Old woman Han was nearly sixty years old. In mid-winter, she contracted tertian malaria (malaria occurring every third day). A certain physician employed a warming and dispersing method and allowed her to drink ginger-date decoction freely. After ten days, she could not consume even a grain of rice, and suffered severe vomiting during malaria paroxysms.
The same physician then advanced with hot-tonifying medicines, causing the condition to worsen significantly. Ten days later, Wang Mengying examined her. She presented with chest oppression and stagnation as if blocked by an object, yellow tongue coating, bitter taste in the mouth, thirst, urine dark red like hot water, wiry and slippery pulse, more pronounced on the right side, and leukorrhea pouring forth like a stream. He prescribed Xiao Xian Xiong Tang (Minor Chest-Harmonizing Decoction) combined with Wen Dan Tang (Warm Gallbladder Decoction) plus Xie Bai (Allii Macrostemonis Bulbus). (In this syndrome, pathogenic heat invades the Ying level from the Qi level, hence the profuse leukorrhea. Treating disease requires seeking its root; here, one should first clear phlegm-heat. Trichosanthis Fructus Semen, pounded, 4 qian; Ginger-processed Coptidis Rhizoma, 2 qian; Processed Pinelliae Tuber, 1.5 qian; Lai Ju Hong (Citri Reticulatae Pericarpium), 0.8 fen; Fried Immature Bitter Orange Fruit, 1.5 qian; Ginger-processed Bamboo Shavings, 4 qian; Fresh Allii Macrostemonis Bulbus, 1.5 qian.) After taking the medicine, she vomited large amounts of gelatinous phlegm. After more than ten days, the chest oppression subsided. The prescription was changed to a sweet-cooling formula, and the malaria gradually ceased. (Sweet-cooling formula: Fresh Reed Rhizome, 2 liang; Ginger-processed Bamboo Juice, one large wine cup mixed; Fresh Loquat Leaf, brushed and wrapped for decoction, 3 qian; Fresh Bamboo Leaf, 2 qian; Dendrobii Caulis, decocted first, 1 liang; Processed Pinelliae Tuber, 2 qian; Dioscoreae Hypoglaucae Rhizoma, 8 qian; Anemarrhenae Rhizoma, wine-fried, 3 qian; Fresh Allii Macrostemonis Bulbus, 1.5 qian.) Subsequently, yin-nourishing methods were incorporated, leading to complete recovery. (Yin-nourishing formula: Scutellaria Barbatae Radix soaked and strained, 8 qian; Lycii Cortex Radicis, 5 qian; Fritillariae Cirrhosae Bulbus, pounded, 4 qian; Benincasae Semen Exsiccatum, 4 qian; Processed Pinelliae Tuber, 2 qian; Ophiopogonis Japonici Tuber, 4 qian; Dioscoreae Hypoglaucae Rhizoma, 2 liang; Melia Toosendan Seed, pounded and decocted first, 3 qian; Charred Prunus Mume Seed, 1 qian.)
Wei Xilin’s niece had lochia persisting for two months postpartum. Subsequently, hearing of the deaths of her husband, father, and two younger brothers, she was overcome with grief that could not be released. She developed dry cough with hemoptysis, headache on the left side, anorexia, insomnia, constipation, thirst with drinking, but urination only once every other day. Despite long-term treatment without effect, Wang Mengying palpated the pulse as deficient, wiry, hollow, and large. He prescribed Gan Mai Da Zao Tang (Licorice, Wheat, and Jujube Decoction) plus Rehmanniae Radix Preparata, Polygoni Multiflori Caulis, Testudinis et Chelonis Carapax et Plastrum, Er Zhi Wan (Two Solitudes Pill), Chrysanthemi Indici Flos, Inulae Flos, Paeoniae Radix Alba, Fritillariae Cirrhosae Bulbus, Cannabis Fructus, and Natrii Chloridum. (The pathology is yin deficiency with liver qi rebellion, failure of lung and stomach qi to descend, and deficiency of yang within yin. Testudinis et Chelonis Carapax et Plastrum, 2 liang, pounded and decocted first in eight bowls of water, using the decoction to cook the herbs. Raw Glycyrrhizae Radix et Rhizoma, 3 qian; Northern Triticum Semen, 4 qian; One Ziziphus Jujuba Fruit, torn open and decocted first; Large Prepared Rehmanniae Radix, 8 qian; Processed Polygoni Multiflori Caulis, 2 qian; Ligustri Lucidi Fructus, pounded, 5 qian; Ecliptae Herba, 4 qian; Hangbaiju (Chrysanthemi Indici Flos), 2 qian; Inulae Flos, wrapped and decocted first, 3 qian; Wine-fried Paeoniae Radix Alba, 1.5 qian; Fritillariae Cirrhosae Bulbus, pounded, 3 qian; Cannabis Fructus, pounded, 3 qian; Natrii Chloridum, 1 fen.) After administration, the pulse gradually became contained, and bleeding stopped. After seven or eight doses, the headache finally subsided. Ten days later, bowel movements normalized, and appetite returned.
