The swelling around the wound is extensive, indicating that the pathogenic toxin is extremely intense. If the ulceration penetrates to the level of the heart and invades the internal organs through the membranes, even a divine physician would find it difficult to save the patient. I believe that moxibustion (Moxibustion) is the best method in the initial stage, fumigation is the best method in the intermediate stage; if there is necrotic tissue but no suppuration, Baixiang Dan (White Reduction Elixir) and Hongsheng Dan (Red Ascension Elixir) should be used to extract pus and remove necrosis. Wait until the toxin is exhausted and new flesh grows, then proceed with generating muscle and healing the wound, thereby avoiding the complication of "locking mouth" (a type of stubborn ulcer). (Qingnang)
Wang Shengzhi said: To distinguish Fa Bei (Back Carbuncle), ask the patient to use both hands to feel up and down. The area that cannot be touched by hand is Fa Bei. If it can be touched, it is classified as Upper Da (Upper Attachment), Lower Da (Lower Attachment), Left Da (Left Attachment), Right Da (Right Attachment), or Middle Da (Middle Attachment), all of which can be treated with manual massage. If it cannot be touched by hand, it is a true Fa Bei, requiring simultaneous internal and external treatment and urgent medical intervention to prevent complications. (Lili)
Chen Shigong said: Lesions above the back region corresponding to the Heart are dangerous because they belong to the Zang organs. Generally, the Heart, Liver, Spleen, and Lung are connected to the back, whereas the Kidney meridian resides alone in the lower part. Although located inferiorly, its essence, fluids, Yuan Qi (Original Qi), and Shen (Spirit) irrigate and nourish the upper body. Thus, the fire of the four Zang organs relies on the water of this one organ for balance. This explains why the root of the five Zang organs is associated with the back. Moreover, the back is governed by the Taiyang Bladder Meridian (Taiyang Bladder Meridian) and the Du Mai (Governor Vessel). The Taiyang channel is the leader of the Six Channels, and the Du Mai is the commander of the Twelve Meridians. When sores occur on the back and toxins invade this area, treatment must differentiate between Yin and Yang and Deficiency and Excess. (Zhengzong)
He also said: Fa Bei occurring below the back region corresponding to the Heart and above the Shenshu point (Shenshu, BL23) is mild because it belongs to the Fu organ domain. Therefore, lesions below the Heart region often present as favorable conditions (Shun Zheng), as their form is inferior, their Qi governs the exterior, and their pathology is an abscess (Yong). (Zhengzong)
Wang Shengzhi said: Cancer (referring here to a specific severe ulcerative condition, not necessarily modern carcinoma) occurs at the Shendao point (Shendao, GV11) and Lingtai point (Lingtai, GV10), involving the Du Mai and the Bladder Meridian. In the early stage, there is no aversion to cold, fever, or pain; the lesion appears purple-black, does not rupture internally but turns black and necrotic spontaneously. It arises from accumulated heat due to sexual exhaustion after the age of twenty. Cases with black skin are incurable. (Lili)
Hu Gongbi said: Dui Xin Fa (Opposite Heart Eruption), Feng Ke Fa (Honeycomb Eruption), Lian Zi Fa (Lotus Seed Eruption), and Liang Tou Jian (Two-Pointed Tip) are all alternative names for Fa Bei, distinguished by the depth and opening of the sore. This condition strictly forbids the consumption of shiitake mushrooms, distilled liquor, and meats such as pork, mutton, beef, and donkey meat. Throughout the course of the disease, cold-natured medicines must be avoided to prevent the toxin from sinking inward. (Qingnang)
