What is the cause of edema in the four limbs after childbirth? The answer is: Postpartum retained blood (stasis of lochia), taking advantage of the body's deficiency, stagnates and accumulates in the five zang organs. It flows along the meridians into the four limbs, where it remains and accumulates deeper day by day, unable to return and dissipate. The blood rots and transforms like water, thus causing a yellowish complexion and edema in the four limbs. If physicians do not recognize this postpartum pathogenesis and treat it as general water qi disease, they will often use diuretic drugs. For extremely deficient patients, using such drugs further depletes the already deficient postpartum qi and blood. This is called "deficiency edema," which often leads to premature death. Simply administering Xiao Tiaojing San (Minor Regulation of Menstruation Powder) allows the blood to circulate freely, and the swelling will subside, leading to recovery.
After childbirth, labor damages qi and blood. When the cou li (skin pores and interstitial spaces) are empty, wind pathogens take the opportunity to invade. The pathogen clashes with qi, preventing its dissemination and venting, thus causing deficiency edema that is light and floating. This is because the pathogen clashes with qi, resulting in "qi edema." If the skin becomes shiny and tight like a ripe plum, and the condition worsens, it transforms into water edema. Qi edema is cured by sweating; water edema requires promoting urination to improve.
Formulas
Xiao Tiaojing San (from *Jingyan Liangfang*): Treats various types of postpartum edema. Some women swell during pregnancy and do not recover after childbirth. Others suffer from poor postpartum care, external contraction of cold, heat, wind, and dampness, or internal injury from anger, joy, worry, and fright. This causes blood and qi to clash and stagnate in the meridians. Whether the pathology lies in the qi level or blood level, treatment must be based on the specific syndrome for recovery. Xiao Tiaojing San is effective for blood-level disorders but has shallow efficacy and may not be sufficient alone. It is better to combine it with Wuzhuyu Tang (Evodia Decoction), Zhizhu Duohun San (Bitter Orange Soul-Plucking Powder), and Da Tiaojing San (Major Regulation of Menstruation Powder), all of which are important formulas. It also treats postpartum retained blood rushing up to the heart, causing mental restlessness, inability to lie down or sit still, hallucinations (seeing ghosts/spirits), and disordered speech.
Ingredients: Myrrha, Amber, Cinnamon Heart, Peony Root, Angelica Sinensis (each 1 qian); Asarum, Musk (each 0.5 qian; one prescription omits this). Grind into powder. Take 0.5 qian per dose, mixed with a little ginger juice and warm wine. Generally, postpartum retained blood stagnates in the five zang organs and flows into the four limbs, causing edema. Physicians who mistake this for water qi disease and use diuretics harm the already deficient patient. Since postpartum deficiency is compounded by drug-induced depletion, it is a double deficiency, often leading to death. Only this formula promotes blood circulation and resolves swelling. For excessive blood loss causing heart deficiency and palpitations, add Borneol (Longnao). For edema in the four limbs, add Licorice Root. Commentator Bao states: Postpartum edema is diverse; one must treat the root cause. Differentiate between blood-level and qi-level disorders. While Tiaojing San is effective for blood-level issues, its power is weak.
Treats generalized cyanotic swelling and pain after childbirth, and postpartum blood-water diseases. (From *Daquan Liangfang*)
Ingredients: Dried Resin of Toxicodendron (Ganxi), Barley (equal parts). Grind each into fine powder. In a new clay pot, layer barley and Ganxi repeatedly until full. Seal with salt mud and fire-roast until red hot. Cool and grind into powder. For any postpartum disease, take 2 qian mixed with hot wine.
Treats postpartum wind edema and blood-deficiency water edema. (From *Daquan Liangfang*)
Ingredients: Leaf of Lycopodium Japonicum (Zelan Ye), Saposhnikovia Root (Fangfeng). Grind into powder. Take 1 qian per dose with warm wine. If alcohol is not tolerated, vinegar water can be used.
