The so-called Cold-Heat Dysentery (Leng Re Li) arises from the weakness of the intestines and stomach, where cold pathogens have been retained internally and are subsequently injured by external heat pathogens. Cold and heat mutually contend, resulting in dysenteric discharge that alternates between yellow and white. If heat contends with blood, and blood seeps into the intestinal tract, it transforms into Bloody Dysentery (Xue Li). Conversely, if cold lurks within the intestines and externally affects the Qi, causing Qi stagnation and impeding the movement of body fluids, it transforms into White Dysentery (Bai Li). Regarding treatments involving decoctions, moxibustion/heat application, acupuncture, and stone therapy, there are separate specific formulas. Methods for tonification, nourishment, and guiding/exercising are attached below. The Guiding and Pulling methods in health preservation texts state: For diarrhea accompanied by cold, one should gently guide the Qi to calm the interior, exhale slowly, and inhale clear Qi through the nose; when the Qi is sufficient and returns to normal, the illness will heal. For those with heat, guiding the Qi can also eliminate it.

*Aconiti Lateralis Radix Preparata* (processed seven times, soaked in water, peel removed, made into powder), *Coptidis Rhizoma* (remove roots, each 1 liang, made into powder), *Boswelliae Resina* (ground, 1 fen). For Cold-Heat Dysentery, take 0.5 qian each of *Coptidis Rhizoma* and *Aconiti Lateralis Radix Preparata*, plus 1 fen *Boswelliae Resina*, mixed with aged rice water to stop dysentery permanently. For subsequent doses, use green tangerine peel water. For red dysentery, take 0.5 qian *Aconiti Lateralis Radix Preparata*, 1 qian *Coptidis Rhizoma*, 1 qian *Boswelliae Resina*. For white dysentery, take 0.5 qian *Coptidis Rhizoma*, 1 qian *Aconiti Lateralis Radix Preparata*, 1 fen *Boswelliae Resina*, mixed with rice water. If not stopped, add 7 black beans, decoct in water to stop it.
Treat imbalance of cold and heat, watery, purulent, or five-color bloody discharge. (From *Qianjin Fang*)
Use 5 sour pomegranates, crush including the shell, squeeze out 2 sheng of juice, and take 0.5 sheng for effect. Another formula: for blood before stool causing sallow complexion, use the shell powdered, taken with eggplant branch water. Adjust for children.
Treat Cold-Heat Dysentery.
Crush red beetroot, squeeze juice, and take.
Jianchang Wan (Intestine-Firming Pills) [From *Shibian Liangfang*]: Treats all types of dysentery, regardless of red, white, pus, or blood.
Currently, patients with dysentery present with minute pulses, deep and choppy upon repeated palpation; this is one sign. If discharging white dysentery, even if severe, it may be treatable. If discharging blood, fever throughout the body is strictly contraindicated; fever indicates death, and fever lasting seven days leads to death. The above explains only the superficial aspects, not the root cause. Scholars argue that in every 60-year cycle (from Jiazi to Guihai), there is no year without dysentery. Yet recently, dysentery has ceased for a year among several years. Why? Because in 60 years, there is always Wood-Earth conflict, Earth-Fire stagnation, and Water-Fire clash. However, the Five Movements act like a scale; if the five elements balance each other annually, disease does not arise. If one Qi becomes excessive and one Zang organ surplus, dysentery arises instantly. I detail this to enlighten students and avoid diagnostic errors. Among toxic dysenteries, Water Evil invading the Heart is most severe. Initially, patients have chills and fever. If fever persists after medication, becomes severe, causes vomiting upon eating, continuous dysentery, heart heat like fire, desire for cool places and cold water, manic running, muscle pain, and touch intolerance, only 3-4 out of 10 cases are treatable.
Formula for Epidemic Toxic Dysentery: Read carefully to understand the pathology before medicating. Initial symptoms include chills, fever, headache, high fever, frequent bowel movements with red dysentery, mixed red-white, pure white, or transformation from white to red or red to white. All are suitable for this formula. Initial chills, fever, and headache indicate Cold Evil invading the Heart, with Water-Fire battle. Blood changes internally, leading to red dysentery resembling purple grass water or amaranth water. Lack of luster indicates severe Cold Evil invasion. Pure white discharge indicates Cold Evil has not yet moved. Transformation from white to red indicates worsening Cold Evil. Transformation from red to white indicates diminishing Cold Evil. Equal red and white indicates balanced Water-Fire and transmission of cold/warm Qi. Variations in red/white ratio reflect the severity of Cold Evil and toxicity. White predominance is mild; red predominance is severe. Treatment first addresses Cold. Based on Tai Sui (Year Pillar), Bing Zi, Bing Wu, Jia Zi, Jia Wu, Geng Wu, Geng Zi, Bing Yin, Bing Shen, Jia Yin, Jia Shen, Geng Yin, Geng Shen, Chen Xu years, and Geng Yun periods are particularly harmful. Chou Wei years also show this pattern. Regardless of Tai Sui, heaven and earth changes are complex. In spring, if weather is imbalanced with cold transformations and little sunlight, cold and heat contend. In summer, if cold-warm transformations occur, Cold Evil invades the Heart. Initial chills, fever, headache, frequent stools, red/mixed/white dysentery are treated with Tongshen San. Fever should subside after 2-3 doses. If not, use other formulas below.
