Shanghan disease. If the patient has already undergone sweating, vomiting, or purging treatments, or after taking medication, and presents with a pulse that is surging (Hong), solid, and rapid (Shu), abdominal distension and fullness, dark yellow urine, difficult defecation, irritability or thirst, and reddish complexion, this is due to accumulated heat in the fu organs.
【English Translation】
In patients suffering from Shanghan (Cold Damage) disease, if they have already been treated with diaphoresis (sweating), emesis, or catharsis (purging), or after ingesting medicine, and exhibit a pulse that is surging (Hong), firm, and rapid (Shu), accompanied by abdominal distension and fullness, dark yellow urine, difficulty in defecation, irritability or thirst, and a reddish facial complexion, this indicates the presence of accumulated heat within the fu organs.
Those with lingering residual heat after Shanghan fail to resolve because yin and yang are not yet harmonized, and pathogenic factors have not been completely eliminated. The pathogens contend and remain in the meridians, accumulating and generating heat. Symptoms include tidal fever resembling malaria, epistaxis (nosebleed), irritability, red face, yellow eyes, abdominal fullness, dysuria, dry and rough stools, or even delirium (admonitory speech), thirst for water, and a deep (Chen) and rapid (Shu) pulse. In such cases, one should seek the diagnosis based on interior syndromes (Li Zheng). If the pulse is merely floating (Fu), one should consider and treat the exterior syndrome.
【English Translation】
The reason why residual heat lingers and fails to resolve after Shanghan is that yin and yang are not yet harmonized, and pathogenic factors have not been entirely cleared. The pathogens contend and remain lodged in the meridians, accumulating and generating heat. Clinical manifestations include tidal fevers resembling malaria, epistaxis, irritability, facial flushing, yellowing of the eyes, abdominal fullness, impaired urination, dry and rough stools, or even delirium, thirst for water, and a deep (Chen) and rapid (Shu) pulse. In these instances, differentiation should be conducted based on interior syndromes (Li Zheng). If the pulse is solely floating (Fu), the treatment should focus on the exterior syndrome after careful consideration.
It is also stated that if heat persists after purging, it is because the patient's pulse was initially faint (Wei) and choppy (Se), and the physician caused harm by inducing excessive sweating, leading to a deficiency of yang qi, and by aggressive purging, leading to a deficiency of yin qi. The patient loses blood. The condition involves aversion to cold, followed by continuous fever without cessation. This is because deficient yang causes aversion to cold, and deficient yin causes fever. For aversion to cold due to deficient yang, Si Ni Tang (Frigid Extremities Decoction) is indicated. For fever due to deficient yin, which constitutes internal heat, Ting Li Ku Jiu Tang (Descurainia Seed and Vinegar Decoction) is indicated.
Generally, if high fever occurs more than eight days into Shanghan, it is difficult to treat. Zhang Zhongjing said: "If the pulses at the cun, guan, and chi positions are both deficient, and the fever does not cease, death will occur." Furthermore, many physicians use pill preparations for purging, resulting in persistent body heat and slight irritability. In such cases, Zhi Zi Gan Jiang Tang (Gardenia and Dried Ginger Decoction) is indicated. After five or six days of Shanghan, following aggressive purging, if the heat does not subside and there is painful obstruction in the heart region, the disease has not yet resolved. Zhi Zi Tang (Gardenia Decoction) is indicated.
