A patient presents with dizziness and vertigo of the head and eyes. If the pulse at the right Cun (Cun position) and Guan (Guan position) is floating and forceless, this indicates deficiency of Spleen and Lung Qi; treat with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If the pulse at the left Guan and Chi (Chi position) is rapid and forceless, this indicates Wind-Heat due to Liver Blood deficiency; treat with Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) to nourish Liver Blood. For cases of Qi deficiency, use Sijunzi Tang (Four Gentlemen Decoction) plus Danggui (Angelica Sinensis) and Huangqi (Astragalus). For excess Heat in the Liver channel, use Chaihu Qinggan San (Bupleurum Liver-Clearing Powder). For deficient Heat in the Liver channel, use Liuwei Dihuang Wan. For Spleen Qi deficiency, use Buzhong Yiqi Tang. For Spleen deficiency with Phlegm-Dampness, use Banxia, Baizhu, Tianma Tang (Pinellia, Atractylodes, and Gastrodia Decoction). For cases caused by excessive bloodletting via Bian (stone needle therapy), use Xiong Gui Tang (Ligusticum and Angelica Decoction). For cases presenting with fever and aversion to cold, use Shengyu Tang (Sage Renewal Decoction). Generally, fever injures True Qi; bitter-cold herbs should not be used, as they may further damage the Spleen and Stomach.
A patient presents with deviation of the mouth and eye. If accompanied by twitching or convulsions of the limbs, or itching and twitching of the brow ridge, this belongs to Wind-Heat due to Liver Blood deficiency; treat with Jiawei Xiaoyao San and Liuwei Dihuang Wan to nourish Liver Blood. If there are tendon cramps and bone pain, or inability to move and walk, treat with Liuwei Dihuang Wan and Buzhong Yiqi Tang to nourish the source of transformation and generation. If caused by taking drying drugs, treat with Siwu Tang (Four Substances Decoction) plus raw Gancao (Licorice) and Jinyinhua (Honeysuckle) to resolve heat toxicity and benefit Yin Blood. All these cases belong to dryness in the Liver channel; the primary treatment must be Liuwei Dihuang Wan and Buzhong Yiqi Tang. If exacerbated by anger or sexual exhaustion, use Liuwei Dihuang Wan and Shiquan Dabu Tang (Ten Complete Tonifying Decoction) as the main formula. If exacerbated by labor injuring the body form, use Buzhong Yiqi Tang and Shiquan Dabu Tang as the main formulas.
During summer and autumn, when Damp-Heat prevails, if there is lack of appetite, dizziness and vertigo, generalized soreness and weakness, or numbness in both legs, thirst with spontaneous sweating, shortness of breath with fever, rapid breathing and panting, body heat with a large pulse, this indicates internal injury to Yuan Qi (Original Qi); treat with Buzhong Yiqi Tang. If there is vomiting, poor appetite, abdominal distension and stuffiness, and loose stools, this indicates injury to the Spleen and Stomach; treat with Liujunzi Tang (Six Gentlemen Decoction). If there is discomfort in the chest and abdomen, poor appetite, and incessant expectoration of phlegm, this indicates deficiency-type fullness in the Spleen and Stomach; treat with Sijunzi Tang. If there is fatigue of form and Qi, numbness of limbs, and poor appetite, this indicates injury to Yuan Qi by Heat; treat with Renshen Yiqi Tang (Ginseng Qi-Augmenting Decoction).
A patient presents with fever, thirst, and turbid urine. If peaceful at night but febrile during the day, the Heat is in the Qi level; treat with Qingxin Lianzi Yin (Heart-Clearing Lotus Seed Decoction). If peaceful during the day but febrile at night, the Heat is in the Blood level; treat with Siwu Erlian Tang (Four Substances and Two Lianas Decoction), both assisted by Liuwei Dihuang Wan. If there is alternating chills and fever, this indicates Liver Blood deficiency; treat with Jiawei Xiaoyao San and Liuwei Dihuang Wan.
