[Prognosis of Stroke]
If the pulse at the Cun Kou (Inch Port) appears level but sudden stroke occurs, it is a critical death prognosis. This is due to the solitary extinction of vital energy and sudden collapse. The Qi of the Five Zang Organs dissipates within a few breaths, similar to falling from a height or drowning; how can one predict the time of death? If the pulse beats seven or eight times per respiration, provided the pulse is neither too large nor too small, it may still be treatable. If the pulse is excessively large or minute, with a floating pulse indicating death during the day and a deep pulse indicating death at night, or if the pulse ceases entirely, there is no doubt about death. The simultaneous appearance of Collapse syndromes are all death prognoses, manifesting as shaking of the head, upward rushing of Qi, long sighing, panting with sweat like oil, rattling phlegm resembling sawing wood, emaciated flesh, painful sinews, and dry, straight hair.
[Annotation] "Level pulse at Cun Kou" refers to individuals whose Cun, Guan, and Chi pulses are all level. Sudden death from stroke in such cases is caused by pathogenic factors being too severe, blocking the Nine Orifices, preventing the True Heaven Qi from communicating with the body's vital energy, leading to its solitary extinction internally. It is like falling or drowning; how can one anticipate death? A pulse beating seven to eight times per respiration, though not excessively large or small, indicates a difficult but treatable condition. If the pulse is chaotic and large, or minute like a thread, a floating pulse indicates death by day, and a deep pulse indicates death by night; these are untreatable. When three or four Zang organs show signs of collapse, along with symptoms like head shaking and upward Qi rush, these are death prognoses.
Tongguan San (Pass-Through Powder), Kaiguan San (Open-Switch Powder), Nasal Fumigation Method, Speech-Resolving Method:
Tongguan San contains Tian Nan Xing (Arisaema Rhizome), Zao Jiao (Gleditsia Sinensis Fruit), Xi Xin (Asarum Root), Bo He (Mentha Haplocalyx), and Sheng Ban Xia (Raw Pinellia Rhizome) ground into powder. Blowing into the nose to induce sneezing indicates treatability. Kaiguan San contains Wu Mei (Mume Fruit Flesh), Bing Pian (Borneol), and Sheng Nan Xing (Raw Arisaema Rhizome) ground into powder; rubbing on the teeth can open trismus. Burning Ba Dou (Croton Seed) oil-paper wrapped with Zao Jiao powder to fumigate the nose can restore consciousness. Applying Turtle Urine under the tongue facilitates speech.
Sansheng San (Three Saints Powder), Goudi San (Melon Peduncle Powder), Quanxie San (Scorpion Powder), Wuyuan San (Five Origins Powder), Bafan Wan (Barium Alum Pill):
For those without sweat, expectorants like Fang Feng (Saposhnikovia Root), Luli (Veratrum Nigrum), and Gou Di (Melon Peduncle) are suitable. For those with sweat, Gou Di combined with Quan Xie (Scorpion) is used. Heavy agents include Luli, Chi Xiao Dou (Adzuki Bean), Bai Fan (Alumen), Zao Jiao, and Dan Fan (Blue Vitriol). For phlegm obstruction, Bafan Wan (Ba Dou and Calcined Alumen) induces vomiting.
[Annotation] Excessive phlegm-saliva accumulation with no sweat and exterior excess requires Sansheng San, composed of Fang Feng, Luli, and Gou Di to induce vomiting. With sweat and interior excess, Goudi San (Gou Di and Chi Xiao Dou) or Quanxie San (Goudi San plus Scorpion) is used. These are mild emetics. For severe cases, Wuyuan San (Luli, Chi Xiao Dou, Bai Fan, Zao Jiao, Dan Fan) or Bafan Wan (Ba Dou and Calcined Alumen) is used for vomiting.
Wuyao Shunqi San (Costus Regulating Qi Powder):
Wuyao Shunqi treats Excess Stroke affecting Collaterals, characterized by facial paralysis, numbness, and wind-induced joint pain. It contains Ma Huang (Ephedra Herb), Zhi Ke (Aurantium Fructus), Jie Geng (Platycodon Root), Wu Yao (Lindera Root), Jiang Can (Bombyx Batryticatus), Bai Zhi (Angelica Dahurica Root), Gan Jiang (Zingiberis Rhizoma), Chen Pi (Citrus Reticulatae Pericarpium), Gan Cao (Licorice Root), and Chuan Xiong (Ligusticum Chuanxiong Rhizome).
