Discussion on Heat Syndromes of Cold Damage: The patient crosses hands over the heart, gropes along clothing, grasps the bedrail, speaks incoherently (delirium), is mentally confused and lethargic, and loses consciousness. Quack doctors fail to recognize this syndrome; seeing these symptoms, they mistakenly diagnose it as a Wind syndrome and use wind-dispelling herbs, causing many errors. They do not realize that this is Liver heat invading Lung metal, with deficient Yuan Qi unable to maintain control. It is named "Groping at Empty Air Syndrome" (Cuokong Syndrome). If urination is free, it is treatable; if not, it is untreatable.
Shengyang Sanhuo Tang (Ascending Yang and Dispersing Fire Decoction)
Ren Shen (Ginseng Radix et Rhizoma, 2 qian) Dang Gui (Angelicae Sinensis Radix, 3 qian) Huang Qin (Scutellariae Radix, 2 qian) Chai Hu (Bupleuri Radix, 8 fen) Mai Men Dong (Ophiopogonis Tuber, 3 qian, remove heart) Shao Yao (Paeoniae Radix, 2 qian)

Crush the above ingredients. Decoct with fresh ginger and jujube. Add gold jewelry and decoct together. Take hot. If there is phlegm, add Ban Xia (Pinelliae Rhizoma, 2 qian). If stool is dry and hard, with delirium and thirst, add Da Huang (Rhei Radix et Rhizoma, 1 qian). For diarrhea, add Sheng Ma (Cimicifugae Rhizoma, 3 fen) and stir-fried Bai Zhu (1 qian).
Discussion on headache, fever, stiffness of the neck and spine, aversion to cold, and absence of sweating: After taking two or three doses of diaphoretic herbs, if sweat does not appear, this is due to Yang deficiency failing to produce sweat. It is named "Yang Absence Syndrome" (Wuyang Syndrome). Use Zaizao San (Reconstruction Powder).
Zaizao San (Reconstruction Powder)

Crush the above ingredients. Decoct with one jujube in water. Take warm. In summer, add Huang Qin (Scutellariae Radix) and Shi Gao (Gypsum Fibrosum). In winter, no need to add.
Discussion on hardness and pain below the heart, purging of clear water only, delirium, thirst, and body heat: Quack doctors fail to recognize this syndrome. Seeing only diarrhea, they call it "Leaky Bottom Cold Damage" (Loudi Shanghan) and immediately use hot herbs to stop the diarrhea. This is like carrying firewood to save a fire, leading to many deaths. They do not realize this is caused by pathogenic heat transmitting inward, resulting in dryness and heat accumulation in the stomach. This diarrhea is not due to interior cold but results from daily intake of herbal decoctions. Mild purging is appropriate. It is named "Heat-Binding Diarrhea Syndrome" (Jiere Li Syndrome). For those with body heat, use this decoction.
Huanglong Tang (Yellow Dragon Decoction)
Da Huang (Rhei Radix et Rhizoma, 2 qian) Mang Xiao (Natrii Sulfas, 1 qian) Zhi Shi (Aurantii Fructus Immaturus, 1 qian) Hou Po (Magnoliae Officinalis Cortex, 8 fen) Gan Cao (Licorice Root, 8 fen) Ren Shen (Ginseng Radix et Rhizoma, 2 qian) Dang Gui (Angelicae Sinensis Radix, 3 qian)
Crush the above ingredients. Decoct with fresh ginger and jujube. Add Jie Geng (Platycodonis Radix, 8 fen) and boil once more. Take hot. For elderly patients with deficient Qi and blood, remove Mang Xiao.
Discussion on headache, body heat, aversion to cold, slight thirst, shivering sweating, body ache, soreness and pain in legs, weakness, heaviness, fatigue, and pulse that is empty, floating, and forceless: Quack doctors fail to recognize this. Seeing headache, aversion to cold, and fever, they diagnose it as standard Cold Damage and induce heavy sweating. Thus, mild conditions become severe, harming more people. They do not realize this is internal injury from labor depleting Qi and blood, compounded by external cold invasion. Sweet-warm formulas should be used for recovery. It is named "Labor-Induced Cold Damage Syndrome" (Laoli Shanghan Syndrome). Use Tiaorong Yangwei Tang (Regulating Blood and Nourishing Defense Decoction).
Tiaorong Yangwei Tang (Regulating Blood and Nourishing Defense Decoction)
Huang Qi (Astragali Radix, 2 qian) Ren Shen (Ginseng Radix et Rhizoma, 3 qian) Bai Zhu (Atractylodis Macrocephalae Rhizoma, 2 qian) Chen Pi (Citri Reticulatae Pericarpium, 1.5 qian) Dang Gui (Angelicae Sinensis Radix, 3 qian) Chai Hu (Bupleuri Radix, 8 fen)

