According to *Chao Shi Bing Yuan* (Source of Diseases by Master Chao) regarding the syndrome of Zhuang Re (Intense Fever): When a child presents with Zhuang Re (Intense Fever), it is because the child's blood and Qi are vigorous, generating internal heat in the five Zang organs. This heat steams and radiates outward, causing intense fever throughout the body. Generally, this resembles Wen Zhuang (Persistent Mild Fever), but there are slight differences; it may be accompanied by latent heat or residual cold. In cases accompanied by latent heat, the stool is yellow and foul-smelling; in cases accompanied by residual cold, the stool is white and has a sour odor. Both conditions result from disharmony of the Fu and Zang organs, where cold and hot qi simultaneously invade the stomach and intestines, accumulating and gradually manifesting. The condition characterized by a distinct sense of warmth and extreme heat intensity is called Wen Zhuang (Persistent Mild Fever). In contrast, Zhuang Re (Intense Fever) arises from vigorous blood and Qi, with heat radiating outward without a gradual onset, directly presenting as high fever; this is the distinction. If Zhuang Re does not subside, it may transform into Jing Feng (Pediatric Convulsion); in severe cases, it may evolve into Xian Zheng (Epilepsy).
Qian Yi discussed the similarities between Wind-Warmth, Tide Fever, and Zhuang Re: Tide Fever manifests as fever in the evening that subsides after a certain time, recurring at the same time the next day; this indicates an impending onset of convulsions. Zhuang Re involves continuous fever without cessation, which can lead to severe convulsions and epilepsy. Wind-Heat presents with body heat and warm breath, accompanied by wind-pathogen symptoms. Wen Zhuang is merely mild warmth without significant heat.
In his supplementary formulas, Qian Yi noted that pediatric Zhuang Re with lethargy, Wind-Cold, Wind-Heat, rash, and food stagnation present similarly. During the period of uncertainty in differentiation, administering Shengma Gegen Tang (Burdock and Kudzu Decoction), Xingxing San (Awakening Powder), or Xiao Chaihu Tang (Minor Bupleurum Decoction) yields excellent results. These formulas provide general treatment for these conditions, minimizing the risk of misdiagnosis. However, if the cause is food stagnation, characterized by sour, foul-smelling, undigested stool, aversion to eating, or vomiting, purgative methods should be used.
Zhang Huan cautiously noted: In late winter and early spring, children wearing overly thick clothing, or wet nurses consuming excessive alcohol, wheat products, sweet and fatty foods, can cause disharmony between the Heart and Lungs, leading to the accumulation of pathogenic heat internally. Upon removing heavy clothes, wind pathogens attack the skin and hair, transmitting into the Fu and Zang organs. This causes phlegm stagnation in the chest and diaphragm, Zhuang Re (Intense Fever), and mental restlessness with palpitations. This presentation resembles Tai Yang Disease (Exterior Syndrome), epidemic febrile diseases, and rashes, and is thus named the Wind-Heat Syndrome.
*Zhuang Shi Jia Chuan* records seven syndromes of Zhuang Re in children:
1. Tai Yang Disease Syndrome: Intense fever throughout the body, frequently protruding the tongue to lick the lips.
2. Macule/Rash Syndrome: Cough, hoarseness, intense fever throughout the body, but the ears and nose remain cool; otherwise, hiccups may occur.
3. Wind-Heat Syndrome: Intense fever throughout the body, twitching of the eyelids on both sides of the nose, rapid breathing. In severe cases, sores form inside the nose, and blood streaks appear on the hands.
4. Qi Accumulation Syndrome: Intense fever throughout the body, cold limbs in the morning, heaviness and oppression in the early morning, and tide fever in the evening.
5. Deficiency Accumulation Syndrome: Intense fever throughout the body, cold feet but warm hands. If stagnation is in the left hypochondrium, the right cheek becomes red; if in the right hypochondrium, the left cheek becomes red.
6. Heat due to Fright Syndrome: Also presents with intense fever throughout the body, excessive sleep, delirium, sleeping on the back, startle jumps, crying, shaking the head, playing with the tongue, and upward gazing.
7. Throat Bi Syndrome: Also presents with intense fever throughout the body, frequent coughing, and a desire to drink cold water. When the throat hurts, there is a sensation of slight chills. For the above seven syndromes, medication should be administered after careful examination.
*Ge Shi Zhou Hou* Xu Wang Shen Xiao Fang: A formula for infants under one hundred days suffering from Zhuang Re and rapid breathing, who can only sleep on their side, have eyes that will not open, and have yellow-red urine.

If cold qi enters the stomach causing dysentery, or desire to pass red-and-white mucus frequently with severe lower abdominal pain and Zhuang Re, and the pulse is surging or rapid, this formula stops the heat and cures the dysentery. It is also indicated for Zhuang Re without vomiting or diarrhea.
