Mr. Songluan, an elderly gentleman of high moral character and benevolent heart, frequently suffered from chest and diaphragm distension and pain at the fifth watch (around 1-5 AM). Renowned physicians in the Sanwu region were consulted, and various cold, hot, warm, and cooling herbs were tried, yet none proved effective. Upon my examination, the pulse at the right Cun position was weak and soft, the left pulse was level, and both Chi positions were also weak. I stated: "This is a deficiency of the Lung and Kidney meridians. It can be cured by tonifying and astringing." The prescription consisted of Psoraleae Fructus (Buguzhi), Corni Fructus (Shanzhuyu), and Ginseng Radix et Rhizoma (Renshen), each 3 liang; Cervi Cornu Colla (Lujiaojiao) and Cervi Cornu Degelatinatum (Lujiaoshuang), each 5 liang; Eucommiae Cortex (Duzhong), Morinda Officinalis Radix (Ba Ji), Poria (Fuling), and Plantaginis Semen (Cheqianzi), each 1.5 liang; and Dioscoreae Rhizoma (Shanyao) 2 liang. The Cervi Cornu Colla was melted with wine to form pills, taken on an empty stomach with light salt water. Additionally, Papaveris Pericarpium (Yumiqiao, processed with honey water after removing fibrous tissue) 3 liang, Terminalia Chebula Retzui (Hezi, roasted in flour and pitted) 1 liang, and Citri Reticulatae Pericarpium (Chenpi) 1.5 liang were made into pills with refined honey, taken one qian before bed at the fifth watch with plain boiled water. After one month of treatment, the disease did not recur. The old gentleman became very pleased with me. Upon seeing the manuscript of my work *Xuanzhu*, he greatly appreciated it, saying, "Physicians usually keep prescriptions secret to appear superior, but you alone wish to share them with the public, showing your intent to save all living beings." He urgently arranged for printing and offered to subsidize the publication. I personally transcribed over a hundred of my formulas, prepared them as pills and powders for free distribution, without any expectation of gratitude.
The respected wife of Wu Yonglan, Magistrate of Lishui, experienced retrograde fullness (feeling of food stagnation) every fifth watch, followed by a bowel movement, accompanied by frequent abdominal distension and occasional pain. Her pulses at both Cun and Guan positions were surging and slippery, while both Chi positions were deep and hidden. I stated: "This is due to food retention and phlegm-fluids in the intestines and stomach." I prescribed Zongguan Wan (General Manager Pill) 3 qian, taken with ginger decoction. Although stool was passed, it was not smooth. Therefore, Shenzhou Xianglian Wan was added. External management included Shi Gypsum (Huashi), Glycyrrhizae Radix et Rhizoma (Gancao), Aucklandiae Radix (Muxiang), Aurantii Fructus (Zhike), Crataegi Fructus (Shanzha), Citri Reticulatae Pericarpium (Chenpi), Paeoniae Radix Alba (Baishao), and Coptidis Rhizoma (Huanglian, wine-processed) for regulation, leading to recovery.
The wife of Mr. Li Yuzhai suffered from severe pain in the chest, hypochondrium, and abdomen, along with delirium resembling madness. Symptoms were slightly lighter during the Yin, Mao, and Chen hours (3 AM - 9 AM), but worsened in the afternoon and night. She could not sleep day or night and had not eaten for eighteen days. Investigation revealed she originally had conditions of phlegm-fire, headache, and toothache. Three days after her menstruation began, she developed headache with alternating chills and fever. Physicians treated it as malaria. Due to extreme heat intolerance, three or four people fanned her, and she immersed her hands in cold water, holding water in her mouth without swallowing, with slight epistaxis. She often wept and felt sorrowful, her eyes swollen from crying. During severe pain, she wished to fly onto the wall; at its worst, she bit people. Urine flowed freely without control, and she felt a lump in the throat, making swallowing difficult. The pulse was wiry and rapid on the left, and surging and slippery at the right Guan position. I stated: "This is Heat Entering the Blood Chamber syndrome. Misuse of drying and acrid medicines for malaria agitation has stirred up phlegm-fire, causing simultaneous manifestation of root and branch symptoms." Others argued: "Heat Entering the Blood Chamber typically presents with delirium and hallucinations at night, like seeing ghosts. How can it cause severe chest/hypochondriac pain and biting?" I replied: "Zhang Zhongjing stated that diseases occurring when menstruation is just starting or stopping should be treated as Heat Entering the Blood Chamber, using the same method as Shaoyang disorders, primarily with Xiaochaihu Tang (Minor Bupleurum Decoction), plus blood-cooling and blood-invigorating herbs. This is an established ancient method." The severe pain and biting are due to stomach deficiency causing intestinal worms to move, seeking food but failing, resulting in a sensation of obstruction in the throat. I immediately prescribed Xiaochaihu Tang with Peach Kernel (Taoren) and Moutan Cortex (Danpi), which reduced the delirium. The next day, Anwei Tang (Calming Worms Decoction) was administered, stopping the pain, restoring appetite, and bringing vitality back.
