (Xi Sharen) 4.5g (mixed), Bupleuri Radix (Chao Chaihu) 4.5g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 1.5g, Sorghum Indicum Concretio (Jing Sumi) 6g, Zingiberis Rhizoma Recens (Shengjiang) 1 small slice, Jujubae Fructus (Xiaozao) 5 pieces.
Second visit: After one dose of the above prescription, symptoms did not improve, instead showing fever, irritability, groaning, cold limbs, spontaneous sweating, occasional confusion. Pulse fine, rapid, and slippery; tongue coating thin and greasy; fingerprint cyanotic and dark, gradually penetrating the three gates. Hand and foot edema, convulsions occurred twice. Self-administered Yan She San, condition still severe. Early next morning, urgent follow-up visit. This indicates Yangming accumulation not resolved, Spleen weak, Liver hyperactive generating wind. Change to prescription for eliminating accumulation, guiding stagnation, expelling wind, and calming fright.
Codonopsis Radix (Su Tiaoshen) 9g, Pinelliae Rhizoma Preparatum (Fa Banxia) 6g, Red-colored Citri Reticulatae Pericarpium (Guang Hahong) 3g, Poria Wood (Fushenmu) 9g, Uncariae Ramulus cum Uncis (Shuang Gouteng) 4.5g, Muscovite (Yunmushi) 6g, Aurantii Fructus Immaturus (Chao Zhiqiao) 4.5g, Magnoliae Officinalis Cortex (Chao Houpo) 4.5g, Mori Rubi Ramulus (Sangzhi) 9g, Sorghum Indicum Concretio (Jing Sumi) 9g, Amomi Fructus Rotundus (Xi Sharen) 6g (mixed), Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 1.5g, Gallus Gallus Domesticus Vermis (Shao Jijin) 1 piece, Snake Gallbladder and Tangerine Peel Powder one tube (mixed in three portions).
Third visit: After one dose of the above prescription, fever slightly reduced, but limbs cold and reversed, spontaneous sweating continuous, hand and foot edema. Coma, frightened crying, frequent convulsions, cyanotic-pale face. Pulse deficient, rapid, and powerless; tongue pale with white coating; fingerprint cyanotic and dark, penetrating the gate and shooting to the nail. This is due to prolonged illness damaging Yuan Qi daily, Ying and Blood depletion, Spleen deficiency generating wind, disease progression from Yang to Yin, transforming into critical slow convulsive disease. Urgently formulate the following prescription for treatment.
Panax Ginseng Radix (Hong Renshen) 9g (decocted separately, mixed), Atractylodis Macrocephalae Rhizoma (Chao Baizhu) 9g, Aconiti Lateralis Radix Praeparata (Chuan Fupian) 12g (simmered thoroughly in boiling water first), Astragali Membranacei Radix (Sheng Huangqi) 12g, Angelicae Sinensis Radix (Quan Danggui) 9g, Lycii Fructus (Da Guoqi) 9g, Rehmanniae Radix Preparata Carbonized (Shudi Tan) 9g, Ziziphi Spinosae Semen (Chao Zaoren) 6g (mixed), Sesami Nigris Semen (Heima) 6g, Corni Fructus (Zaopi) 3g, Prepared Zingiberis Rhizoma (Pao Chuanjiang) 6g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 3g.
Fourth visit: After two consecutive doses of the above prescription, reversal reversed, limbs warm, convulsions and frightened crying gradually calmed.
Heat intensity reduced, spontaneous sweating gradually stopped, able to lie peacefully. Fingerprint retreated to the Qi Gate, pulse fine and weak, tongue turned pink and moist. This indicates Yuan Qi gradually recovering, disease turning for the better. Continue with modifications of the above prescription.