Han Fangji San (Han Fangji Powder) (From *Daquan Liangfang*)
Treats postpartum qi deficiency with blood congestion attacking the heart, causing facial and head edema.
Ingredients: Stephania Tetrandra (Han Fangji), Polyporus (Zhuling), Bitter Orange (Zhike), Phytolacca Root (Shanglu), Licorice Root (Gancao), Mulberry Root Bark (Sangbaipi) (each 1 liang). Coarsely grind. Take 4 qian per dose with 1.5 cups of water and 3 slices of fresh ginger. Decoct to 0.7 cup, strain, and drink warm on an empty stomach.
Chenxiang San (Agarwood Powder) (From *Bianchan Xu Zhi*)
Treats postpartum incomplete expulsion of blood, which disperses into the four limbs causing edema, abdominal distension, and rapid breathing.
Ingredients: Agarwood (Chenxiang) (3 qian), Ligusticum Chuanxiong (Chuanxiong) (0.5 liang), Cinnamon Heart (Guixin), White Peony Root (Baishao), Licorice Root (Gancao), Angelica Sinensis (Danggui) (each 3 qian), Moutan Bark (Mudanpi) (11 zhu), Pollen Typhae (Puhuang) (0.5 liang, stir-fried). Grind into fine powder. Take 2 qian mixed with warm wine. Efficacy is indicated by the disappearance of swelling as blood is expelled.
Duohun San (Soul-Plucking Powder) (From *San Yin Fang*)
Treats postpartum deficiency edema with panting. Promoting urination leads to recovery.
Ingredients: Fresh Ginger (3 liang, juice extracted), Wheat Flour (3 liang), Pinellia Tuber (Banxia) (7 pieces, washed seven times in boiling water). Use ginger juice to knead the flour and wrap the Pinellia, forming seven cakes.
Da Tiaojing San (Major Regulation of Menstruation Powder)
Treats postpartum abdominal fullness and edema, panting, thirst, difficulty urinating, alternating chills and fever, and disharmony of Ying and Wei levels. Do not underestimate this. If retained blood does not dissipate, it affects not only the Heart and Spleen. Generally, if one Yin obstructs one Yang, cold arises; if Yang prevails, heat arises. Retained blood flowing along meridians obstructs Yang (causing heat) or Yin (causing cold). When Ying and Wei harmonize, the body resists re-infection. If other formulas fail, use Da Tiaojing San. Blood deficiency with unexpelled lochia, stagnant qi, congealed turbid blood, disharmonious Ying and Wei, and mutual invasion of Yin and Yang cause aversion to cold, fever, spontaneous sweating, or edema. All are due to unbalanced Ying and Wei.
Ingredients: Soybean (Stir-fried, peeled, 1.5 liang), Polygala Root (Fushen) (1 liang), Amber (1 qian). Grind into powder. Take with thick decoction of black soybeans.
Treats deficiency edema in the four limbs of women after childbirth.

Coarsely crush and sieve. Take 3 qian per dose with 1 cup of water and 2 halved jujubes. Decoct to 0.7 cup, strain, and drink warm. Stop when edema subsides through draining. Commentary: Postpartum qi and blood are both deficient. Cold-dampness invades, weakening the Spleen and Stomach, impairing their ability to distribute qi. Water and blood mix and overflow into the skin, causing edema. Promoting urination cures the disease.

Physician's Commentary: Postpartum edema is diverse. Some swell during pregnancy and do not recover after childbirth. Others suffer from poor postpartum care, external contraction of cold, heat, wind, and dampness, or internal injury from anger, joy, worry, and fright. Blood and qi clash and stagnate in the meridians. One must differentiate between qi-level and blood-level disorders. Tiaojing San only treats blood-level issues. If swelling persists, Zhizhu Tang, Duohun San, and Da Tiaojing San are essential. Zhizhu Tang treats hardness under the heart as large as a plate with edges like a spinning top, called "qi level."
