Pulse string-like indicates wind. Pulse soft and weak indicates dampness. Pulse surging and rapid indicates heat. Pulse slow and choppy indicates cold. Pulse faint and slippery indicates deficiency. Pulse firm and hard indicates excess; if knotted, it is due to qi stagnation; if scattered, it is due to excessive worry; if tight, it is due to anger; if thin, it is due to sorrow.
Hongjiao (a disease characterized by swelling, pain, numbness, or spasms of the feet) arises from damp-heat accumulating in the feet, causing qi obstruction and pain. When damp-heat contends, dominance of dampness causes aversion to cold, while dominance of heat causes high fever. These are its symptoms. There may also be accompanying headache, resembling Taiyang disease (diseases caused by pathogenic factors affecting the Taiyang channel). However, at the onset, the etiology stems from Hongjiao, which distinguishes it from Taiyang disease. One should not rely solely on foot swelling for diagnosis; there are cases with pain but no swelling, called Dry Hongjiao; cases with flaccid paralysis and inability to move freely, called Huanfeng (Flaccid Wind); cases with pain and insensitivity, called Shibì (Damp Bi); and cases with muscle spasms and cramping, called Fengdu (Wind Toxin). Physicians must carefully observe and differentiate these syndromes. Each has different treatment methods. Generally, Hongjiao is a disorder of obstruction and congestion; it favors unobstructed flow and dislikes blockage. Therefore, Sun Simiao said: Hongjiao patients often die from excess qi (pathogenic excess), and never die from deficiency caused by medication. Thus, Hongjiao patients should neither receive strong tonification nor strong purgation. In this formula, Mutong (Akebia Stem), Fangji (Stephania Root), and Binglang (Areca Seed) are draining agents that can clear heat; Cangzhu (Atractylodes Rhizome) and Baizhu (White Atractylodes Rhizome) are drying agents that can remove dampness. However, Chuanxiong (Ligusticum Rhizome) can disperse qi stagnation in the blood, and Xijiao (Rhinoceros Horn) can promote blood circulation within the qi. If pain precedes swelling, qi has injured blood; use heavy doses of Chuanxiong. If swelling precedes pain, blood has injured qi; use heavy doses of Xijiao. If stool is dry, add Taoren (Peach Kernel); if urination is difficult, add Niuxi (Achyranthes Root); if there is heat, add Huangqin (Scutellaria Root) and Lianzi (Coptis Rhizome); if there is intermittent fever, add Shigao (Gypsum Fibrosum); if there is phlegm, add Zhuli (Bamboo Sap). Flexibility is key; do not be rigid. If Hongjiao attacks the heart, causing incessant panting and vomiting, it is fatal, due to water invading fire (Kidney Water overwhelming Heart Fire).
Another discussion focuses on treating Hongjiao with aversion to cold and high fever, caused by damp-heat in the feet leading to qi pain.
Fangji Yin (Stephania Decoction)
Cangzhu (stir-fried with salt water), Baizhu, Huangbai (Phellodendron Bark, stir-fried with wine), Fangji, Shengdihuang (Rehmannia Root, stir-fried with wine), Chuanxiong, Binglang, Mutong, Xijiao, Gancao Gao (Licorice Root Tip).
Crush the ingredients, decoct in water, and take on an empty stomach.
Another discussion: When Hongjiao first appears, causing whole-body pain or swelling and pain in limbs and joints, with blocked urination and defecation, first use this medicine to guide the pathogen out, then administer Danggui Niantong Tang (Angelica Pain-Relieving Decoction) to cure.
Qianghuo Daotan Tang (Notopterygium Phlegm-Guiding Decoction)
Qianghuo (Notopterygium Rhizome), Duhuo (Angelica Pubescens Root, each 1 qian), Fangji (stir-fried 1 qian), Danggui (Angelica Sinensis Root, 1 qian), Dahuang (Rhubarb Root, decocted with wine 1.5 qian), Zhishi (Immature Bitter Orange, stir-fried 1 qian).
Crush the ingredients, decoct in water, and take.
Another discussion: For Hongjiao caused by damp-heat, causing vexing pain in limbs and joints, heaviness in shoulders and back, discomfort in chest and hypochondrium, accompanied by whole-body pain, dampness sinking to cause swelling and pain in calves and shins, sores on feet and knees with redness and swelling, internal and external sores with continuous pus discharge, itching or pain, all are suitable for Danggui Niantong Tang.
Danggui Niantong Tang (Angelica Pain-Relieving Decoction)
Qianghuo, Yichen (Artemisia Capillaris, each 1 qian), Fangfeng (Saposhnikovia Root), Zhuling (Polyporus), Kushen (Sophora Root), Baizhu (de-rooted, each 0.5 qian), Gegen (Pueraria Root, 0.4 qian), Huangqin (stir-fried with wine), Zexie (Alisma Root), Danggui (wine-rinsed), Zhimu (Anemarrhena Rhizome, de-hairy), Renshen (Ginseng Root), Shengma (Cimicifuga Rhizome), Cangzhu (soaked in rice water and stir-fried, each 0.4 qian), Gancao (Licorice Root, 1 qian).
