The disease of Huhuo (Fox-Hu Syndrome), in its initial stage, presents symptoms similar to Shanghan (Cold Damage). It may arise from a transformation following Shanghan. However, it is essentially a parasitic syndrome. When parasites erode the throat, it is called "Huo" (Hu), causing hoarseness. When parasites erode the lower part (genitals or anus), it is called "Hu" (Fox), causing dryness of the throat. The clinical manifestations include a silent demeanor with a desire to sleep yet an inability to rest peacefully, restlessness in daily activities, aversion to food and drink, and facial complexion that alternates between red, white, and black. This occurs because internal heat from Shanghan reduces food intake, leaving the intestines and stomach empty, allowing parasites to move restlessly up and down with the ingested food, hence the name Huhuo.
Patients exhibit a silent demeanor with a desire to sleep, yet their eyes cannot close; they are restless in daily activities, have hoarse voices, or suffer from dry throats. This is termed Huhuo Shanghan. Both Huhuo and Shibai (Damp-Bi) are parasitic syndromes. The initial symptoms resemble Shanghan, or it may transform into this disease after Shanghan. Manifestations include a silent demeanor with a desire to sleep, inability to close the eyes, and restlessness. Parasites eroding the throat cause Hoarseness (Huo); parasites eroding the lower part cause Dry Throat (Hu). Patients with Huhuo generally dislike food and drink, and the alternating facial complexion of red, white, and black are characteristic signs. Generally, patients with Shanghan have internal abdominal heat, eat little, and have empty intestines and stomachs. The "Three Worms" (a general term for intestinal parasites) move about seeking food, eroding the five zang organs and the lower parts, thereby causing disease. Signs include teeth lacking luster, a tongue coated entirely with white fur, severe cases showing black lips with sores, heavy limbs, mental confusion with a tendency to sleep. If parasites erode the anus to the point where the five zang organs are visible, death ensues. One should frequently examine the upper and lower lips: sores on the upper lip indicate erosion of the viscera; sores on the lower lip indicate erosion of the anus. This disease kills rapidly, often resulting from diarrhea.
Treatment:
If Shanghan is not treated with sweating methods, it may transform into Huhuo. The qi dynamics resemble Shanghan, with a silent demeanor, desire to sleep, inability to close eyes, and restlessness. Toxicity located in the throat constitutes the Huo disease; toxicity in the yin/anus constitutes the Hu disease. Patients with Huhuo dislike food and drink, do not wish to smell food odors, and have faces that alternate between red, white, and black. Toxicity attacking the upper part causes hoarseness (also written as sha). Toxicity attacking the lower part causes dry throat. This is caused by warm toxic qi. For toxicity in the upper part, Xixin Tang (Drain the Heart Decoction) is the primary treatment. For toxicity in the lower part, Kushen Tang (Sophora Decoction) is used for soaking/washing. For toxicity outside the anus, Xionghuang (Realgar) fumigation is used. Specifically, place three pieces of Realgar in a small clay bottle, burn with charcoal ash fire, and fumigate the anus towards the smoke. Simultaneously administer decoctions.
A verse states:
"Huhuo Shanghan brings hoarse and raspy voice,
Sores on lips and mouth are truly fearful.
Upper lip sores erode the viscera, lower lip sores erode the anus,
Physicians' treatments should be priceless."
Another discussion states: Sores on the upper lip indicate worms eroding the viscera, or throat, called "Huo". Sores on the lower lip indicate worms eroding the anus, or yin, called "Hu". They are used interchangeably.
For multiple sleeping conditions, first observe the pulse. If it is Fengwen (Wind-Warmth), the condition is silently heavy. When the Taiyang exterior syndrome is resolving, consider Shaoyin disease and Huhuo. Floating pulse with spontaneous sweating indicates Wind-Warmth; use Weiyu Tang (Jade Yucca Decoction). Fine pulse indicates Taiyang exterior has resolved; use Xiaochaihu Tang (Minor Bupleurum Decoction). Floating pulse without sweating indicates Taiyang excess; use Mahuang Tang (Ephedra Decoction). Deep and fine pulse indicates Shaoyin disease; use Sini Tang (Frigid Extremities Decoction).
A verse states:
"Jueyu (Reversal Cold) uses Wumei Wan (Mume Pill) and Lizhong Tang,
Visceral cold, aversion to cold, vomiting long worms.
Dry throat and hoarse voice named Huhuo,
Shibi lip sores have limited duration."
Another discussion states: Jueyu syndrome belongs to the Jueyin channel. If the patient's family has cold, and sweating is wrongly induced, or if fever persists after sweating and sweating is repeated, it leads to deficiency-cold in the stomach. Thus, long worms rebel upwards. The patient feels hungry but does not want to eat; eating causes vomiting of worms. The patient alternates between quiet and irritable because the worms sometimes move up and stop. Worms emerge upon smelling food, hence vomiting worms after eating. Zhang Shi said: Jueyin disease involves thirst, qi rushing up to the heart, hunger without desire to eat, and vomiting roundworms after eating. Since the roundworms have been vomited out of the stomach, why is there still a thirst symptom in the abdomen? This is because heat is in the Upper Jiao, causing deficiency-cold and lack of heat in the Lower Jiao. If stool is hard and knotted, this is also due to accumulated toxicity, not merely dry feces. Use raw material Lizhong Tang plus Dahuang (Rhubarb Root), adding honey to promote bowel movement. Baizhu (Atractylodes Rhizome) and Ganjiang (Dried Ginger) assist Dahuang. Treatment method: First take Lizhong Wan (Regulate the Middle Pill), then use Wumei Wan. If mistakenly purged and diarrhea continues, use Sini Tang. Another method: Lizhong Wan plus Fuling (Poria), Zhike (Bitter Orange Peel), Wumei (Mume Fruit). If thirsty, add Gualougen (Trichosanthes Root). Huhuo and Shibi are both parasitic syndromes. Symptoms resemble Shanghan, often resulting from deterioration after Shanghan-induced diarrhea. Because there is heat in the abdomen, little food is eaten, intestines and stomach are empty, so the Three Worms seek food and eat the five zang organs. Signs include heavy limbs, aversion to food, silent desire to sleep, inability to close eyes, white tongue coating, dull teeth, facial complexion changing irregularly among red, white, and black colors. Parasites eroding the lower part constitute Hu, with sores on the lower lip and dry throat; parasites eroding the viscera constitute Huo, with sores on the upper lip and hoarse voice. Regarding Shibi regulations, who regulates it effectively? This is why Yue Ren (Bian Que) was startled upon observation. Treatments include Taoren Tang (Peach Kernel Decoction), Huanglian Xijiao Tang (Coptis and Rhinoceros Horn Decoction), Xionghuang Ruisan (Realgar Sharp Powder), for backup use. Jin Ye Dan (Golden Liquid Pill) is also a method for treating Shibi. Essentially, Sulfur can kill various worms in the abdomen; if no decoction is available, give it directly.
