Gong Zicai treated a pediatric patient suffering from Pi Xi (痞癖, abdominal masses and accumulation). After administering drugs such as Areca Seed (Binglang), Curcuma Aromatica Rhizome (Pengzhu), Immature Bitter Orange (Zhishi), and Coptis Rhizome (Huanglian), the Pi condition worsened. Gong stated: "This is Pi caused by Blood deficiency in the Spleen Meridian; it cannot be treated with purgative or attacking methods." He subsequently prescribed Liu Jun Zi Tang (Six Gentlemen Decoction) added with Angelica Sinensis Root (Danggui), which cured the patient after several doses.
In cases where Stomach Qi is exhausted and depleted, Spleen and Stomach are damaged, Qi and Blood are dried up, limbs are emaciated, complexion is sallow, abdomen is distended with visible veins, body heat with spontaneous sweating, panting and shortness of breath, diarrhea with abdominal distension, edema, and anorexia, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) should be administered for a prolonged period to achieve recovery.
Wan Mizhai treated a one-year-old infant who developed accumulation due to eating chicken meat too early. The child became increasingly emaciated and lost appetite for milk and food. Upon detailed inquiry from the father, Wan prescribed Yang Pi Qu Ji Wan (Nourish Spleen and Remove Accumulation Pill), composed of Atractodis Macrocephalae Rhizoma (Baizhu), Tangerine Peel (Chenpi), Atractodis Rhizoma (Cangzhu), Magnoliae Officinalis Bark (Houpo), Aurantii Fructus Immaturus (Zhiqiao), Pinelliae Tuber (Banxia), Citri Reticulatae Pericarpium Viride (Qingpi), Medicated Leaven (Shenqu), Hordeum Vulgare Sprout (Maiya), Crataegi Fructus (Shanzha), and Licorice Root (Gancao). The infant took this for three days, followed by Ding Xiang Pi Ji Wan (Clove Abdominal Mass Pill), taken with chicken broth. A piece of undigested chicken meat was expelled. Subsequent administration of Yang Pi Wan led to full recovery.
The son of Mr. Wang suffered from pain at the epigastrium directly below the heart for seven days despite treatment. Palpation revealed that only the area below the heart was intolerable to touch. Wan remarked: "Previous treatments were erroneous, hence ineffective. Pain that can be pressed is deficient pain; pain that cannot be pressed is excess pain. Excess pain is either due to food stagnation or phlegm-fluid." He formulated a new prescription, extracting Zhi Shi, Huanglian, and Banxia (each 2 qian) from Zhi Shi Dao Yin Wan and Kong Yan Dan, along with equal parts of Aucklandia Root (Muxiang), Black Morning Glory Seed Powder (Hei Qianniu Toumo), Stir-fried White Mustard Seed (Baijiezi Chao), and Gancao. These were powdered and mixed with fresh ginger juice and Shenqu to form pills the size of hemp seeds. They were taken with water infused with Agarwood (Chenxiang), Muxiang, and Areca Seed (Binglang), or alternatively with ginger soup. Initially, 21 pills were taken; shortly after, the pain shifted to the Zhongwan point. Seven more pills were taken, moving the pain to below the umbilicus. Five additional pills resulted in the discharge of clear fluid, stopping the pain. It was then determined to be Spleen pain. Finally, Qing Pi Wan, added with Qingpi, Chenpi, Muxiang, Amomum Fruit (Sharen), Shenqu, Ophiopogon Tuber (Maidong), and Shanzha, was used for regulation, leading to recovery.
Li Shizhen treated the grandson of the Duke of Fushun, who had a craving for the smell of lamp wick oil. Whenever he smelled it, he would cry incessantly. Diagnosis indicated Pi Ji (癖积, specific type of accumulation). Treatment with vermifuge and anti-accumulation pills cured him after one course. (Source: *Bencao Gangmu*).
Lu Yangyu treated the son of Vice Minister Pan, aged fourteen. Initially, he contracted a cold, which resolved after medication. Later, he developed night fever and yellow urine, with slight daytime fever. Some physicians attributed this to Qi and Blood deficiency following diaphoresis and prescribed tonics, but the fever intensified. He was diagnosed with "Tongzi Lao" (Juvenile Consumption) and Yin-deficiency night fever. Treatment with Yin-nourishing and fire-reducing herbs caused abdominal distension, muscle wasting, reduced intake, and continuous day-and-night fever. Pulse diagnosis showed a harmonious Renying pulse, a tight and full Qikou pulse, and surging and slippery pulses at both Chi positions. This indicated Food Stagnation. Treatment focused on digestion and guidance, using Zhishi, Huanglian, Binglang, Shenqu, Maiya, Shanzha, Poria (Fuling), Alisma Rhizome (Zexie), and Gancao. After several doses, distension decreased and fever subsided, with gradual recovery of spirit. Binglang, Zexie, and Maiya were removed, and Ginseng Root (Renshen), Baizhu, and Dried Ginger (Ganjiang) were added. Several doses led to complete recovery.
