Excess qi is termed fire; when it moves recklessly, one must know it scorches the supreme yin. For excess conditions, purgation is indicated; for deficiency conditions, tonification is indicated. The principle of heavy ascending and light descending is indeed profound. Yin deficiency with fever and excessive yang presents with a rapid but forceless pulse, indicating involvement of the Heart and Kidney. Internal injury fever due to yang deficiency presents with a large but forceless pulse, indicating involvement of the Lung and Spleen. Qi rising from the left indicates Liver fire; yin fire arises from below the navel. Heat rising from the feet invading the abdomen indicates extreme deficiency, where medicine is difficult to balance. To purge fire, use Huanglian Jiedu Tang (Coptis Decoction for Alleviating Toxicity); to tonify deficiency, Ginseng, Atractylodes Macrocephala Rhizome, and Raw Licorice Root are excellent. For constrained fire following the meridians, dispersion is appropriate; for external wind-cold, dispersion is also appropriate. Heart fire is treated with Coptis Rhizome (Huanglian), Lung fire with Scutellaria Root (Huangqin), Liver and Gallbladder fire with Bupleurum Root (Chaihu). Scutellaria and Coptis can purge Sanjiao fire; organ fire is preferably treated with Indigo Naturalis (Qingdai). Paeonia Root (Shaoyao) purges Spleen fire, Anemarrhena Rhizome (Zhimu) purges Kidney fire, Gypsum Fibrosum (Shigao) clears Stomach fire and nourishes yin. To purge Bladder fire, Phellodendron Bark (Huangbai) and Asarum Root (Xixin) must be added. Akebia Stem (Mutong) specifically purifies Small Intestine fire; for constrained fire, Cimicifuga Rhizome (Shengma) and Kudzu Root (Gegen) are used. If treating rootless, wandering fire, how can Rehmannia Root (Xuanshen) be omitted from the formula?
Huanglian Jiedu Tang (See details below)
Additional Remarks: Cool medicines should not be used abruptly for non-excess fire; warm dispersion must first be employed. For external wind-cold, dispersion is suitable; for constrained fire, dispersion is also suitable. Raw Licorice Root can moderate fire pathogens, and Gardenia Seed (Zhizi Ren) greatly descends fire.
It was originally known that heart pain is not true heart pain; Stomach heat must be distinguished as new or old. New cases involve damage to the Stomach by cold food; old cases become damp-heat and linger. Excess heat causes the left wrist pulse to be rapid; a choppy pulse indicates blood stasis. A full pulse at the right wrist indicates phlegm accumulation causing pain; a surging pulse in chronic cases indicates blood disasters. The treatment method generally employs Pingwei San (Regularize the Stomach Powder), containing Dried Ginger, Licorice Root, Amomum Fruit, Pinellia Tuber, Aucklandia Root, and Platycodon Root. This treats new cases with divine efficacy. If it is a chronic case, add Corydalis Rhizome (Yanhusuo), Platycodon Root, Coptis Rhizome, Pinellia Tuber, Aucklandia Root, and Gardenia Fruit decocted together; then believe in the miraculous medicine of physicians.
Differentiation: There are nine types of stomach pain: Drink, Food, Wind, Cold, Qi, Palpitation, Worms, Zhu (a type of infectious disease), and Wandering Heat Pain. Hejian said: Sudden pain is not heat; chronic pain has no cold. Some cases arise from Qi stagnation and food retention, suddenly developing pain, constipation, elevated chest and heart area, worsened by pressure, and loss of appetite. Urgently use Zhuhuang Wan (Boiled Yellow Pill) to purge, then use Ben Tang (This Decoction) afterward to remove residual pathogens. Cold jue heart pain presents with inverted cold limbs, cold sweat on the body, loose stools, clear urine, and weak energy; this is not a chronic condition, potentially fatal from morning to evening. Urgently warm with Shufu Tang (Atractylodes and Aconite Decoction). Heat jue heart pain presents with body heat and cold feet, severe pain, irritability, vomiting, and spontaneous sweating on the forehead. Moxibustion should be applied to Taixi (KI3) and Kunlun (BL60), purging both interior and exterior, leading to recovery through general sweating. After moxibustion, take Jinlingzi San (Melia Melia Powder); once pain stops, take Zhizhu Wan (Immature Bitter Orange and Atractylodes Pill) to remove pathogens. Medical Mirror records: White spots on the face with red lips, ability to eat after pain, intermittent pain indicates worm pain. Erchen Tang (Two-Cured Decoction) with Toosendan Fruit Bark decocted and taken. Large belly with blue veins is treated with Chongwan (Worm Pill).
