Deficiency consumption and emaciation result from damage to the qi of the five zang organs. As stated in the classics: The first degree of damage affects the skin and hair, manifesting as dry skin and hair loss; the second degree damages the blood vessels, resulting in deficient and scanty blood vessels that cannot nourish the five zang and six fu organs; the third degree damages the muscles, causing muscle wasting where food intake fails to transform into flesh; the fourth degree damages the sinews, leading to flaccid sinews and inability to control limb movement; the fifth degree damages the bones, causing bone weakness and inability to rise from bed. How then should deficiency be treated? For lung deficiency, tonify its qi; for heart deficiency, harmonize Ying (Nutritive) and Wei (Defensive) Qi; for spleen deficiency, regulate diet and adapt to cold and warmth; for liver deficiency, soothe the liver's constraint; for kidney deficiency, replenish kidney essence. These are the principles for treating deficiency.
Deficiency consumption and emaciation are signs of deficiency and damage in the five zang and six fu organs. If people indulge in sexual desires, crave sweet and greasy foods, lack restraint, and act on their impulses, while also being exposed to external wind-cold pathogens and suffering internal injury to qi and blood, it leads to simultaneous exterior and interior disease, with yin and yang clashing. If left untreated for a long time, it eventually forms chronic consumptive disease (Lei Zhai). This arises from the imbalance of cold and hot qi and blood, belonging to the manifestations of Five Labors and Six Extremes. Consequently, spleen and kidney labor and injury occur, causing muscles and bones to wither, thus forming the disease of consumption and emaciation.
The elderly and children suffer from weakness due to insufficient qi and blood. Cold qi accumulates alone in the lower part (earth), while heat qi dissipates alone in the upper part (heaven). Heaven and Earth do not interact, so the body becomes obstructed. When the body is obstructed, muscles lose moisture, and interstices become blocked, leading to chest oppression, rapid breathing, and abdominal distension. This is Yin qi attacking Yang qi, preventing Yang from connecting. Alternatively, there may be hard accumulation in the Bladder and heaviness in the limbs, because Yang qi cannot descend to nourish, leaving Yin qi excessively strong. This causes stagnation in the Large Intestine, making it impossible to take decoctions, or failure to digest food and water, exacerbating shortness of breath. If Guan-Ge (obstruction of upper and lower pathways) is not resolved, death is inevitable.