[Annotation] "Excess Stroke affecting Collaterals" refers to individuals with Wind-Evil invading collaterals but possessing robust constitution. Facial paralysis involves deviation of the mouth and eyes. "Numbness" refers to skin and muscle insensitivity. "Wind-injected pain" refers to bone and joint pain caused by Wind pathogen. This formula includes Ma Huang, Zhi Ke, Jie Geng, Wu Yao, Jiang Can, Bai Zhi, Chen Pi, Gan Jiang, Gan Cao, and Chuan Xiong.
Da Qin Jiao Tang (Large Ligusticum Decoction):
Da Qin Jiao Tang treats Deficient Stroke affecting Collaterals, involving facial paralysis and hemiplegia. It modifies Bazhen Tang (Eight Treasures Decoction) by removing Ren Shen (Ginseng Radix) and adding Qin Jiao (Gentiana Macrophylla Root), Sheng Di Huang (Rehmannia Root), Shi Gao (Gypsum Fibrosum), Qiang Huo (Notopterygium Rhizome), Du Huo (Angelica Pubescens Root), Bai Zhi, Fang Feng (Saposhnikovia Root), Xi Xin, and Huang Qin (Scutellaria Root).
[Annotation] "Deficient Stroke affecting Collaterals" refers to individuals with Wind-Evil invading collaterals but possessing weak constitution. Hemiplegia refers to paralysis of half the body. Modifying Bazhen Tang implies removing Ginseng and adding the listed herbs. Hemiplegia is a syndrome of Meridian involvement but is treatable because this formula nourishes blood and benefits sinews, serving as a conditioning agent for chronic wind diseases.
Huangu Dan (Bone-Replacing Pill):
Huangu Dan is indicated for Excess Stroke affecting Meridians, manifesting as facial paralysis and paralysis. It contains Bai Zhi, Chuan Xiong, Fang Feng, Bing Pian, She Xiang (Musk), Zhu Sha (Cinnabar), Mu Xiang (Aucklandiae Radix), Huai Jiao (Sophorae Tonkinensis Fruit), Ku Shen (Sophora Flavescens Root), Wu Wei Zi (Schisandra Fruit), Ling Xian (Salvia Miltiorrhiza Root), Ren Shen, Ma Huang Gao (Ephedra Paste), He Shou Wu (Polygonum Multiflorum Root), Man Jing Zi (Vitaliae Fructus), Cang Zhu (Atractylodes Rhizome), and Sang Bai Pi (Morus Root Bark). Ma Huang Gao is made by boiling Ephedra into a paste and mixing it with decocted medicine to form pills; Zhu Sha is used as a coating.
[Annotation] "Excess Stroke affecting Meridians" refers to individuals with Wind-Evil invading meridians but possessing robust constitution. Paralysis (Tan) refers to left-side immobility; flaccidity (Huan) refers to right-side immobility.
Xiao Xu Ming Tang (Minor Continuation of Life Decoction):
Xiao Xu Ming Tang treats Deficiency affecting Meridians and Collaterals, covering Eight Winds and Five Bi Syndromes. Ma Huang, Xing Ren (Apricot Seed), Gui Zhi (Cinnamon Twig), and Bai Shao (White Peony Root) regulate Ying and Wei. Ren Shen, Gan Cao, Dang Gui (Angelica Sinensis Root), and Chuan Xiong promote Qi and Blood circulation. Fang Feng addresses Wind excess, Yi Zhi Ren (Alpinia Oxyphylla Fruit - *Note: Text says 'Ji' likely referring to Yi Zhi or similar damp-draining herb contextually, but standard text often implies specific damp herbs; here strictly translating text 'Yi'*), Huang Qin (Scutellaria Root) addresses Heat excess, and Fu Zi (Aconite Lateral Root) addresses Cold excess. In Spring and Summer, add Shi Gao and Zhi Mu (Anemarrhena Rhizome); in Autumn and Winter, double Gui Zhi and Fu Zi.
[Annotation] "Deficiency affecting Meridians" refers to Wind-Evil invading meridians in individuals with deficient constitution. "Eight Winds" refers to pathogenic winds from eight directions. "Five Bi" is detailed in the Bi Syndrome Essentials chapter.
