Wei San (Stomach-Scattering Powder), composed of [missing character], Baizhu (Atractylodis Rhizoma), Baizhi (Angelicae Dahuricae Radix), Renshen (Ginseng Radix et Rhizoma), and Shanyao (Dioscoreae Rhizoma). (Note: The original text has a missing character here. Based on context and common prescriptions, it is likely Renshen or similar Qi-tonifying herbs, but strictly following the instruction not to add content, it is translated as "[missing character]". In TCM textual criticism, this is often a lacuna. For fluency, I will translate literally noting the absence.) These formulas are generally difficult to apply directly, but their theoretical intent is worthy of reference.
When one feels such symptoms appearing, treatment should be initiated promptly; delay may render it too late. The method of treatment involves two aspects: first, killing parasites to eliminate the root cause; second, tonifying deficiency to restore the True Yuan (Original Qi). Medications should be administered according to the meridians, with clear organization, much like Cook Ding carving an ox, striking precisely at the vital points, ensuring no case remains unhealed. However, if one waits until the disease is severe and the Yuan Qi is depleted, even if ancient methods of using parasite-killing and tonifying drugs are employed, fewer than one in a hundred patients will survive. Such cases may only serve to prevent the transmission of notes on this disease to future generations; students of medicine should examine this carefully. (Zhengchuan)
At the initial stage of this syndrome, one should only aim to soothe the emotions and avoid depression. Use herbs that disperse stagnant heat to prevent internal steaming, such as Xiaoyao San (Free and Easy Wanderer Powder). Externally, moxibustion can be applied to the Gohuang (Bladder Meridian; GV-43 area/Gaohuang points) and Baihui (GV-20) acupoints to prevent the generation of parasites. Additionally, pounding the heads of peach and willow branches and rubbing them on the spine and between the limbs, wrists, and feet can help kill erosive pests. Concurrently, take the subsequent formula (Note: The subsequent formula is difficult to use and thus not recorded), and follow proper regimens for conditioning and tonification to seek a slim chance of recovery. It is better to nurture the spirit, maintain tranquility and simplicity, and rest quietly, which is the most urgent approach to seeking life. (Baiwen)
Is Renshen (Ginseng Radix et Rhizoma) suitable? Renshen can tonify Original Yang, generate Yin Blood, and drain Yin Fire; this is supported by Li Dongyuan. It can be used for those with white, yellow, or blue complexions who are emaciated and haggard, indicating insufficiency of Spleen, Lung, and Kidney Qi. It should not be used for those with red-black complexions who have strong Qi and robust Spirit. Pulse patterns that are floating and hollow (Kou), soft (Ru), deficient, large, slow, and weak, or deep, slow, choppy, weak, thin, knotted, intermittent, and weak, all indicate deficiency and insufficiency, making Renshen suitable. Conversely, pulses that are wiry (Xian), long, tight, solid, slippery, rapid, and forceful indicate internal excess of Fire stagnation, making Renshen unsuitable. Jiegu stated that Renshen should not be used for wheezing and coughing caused by phlegm excess and Qi congestion. However, for wheezing and shortness of breath due to Kidney deficiency and Qi shortness, it must be used. Jiezhai stated that Renshen should not be used for Yin deficiency with hyperactive Fire, characterized by blood deficiency, fire hyperactivity, good appetite, wiry and rapid pulse, where cooling herbs damage the Stomach and warming herbs damage the Lung, rendering the patient unable to tolerate tonification. However, for those with spontaneous shortness of breath, cold limbs, and deficient pulse, it must be used. (Dianxue) (Excerpt)
Xuanshu states: Only when there is no accumulation and the pulse is powerless upon both pressing and lifting can tonification be applied. (Bingji Shazhuan)
Harmonizing Formulas (Including Tonifying and Clearing Formulas)
Jiegu Wu Zheng Tang (Decoction for Resolving Five Steams) from Gujin Lu Yan:
Gancao (Licorice Root, 1 liang, honey-fried)
Fuling (Poria, 3 liang)
Renshen (Ginseng Root, 2 liang)
Zhuye (Bamboo Leaf, 2 bunches)
Gegen (Kudzu Root)
Sheng Dihuang (Rehmannia Root, dried, 3 liang each with Gegen)
Zhimu (Anemarrhenae Rhizoma, 2 liang)
Huangqin (Scutellariae Radix, 2 liang)
Shigao (Gypsum Fibrosum, 5 liang, crushed)
Jingmi (Oryza Sativa, 1 he)
Cut the above ten ingredients. Boil in 9 sheng of water, reduce to 2.5 sheng, and divide into three doses. Alternatively, boil 1 sheng of wheat in 3 sheng of water first, then add the herbs. Avoid consuming Haizao (Sargassum), Songcai (Chinese Cabbage), Wuyi (Chrysanthemi Fructus/Artemisia?), and strong vinegar. (Same as Fan Wang's prescription. Another version lacks Gancao, Fuling, Renshen, Zhuye, containing only six ingredients.) (Waitai)
Qianjin Zhuye Tang (Bamboo Leaf Decoction from Qianjin Fang): Treats heat in the five centers, vexing pain in hands and feet, dry mouth and lips, heat in the chest. Modify the base formula by removing Gegen, Dihuang, and Jingmi, and adding Maimendong (Ophiopogonis Radix), Shengjiang (Fresh Ginger), Gualougen (Trichosanthis Radix), and Banxia (Pinelliae Rhizoma).