Panax Ginseng Radix (Hong Renshen) 6g (decocted separately, mixed), Atractylodis Macrocephalae Rhizoma (Chao Baizhu) 9g, Aconiti Lateralis Radix Praeparata (Chuan Fupian) 12g (simmered thoroughly in boiling water first), Angelicae Sinensis Radix (Danggui) 9g, Astragali Membranacei Radix (Sheng Huangqi) 9g, Pinelliae Rhizoma (Jing Banxia) 9g, Citri Reticulatae Pericarpium (Guang Chenpi) 6g, Amomi Fructus Rotundus (Xi Sharen) 6g (mixed), Prepared Zingiberis Rhizoma (Pao Chuanjiang) 4.5g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 3g, Small Jujubae Fructus (Xiaohongzao) 9 pieces.
Fifth visit: After two doses of the above prescription, fever subsided, reversal reversed, sweating stopped, convulsions completely ceased.
Gradually desiring food, fingerprint turned red, retreated to the Wind Gate, pulse weak and slow, tongue red and moist. Long-term illness left Qi and Blood still deficient, Liver and Spleen disharmonious. Continue warming and harmonizing.
Panax Ginseng Radix (Hong Renshen) 6g (decocted separately, mixed), Bleached Atractylodis Macrocephalae Rhizoma (Biaobaizhu) 9g, Aconiti Lateralis Radix Praeparata (Chuan Fupian) 12g (simmered thoroughly in boiling water first), Poria (Fuling) 9g, Rice-fried Astragali Membranacei Radix (Miche Huangqi) 9g, Amomi Fructus Rotundus (Xi Sharen) 3g (mixed), Rice-fried Angelicae Sinensis Radix (Miche Danggui) 9g, Licorice Root (Zhi Gancao) 3g, Citri Reticulatae Pericarpium (Chenpi) 3g, Small Jujubae Fructus (Xiaozao) 9 pieces.
Sixth visit: After three doses of the above prescription, all symptoms reduced, desiring food, face ruddy, occasional phlegm congestion causing cough. Urine clear and long, stool dry, once daily. Pulse harmonious and slow, tongue red and moist. Formulate another prescription for regulation.
Aconiti Lateralis Radix Praeparata (Chuan Fupian) 12g (simmered thoroughly in boiling water first), Rice-fried Angelicae Sinensis Radix (Miche Danggui) 9g, Rice-fried Codonopsis Radix (Miche Dangshen) 9g, Atractylodis Macrocephalae Rhizoma (Chao Baizhu) 9g, Poria (Fuling) 9g, Pinelliae Rhizoma (Jing Banxia) 6g, Citri Reticulatae Pericarpium (Guang Chenpi) 4.5g, Paeoniae Radix Alba (Chao Hangshao) 6g, Amomi Fructus Rotundus (Xi Sharen) 4.5g (mixed), Nelumbinis Semen (Jian Lianzi) 9g, Coix Seed (Chao Yiren) 9g, Licorice Root (Zhi Gancao) 3g, Small Jujubae Fructus (Xiaohongzao) 7 pieces.
[Commentary] This pediatric case involved prolonged Ying and Blood depletion and Yuan Qi damage. The symptoms could not be resolved by harmonizing Shaoyang or guiding Yangming accumulation, instead deteriorating rapidly, moving from Yang to Yin, transforming into critical slow convulsive disease. Observing hand and foot edema, sallow face, and deficient-rapid pulse indicates Ying and Blood depletion. Coma, reversal, cold limbs, diarrhea, and continuous spontaneous sweating indicate decline of Yuan Yang. At this critical moment of pathological deterioration, directly using a large dose of Modified Lizhong Dihuang Tang with additions and subtractions to support Yuan Qi, nourish Ying and Blood, consolidate the root, and rescue the interior fortunately saved the critical condition.
Slow Convulsive Disease
Yang XX, male, 3 years old, residing in Kunming City. 1969.
