Huoxiang Zhengqi San (Agastache Powder to Rectify the Qi) (see "Stroke" chapter), Wuji San (Five Accumulations Powder) (see "Cold Damage").
Fuyang Zhuwei Tang (Yang-Supporting and Stomach-Assisting Decoction): Luo Qianfu treated Cui Yunqing, eldest son of the Transport Commissioner. The patient was 25 years old, originally robust and plump, accustomed to rich food, often presenting heat signs. Friends advised eating cold foods, so he took cold medicines. In autumn of the Gengchen year, malaria occurred. Doctors treated him with arsenic and other drugs, administered with fresh cold water, and prohibited hot foods. The malaria did not resolve; instead, vomiting and diarrhea were added, further injuring the spleen and stomach, weakening middle qi. Abdominal pain and borborygmus occurred, along with frequent pain in the epigastrium corresponding to the heart, unbearable in suffering. Multiple treatments failed, and by winter, there was no improvement. By April, due to excessive labor and worry, the previous symptoms worsened significantly. Upon examination, the pulse was wiry, thin, and faint; hands and feet were slightly cold; complexion was greenish-yellow and dull; mood was depressed, aversive to people and noise; appetite decreased; slight fullness caused subcardiac stuffiness; sour fluid was vomited; cold sweat appeared during attacks; breathing was rapid and chaotic with anxiety, requiring leaning forehead-to-forehead with someone. I reflected on the *Neijing*: "Deficiency of middle qi leads to changes in urination and defecation, and intestinal borborygmus; deficiency of lower qi leads to flaccidity, jue, and heart oppression." It also states: "When cold qi lodges between the stomach and intestines, sudden pain occurs, which resolves with warmth." Only sweet, pungent, and greatly hot agents can cure this, so this formula was prepared.
Herbs: Fuzi (Processed Aconite Root, peeled and de-navel, 2 qian), Ganjiang (Dried Ginger, processed, 1.5 qian), Caodoukou (Black Cardamom Seed), Yizhiren (Alpinia Oxyphylla Fruit), Jianshen (Selected Ginseng Root), Gancao (Honey-fried Licorice Root), Guanzi Gui (Cassia Bark), Baishaoyao (White Peony Root, each 1 qian), Wuzhuyu (Evodia Fruit), Chenpi (Tangerine Peel), Baizhu (Atractylodes Macrocephala, each 0.5 qian).
Formula Analysis: The *Neijing* states: "For cold invading internally, treat with pungent-hot substances, assisted by bitter-warm ones." Fuzi and Ganjiang are greatly pungent and hot, warming the middle and dispersing cold, thus serving as Sovereign herbs. Caodoukou and Yizhiren are pungent, sweet, and greatly hot, treating guest-cold invading the stomach, serving as Minister herbs. For spleen insufficiency, tonify with sweet flavors. Honey-fried Gancao is sweet and warm; Baizhu and Chenpi are bitter and warm, tonifying the spleen and nourishing qi. Water qi carrying wood qi may insult earth, causing acute pain. Guizhi is pungent and hot to dispel cold water; Shaoyao is sour to drain wood qi that overacts on earth; Wuzhuyu is bitter and hot to vent rebellious jue qi ascending to the chest, serving as Envoy herbs. Crush herbs to the size of hemp seeds. Combine as one dose, add 2 zhan of water, 3 slices of ginger, and 2 jujubes. Decoct to 1 zhan, remove dregs, take warm before meals. After three doses, the major condition subsided, and pain halved. In autumn, moxa-foxilate the Zhongwan point (37 moxa cones) to assist stomach qi. Then moxa-foxilate the Qihai point (over 100 cones) to generate yuan qi and nourish the hundred vessels. Administer Yuanshao Dan to improve appetite, build muscle, and moisten skin. In spring of the next year, moxa-foxilate the Zusanli point (14 cones), the He-sea point of the Stomach, to assist stomach qi and guide qi downward. Use aromatic herbs to assist the spleen, administering Yuqi Tang with added Baitanxiang (Sandalwood) for balanced treatment. Advised to restrain anger, curb desires, guard speech, and regulate diet. Recovered completely after one year.
Caodoukou Wan (Cardamom Pill) (Li Dongyuan's formula): Treats guest-cold invading the stomach; suitable even if heat is present, but only for one or two doses.