Cui Shi Zhu Gu Zheng, Chungan Kouzao, Desire to Drink Water to Quench Thirst, Zhuye Yin (Bamboo Leaf Drink):
Zhuye (Bamboo Leaf, one handful)
Maimendong (Ophiopogonis Radix, 1 sheng, heart removed)
Dazao (Jujube, 20 pieces, split)
Gancao (Licorice Root, 3 liang, honey-fried)
Banxia (Pinelliae Rhizoma, 1 sheng, washed in hot water until smooth)
Jingmi (Oryza Sativa, 5 he)
Shengjiang (Fresh Ginger, 3 liang)
Cut the above seven ingredients. Boil in 5 sheng of water, reduce to 2.5 sheng, and divide into three warm doses. Avoid mutton, malt sugar (Ying), Haizao, and Songcai. (Ibid.)
Treats pathogenic heat lodging in meridians, muscle heat, phlegm-cough, heat in the five centers with vexation, dizziness and headache, night sweats. This medicine harmonizes and tonifies True Qi, relieves fatigue, treats women's blood heat, consumptive bone-steaming fever, etc. Ren Shen San (Ginseng Scattering Powder) is recommended.
Renshen (Ginseng Root, rhizome removed)
Baizhu (Atractylodis Rhizoma)
Baifuling (Poria, peel removed)
Chaihu (Bupleuri Radix, sprouts removed, washed)
Banxiaqu (Pinelliae Fermented Paste)
Danggui (Angelicae Sinensis Radix, washed, rhizome removed, thinly sliced, baked dry, weighed)
Chishao (Red Peony Root)
Gangen (Kudzu Root)
Gancao (Licorice Root, 1 liang each, honey-fried)
Ziqin (Scutellariae Radix, 0.5 liang, peel removed)
Grind into fine powder. Take 3 qian per dose, with 1 cup of water, 4 slices of fresh ginger, and 2 jujubes. Decoct to 80% volume. Take warm, regardless of meal times. Suitable for any labor-heat syndrome; stop once heat subsides. Generally, for releasing muscle layer heat, Gegen is primary, followed by Chaihu. Thus, Shengma Gegen Tang is the premier formula for releasing the muscle layer. (Benshi) (Note: Chaihu San in Chuanjia Mibao for labor-heat contains Chaihu, Ziqin, Gancao, Gangen, Huanglian, Niuhuang ground into powder, taken with fresh well-water. This formula may adopt its principle.)
For chills and fever progressing towards consumption (Lao Zhai), Pingjiang Physician Monk Gao's Perfect Ten Formula (Shiquan Yinzi), add Huanglian. Decoct and take according to method. If heat is in bone joints, add Qinghao (Artemisiae Annuae Herba) or Biejia (Testudinis Carapax et Plastrum) decoction. (Shizhai) (Note: Shiquan Yinzi is equivalent to Shiquan Dabu Tang.)