Crush the ingredients, decoct in water, and take on an empty stomach.
Another discussion: For Hongjiao involving the three yang channels, flowing to the ankles, causing red, hot, and swollen areas, alternating chills and fever like malaria, spontaneous sweating, and aversion to wind, use Ren Shen Bai Du San (Ginseng Detoxifying Powder), 1 qian each. (Formula see Cold Section). Add Cangzhu (rice water stir-fried) and Dahuang (wine-steamed), 2 qian each, as one dose. Add fresh ginger, decoct, and take on an empty stomach. For skin itching and red rashes, add Chantui (Cicada Slough).
Another discussion: For wind-damp flowing into meridians, causing flaccid paralysis of limbs and joints, and elderly people with painful knees and feet unable to walk.
Qisheng San (Seven Sages Powder)
Chuan Niuxi (Cyathula Root, soaked in wine), Duzhong (Eucommia Bark, stir-fried with ginger and wine), Xuduan (Dipsacus Root), Chuan Bixie (Dioscorea Hypoglauca Rhizome), Fangfeng, Chuan Duhuo, Gancao (each 3 liang).
Dry by baking, grind into powder, take 2 qian each time, mixed with wine.
Another miraculous prescription for relieving Hongjiao pain.
Ruxiang (Frankincense), Moyao (Myrrh), Tianma (Gastrodia Rhizome), Baifuzi (Typhonium Rhizome), Jiangcan (Bombyx Batryticatus).
Crush the ingredients, equal parts, grind into fine powder, take 0.5 qian each time, on an empty stomach, mixed with wine.
Another treatment for dampness attacking and injecting, causing waist and foot pain and difficulty walking.
Danggui (wine-rinsed), Rougui (Cinnamon Bark), Yuanhusuo (Corydalis Rhizome), Bixie, Moyao (each 3 liang), Duzhong (wine-stir-fried 1.5 liang).
Grind into fine powder, take 3 qian each time, on an empty stomach, with warm wine.
Another treatment for Hongjiao edema.
Chuan Niuxi (de-rooted, wine-rinsed 0.5 qian), Weilingxian (Swallowwort Root, 1 qian), Han Fangji (Stephania Tetrandra Root, 1 qian), Wujia Pi (Acanthopanax Bark, 1.5 qian), Chuan Duhuo (1.5 qian), Cangzhu (soaked in rice water 1.5 qian), Danggui Shen (Angelica Root Body, 1 qian), Chuan Huangbai (Phellodendron Bark, salt and wine stir-fried 1 qian).
Crush one dose, decoct with fresh ginger, when cooked, add one cup of wine and take together.
One person felt qi like fire rising from feet to abdomen, indicating extreme deficiency. The fire originates from the depths (Kidney). This disease is rarely curable (one in ten survives). Treatment uses Siwu Tang (Four Substances Decoction) plus Huangbai and Zhimu to descend fire. Externally, apply large Fuzi (Aconite Root) powder mixed with saliva to Yongquan (KI1) acupoint on the sole to guide fire downward.
Note: This discussion is incomplete. If labor fatigue, consumption, or yin deficiency patients have this, treat as yin deficiency. If robust individuals have this, it is dampness郁 transforming into heat. I once walked in rain, got wet, and developed this syndrome. Later, my Jiawei Ermiao Wan (Modified Two Marvels Pill) cured me. Subsequent physicians verified its efficacy. Misdiagnosing as yin deficiency would lead to Wei syndrome (flaccidity).
Another treatment for heel pain. With phlegm, or blood heat. For blood heat, use Siwu Tang plus Huangbai, Zhimu, Niuxi. With phlegm sputum, use Wuji San (Five Accumulations Powder) plus Mugua (Chaenomeles Fruit).
Another discussion: Damp-bi pain in both feet, or burning heat starting from dorsum pedis spreading to waist and hips, or numbness and flaccidity, all caused by damp-heat due to indulgence in alcohol and desires, and exposure to wind-cold.
Jiawei Ermiao Wan (Modified Two Marvels Pill)
Cangzhu (rice water processed 4 liang), Huangbai (wine-soaked and sun-dried 2 liang), Niuxi (1 liang), Danggui Wei (Angelica Root Tail, wine-rinsed 1 liang), Bixie (1 liang), Fangji (1 liang), Guiban (Tortoise Shell, vinegar-roasted 1 liang, or replace with Shudihuang 2 liang).
Grind into fine powder, make pills with good wine paste, size of wutong seeds. Take 70 pills each time, on an empty stomach, with salt and ginger water.
