补中益气汤(方见溃疡发热门)
Pain, if the radial pulse is tight and powerless, employ the supporting method. If swelling is hard and pain is severe, apply garlic-separated moxibustion and combine with detoxifying herbs. If pus does not form but pain persists, prioritize detoxification. If pus does not form, prioritize tonifying qi and blood to support toxin expulsion. If it does not ulcerate, or ulcerates but fails to heal, prioritize tonifying qi and blood to support toxin expulsion.
A man suffered from a carbuncle on the buttock, with swelling, hardness, and pain; the radial pulse was floating and tight, powerless upon deep pressure. After one dose of Neituo Qianghuo Tang (Internal Supporting Notopterygium Decoction), the pain ceased. After four doses of Jinyinhua San (Honeysuckle Powder), pus drained and he recovered.
A man suffered from a carbuncle on the buttock, with severe swelling, hardness, and pain. He underwent garlic-separated moxibustion. Subsequently, he took Xianfang Huoming Yin (Immortal Recipe for Life-Preserving Infusion); after two doses, the pain stopped. Then, using Tuoli Xiaodu San (Supporting Interior Detoxifying Powder), pus drained and he healed.
A man suffered from a carbuncle on the buttock, without pus formation, decreased appetite, and poor desire for food. First, Liu Junzi Tang (Six Gentlemen Decoction) was administered with Chuanxiong, Danggui, and Huangqi; his diet gradually improved. Then, Tuoli Xiaodu San (Supporting Interior Detoxifying Powder) was given; pus drained and he recovered.
A man’s ulcer had opened, but the pus was clear and thin, failing to heal. He was treated with Fermented Soybean Cake Moxibustion. Subsequently, Shiquan Dabu Tang (Ten Complete Tonifying Decoction) was drunk; after more than two months, he recovered. Generally, when sores fail to form pus, fail to ulcerate, or ulcerate but fail to heal, it is due to deficiency of qi and blood. If the pus is clear and thin, it indicates especially severe deficiency (see the section on Ulcer Pain for details on excess and deficiency).
A man suffered from a carbuncle on the buttock, with continuous clear pus, gradual muscle wasting, decreased appetite, and a slightly wiry stomach pulse. After several doses of Liu Junzi Tang (Six Gentlemen Decoction) added with Huoxiang and Danggui, his diet improved. He drank Shiquan Dabu Tang (Ten Complete Tonifying Decoction) and received Fermented Soybean Cake Moxibustion; after more than two months, the wound closed.
A frail person suffered from a carbuncle on the buttock, where pus had formed but failed to open. After several doses of Shiquan Dabu Tang (Ten Complete Tonifying Decoction), the pathogen was supported outwardly, then punctured with a needle; after another twenty-plus doses, he recovered. The buttock is in a remote location where qi and blood rarely reach. For elderly and weak persons suffering from this, it is especially necessary to tonify qi and blood to ensure recovery.
A man suffered from a carbuncle on the medial thigh. Before pus formation, there was swelling and pain. After two doses of Neituo Huangqi Chaihu Tang (Internal Supporting Astragalus and Bupleurum Decoction), symptoms slightly improved; after two more doses, it resolved.
A man suffered from diffuse swelling on the buttock, with unchanged skin color, and a slippery, rapid, yet powerless pulse; this was a buttock carbuncle. Pus was about to form but remained internal. I intended to treat with interior-supporting medicines, waiting for the carbuncle toxin to consolidate. For those that do not rise, only tonifying qi and blood is needed to hasten pus formation before puncturing; internal resolution methods should not be discussed. Once pus has formed, one must distinguish between "raw" and "ripe": if pressing causes immediate rebound, there is pus; if pressing does not cause rebound, there is no pus. If swelling is high and soft, it originates in the blood vessels; if low and hard, it originates in the sinews and bones; if skin color remains unchanged, it originates in the bone marrow.
A man suffered from a carbuncle on the lateral thigh, with diffuse swelling and severe pain. After two doses of Neituo Huangqi Wine-Infused Decoction, symptoms slightly improved. Then, several doses of Tuoli San (Supporting Interior Powder) were given; it ulcerated and healed.
A woman suffered from a leg carbuncle that failed to heal for a long time. The sore opening was purple and sunken, with clear, thin pus. I considered this a deficiency. She did not believe me and instead took purgative medicines, causing deficiency syndromes to arise. This is extreme deficiency of qi and blood, requiring strong tonification; otherwise, it will become a failure syndrome. If other syndromes occur subsequently, healing will be particularly difficult.
