Nash's *Bingyuan* (Source of Diseases) on the condition of pediatric abdominal distension: Abdominal distension is caused by cold qi invading the zang-fu organs. Children's zang-fu are tender and weak; if wind-cold pathogenic factors invade and contend with the organ qi, it leads to abdominal distension.
If spleen deficiency and cold shift into the stomach, food cannot be digested; if the intestines are deficient and cold qi takes advantage, it transforms into diarrhea.
Qian Yi discussed abdominal distension, attributing it to spleen and stomach deficiency with rebellious qi attacking. In excess patterns, there is chest oppression, restlessness, fullness, and wheezing; purgation can be used, employing Zishuang Wan (Purple Frost Pill) or Baibing Zi (White Cake). (Zishuang Wan is seen in the Accumulation and Mass section; Baibing Zi formula is seen in the Convulsion section.) If there is no wheezing, it is a deficiency pattern, and purgation should not be used. If mistakenly purged, spleen qi becomes deficient and rebels upward, attaching to the lung and traveling with it, causing both mother (spleen) and child (lung) organs to be deficient. The lung governs the eyelids and cheeks; the spleen governs the four limbs. When maternal qi is extremely deficient, eyelids and cheeks swell. Yellow complexion belongs to the spleen. Treatment uses Taqi Wan (Collapse Qi Pill) to gradually resolve symptoms (formula seen in the Stomach Qi Disharmony section); if not cured, gradually increase the number of pills. Do not treat with large warming and dispersing herbs such as Dingxiang (Clove), Muxiang (Aucklandia Root), Jupi (Tangerine Peel), and Doukou (Cardamom). Why? Because spleen deficiency qi has not yet exited, presenting as abdominal distension without wheezing; powder preparations can be used to disperse qi upwards and downwards, leading to recovery. If deficient qi has already exited and attaches to the lung, it indicates internal weakness of spleen and stomach, frequently generating deficient qi that enters the four limbs and face. Children easily shift between deficiency and excess; spleen deficiency cannot tolerate cold or warmth—taking cold generates raw/cold conditions, taking warmth generates heat. This must be understood to avoid error. Stomach qi deficiency with heat often generates jaundice disease. Or excessive thirst occurs. Spleen deficiency fails to control kidney qi, which follows lung qi upward to the four limbs like water; kidney qi invading and floating to the lung causes severe wheezing. This situation requires taking Taqi Wan. (Formula seen above.) After the illness resolves, if the face is not ruddy, it is because deficiency and decline have not yet recovered.
Treating abdominal distension is like fighting bandits in a forest; if the bandits have not left the forest, attacking with troops will surely capture them; if they have left the forest, do not attack hastily, for attack will result in loss; one should gradually subdue them with intent, which is compliant with the condition.
To treat deficiency-type abdominal distension, first take Taqi Wan (formula seen above); if not cured, and there is food accumulation and hardened stool in the abdomen, yellow urine, occasional mild wheezing, pulse is deep and excess, frequent desire to drink but inability to eat, purgation can be used. This is because the spleen was initially deficient and then accumulated stagnation; treatment should first tonify the spleen, then purge. After purging, tonify the spleen again, and it will heal. Tonifying the lung may generate deficient wheezing.
