After hematemesis (vomiting blood), the zang-fu organs are depleted and weak, the Ying and Wei (nutritive and defensive) qi fail to regulate properly, and yin and yang become severed. Yang deficiency in the upper jiao results in a sensation of deficient heat in the body, chest fullness and obstruction (chest痞), and dryness of the mouth.
The Heart governs blood; the Liver stores blood. Sorrow, worry, and overthinking damage the Heart; anger with counterflowing qi that rises but does not descend damages the Liver. When both the Liver and Heart are damaged, blood flows and scatters without stopping, and qi counterflows, leading to vomiting and bleeding.
Hemoptysis (spitting blood) is caused by injury to the Lung. The Lung acts as the canopy for the five zang organs and is easily injured; if subjected to pathogenic heat, it causes spitting of blood. If the sputum contains blood threads resembling red silk, this indicates lung injury; if accompanied by pain in the hypochondrium and spitting of fresh bright red blood, this indicates liver injury.
A pulse that is faint and cun-kou (cun position at the guan section) suggests hemoptysis. A deep and weak pulse indicates survival/prognosis is good, whereas a firm and solid pulse indicates death/poor prognosis. Regarding treatments involving decoctions, external warming (tang-wen), acupuncture, and stone therapy, there are separate specific formulas. Methods for tonification, nourishment, and guidance/exercise are attached hereafter.
The "Yangsheng Fang · Daoyin Method" states: Extend both legs, and use both hands' fingers to touch the five toes of the feet; this can cure conditions where the waist is broken/injured and cannot bend forward. For hemoptysis or chronic pain, extend both legs for a long time, and grasp the five toes of the feet with both hands seven times. This can cure inability to bend or extend the waist due to injury, as well as hemoptysis, chronic pain, and blood disorders. Frequent walking allows the body to become flexible and rollable.
The Heart governs the vessels and manifests in the tongue. If there is heat in the Heart, blood will bleed from the tongue like a gushing spring.
This condition is caused by damage to the five zang organs. Once the organ qi is injured, wind pathogens easily enter, and internal heat may also cause blood to pass down through the stool, which is bright red and accompanied by abdominal pain. If cold qi accumulates internally, it also causes blood to pass down through the stool, with a color resembling mung bean juice, and pain upon discharge that is not severe.
Bleeding after defecation indicates the source of bleeding is distant; bleeding before defecation indicates the source is near. "Distant" and "near" refer to pathology in the Upper Jiao or Lower Jiao respectively. This causes the face to lose its ruddy color, with alternating chills and fever.
A floating and weak pulse that disappears upon heavy pressure indicates lower bleeding (hematochezia).
The Heart governs blood and harmonizes with the Small Intestine. If heat accumulates in the Heart and binds to the Small Intestine, it causes hematuria (blood in urine).
A rapid and wiry pulse in the lower positions indicates wind pathogens entering the Shaoyin channel, leading to hematuria. A faint and cun-kou pulse at the Chi position also indicates hematuria.
The "Yangsheng Fang" states: After consuming sweet cheese/yogurt, one should not consume strong vinegar, otherwise it will inevitably transform into hematuria.