A man suffered from an internal ulceration of a leg carbuncle. Puncture yielded approximately four to five bowls of pus. He experienced aversion to cold and food, and his pulse was thread-like; this indicated slight yang qi. He took Sijunzi Tang (Four Gentlemen Decoction), tonifying medicines, and received Fuzi Cake Moxibustion; after two months, he recovered.
An elderly person suffered from a leg carbuncle that spontaneously ulcerated. Suddenly, he experienced dizziness and blurred vision, with a fine and faint pulse; this indicated extreme deficiency of qi and blood. He revived after taking strong tonifying agents.
A frail person suffered from a leg carbuncle. Taking one qian of Renshen (Ginseng Root), symptoms slightly improved after two doses. Using strong tonifying agents, he recovered after more than a month. Generally, after massive loss of pus and blood, strong tonification of qi and blood must be prioritized. Even if other syndromes exist, they should be treated secondarily. Generally, after a carbuncle ulcerates severely, fever and aversion to food indicate extreme deficiency of qi and blood. If the left-hand pulse is insufficient, blood-tonifying herbs should outweigh qi-tonifying herbs; if the right-hand pulse is insufficient, qi-tonifying herbs should outweigh blood-tonifying herbs. One must absolutely avoid releasing the exterior.
A woman suffered from lower back pain and weak legs, flaccidity preventing movement. She recovered after taking Renshen Baidu San (Ginseng Detoxifying Powder) added with Cangzhu, Huangbai, and Zexie.
A woman suffered from pain at the Huantiao acupoint, with unchanged skin color and a tight, rapid pulse; this was deep-bone abscess (Guji). Before pus formation, Neituo Huangqi Wine-Infused Decoction was taken with Qingpi, Longdancao, and Zhizi; after several doses, it resolved.
A man suffered from a leg carbuncle accompanied by tendon spasms and pain, with a wiry-tight pulse. He was treated with Wuji San (Five Accumulations Powder) added with Huangbai, Chaihu, and Cangzhu, and recovered.
A woman suffered from deep-bone abscess (Guji) that failed to heal for a long time, with continuous pus, skin itching, and flaccid limbs. I considered this deficiency and proposed tonification. The patient doubted, thinking it was wind. Heart, Liver, Spleen, Lung, and Kidney govern bodily functions. If drugs are used recklessly according to emotions, or if anger, joy, labor, and rest are imbalanced, leading to depletion of essence and blood in the five zang organs, the skin, blood, sinews, bones, and flesh become flaccid and unable to move, resulting in Wei syndrome (flaccidity). Its appearance is similar to Roufeng Hongkong Foot (soft wind gout). Roufeng Hongkong Foot is caused by external factors, whereas Wei syndrome is caused by insufficiency of the five internal organs.
A man suffered from deep-bone abscess (Guji), with swelling, hardness, fever, bone pain, and tendon spasms, with a rapid and deep pulse. He recovered after taking Danggui Niantong Tang (Angelica Joint-Pain Relieving Decoction).
A man suffered from pain penetrating to the bone near the Huantiao acupoint at the root of the leg, with normal outer skin, and a rapid, slightly slippery pulse; this was deep-bone abscess (Guji). Pus was about to form. Six doses of interior-supporting medicine were given. Swelling rose and became painful, pulse became slippery and rapid, indicating pus had formed. It was punctured, yielding about one bowl of pus. Additional tonifying agents were given; he recovered after more than a month.
A man suffered from a leg carbuncle with diffuse swelling and pain, cold extremities, throat obstruction sensation, and alternating chills and fever. Various treatments failed; this was due to pathogenic factor stagnation in the meridians. Wuxiang Lianqiao Tang (Five Fragrant Forsythia Decoction) was taken; after one dose, symptoms slightly receded. Upon further intake, bowel movements occurred twice, all symptoms receded, and he recovered.
A woman suffered from pain in both legs, unable to extend them, with a wiry-tight pulse, rough upon deep pressure. First, Wuji San (Five Accumulations Powder) was taken; after two doses, pain slightly stopped, and after one more dose, it ceased. Shenying Yangzhen Dan (Divine Response Nourishing True Pill) was then taken, allowing extension and flexion.
A man suffered from leg pain, slight knee swelling, floating upon light palpation, and wiry-tight upon deep pressure; this was Crane Knee Wind (Xixifeng). Da Fangfeng Tang (Great Saposhnikovia Decoction) was given; after two doses, symptoms receded by two or three parts.
Shortness of breath and rapid breathing indicate damage to true qi. Lethargy and propensity to lie down indicate decline of spleen qi. Incontinence of urine indicates bladder inability to store. Occasional irritability and heat, empty fullness below the heart, indicate stomach qi failing to nourish upwards. Absent-mindedness and forgetfulness indicate disorder of spirit and mind. If untreated, the outcome was indeed as predicted. This syndrome often affects those with deficiency; thus, Jiajian Xiao Xuming Tang (Modified Minor Continuing Life Decoction) and Da Fangfeng Tang (Great Saposhnikovia Decoction) are effective. Using toxic purgatives would certainly be a mistake.