A patient with dual deficiency of Qi and Blood, alternating chills and bone-steaming fever, normal stool, fine and rapid pulse, poor appetite. Babawu Tang (Eight Substance Decoction - *Correction: Babawu usually refers to Siwu plus Sijunzi or similar variations, here likely referring to a specific eight-herb combo*), add Chaihu, Zhimu, Huangbai. (Danxi)
When I was twenty, due to prolonged cold-induced cough and violating sexual abstinence, I developed bone-steaming fever. My skin felt like burning embers. I expectorated about a bowl of phlegm daily. In summer, I suffered from vexation and thirst, nearly abandoning sleep and food. My six pulses were floating and surging. I took various medicines including Chaihu, Maimendong, and Jingli (Bamboo Sap) for over a month, worsening significantly. Everyone believed I would surely die. My father recalled Li Dongyuan's treatment for lung heat like burning embers, vexation, excessive drinking, worse during the day, identifying it as Heat in the Qi Level. He recommended Yiwai Huangqin Tang (Single Herb Scutellaria Decoction) to drain Fire from the Lung Qi level. I followed the prescription, using 1 liang of Pianxin (Scutellaria), boiled in 2 zhong of water to 1 zhong, taken in one dose. The next day, body heat completely subsided, and phlegm-cough healed. The medicine hit the mark precisely, like a drum responding to a stick. The wonder of medicine is indeed so! (Bencao Gangmu)
Clearing Bone Formulas (Including Lung-Benefiting Formulas Attached)
Treats bone-steaming consumptive Qi, vexing heat, weakness in four limbs, nocturnal empty sweating, dry lips and mouth, pale complexion, gradual emaciation. Suitable for men and women. Sansheng San (Three Saints Powder):
Huhuanglian (Coptidis Rhizoma substitute / Picrorhizae Rhizoma)
Chaihu (Bupleuri Radix)
Biejia (Testudinis Carapax et Plastrum, 2 liang each)
Pound the above herbs, sieve finely into powder. Take 1 qian mixed with ginger wine per dose. Take once in the morning, noon, and before bed. (Shenghui)
Treats bone-steaming in young men and women, women's wind-blood attacking limbs, chest and heart vexation and fullness. Maijian San (Wheat Decoction Powder):
Biejia (Testudinis Carapax et Plastrum, vinegar-fried)
Dahuang (Rhei Radix et Rhizoma, wrapped in wet paper, baked ripe)
Changshan (Polygalae Radix)
Chaihu (Bupleuri Radix)
Chifuling (Poria with red wood, 1 liang each)
Danggui (Angelicae Sinensis Radix, soaked in wine overnight)
Gan Shengqi (Dry Raw Lacquer)
Baizhu (Atractylodis Rhizoma)
Shigao (Gypsum Fibrosum)
Sheng Dihuang (Rehmanniae Radix, 1 liang each)
Gancao (Licorice Root, honey-fried, 0.5 liang)
Grind into powder. Take 2 qian per dose, with 50 grains of wheat and 1 cup of water. Decoct to 60%. Take warm after meals and before bed. If empty sweating exists, add Mahuanggen (Ephedrae Radix et Rhizoma without epidermis, 1 liang). This is the prescription of Judge Wu of Huangzhou. (Sushen) (Note: Yifang Kao states: This is an attacking formula for depressive consumption. Young men desiring women without success retain essence; young women desiring men without success retain blood; widows with lingering thoughts have Qi stagnation and retained stasis. The principle is the same. "Retention" means essence and blood have left their proper places but remain in key areas of meridians, blocking Qi and Blood flow. Qi is Yang; blocked and obstructed, accumulated Yang generates heat, causing steaming bone heat. Although, this is an attacking agent. It can only be used for young men, women, and widows with intact and solid True Qi. Those weakened by sexual intercourse are still prohibited from using it.)
Chaihu San from Chuanjia Mibao for children's bone-steaming heat-labor, also resolving muscle heat post-typhoid: Remove Fuling, Danggui, Baizhu, Shigao, Dihuang from the base formula. Add Qinjiao (Gentianae Macrophyllae Radix) (Biejia 3 liang, others 1 liang each). Add a pinch of wheat. Decoct in water.
Biejia San from Weisheng Jiabao: Add Shaoyao (Paeoniae Radix) to the base formula.
Jielao San (Relieving Labor Powder): Treats consumptive labor, hard accumulated Qi, choking sensation in chest and hypochondrium, radiating back pain.
Baishaoyao (White Peony Root, 1.5 liang)
Chaihu (Bupleuri Radix, sprouts removed)
Biejia (Testudinis Carapax et Plastrum, vinegar-soaked, fried yellow)
Zhike (Aurantii Fructus, pith removed, bran-fried, 1 liang each)
Gancao (Licorice Root, honey-fried, 0.5 liang)
Chifuling (Poria with red wood, peel removed, 0.5 liang)
Cinch the herbs. Take 5 qian per dose, with 1.5 cups of water, 3 slices of ginger, and 1 jujube. Decoct to 70%. Take warm after meals. (Yangshi)
Xijiao Wan (Rhinoceros Horn Pill): Treats consumptive labor bone heat, vexing pain in four limbs, red-yellow urine, gritty eyes, insomnia.
Xijiao (Rhinocerotis Cornu, shaved)
Huhuanglian (Picrorhizae Rhizoma)
Chishao (Red Peony Root)