Herbs: Caodoukou (Black Cardamom Seed, 1.4 qian, wrapped in flour, roasted hot, peeled), Wuzhuyu (Evodia Fruit, soaked in hot water to remove bitterness), Yizhiren (Alpinia Oxyphylla Fruit), Jiangcan (Silkworm, fried, each 0.8 qian), Dangguishen (Angelica Sinensis Root Body), Qingpi (Green Tangerine Peel, each 0.6 qian), Shenqu (Medicated Leaven), Jianghuang (Turmeric Rhizome, each 0.4 qian), Sheng Gancao (Raw Licorice Root, 0.6 qian), Taoren (Peach Kernel, peeled, 7 pieces), Banxia (Pinellia Tuber, soaked in hot water seven times, 1 qian), Zexie (Alisma Rhizome, 1 qian; reduce by half if urination is clear, or 0.1 qian, suspected error), Mai (Wheat Bran, fried yellow, 1.5 qian), Zhi Gancao (Honey-fried Licorice Root, 0.6 qian), Chaihu (Bupleurum Root, 0.4 qian, adjust based on hypochondriac pain), Renshen (Ginseng Root), Huangqi (Astragalus Root), Chenpi (Tangerine Peel, each 0.8 qian).
Preparation and Administration: Except for Taoren which is ground separately into paste, grind the rest into extremely fine powder, mix evenly, and form pills using steamed dough soaked in hot water, size of paulownia seeds. Take 30 pills each time, delivered with cooked white water, well after meals, adjusting dosage as needed.
Da Chaihu Tang (Major Bupleurum Decoction) (see "Shaoyang Syndrome Alternating Cold and Fever"), Dachengqi Tang (Major Order the Qi Decoction) (see "Shaoyang Tide Fever"), Xiaowei Dan (Minor Stomach Pill) (see "Phlegm-Fluids"), Nongpu Wan (Magnolia Bark Pill) (see "Accumulation"), Ziwan Wan (Kadsura Pill) (see "Accumulation").
Xingban Anzhong Tang (Arisaema and Pinellia Stabilizing Middle Decoction) (from *Tongzhi Fang*): Treats pain caused by phlegm accumulation.
Herbs: Nanxing (Arisaema Cum Bile), Banxia (Pinellia Tuber, each 1.5 qian, both soaked in ginger juice), Huashi (Talcum), Xiangfu (Cyperus Rhizome), Zhike (Immature Bitter Orange, bran-fried), Qingpi (Green Tangerine Peel, vinegar-fried), Muxiang (Vladimiria Root), Cangzhu (Atractylodes Lancea, soaked in rice wash overnight, fried), Sharen (Black Cardamom Seed), Shan Zhi (Gardenia Fruit, fried black), Fuling (Poria), Juhong (Red Tangerine Peel, each 1 qian), Gancao (Honey-fried Licorice Root, 0.5 qian).
Modifications: For qi-attacking pain, remove Nanxing and Huashi, add Nongpu (Magnolia Bark) and Yanhusuo (Corydalis Rhizome) 1 qian each. For severe phlegm, add 1 qian of calcined White Luoke (Concha Meretricis) at administration.
Administration: Add 2 zhong of water and 3 slices of ginger, decoct to 0.8 zhong, take before meals.
Haige Wan (Clam Pill) (Zhu Danxi's formula): Treats heart pain due to phlegm-fluids.
Herbs: Haige (Clam Shell, burned to ash, ground extremely fine, left for several days to dissipate fire toxicity), Gualouren (Trichosanthes Seed, ground with pulp).
Preparation and Administration: Place Haige inside Gualou, adjust moisture content appropriately, and form pills. Take 50 pills each time.
Qingzhong Tang (Clearing Middle Decoction) (from *Tongzhi Fang*): Treats fire pain.
Herbs: Huanglian (Coptis Rhizome), Shan Zhi (Gardenia Fruit, fried, each 2 qian), Chenpi (Tangerine Peel), Fuling (Poria, each 1.5 qian), Banxia (Pinellia Tuber, 1 qian, soaked in ginger juice seven times), Caodoukou Ren (Black Cardamom Seed, crushed), Gancao (Honey-fried Licorice Root, each 0.7 qian).