Add Chuan Xiong (Ligustici Chuanxiong Rhizoma), Qiang Huo (Notopterygii Rhizoma et Radix), and Fang Feng (Saposhnikoviae Radix).
Crush the above ingredients. Decoct with fresh ginger, jujube, and white part of scallion. For internal injury complicated by external contraction, use the eight herbs of Buzhong Yiqi Tang as the main base, modifying according to symptoms seen in the Six Channels. If Taiyang syndrome appears (head/neck pain, stiff waist/spine), add Qiang Huo (1 qian), Gao Ben (Ligustici Wallichii Rhizoma et Radix, 2 qian), and Gui Zhi (8 fen). If Yangming syndrome appears (body heat, eye pain, dry nose, insomnia), add Ge Gen (Puerariae Lobatae Radix, 1 qian) and double Sheng Ma. If Shaoyang syndrome appears (hypochondriac pain, deafness), add Huang Qin (2 qian), Ban Xia (Pinelliae Rhizoma, 2 qian), and double Chai Hu. If Taiyin syndrome appears (abdominal fullness, dry throat), add Zhi Shi (1 qian) and Hou Po (8 fen). If Shaoyin syndrome appears (dry mouth/tongue), add raw Gan Cao (8 fen) and Jie Geng (8 fen). If Jueyin syndrome appears (irritation/fullness, retracted testicles), add Chuan Xiong (1.5 qian). If complications include macules/rash, add Ge Gen (1 qian), Xuan Shen (Scrophulariae Radix, 2 qian), and double Sheng Ma. For internal injury with phlegm, add Ban Xia and Zhu Li (Bambusae Succus), still adding Jiang Zhi (Fresh Ginger Juice) to guide the medicine.
Discussion on gradual onset of coma and muteness in Cold Damage, or solitary talking in sleep, red eyes and scorched tongue after 10-20 days, swallowing water when offered but refusing otherwise, resembling drunkenness: This is pathogenic heat transmitting to the Heart channel. Heart fire forcing upward onto Lung metal causes mental confusion, hence named "Crossing Channel Syndrome" (Yuejing Syndrome). Use Xiexin Daochi Tang (Draining Heart and Guiding Out Redness Decoction).

Crush into powder. Decoct with fresh ginger, jujube, and Deng Xin Cao (Junci Medullae). Before administration, add Sheng Di Huang juice (Radix Rehmanniae Recens, 2 spoons).
Discussion on Cold Damage without headache or aversion to cold, slight body heat, flushed face, slight thirst, dull eyes, incoherent speech, rapid but forceless pulse: This is due to excessive sweating and purging, weakening the lower origin. Rootless deficient fire floats upward, named "Floating Yang Syndrome" (Daiyang Syndrome). Use Fuyuan Tang (Restoring Origin Decoction).
Fuyuan Tang (Restoring Origin Decoction)
Shu Fu Zi (Prepared Aconiti Lateralis Radix, 8 fen)
Gan Cao (Licorice Root, 8 fen) Gan Jiang (Zingiberis Rhizoma, 8 fen) Ren Shen (Ginseng Radix et Rhizoma, 2 qian) Wu Wei Zi (Schisandrae Fructus, 4 fen) Mai Men Dong (Ophiopogonis Tuber, 3 qian) Huang Lian (Coptidis Rhizoma, 8 fen) Zhi Mu (Anemarrhenae Rhizoma, 1.5 qian) Shao Yao (Paeoniae Radix, 2 qian)
Crush into powder. Decoct with fresh ginger, jujube, and scallion. Before retiring, add Tong Bian (Child's Urine, 3 spoons). Take warm.
Discussion on early stage of Cold Damage without heat, but with manic speech, irritability, and spirit lacking normal vigor: Do not mistake this for mania and use purgatives, as many die. This is actually pathogenic heat binding in the Bladder, named "Like-Mania Syndrome" (Rukuang Syndrome). Use Guiling Yinzi (Cassia and Poria Drink).
Guiling Yinzi (Cassia and Poria Drink)

Crush. Decoct with fresh ginger and 24 stalks of Deng Xin Cao (Junci Medullae).
Discussion on no headache, no aversion to cold, only intense thirst, free urination, black stool, and incoherent speech: This is an internal injury syndrome of blood stasis in Liver and Spleen, causing coma, heaviness, and erroneous speech, named "Blood-Clasping" (Jiaxue).
Danggui Huoxue Tang (Angelica Blood-Invigorating Decoction)
Dang Gui (Angelicae Sinensis Radix, 3 qian)
Chi Shao (Paeoniae Radix Rubra, 2 qian) Gan Cao (Licorice Root, 8 fen) Hong Hua (Carthami Flos, 8 fen) Gui Zhi (Cinnamomi Ramulus, 8 fen) Gan Jiang (Zingiberis Rhizoma, 8 fen) Zhi Ke (Aurantii Fructus, 8 fen, bran-fried) Chai Hu (Bupleuri Radix, 8 fen) Ren Shen (Ginseng Radix et Rhizoma, 1 qian) Sheng Di Huang (Radix Rehmanniae Recens, 3 qian) Tao Ren (Persicae Semen, 8 fen, paste)
Crush. Decoct with one slice of fresh ginger in water. Add three spoons of wine and take together. After three doses, remove Tao Ren, Hong Hua, Gan Jiang, and Gui Zhi. Add Bai Zhu (1 qian) and Fu Ling (Poria, 3 qian).
Discussion on aversion to cold with high fever, headache with confusion and oppression, shortness of breath with wheezing, and salivation: This is due to internal injury from Seven Emotions causing phlegm to confuse the Heart orifices, Spirit failing to reside, leaving the residence empty, thus generating phlegm, named "Phlegm-Clasping" (Jiatan). Phlegm syndromes resembling Cold Damage are treated similarly.
Jiawei Daotan Tang (Modified Expectoration-Resolving Decoction)

Discussion on Food Stagnation resembling Cold Damage: Headache, body heat, aversion to cold, tight and full pulse at Qi Kou (cun position), but no body pain, which is the difference.