A patient presents with difficulty in urination. If caused by taking drying drugs, treat with Siwu Tang plus chao hei Huangbai (Phellodendron) and Zhimu (Anemarrhena), and raw Gancao to nourish Yin Blood. If frequent with yellow color, treat with Siwu Tang plus Renshen (Ginseng), Baizhu (Atractylodes), Maimendong (Ophiopogon), and Wuweizi (Schisandra) to generate Blood and Qi. If scanty with yellow color, treat with Buzhong Yiqi Tang plus Shanyao (Dioscorea), Maimendong, and Wuweizi to nourish the source of transformation and generation. The Classic states: Without Yin, Yang cannot be generated; without Yang, Yin cannot be transformed.
A patient presents with constipation. If due to Blood deficiency and internal Heat causing dryness and stagnation, treat with Siwu Tang plus Maimendong and Wuweizi to generate Blood and moisten dryness. If caused by hot-drying drugs, treat with Siwu Tang plus Lianqiao (Forsythia) and raw Gancao to generate Blood and clear Heat. If caused by purgative/draining drugs, treat with Sijunzi Tang plus Xiong (Ligusticum) and Gui (Angelica) to assist Qi in generating Blood. If thirsty and desiring cold drinks, this is Heat flooding internally; treat with Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) to clear Stomach Fire. If thirsty and desiring hot drinks, [method implied: cool blood and moisten dryness]. If the intestines and stomach are full and dry, with dryness obstructing the rectum, use pig bile for enema induction. If intestinal Qi is deficient and Blood is exhausted causing obstruction, treat with Shiquan Dabu Tang. If Liver and Gallbladder pathogenic factors are excessive,侮 (insulting) the Spleen Earth so it cannot transport and transform, treat with Xiao Chaihu Tang (Minor Bupleurum Decoction) plus Zhizi (Gardenia), Yuliren (Pericarpium Pruni Semen), and Zhike (Aurantii Fructus).
A patient presents with palpitations and insomnia, or accompanied by epistaxis and hematochezia. If there is internal heat, afternoon tidal fever, thirst desiring hot drinks, and limb fatigue, this is Spleen Blood deficiency with moving fire; treat with [missing text, likely Anshen formulas] and Yuanzhi (Polygala). If due to Heart Blood deficiency and damage, treat with Baiziren San (Biota Seed Powder). Generally, these symptoms are caused by insufficient Heart and Spleen Blood; by regulating and tonifying Stomach Qi, Phlegm will clear and Spirit will naturally settle; there is no need to specialize in clearing Heat and resolving Phlegm.
A patient presents with fever and aversion to cold. If there is limb fatigue, irritability, thirst, rapid breathing and panting, headache, and spontaneous sweating, this is internal injury to Qi and Blood; treat with Buzhong Yiqi Tang and Yiwei Tang (Benefit Stomach Decoction). If there is irritability, thirst, body fatigue, poor appetite, or indigestion after eating, this is Spleen Qi deficiency with Heat; treat with Liujunzi Tang.
If fever occurs in the morning and the pulse is rapid and forceful, this is Heat in the Qi level; treat with Qingxin Lianzi Yin. If the pulse is rapid and forceless, this is Yang Qi deficiency; treat with Buzhong Yiqi Tang. If heat rises from the hypochondrium, this is Liver deficiency; treat with Siwu Tang plus Renshen, Baizhu, and Huangqi. If heat rises from below the umbilicus, this is Kidney deficiency; treat with Siwu Tang plus Renshen, Baizhu, Huangbai, Zhimu, Wuweizi, Maimendong, Rougui (Cinnamon Bark), or Liuwei Wan. If heat appears during the day and hides at night, or appears at night and stops during the day, or comes and goes without fixed time, or arises without fixed location, or starts from the feet, this is rootless deficient fire; use Jiawei Bawei Wan (Modified Eight-Ingredient Pill) and Shiquan Dabu Tang plus Maimendong and Wuweizi, and apply Fuzi (Aconite) powder mixed with saliva to Yongquan (KI 1) acupoint. If the form fears cold and prefers hot food and drink, this is Yang deficiency and Cold; urgently use Bawei Wan (Eight-Ingredient Pill).
