Patients suffering from carbuncles and sores (Yongju, 痈疽) have obstructed meridians and collaterals, with qi and blood condensing and accumulating. Ying (Nutritive Qi) and Wei (Defensive Qi), along with body fluids, become deficient and dry due to this. Pathogenic heat surges internally and flows into the Large Intestine and Small Intestine. Since the flow of qi is not unobstructed, both urination and defecation are blocked. The treatment method should focus on clearing the obstruction and guiding out pathogenic toxins, preventing the disease from penetrating deeply into the Zang-Fu organs. This is the essential method of treatment. Regarding cases where a back carbuncle (Fabei, 发背) leads to urinary and fecal retention, it is caused by cold pathogens lodging in the meridians. Blood stasis causes friction and generates heat, which accumulates to form a carbuncle. Qi becomes stagnant in the Fu organs, and heat enters the stomach and intestines, thus causing urinary and fecal retention.
Formulas
Xuan Shen Wan (Scrophularia Pill): Treats back carbuncles and various swellings, urinary and fecal retention, chest and abdominal fullness, oppression, and irritability.