First visit: Illness for half a month, initially fever and cough, vomiting and diarrhea. After taking Chinese and Western medicines, fever gradually subsided but diarrhea persisted, vomiting remained frequent. Further administration of cooling antipyretics resulted in intermittent convulsions. Three days later, coma and convulsions, cyanotic-dark face, eyes, and nails, fingerprint cyanotic purple penetrating the three gates. Also spontaneous sweating, loose stools, vomiting and reflux, cold hands and feet. Tongue pale with white coating, pulse fine and minute. This is due to post-fever Spleen and Stomach weakness, mistaken administration of cold-cooling herbs injuring Middle Yang, developing into slow wind syndrome. Urgently formulate the following prescription:
Aconiti Lateralis Radix Praeparata (Chuan Fupian) 9g (simmered thoroughly in boiling water first), Scorched Atractylodis Macrocephalae Rhizoma (Jiao Baizhu) 9g, Poria (Fuling) 9g, Codonopsis Radix (Lu Dangshen) 9g, Pinelliae Rhizoma Preparatum (Fa Banxia) 9g, Citri Reticulatae Pericarpium (Guang Chenpi) 3g, Amomi Fructus Rotundus (Xi Sharen) 3g (mixed), Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 2.1g, Zingiberis Rhizoma (Chuan Ganjiang) 4.5g, Stir-fried Old Rice (Chao Laomi) 6g.
Second visit: After one dose of the above prescription, coma not fully recovered, convulsions still occurring, but pulse slightly rose, slightly taking food, crying voice not loud. This indicates Spleen and Stomach Yang deficiency, fright-wind not calmed. Use original prescription with modifications:
Aconiti Lateralis Radix Praeparata (Chuan Fupian) 9g (simmered thoroughly in boiling water first), Zingiberis Rhizoma (Chuan Ganjiang) 4.5g, Codonopsis Radix (Lu Dangshen) 9g, Scorched Atractylodis Macrocephalae Rhizoma (Jiao Baizhu) 9g, Poria (Fuling) 9g, Evodiae Fructus (Chao Wuyu) 1.5g, Amomi Fructus Rotundus (Xi Sharen) 3g, Uncariae Ramulus cum Uncis (Shuang Gouteng) 2.4g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 3g, Stir-fried Old Rice (Chao Laomi) 6g, Gallus Gallus Domesticus Vermis (Shao Jijin) 1 piece.
Third visit: After two doses of the above prescription, consciousness fully recovered, convulsions, vomiting, and diarrhea stopped. Hands and feet turned warm, face became moist, cyanotic-dark nails, lips disappeared, crying voice clear. Fingerprint light red, retreated to the Wind Gate, tongue moist, pulse harmonious. This indicates fright-wind calmed, Middle Yang gradually recovering. Still Qi-deficient and Spleen-weak, continue warming and regulating. Prescription:
Codonopsis Radix (Shang Dangshen) 9g, Scorched Atractylodis Macrocephalae Rhizoma (Jiao Baizhu) 9g, Poria (Fuling) 9g, Amomi Fructus Rotundus (Xi Sharen) 3g, Zingiberis Rhizoma (Chuan Ganjiang) 4.5g, Paeoniae Radix Alba (Chao Hangshao) 3g, Glycyrrhizae Radix et Rhizoma (Sheng Gancao) 3g, Jujubae Fructus (Dazao) 2 pieces, Polygonati Odorati Rhizoma (Chao Yuzhu) and Stir-fried Old Rice (Chao Laomi) 6g each, Aconiti Lateralis Radix Praeparata (Chuan Fupian) 9g (simmered thoroughly in boiling water first).
Taking the above prescription continuously for five doses. Disease cured.
[Commentary] Fever, vomiting, and diarrhea, mistakenly treated with cooling suppression, led to Spleen deficiency, Qi weakness, and difficulty dispersing Yin-cold. Heart Yang not vigorous, Spirit不安 (unsettled), sinews and vessels lacking nourishment, thus convulsions developed. The Nei Jing states: "Yang Qi is like the sky and the sun; if it loses its proper function, lifespan is shortened and brilliance obscured. Therefore, heavenly movement should be bright like the sun." The formula administered was Modified Lizhong Tang, warming cold and strengthening transport, dissipating yin-turbidity, like the sun shining brightly in the sky, thus the disease healed.