Huang Qi Wu Wu Tang (Astragalus Five Substance Decoction):
Huang Qi Wu Wu Tang treats Deficiency affecting Meridians, causing hemiplegia, weakness, and flaccid paralysis due to deficient Wind. Clear mind and slurred speech due to soft tongue indicate deficiency; stiff tongue and turbid spirit indicate Fire-Phlegm. To tonify Wei and restore movement, use Huang Qi (Astragalus Root) without modification. To benefit Ying, decoct Gui Zhi, Bai Shao, Da Zao (Jujube), and Sheng Jiang (Fresh Ginger). Add Dang Gui for left-sided issues and Niu Xi (Achyranthes Root) for lower limb weakness. Add Mu Gua (Chaenomeles Fruit) for stiff sinews, Hu Gu (Tiger Bone - *historical term*) for weak bones, and Pao Fu Zi for unblocked meridians or cold conditions.
[Annotation] Huang Qi Wu Wu Tang treats hemiplegia caused by Wind invading meridians due to deficiency. If the patient has a stiff tongue, difficulty speaking, and unclear spirit, it is Phlegm-Fire disease, and this formula is unsuitable. If the mind is clear, speech is slurred, and the tongue is soft and weak, it is Ying-Wei deficiency, and this formula is appropriate. The Classics state: Wei deficiency leads to immobility; Ying deficiency leads to insensitivity. This formula uses Huang Qi as the sovereign herb to tonify Wei and restore movement; Gui Zhi and Bai Shao as minister herbs to benefit Ying and treat insensitivity; Sheng Jiang and Da Zao as assistant herbs to harmonize Ying and Wei. Immobility and insensitivity on the right belong to Qi; increase Huang Qi. On the left, they belong to Blood; add Dang Gui. Weakness in both legs and knees requires Niu Xi. Weak bones requiring inability to stand long require Hu Gu. Stiff sinews requiring difficulty in flexion/extension require Mu Gua. Unblocked meridians anywhere on the body require Pao Fu Zi, especially if cold is present. This formula is highly effective for external tonification; thus, Ren Shen and Gan Cao are omitted to avoid internal/middle tonification interference.
San Hua Tang (Three Transformations Decoction), Soushun Fengqi Wan (Search-for-Gentleness Wind-Qi Pill):
San Hua Tang treats Excess Qi with Wind attacking the Fu Organs, manifesting as coma, constipation, and abdominal fullness. It modifies Xiao Cheng Qi Tang (Minor Order of the Qi Decoction) with Qiang Huo. For individuals with both Qi and Form deficiency, or Wind-Dryness, Soushun Fengqi Wan promotes natural recovery.
[Annotation] "Excess Qi Wind attacking Fu Organs" refers to Wind-Evil attacking Fu organs in individuals with robust constitution. Coma refers to unconsciousness. Constipation and fullness refer to blocked bowel/bladder and abdominal distension. Xiao Cheng Qi Tang consists of Hou Po (Magnolia Bark), Zhi Shi (Immature Bitter Orange), and Da Huang (Rhubarb Root); adding Qiang Huo creates San Hua Tang. For Qi/Form deficiency, use Soushun Fengqi Wan slowly. Chronic wind patients often have dry stools (Wind-Dryness) due to excessive sweating from Xu Ming Tang or purging from San Hua Tang, depleting fluids. Regardless of whether the stroke affects meridians/collaterals or Zang/Fu, if there is bowel/bladder blockage and insufficient Qi/Form resisting purgatives, this method is suitable to search out Wind from Six Fu organs and circulate Qi in intestines/stomach, naturally relieving blockages.
Niu Huang Qing Xin Wan (Calculus Bovis Heart-Clearing Pill):
Niu Huang Qing Xin Wan treats Excess Stroke affecting Zang Organs, manifesting as phlegm obstruction, coma, muteness, facial paralysis, robust constitution, clenched hands, and locked jaw.
[Annotation] Niu Huang Qing Xin Wan treats Wind-Evil attacking Zang organs in individuals with robust constitution. Symptoms include phlegm obstruction, coma, muteness, facial paralysis, full constitution, clenched hands, and emergency lockjaw (Bi Syndrome). It is suitable for these conditions.
Shen Fu Tang (Ginseng and Aconite Decoction):
Shen Fu Tang treats Deficient Stroke affecting Zang Organs, manifesting as relaxed lips, drooling, muteness, coma, hemiplegia, and concurrent Five Collapse syndromes. Use doubled Ren Shen first to secure collapse, then treat Wind.
[Annotation] Shen Fu Tang consists of Ren Shen and Fu Zi. It treats Wind-Evil attacking Zang organs in individuals with deficient constitution. Symptoms include relaxed lips, flowing saliva, coma, muteness, hemiplegia, or concurrent Five Collapse syndromes. Use a large dose of Ren Shen first to secure against collapse, then treat the Wind pathogen.