Another treatment for all cold-damp deficient-cold Hongjiao, with swelling, pain, wasting, bedridden for years, unable to walk. Use Duhuo Jisheng Tang (Notopterygium and Dogwood Decoction) in equal parts, cooked with good wine. Effective. (Formula see Damp-Cold Section).
Another discussion: For men and women with five labors and seven injuries, kidney qi failure, spirit exhaustion, wind-damp flowing, painful and sore knees and feet, difficult walking, loss of taste, deafness, dim vision, dry skin, uterine cold causing infertility, foul lower body, intestinal wind and hemorrhoids, hematemesis and hematochezia, and various qi disorders, all treated.
Jingjin Dixian Dan (Imperial Earth Immortal Pill) Verified many times by Xiyuan Gong.
Huangqi (Astragalus Root, stir-fried 1.5 liang), Renshen (1 liang), Baifuling (Poria, peeled 1 liang), Baizhu (de-rooted 1 liang), Dafuzi (Aconite Root, processed and peeled 4 liang), Fupenzi (Rubus Fruit, 2 liang), Shouwu (Polygonum Multiflorum Root, 2 liang), Chuanwu (Aconite Root, processed 1 liang), Baifuzi (Typhonium Rhizome, processed 4 liang), Niuxi (wine-soaked 4 liang), Roushouyu (Cynomorium, wine-soaked 4 liang), Chuan Bixie (2 liang), Tusizi (Cuscuta Seed, wine-processed 2 liang), Wuyao (Lindera Root, 2 liang), Tiannanxing (Arisaema Rhizome, processed 2 liang), Gusui Bu (Drynaria Rhizome, de-hairy and stir-fried 3 liang), Fangfeng (2 liang), Chixiaodou (Adzuki Bean, 2 liang), Mubiezi (Momordica Seed, de-shelled 2 liang), Qianghuo (2 liang), Chuanjiao (Sichuan Pepper, de-eye and stir-fried 4 liang), Dilong (Earthworm, de-earth 3 liang), Jinmao Gouji (Cibotium Rhizome, de-hairy 2 liang), Gancao (Licorice Root, 2 liang).
Grind into fine powder, make pills with wine-boiled wheat flour paste, size of wutong seeds. Take 30-40 pills each time, on an empty stomach, with warm wine.
Tao Hongjing included this Dixian Dan in the Daoist Canon. Someone had wind-qiu disease since childhood, untreatable for over 50 years. Living in seclusion, he took three doses daily, and all diseases vanished. People say: Study prescriptions for three years, no disease is incurable; treat diseases for three years, no medicine is useless. Alas, true indeed. I recently suffered knee and foot pain, took Jingjin Dixian Dan, and recovered in three months. Thus, there is no incurable disease in the world, and no useless prescription in medical books; it depends on finding a wise physician.
Ershisiwei Qingjiao Wan (Twenty-Four Ingredient Light Foot Pill) General for Hongjiao.
Danggui (wine-rinsed 2 liang), Chuanxiong (1 liang), Bixie (boiled in clean water until dry 1 liang), Muxiang (Costus Root, 0.7 qian), Haitongpi (Tinospora Bark, 0.7 qian), Xixin (Asarum Root, 1 liang), Niuxi (wine-rinsed 1 liang), Zhiqiao (Bitter Orange, 1 liang), Cangzhu (rice water soak and stir-fry 0.7 qian), Fangfeng (0.7 qian), Shinanteng (Tinospora Vine, 2 liang), Mahuang (Ephedra Herb, 0.7 qian), Duzhong (ginger-stir-fried 1 liang), Mugua (0.7 qian), Weilingxian (0.7 qian), Qianghuo (1 liang), Yiyiren (Coix Seed, 1 liang), Ruxiang (0.5 qian), Xuduan (wine-rinsed 0.7 qian), Binglang (1 liang), Wujia Pi (0.7 qian), Duhuo (0.7 qian), Wulingzhi (Trogopterus Dung, 0.7 qian), Moyao (0.7 qian).
Grind into fine powder, make pills with wine-soaked snow fat, size of wutong seeds. Take 50 pills each time, with Spicy Cinnamon and Schizonepeta tea, or Bitter Orange and Chaenomeles tea.
Another discussion: Cold-damp Hongjiao with pain and numbness, two chi pulses deep and thin, is Bi syndrome. Nei Jing says: Excess cold causes Tongbi (painful Bi), excess dampness causes Zhuobi (fixed Bi). Now pain and numbness indicate cold and fixed dampness. Two chi pulses govern the feet; deep pulse indicates interior, slow pulse indicates cold. In this formula, Guizhi (Cinnamon Twig) and Fuzi treat cold, Cangzhu treats dampness, Gancao and Fuling are spleen herbs, supporting insufficient spleen qi and controlling excess dampness.
