General Principles
Lung heat causes the lung leaves to wither and dry. The five zang organs are thereby affected, leading to flaccidity syndromes (Wei syndrome). (From *Neijing*) The Lung governs all qi and fears fire; the Spleen governs the four limbs and fears wood. If desires are unregulated, water loses its nourishment, causing fire to lack restraint and overact upon what it normally controls. When the Lung is invaded by fire pathogen, heat arises. Once the Lung is heated, metal loses its nourishment; wood lacks restraint and overacts upon what it normally controls, injuring the Spleen. Lung injury impairs its ability to regulate the whole body; Spleen injury impairs the function of the four limbs, resulting in various flaccidity syndromes. (From Zhu Danxi)
Internal Causes
Flaccidity syndromes are categorized into five types corresponding to the skin, vessels, sinews, muscles, and bones, which correspond to the five zang organs. The Lung governs the skin and hair; the Spleen governs the muscles; the Heart governs the blood vessels; the Liver governs the sinews; the Kidney governs the bone marrow. Excessive anger, joy, labor, and sexual indulgence cause internal depletion of the five zang organs, preventing the skin, vessels, muscles, sinews, and bone marrow from receiving transport and nourishment, thus leading to flaccidity. (From *Zhengzhi Huibu*)
External Manifestations
Skin flaccidity presents with withered complexion and falling hair, incessant panting and calling, indicating Lung heat. Vessel flaccidity presents with red complexion and overflowing vessels, flaccid calves unable to bear weight, indicating Heart heat. Sinew flaccidity presents with blue-green complexion and bitter taste, withered nails and contracted sinews, indicating Liver heat. Muscle flaccidity presents with yellow complexion and emaciated muscles, numbness and insensitivity of the skin and muscles, indicating Spleen heat. Bone flaccidity presents with black complexion and scorched ears, difficulty in lifting the waist and knees, indicating Kidney heat. (From *Zhengzhi Huibu*)
Pulse Diagnosis
Flaccidity belongs to Lung heat transmitting to the five zang organs. The pulse is mostly floating and large, or the Chi pulse is weak, or the pulse is slow, choppy, and tight. (From *Xuanyao*)
Accompanying Symptoms of Flaccidity
Internal heat causing flaccidity discusses the root of the disease. If there are triggering factors, they must be due to accompanying pathogens. There are cases accompanied by damp-heat, damp-phlegm, qi deficiency, blood deficiency, yin deficiency, dead blood (blood stasis), and food stagnation obstructing the pathway of qi ascent and descent. These must be clearly differentiated.
Damp-Heat Flaccidity
Damp-heat flaccidity results from rain and dampness infiltrating, where pathogenic steam steams the Spleen and flows to the four limbs. Patients feel reverse qi rising from the shins, or soreness, softness, swelling, and pain in the four limbs, or numbness and stiffness in the toes, with dark and scanty urine. The pulse is deep, soggy, and rapid. This is due to damp-heat in the lower jiao. As stated, "If damp-heat is not eliminated, the large sinews shorten and contract, while the small sinews relax and lengthen." Shortening leads to spasm; relaxation leads to flaccidity. Treatment should focus on raising yang and drying dampness. Filling and cloying agents are contraindicated. (From *Zhengzhi Huibu*)
Damp-Phlegm Flaccidity
Damp-phlegm flaccidity occurs in obese individuals whose qi and blood cannot move the phlegm, causing damp-phlegm to accumulate internally and lodge in the meridians, leading to numbness of the waist and knees and weakness of the four limbs. The pulse is deep and slippery. This is due to rich food, sweet wine, and dampness. As stated, "Excess earth causes people to be unable to lift their four limbs." Treatment should focus on drying the Spleen and resolving phlegm. (From *Zhengzhi Huibu*)
Qi Deficiency Flaccidity
Qi deficiency flaccidity results from hunger and fatigue, causing Stomach qi to become deficient and Lung qi to fail first. All bones and valleys lose nourishment, leading to flaccidity of the Zongjin (major sinews) and emptiness of the joints. Post-illness weakness of the hands and feet belongs to qi deficiency. As stated, "When the Spleen is diseased, it cannot transport fluids for the Stomach, and the four limbs do not receive grain qi and cease to function." Treatment should focus on tonifying middle and boosting qi. (From *Zhengzhi Huibu*)
Blood Deficiency Flaccidity
Blood deficiency flaccidity typically occurs after childbirth or blood loss, presenting with sallow complexion, weakness of the hands and feet, and inability to move. Treatment should focus on nourishing Ying blood. However, since blood is generated by the Spleen, if blood-nourishing herbs fail to cure the flaccidity, it is because Spleen deficiency prevents blood generation. Tonifying the Spleen will cause blood to flourish naturally, curing the flaccidity. (From *Zhengzhi Huibu*)
Yin Deficiency Flaccidity
Yin deficiency flaccidity results from excessive alcohol and sex, causing fire in the Lower Jiao Liver and Kidney to scorch the sinews and bones. Patients feel extreme heat in both feet rushing up to the thighs and knees, with soreness, weakness, and difficulty walking or standing for long periods. The pulse is choppy and weak, or the Left pulse is large but empty upon pressure. Treatment should focus on vigorously tonifying essence and blood to support the Liver and Kidney. (From *Zhengzhi Huibu*)