Qianjin Huan Hun Tang (Thousand Years Return Soul Decoction):
Huan Hun Tang treats Closed Syndrome of Meridians/Collaterals with sudden evil attack, manifesting as rapid breathing, coma, unconsciousness, no sweat, limb stiffness, and unilateral body pain. It contains Rou Gui (Cinnamon Bark), Ma Huang, Gan Cao, and Xing Ren.
[Annotation] "Closed Syndrome of Meridians/Collaterals" refers to closed-type Wind-Evil invasion of meridians/collaterals. Rapid breathing indicates coarse Qi. No sweat, limb stiffness, and unilateral pain indicate exterior pathogen closure. Use Rou Gui, Ma Huang, Gan Cao, and Xing Ren (Return Soul Decoction) to open the exterior.
Duoming San (Life-Seizing Powder):
Duoming San treats Closed Syndrome of Zang/Fu Organs with abdominal fullness and blockage, coma, trismus, and phlegm knot in the throat. In emergencies where decoctions cannot be swallowed, use Ba Dou, Bai Zhi, Ban Xia (Pinellia Rhizome), Ting Li Zi (Lepidium Seed), and Sheng Nan Xing.
[Annotation] "Closed Syndrome of Zang/Fu Organs" refers to closed-type Wind-Evil invasion of Zang/Fu. Abdominal fullness/blockage refers to distension and blocked bowels/bladder. Combined with coma, trismus, and phlegm in the throat preventing swallowing, use this formula to induce vomiting and purging. Ingredients: Ba Dou, Bai Zhi, Ban Xia, Ting Li Zi, Sheng Nan Xing.
Sansheng Yin (Three Living Decoction):
Sansheng Yin treats Wind-Cold Stroke, manifesting as cold limbs, deep/faint pulse, and rising Qi-Phlegm. Uses raw Sheng Nan Xing, Sheng Chuan Wu (Aconitum Carmichaelii Lateral Root), Sheng Fu Zi, and Mu Xing. Add Ren Shen for Qi deficiency; double Ren Shen for extreme deficiency approaching collapse.
[Annotation] "Wind-Cold Stroke" refers to Wind-Evil attacking Zang organs with Cold, regardless of meridian/collateral involvement. Cold limbs (Jue Ni) and deep/faint pulse (Chen Fu) are key signs. Ingredients: Raw Arisaema, Raw Aconite, Raw Prepared Aconite, Aucklandia. Suitable only for severe Cold and Excess Qi. Add Ren Shen for Qi deficiency; double Ren Shen for extreme deficiency to prevent adverse reactions ("rebellious weapon").
Qufeng Zhibao Tang (Wind-Expelling Precious Treasure Decoction):
Qufeng Zhibao Tang treats Wind-Heat Stroke, manifesting as floating/rapid pulse, red face, heat, and irritability. It modifies Fangfeng Tongsheng San (Saposhnikovia Releasing and Unblocking Decoction) by adding Quan Xie, Tian Ma (Gastrodia Rhizome), Xi Xin, Bai Fu Zi (Typhonii Rhizoma), Qiang Huo, Du Huo, Huang Bo (Phellodendron Bark), Huang Lian (Coptis Rhizome), and Jiang Can. Fangfeng Tongsheng San is detailed in the Cold Damage chapter.
[Annotation] "Wind-Heat Stroke" refers to Wind-Evil attacking Fu organs with Heat. Floating/rapid pulse indicates exterior heat. Heat and irritability indicate body heat and mental restlessness. Fangfeng Tongsheng San is modified with Scorpion, Gastrodia, Asarum, Typhonium, Notopterygium, Angelica Pubescens, Phellodendron, Coptis, and Bombyx.
Qingzhou Bai Wan Zi (Qingzhou White Pills):
Qingzhou Bai Wan Zi treats Wind-Phlegm Stroke, manifesting as facial paralysis, flaccid paralysis, and excessive phlegm-saliva. It is also an excellent remedy for pediatric convulsive phlegm. Ingredients: Sheng Bai Fu Zi, Sheng Chuan Wu, Sheng Nan Xing, Sheng Ban Xia, processed into pills.
[Annotation] "Wind-Phlegm Stroke" refers to Wind-Evil attacking the exterior in individuals with Phlegm-Fluid retention. Excessive phlegm-saliva is the key sign. Ingredients: Raw Typhonium, Raw Aconite, Raw Arisaema, Raw Pinellia, processed into pills.