A woman suffered from Hong Kong Foot (gout/polio), with occasional leg tendon spasms and abdominal pain. Various medicines failed, and she gradually became critical. Medical texts state: Bawei Wan (Eight Ingredients Pill) treats Shaoyin... Kidney overacting on Heart, Water overcoming Fire, death imminent; urgent administration is required.
A man suffered from leg pain, accompanied by tendon spasms and bone pain, with a wiry-tight pulse. Da Fangfeng Tang (Great Saposhnikovia Decoction) was taken; after two doses, spasms slightly improved, and after two more doses, swelling subsided. However, swelling arose internally, pulse became slippery and rapid, indicating pus had formed. It was punctured. Shiquan Dabu Tang (Ten Complete Tonifying Decoction) was taken; he stabilized after more than a month.
A woman suffered from knee swelling and pain, worsening with cold, lasting over a month without improvement despite various medicines. Pulse was wiry-tight; this indicated deep-severe cold pathogen. Da Fangfeng Tang (Great Saposhnikovia Decoction) was used. Cold pathogen invades deeply; if true qi is robust, how can pathogen cause harm? Thus, deep-bone abscess (Guji) and Crane Knee Wind (Xixifeng) are often suffered by those with kidney deficiency. Ancients used Fuzi (Aconite Root) to warm and tonify kidney qi, promote drug action, and disperse cold. Also, frail persons sleeping exposed in autumn/summer may be attacked by cold air, leading to latent cold-heat knotting, forming this syndrome, unable to turn, alternating chills and fever without sweat, pain responding to bone upon pressure. If unresolved for a long time, extreme yin generates yang, cold transforms into heat, and it ulcerates. If injured by thief wind, the affected area is not very hot, but shivering with aversion to cold, intermittent sweating, pain relieved by warming requires Da Fangfeng Tang (Great Saposhnikovia Decoction) and Huolong Gao (Fire Dragon Ointment). If treated incorrectly leading to curvature and hemiplegia, with hardness like stone, it is called Stone Abscess (Shiju). If heat is mild, days pass without ulceration, flesh color becomes purple, skin and flesh rot, it is called Slow Abscess (Huanju). Throughout beginning and end, the aforementioned decoction should be taken to dispel cold pathogen and tonify deficiency while supporting interior.
A man suffered from red, swollen, painful right leg, with a deep, rapid pulse. Danggui Niantong Tang (Angelica Joint-Pain Relieving Decoction) was taken, but limb pain worsened, indicating damp-toxin stagnation. Moreover, in the lower body, drugs are hard to reach. It was not a mismatch of drug and syndrome. Pricking the affected area to release toxic blood was performed, followed by the previous medicine; after one dose, symptoms drastically reduced, and after four doses, it resolved.
A man first had leg pain, then pain in all four limbs, wandering and fixed nowhere, worsening at night. Damp-draining and detoxifying agents failed. Pulse was slippery and rough; damp-heat or dead blood flowing into joints. Without acrid-warm agents to open interstices, unblock tunnels, and harmonize qi and blood, how could he recover?
A man suffered from leg pain; each time it hurt, phlegm increased, or epigastric discomfort occurred. Pulse was slippery and rapid. Erchen Tang (Two Aged Agents Decoction) was taken with Shengma, Baizhu, Ze xie, Qianghuo, and Tianxing; he stabilized.
A man had chronic Hong Kong Foot (gout/polio), pain under ribs, fever, dizziness, vomiting, and leg numbness. Detoxifying and heart-protecting medicines failed. Left Guan pulse was tight, Right Guan pulse was wiry; this too was Hong Kong Foot (gout/polio). Banxia Zuojing Tang (Pinellia Left Meridian Decoction) was taken; he recovered.
A man suffered from soft, swollen, painful feet, fever, craving cold drinks, constipation of urine and stool, and a rapid Right Guan pulse; this was damp-heat flowing into the Foot Yangming Meridian. Dahuang Zuojing Tang (Rhubarb Left Meridian Decoction) was taken; he recovered.
A man suffered from pain in the shin and ankle. Treated with Jiawei Baidu San (Modified Detoxifying Powder); he recovered.
A man suffered from pain in both legs, with a slippery, slow pulse; this was caused by damp-phlegm. Erchen Tang (Two Aged Agents Decoction) was taken with Baizhu, Huangbai, Qianghuo, and Zexie; it resolved.