Blood Stasis Flaccidity
Blood stasis flaccidity occurs when lochia remains unfinished postpartum and flows to the waist and knees, or due to trauma where accumulated blood does not dissipate, causing limb pain and immobility, resulting in a choppy and hollow pulse. Treatment should focus on nourishing blood and moving stasis. (From *Zhengzhi Huibu*)
Food Stagnation Flaccidity
Food stagnation flaccidity results from overeating, obstructing pathways and disrupting qi ascent and descent. Spleen qi cannot transport to the four limbs, causing weakness of the hands and feet, abdominal distension and pain, nausea, and belching. The Right Hand pulse is surging, wiry, and slippery. Treatment should focus on promoting Spleen function and digesting stagnation, treating from the perspective of food stagnation. Only after the stagnation resolves should the Spleen be tonified. (From *Zhengzhi Huibu*)
Post-Dysentery Flaccidity
Weakness of the legs and shin pain or knee swelling after dysentery is caused by excessive downward purging leading to Yin loss. Treatment should focus on tonifying the Spleen and using lifting agents. Treating it as wind syndrome will further dry the Yin and make flaccidity difficult to cure. Occasionally, premature use of astringents during dysentery prevents the clear expulsion of accumulated stasis, which then descends into the tunnels and meridians causing flaccidity. In such cases, moving qi and driving out stasis is required, which is distinct from previous conditions. (From *Zhengzhi Huibu*)
General Differentiation of Flaccidity
Flaccidity resembles Roufeng (soft wind) and Hongkong Foot (Charcot-Marie-Tooth-like presentation), but those conditions involve excess pathogens and pain, whereas flaccidity involves internal deficiency and no pain. (From *Sanyin Fang*) However, some flaccidity cases do involve pain, necessarily due to accompanying fire, phlegm, dampness, or stasis. They must not be confused with wind treatments. (From *Zhengzhi Huibu*)
Treatment Methods
Treating flaccidity exclusively focuses on the Yangming channel because the Yangming meridian is the sea of water and grain, governing the transformation of fluids into qi and blood to seep into the valleys and moisten the sinews and vessels. Moreover, the Yangming meridian joins the Zongjin, meets at the Qichuan, belongs to the Dai meridian, and connects to the Du meridian. Therefore, Yangming deficiency means the five zang organs have nothing to rely on, cannot circulate qi and blood, nourish sinews and bones, or benefit joints, leading to flaccidity of the Zongjin and failure of the Dai meridian to draw, resulting in flaccidity. Thus, ancients prioritized Yangming for flaccidity, establishing this law for qi deficiency. Special emphasis on the Liver and Kidney exists because the Kidney governs bones and stores essence, and the Liver governs sinews and stores blood. Liver and Kidney deficiency leads to exhaustion of essence and blood; exhaustion of essence and blood allows internal fire to consume sinews and bones, causing flaccidity. Treatment should tonify and nourish the Liver and Kidney, establishing this law for yin deficiency. A skilled practitioner distinguishes between qi deficiency and yin deficiency and applies appropriate treatments. Regarding the many accompaniments of seven emotions and six desires, whether resolving phlegm and stasis, clearing damp-heat, draining excess, or tonifying deficiency, it depends on the physician's divine understanding. (From *Zhengzhi Huibu*)
Contraindication of Warm Agents for Lung Heat
If Lung metal is congested and yang qi cannot descend, causing cold fear in the feet despite thick wrapping, with persistent heat on the dorsum of the foot and scanty, frequent urination, treatment should clear the Lung and harmonize the Stomach. Mistaking this for yang deficiency and recklessly administering rigid warm agents will worsen the flaccidity. (From *Liuyao*)
Contraindication of Cold Agents for Stomach Deficiency
For those with poor appetite and emaciation, or diarrhea, even if symptoms of internal heat and blood deficiency exist, priority must be given to restoring Stomach qi with aromatic and sweet-warm substances. The Stomach is the mother of all things and the source of generating qi and blood. Once food intake improves, weakness naturally strengthens. Adhering strictly to the theory of "draining South and tonifying North" and长期使用 cold-cooling agents will further deplete grain qi and wither the four extremities. (From *Zhengzhi Huibu*)
Contraindication of Rich Foods for Phlegm-Heat
Deficient patterns of the Spleen and Stomach indeed require nourishment through the five flavors. However, for flaccidity caused by damp-phlegm or damp-heat, rich foods must be strictly avoided to prevent phlegm generation. Animal products belong to yang and rich foods generate heat; without a bland diet, safety is hard to maintain. Thus, deficient and excess patterns differ not only in medication but also in diet. (From *Zhengzhi Huibu*)
Incurable Signs
Bone flaccidity persisting in bedridden state, unable to rise from bed, indicates death.