*Ephedrae Herba*, *Cinnamomi Ramulus* (each 3 fen), *Ligustici Wallichii Rhizoma et Radix* (Da Xiong), *Atractylodis Macrocephalae Rhizoma* (each 2 liang), *Ligustici Chuanxiong Rhizoma* (Ben), *Angelicae Dahuricae Radix*, *Platycodonis Radix*, *Saposhnikoviae Radix*, *Paeoniae Radix Alba*, *Angelicae Dahuricae Radix* (each 0.5 liang), *Moutan Cortex*, *Glycyrrhizae Radix et Rhizoma* (each 1 fen), *Asari Radix et Rhizoma* (8 fen), *Semen Kochiae* (4 qian). Make fine powder. Take 2 qian with hot water, drink warm with dregs, followed by 2-3 cups of wine. If chills/fever persist, stop and use other formulas. If chills/fever resolve and red dysentery reduces, proceed to Formula 2. If chills/fever resolve but red dysentery persists, take 2-3 doses (max 2/day) until red dysentery reduces. If it turns to white dysentery, proceed to Formula 2, then Formula 3 as white dysentery decreases. In Liu Bing/Geng years, spring/summer cold favors this formula. In Liu Jia/Chou Wei years, warmth prevails; add *Alpiniae Katsumadai Semen* (1 liang) if chills/fever occur initially. Regardless of Tai Sui, if spring/summer is cold and chills/fever occur, use this formula.
Treat Toxic Dysentery: After initial chills/fever resolve with previous formula and red dysentery reduces, use Jin Tui Zhen San. If chills/fever/red dysentery persist, do not use this yet. Heaven and earth changes are variable; dysentery patterns are diverse; caution is required.
*Terminalia chebula Retzii Fructus* (5 jin, flour-roasted until hot, not raw/burnt, remove flour, roast peel until dry, remove pit, use only peel). Pound and sieve finely. Take 2.5 qian with 1.5 sheng rice water, reduce to 1 sheng, take warm with dregs on empty stomach. If phlegm/saliva is vomited, continue for several doses until bowels stabilize and discharge reduces, then proceed to Formula 3 to firm the large intestine. If previous formula failed to reduce fever/blood dysentery, use this.
Treat Toxic Dysentery: After previous two formulas reduce severity and discharge is minimal, if sudden clear stool like duck feces or tea soup occurs, use Bo Shang Huang (desanded, 2 liang, ground fine). Mix with 2 liang roasted *Coicis Semen* powder. Add cooked water to form paulownia seed-sized pills. Take 50 pills with rice water on empty stomach.
Treat Toxic Dysentery: Initial chills/fever treated with Tongshen San (3 doses). If after one day/night chills/fever persist, dysentery unchanged, whole body fever, discharge like pus/traces, peach blossom, or red rice glue, taking 2-3 doses of this medicine for one day/night with persistent symptoms means stop. If fever subsides and dysentery reduces after 2 doses, continue gradually (max 2/day). When red dysentery reduces and turns white, switch back to Zhen Huan San. Dysentery has cold/heat signs but same pathology. Previous formula uses warm herbs; this uses cool herbs. Explanation: Water Evil invading Heart causes fever; Earth dampness stifling Fire causes fever; Liver Heat punishing Spleen causes fever. Only experts discern subtle changes. These two formulas correct errors. Water Evil invading Heart is most severe. Patients with increased chills/high fever should take Tongshen San first. If chills/fever persist and dysentery continues, then take this formula. Based on Tai Sui, Ren, Xu, Yin-Shen, Si-Hai years suit this. Xu years have variations due to Fire excess and Cold-Damp clash. Six Qi shifts are unpredictable; do not fixate on years. If spring/summer heat dominates and dampness fails, with wiry Liver pulse and flooding Heart pulse, take this. If 2 doses fail, stop. Remember Water Evil invading Heart at onset; do not recklessly use cool herbs.
*Schizonepetae Spica*, *Moutan Cortex* (remove heart), *Semen Kochiae* (fried), *Aucklandiae Radix*, *Pedicularis Resupinatae Herba*, *Paeoniae Radix*, *Platycodonis Radix*, *Saposhnikoviae Radix*, *Angelicae Dahuricae Radix*, *Sanguisorbae Radix* (each 1 fen), *Coptidis Rhizoma* (honey-soaked). Make fine powder. Take 2 qian, decoct in one sheng of water for one boil, strain immediately, take warm after meals.
Treat Toxic Dysentery: Initial fever with white mucus discharge. Use Tongshen San. If dysentery lasts 1-2 days, chills/fever resolve spontaneously, leaving only white dysentery without effect, use this formula.
*Citri Reticulatae Pericarpium*, *Saposhnikoviae Radix* (each 1 fen), *Glycyrrhizae Radix et Rhizoma* (4 zhu), *Asari Radix et Rhizoma* (3 zhu), *Atractylodis Macrocephalae Rhizoma* (2 fen), *Alpiniae Katsumadai Semen* (1 piece), *Ligustici Wallichii Rhizoma et Radix* (half...

Treat Toxic Dysentery: Initial slight chills/fever, sudden abdominal pain. Post-defecation, small amount of mucus mixed with red dysentery. Mistakenly taking cool herbs leads to massive dysentery from morning to night, sleepiness, sallow complexion, cool body early, flushed face late, massive clear mucus and red dysentery, whole body fever, un...