Wang Shifu’s wife suffered from exogenous pathogen invasion with rash eruption. A certain physician treated with clearing and resolving methods. The fever gradually subsided, but mental clarity was impaired, the tongue was black and difficult to extend, there was frequent sighing, constipation, chest tenderness upon pressure, and the pulse was wiry, slow, and slippery. (Mental cloudiness and the following four symptoms are all signs of phlegm-heat obstructing the lung and stomach. The wiry-slow aspect of the pulse refers to the superficial layer, while the slippery aspect refers to the deep layer; thus, Mang Xiao and Da Huang from Liang Ge San (Cool the Epigastrium Powder) are appropriate. If the text implies wiry-slippery and slow, Liang Ge San should not be used. Scorched Scutellariae Radix, wine-fried, 1.5 qian; Fried Fermented Soybean Seed, 3 qian; Black Gardenia Skin, 3 qian; Mint Tip, 0.8 fen; Fresh Bamboo Leaf, 2 qian; Anemarrhenae Rhizoma, wine-fried, 3 qian; Hyacinth Bean Starch, 4 qian; Fried Immature Bitter Orange Fruit, 2 qian; Ginger-processed Bamboo Shavings, 3 qian; Yuan Ming Fen (Mirabilite), 1 qian; Raw Rheum Palmatum Radix, 3 qian, soaked in boiling water, strained, and taken.) After one dose, the yellow tongue coating receded. After further doses, black stools were passed, mental clarity returned, and the patient began to recover.
Chen Chadang’s wife suffered from exogenous pathogen invasion. She presented with facial redness, insomnia, irritability, delirious speech, sweet taste in the mouth with sticky thirst, dysuria with pain. Multiple doses of clearing-resolving formulas by another physician were ineffective. Wang Mengying palpated the left pulse as wiry, surging, and rapid, and the right pulse as slippery and overflowing. There was chest oppression and stagnation, and no bowel movement. The diagnosis was floating liver yang, failing descent of lung qi, obstruction by phlegm-heat, and lingering pathogenic factors. He prescribed Xiao Xian Xiong Tang combined with Wen Dan Tang, Xue Geng Tang (Snow and Soup Decoction), plus Inulae Flos and Xie Bai. (Left wiry, surging, and rapid pulse indicates yin deficiency with heat; right slippery and overflowing pulse indicates phlegm with wind-yang. Lung pathology is more urgent than liver pathology; clearing phlegm-heat is the method to quell wind-yang. Trichosanthis Fructus Semen, pounded, 4 qian; Ginger-fried Coptidis Rhizoma, 2 qian; Processed Pinelliae Tuber, 2 qian; Fried Immature Bitter Orange Fruit, 1 qian; Ginger-processed Bamboo Shavings, 3 qian; Dioscoreae Hypoglaucae Rhizoma, smashed, 1 liang; Light Sea Cuttlebone, decocted first, 2 liang; Inulae Flos, wrapped and decocted first, 3 qian; Western Xie Bai, smashed and added later, 1.5 qian.) The chest stagnation gradually opened. Ban Xia and Xie Bai were removed, and Dang Gui Long Hui Wan (Angelica Sinensis and Gentian Pill) was administered with the decoction. (Administered Long Hui Wan, 3 qian.) Delirious speech ceased, and sleep was possible. Tong You Tang (Unblock the Lower Orifices Decoction) was used to purge black stools. After three doses, nourishing yin and harmonizing the stomach formula was given, leading to recovery. (Nourishing yin and harmonizing stomach formula: Large Sheng Di Huang, 8 qian; Tian Dong, sliced, 6 qian; Ligustri Lucidi Fructus, pounded, 5 qian; Raw Glycyrrhizae Radix et Rhizoma, 3 qian; Hua Mai Dong, 4 qian; Anemarrhenae Rhizoma, wine-fried, 3 qian; Charred Prunus Mume Seed, 1 qian; One Ziziphus Jujuba Fruit, torn open and decocted first; Fritillariae Cirrhosae Bulbus, pounded, 4 qian.)