Medicinal Usage
For blood deficiency, primarily use Siwu Tang, adding Niuxi, Qinjiao, Duzhong, and Duhuo. For fire, add Huangbai and Zhimu. For blood stasis, add Taoren, Honghua, Danpi, Niuxi, and Yanhusuo. For qi deficiency, use Sijunzi Tang. For deficiency heat, use Buzhong Yiqi Tang modified (Qibing Hezhong Tang). For Kidney deficiency, use Dihuang Wan or Danxi Buyin Wan. For deficiency heat, use Hujian Wan. For deficiency cold, use Huanshao Dan adding Lurong. For food stagnation causing flaccidity, use Erchen Tang adding Shenqu, Shanzha, Maiya, and Zhishi. For damp-phlegm flaccidity, use Erchen Tang combined with Baizhu and Cangzhu, adding Zhuli and Jiangzhi. For phlegm-fire flaccidity, use Erchen Tang adding Huangqin, Shanzhi, or Huangbai and Zhuli. For damp-heat flaccidity, use Dongyuan Jianbu Wan or Qingzao Tang. For rich-food congestion heat, use Chengqi Tang.
[Appendix: Crane Knee Wind]
Crane Knee Wind results from improper regulation and depletion of the three Yin meridians of the foot, allowing wind pathogen to enter via deficiency. This leads to daily muscle wasting, internal heat, reduced appetite, and limb cramping/pain. Eventually, the knee becomes large while the leg becomes thin. If injured in the Spleen and Stomach, use Buzhong Yiqi Tang as primary. If injured in the Liver and Kidney, use Liuwei Dihuang Tang as primary. If suppuration is imminent or post-ulceration, use Shiquan Dabu Tang as primary. All should be assisted with Dafangfeng Tang. Early stage requires Scallion Moxibustion (Congyun) for internal resolution. If symptoms include dry mouth and dizziness, combine with Buzhong Yiqi Tang. If poor diet, chest distension, and diarrhea occur, combine with Liujunzi Tang. If fever comes and goes intermittently, with clear/thin pus and non-healing flesh, combine with Bazhen Tang or Shiquan Dabu Tang. If umbilical/abdominal cold pain, weak legs/knees, dizziness, phlegm vomiting, and frequent urination occur, combine with Bawei Wan. (From Xue Lizhai)
[Appendish: Impotence]
The Kidney represents the Kan trigram, containing both water and fire. Yin and Yang interact to produce skill/function. Excessive sexual activity causes flaccidity of the Zongjin, leading to impotence (Yinwei), which is due to Mingmen fire decline, akin to winter where all plants wither. Alternatively, endless thoughts cause qi stagnation in the Heart and Kidney leading to impotence, which is Lower Jiao fire depression, akin to summer heat where grass and trees droop. For fire decline, use Guifu Bawei Wan. For fire depression, use Zhibai Liuwei Wan. If Kidney meridian fire depression causes impotence, combine with Zhibai clearing fire and strengthening kidney agents for immediate effect. Clinical examination is necessary; do not assume it is solely fire decline. (From *Zhengzhi Huibu*)
[Appendix: Jie Xie]
Jie Xie refers to spinal vein pain accompanied by shortness of breath and reluctance to speak. (From *Neijing*) Physical lethargy, relaxed sinews and vessels, inability to sit or walk properly, with a slow and choppy Chi pulse, belongs to the category of flaccidity. (From *Yitong*)