Lu Zuyu treated his nephew Fei, who suffered from dysentery following malaria in childhood. After initial recovery, dietary injury occurred, causing internal steaming heat, difficulty defecating, frequent urges without passage. Physicians incorrectly diagnosed Spleen deficiency with loose stools due to recent illness and frequent bowel movements, prescribing Renshen and Baizhu tonics. As noted in medical classics, this "augmented the pathogen and aided recurrence," worsening the condition. Post-medication, fever increased, accompanied by irritability and dry throat. With floating and surging pulses throughout, and considering that fever with large pulses in chronic dysentery is ominous, Lu confirmed surging and forceful pulses with an abdomen intolerable to touch. He stated: "Pulse and symptoms both indicate excess, consistent with each other; do not panic. It is merely dehydration after chronic dysentery." He used Rehmannia Root (Shengdi), Danggui, White Peony Root (Baishao), Huanglian, doubled Zhishi, and Shanzha. After one dose, abdominal movement was felt; after two, flatus occurred. Soon after, over ten hard feces were passed, followed by three to four more episodes. Pulses calmed, body cooled, and spirit refreshed. Subsequent nourishment of fluids and tonification of the Spleen for half a month led to recovery.
Xue Lizhai treated a child with intra-abdominal masses causing upward attacking pain and irregular urination. Long Dan Xie Gan Tang (Gentian Liver-Draining Decoction) and Lu Hui Wan (Aloe Pill) cured the condition. Later, the child exhibited emaciation, fever, and thirst, indicating Liver Wood overacting on Spleen Earth. Bu Zhong Yi Qi Tang and Lu Hui Wan restored health.
Mr. Tang treated the daughter of a Vice Minister of Revenue suffering from Pi syndrome, with accumulated stagnation persisting for a year. Symptoms included abdominal fullness, high fever, constipation and urinary retention, and anorexia. Other physicians treated it as deficient heat-dampness or Stomach cold. Since she could not eat, reduced excretion was expected, and some considered it Glandular Heat (Gan Re). Various medicines failed, leaving the situation critical. Tang asked what medicine should be taken and answered: "Gansui and Dahuang." Dr. Zhang exclaimed: "Previous doctors used tonics; this girl has not eaten for long, so purgatives moving the intestines are unsuitable." Tang replied: "Trust me and live; oppose me and die." Zhang asked if there was a formula second only to these without Gansui. Thus, Da Cheng Qi Tang (Major Order the Qi Decoction) was administered, curing her after two doses. On the next day, residual stagnation remained, predicting recurrence of blockage in a few days. The original strong purgatives were needed to eradicate the root. Days later, blockage recurred with abdominal fullness and Pi knotting. Re-administration of the medicine cured her in one dose.
Zhu Danxi treated the son-in-law of Jia Fu's sixth uncle, aged sixteen, with a mass in the left hypochondrium but normal appetite. Prescription: Vinegar-fried Citri Reticulatae Pericarpium Viride (Qingpi), Sparganii Rhizoma (Sanleng), Bupleuri Radix (Chaihu) 3 fen; Cinnamon Twig (Guizhi), Ligusticum Wallichii Rhizome (Chuanxiong), Saposhnikoviae Radix (Fangfeng) each 2 qian; Atractodis Macrocephalae Rhizoma (Baizhu) 2.5 qian; Akebia Caulis (Mutong) 1.5 qian; Sargassum (Haizao) 1 qian; Licorice Root (Gancao) 0.5 fen. Divided into seven doses, decocted to half a cup, taken with 15 Bao He Wan (Preserve Harmony Pills). Avoid all triggering foods.
Jiang Zhongfang treated a seven-year-old boy who, after eating and being startled, developed chills and fever with a mass in the right hypochondrium, causing severe distending pain when heavy, and hardness/fullness when light, lasting three years. He subsequently suffered from San Yin Malaria for over a year, which was stopped with pill medication. Although the malaria ceased, he developed morning chill and evening fever, coughing, emaciation, and severe pain from the Pi mass. Prescription: [Missing Character], Baizhu, Trionycis Carapax (Biejia), Danggui each 4 liang; Renshen, Baishao, Anemarrhenae Rhizome (Zhimu), Moutan Cortex (Danpi), Maiya, Shenqu, Shanzha each 2 liang; Qingpi, Chenpi, Binglang, Muxiang, Cinnamomi Cortex (Guangui) each 1 liang; Sanleng, Curcumae Rhizoma (Ezhu), Chaihu, Peach Kernel (Taoren) each 7 qian. Decocted into a paste, mixed with 4 liang of Maltose (Yitang). Taken at any time. Recovery occurred before the course was finished.