Weng Jiashun’s wife, postpartum, had a mass prolapsing from the vagina, resembling a lung in shape and color, which could not be reduced despite various treatments. On the third day, she sought Wang Mengying’s help. He instructed her to fumigate and wash the area with a strong decoction of two liang of Ze Lan Ye (Lycopus Herba). Then, equal parts of Hai Piao Xiao (Cuttlebone) and Wu Bei Zi (Gallnut) were applied. (Vinegar-processed Hai Piao Xiao, 4 qian; Stir-fried Wu Bei Zi, 1 qian.) These were ground into fine powder and dusted on the area, resulting in immediate reduction. Subsequently, lochia failed to flow, leukorrhea appeared intermittently, lactation was absent, and both legs ached. Wang Mengying said, “This is blood deficiency. Although lactation and lochia are absent, the abdomen must not be distended. The previous condition also belongs to severe deficiency.” Therefore, he prescribed Huang Qi, Dang Gui, Gan Cao, Sheng Di, Du Zhong, Da Zao, Glutinosa Rice, Sesame Seed, Lotus Root, decocted in concentrated mutton soup. After taking it, lactation gradually increased, and long-term use led to robust health. (Formula: Raw Huang Qi, 1 liang; Dang Gui Shen, 2 qian; Raw Glycyrrhizae Radix et Rhizoma, 3 qian; Dry Sheng Di Huang, 5 qian; Mian Du Zhong, 4 qian; Three Ziziphus Jujuba Fruits, torn open; Washed Glutinosa Rice, 5 qian; Pounded Sesame Seed, 5 qian; Sliced Lotus Root, 1 liang; Raw Mutton, 4 liang, boiled to make soup for decocting herbs.)
Tu suffered from nocturnal emission, unresponsive to long-term treatment. Pus and fluid discharged from the ears, eyes produced excessive tears making them hard to open, soreness and pain in shoulders, hypochondrium, chest, and back, slight aversion to cold and fever, decreased appetite, and mental fatigue. Wang Mengying observed the left pulse as wiry and rapid, and the right pulse as deficient and choppy. He prescribed San Cai Feng Sui Dan (Three Capabilities Pill Sealing the Marrow) plus Long Gu, Mu Li, Huang Qi, Sang Ye, Dan Pi, Zhi Zi, and Ju Hua. (Left wiry and rapid pulse indicates liver yin deficiency with heat; right deficient and choppy pulse indicates qi-level yang deficiency. Stir-fried Lu Dang Shen, 3 qian; Stir-fried Xi Yang Shen, 3 qian; Dan Cong Rong, 1.5 qian; Xi Sha Ren, pounded and added later, 6 fen; Wine-fried Huang Bo, 1.5 qian; Large Shu Di Huang, 8 qian; Tian Dong, sliced, 5 qian; Vinegar-calcined Mu Li, pounded, 4 liang; Vinegar-calcined Long Gu, pounded, 1 liang, these two simmered first in eight bowls; Dong Sang Ye, 3 qian; Hang Bai Ju, 2 qian; Jiao Shen Qu, pounded, 1.5 qian; Dan Pi, 2 qian; Black Zhi Zi Pi, 3 qian.)
Li Huafu’s wife suffered from head tremors. Wang Mengying palpated the pulse and determined it was due to liver meridian depression-fire rising. He prescribed Dang Gui Long Hui Wan (Angelica Sinensis and Gentian Pill), which cured the condition. (One Qing Gu with seed, pounded and simmered first; washed down with Long Hui Wan, 3 qian.) However, she could not control her anger. After the illness flared up, she additionally presented with urinary retention, abdominal distension, panting, sweating, and near-collapse. Wang Mengying examined her: the pulse was very wiry and choppy, mouth bitter, tongue coating yellow, tongue body purple and dark. Although menstruation was regular, there was internal blood stasis. He prescribed Xue Geng Tang plus Jin Ling Zi, Xuan Fu Hua, Zhi Zi, Hua Shi, Tao Ren, Chong Wei Zi, Che Qian Zi, and Mu Tong, still swallowing Long Hui Wan. (Differentiating blood stasis by abdominal distension, wiry-choppy pulse, and purple-dark tongue. Light Sea Cuttlebone, decocted first, 2 liang; Dioscoreae Hypoglaucae Rhizoma, 1 liang; Melia Toosendan Seed, pounded and decocted first, 4 qian; Inulae Flos, wrapped and decocted first, 3 qian; Black Zhi Zi Pi, 1.5 qian; Xi Hua Shi, decocted first, 5 qian; Raw Tao Ren, pounded, 3 qian; Chong Wei Zi, pounded and decocted first, 4 qian; Che Qian Zi, pounded and decocted first, 4 qian; Fine Mu Tong, 1 qian; Still administer Long Hui Wan with the medicine, 3 qian.) Externally, three large river snails, six cloves of raw garlic, and a handful of Che Qian Cao were mashed and applied to the lower abdomen. After taking the medicine, black blood was passed first, followed by free urination. Subsequently, stool was also black, and she recovered.