Qiang Huo Yu Feng Tang (Notopterygium Wind-Resolving Decoction):
Qiang Huo Yu Feng Tang treats External Wind Invasion, manifesting as limb weakness, difficulty speaking, slight muscle twitching, and insensitivity. It modifies Da Qin Jiao Tang by adding Ren Shen, Guan Gui (Cinnamon Bark), Huang Qi (Astragalus Root), Du Zhong (Eucommia Bark), Fang Ji (Stephania Root), Zhi Mu, Zhi Ke, Chai Hu (Bupleurum Root), Bo He, Man Jing Zi, Ju Hua (Chrysanthemum Flower), Qian Hu (Peucedani Radix), Cang Zhu, Ma Huang, Ban Xia, Hou Po (Magnolia Bark), Gou Qi (Goji Berry), and Di Gu Pi (Lycium Root Bark). It regulates various wind syndromes.
[Annotation] "External Wind Invasion" refers to Wind entering from outside. Prodromal signs include limb weakness, slurred speech, occasional muscle twitching, and numbness/immobility of the thumb and index finger. Modify Da Qin Jiao Tang by adding the listed herbs. After internal pathogens are cleared and external pathogens fading, take this decoction to guide Qi through meridians, eventually eliminating major Wind, separating clear/turbid Qi, and harmonizing Ying/Wei.
Qing Re Hua Tan Tang (Clear-Heat Resolve-Phlegm Decoction):
Qing Re Hua Tan Tang treats Internal-Onset Disease, manifesting as shortness of spirit, erratic speech, dizziness, and weak legs. It modifies Liu Jun Zi Tang (Six Gentlemen Decoction) with Mai Dong (Ophiopogon Tuber), Huang Qin, Huang Lian (Coptis Rhizome), Shi Chang Pu (Acorus Rhizome), Zhi Shi (Immature Bitter Orange), Zhu Ru (Bamboo Sap), Nan Xing (Arisaema Rhizome), and Mu Xiang (Aucklandiae Radix).
[Annotation] "Internal-Onset" refers to Phlegm-Fire arising internally. Prodromal signs include erratic spirit, abnormal speech, upper excess/lower deficiency, dizziness, and weak legs. Ingredients: Ren Shen, Bai Zhu (Atractylodes Macrocephala Rhizome), Fu Ling (Poria), Gan Cao, Ju Hong (Citrus Reticulatae Pericarpium), Ban Xia, Mai Dong, Huang Qin, Huang Lian, Shi Chang Pu, Zhi Shi, Zhu Ru, Nan Xing, Mu Xiang.
Di Huang Yin Zi (Rehmannia Infusion):
Si Zhi Bu Shou (Four Limbs Uncontrolled) without pain is Fei Syndrome; Hemiplegia with pain is Pian Ku Syndrome. If speech and will remain unchanged, the pathogen is shallow in Fen Cou (interstices), treatable with Huang Qi Wu Wu Tang. Severe inability to speak is Yin Fei Syndrome, indicating Kidney Deficiency and internal depletion, with pathogen entering Zang organs, usually fatal. Di Huang Yin Zi treats Kidney Deficiency internal depletion. Ingredients: Shu Di Huang (Prepared Rehmannia Root), Rou Gui, Fu Zi, Rou Cong Rong (Cistanche Stem), Ba Ji Tian (Morinda Root), Yuan Zhi (Polygala Root), Shan Yu Rou (Cornus Fruit), Shi Hu (Dendrobium Stem), Mai Dong, Wu Wei Zi, Bo He, Shi Chang Pu, Fu Ling.
[Annotation] Fei, Pian Ku, and Yin Fei are external invasions with varying severity. Fei: limbs uncontrolled, no pain. Pian Ku: hemiplegia, pain. Unchanged speech/will indicates shallow pathogen in interstices/Ying-Wei, treated with Huang Qi Wu Wu Tang to tonify Ying-Wei and dispel Wind. Severe muteness/confusion is Yin Fei, caused by Kidney Deficiency internal depletion, failure of Shaoyin reaching extremities, and pathogen entering Zang organs, hence usually fatal. Di Huang Yin Zi treats Kidney Deficiency internal depletion. Ingredients: Prepared Rehmannia, Cinnamon, Aconite, Cistanche, Morinda, Polygala, Cornus, Dendrobium, Ophiopogon, Schisandra, Mint, Acorus, Poria.
Di Tan Tang (Phlegm-Washing Decoction):
Di Tan Tang treats Internal-Onset confusing the Heart Orifice, manifesting as stiff tongue and difficulty speaking. Combine with Wendan Tang (Warm Gallbladder Decoction).