A patient presents with dry mouth. If averse to cold drinks and food, this is Stomach Qi deficiency unable to generate fluid; treat with Qiwei Baizhu San (Seven-Ingredient Atractylodes Powder). If preferring cold drinks and food, this is盛 (excess) Stomach Fire consuming fluids. If these conditions arise after sweating or purging, this is Stomach Qi deficiency; primarily use Bazhen Tang (Eight-Treasure Decoction).
A patient presents with thirst. If irritable and desiring cold drinks, this is Excess Heat in the Upper Jiao; treat with Liangge San (Cool the Diaphragm Powder). With concurrent constipation, this is also Excess Heat in the Upper Jiao; treat with Qingliang Yin (Clearing and Cooling Decoction). If thirst results from using draining drugs, this is Qi and Blood deficiency; urgently use Bazhen Tang and Liuwei Wan.
A patient presents with deafness and tinnitus. If the left Cun and Guan pulses are wiry and rapid, this is deficient Heat in the Heart and Liver channels; treat with Siwu Tang plus Zhizi and Chaihu (Bupleurum) to generate Yin Blood. [Missing text regarding right pulse], add Zhizi, Xiong, and Gui to clear Liver and cool Blood. If worse in the morning, use Xiao Chaihu Tang plus Renshen, [missing herb], Gui, Baizhu to tonify Qi and clear Liver. If worse in the afternoon, use Siwu Tang plus jiu chao hei Huangbai, Zhimu, and Wuweizi to tonify Yin and descend fire. If heat in the soles of the feet belongs to Kidney deficiency, use Liuwei Wan to strengthen the master of Water. If cold feet belong to Yang deficiency, use Bawei Wan to benefit the source of Fire.
A patient presents with neck stiffness, locked jaw, and opisthotonos (arching back), which is Jing Spasm due to Qi and Blood deficiency. Master Zhang Zhongjing stated: Excessive sweating in Taiyang disease causes Jing Spasm. Due to Wind pathogen causing meridians to be stagnant and blood flow to be sluggish, there is no sweat and aversion to cold, called Gang Jing (Stiff Spasm). All are due to internal deficiency combined with sweating losing Jin Ye (Body Fluids) and Blood, leaving tendons without nourishment; all belong to deficiency patterns, not Wind diseases. Primary treatment should be greatly tonifying Qi and Blood. Thus, postpartum patients with ulcers, those with labor-injured Qi and Blood, those with Damp-Heat and Ministerial Fire, and those who mistakenly took draining drugs often suffer from this; its meaning is visible. Recently, "Jing" was miswritten as [error] in transmission.
A female patient presents with amenorrhea. If caused by constrained Fire injuring the Spleen, use Guipi Tang (Restore the Spleen Decoction) plus Zhizi and Danpi (Moutan). If both Qi and Blood are deficient, use Bazhen Tang plus Zhizi and Danpi. If menstruation arrives early, this is Blood deficiency with Heat; use Siwu Tang doubled with Renshen, Baizhu, and Mudanpi. If pain occurs before menstruation arrives, this is Qi deficiency with Blood stagnation; use Siwu Tang [text missing]. If Blood deficiency, use Bazhen Tang. For metrorrhagia (flooding), if caused by Liver Fire failing to store Blood, use Jiawei Xiaoyao San. If caused by Spleen deficiency failing to control Blood, use Buzhong Yiqi Tang. All these are caused by Six Evils and Seven Emotions damaging Yuan Qi; one must examine the cause and prioritize regulating and tonifying Stomach Qi.
