Xin Gan (Heart Caloric Disease), caused by the deficiency of Zang-Fu organs, allows various parasites to move within the intestines and stomach. Taking advantage of this deficiency, they attack and consume the Heart, which is termed Xin Gan (Heart Caloric Disease). Initially, no other symptoms are perceived; the patient experiences mental confusion, excessive sleepiness, and heaviness in the four limbs. If this condition attacks the Heart, it causes mental restlessness, oppression, and pain, before subsequently invading other parts of the body. Palpation reveals a deep and fine pulse, with cold hands and feet, indicating dampness accumulating in the Heart.

Excessive consumption of sweet flavors stimulates the movement of parasites in the intestines and stomach, leading them to invade and consume the Zang-Fu organs; this still falls under the category of Gan disease (Caloric Disease). All foods of the five flavors enter the Stomach, and their Qi returns to the respective Zang-Fu organs according to their flavor preferences. The Spleen and Stomach are exterior-interior pairs, both corresponding to Earth, with a sweet flavor. Sweetness moistens and softens the Spleen and Stomach. When the Spleen and Stomach are moistened, the Qi becomes sluggish; when Qi is sluggish, parasites move, and their movement leads to invasion and consumption, resulting in Gan disease (Caloric Disease). Since the parasites move due to sweetness, it is named Gan (Caloric Disease).

The initial manifestations include: vexing pain in the hands and feet, weakness in the lower back and spine, irritability during sleep at night, drowsiness with frequent dreaming, taciturnity with dry eyes, inverted dreams at night, loss of taste in food, pallor, preference for sleep, dizziness upon rising, heavy body weight, and soreness in the shins. If the parasites ascend to consume the Five Zang, there is internal vexation; if they descend to consume the throat and gums, ulcers form, black blood is discharged, and teeth turn purple-black; if they descend to consume the intestines and stomach, black blood appears in the stool; if they exit via the anus, ulcers form and rot. Deficiency and rebellious Qi of the Stomach lead to vomiting and hiccups. In acute cases, death occurs within a few days; in chronic cases, the patient may only exhibit depression, heaviness and pain in the joints, reduced appetite, and pallor. The invasion continues internally for years until ulcers form on the mouth and teeth from above, and the anus is damaged and rotted from below, leading to death.

It is also stated: Among the Five Gan diseases, the first is Bai Gan (White Caloric Disease), causing dry and rough skin and pallor. The second is Chi Gan (Red Caloric Disease), consuming the Five Zang internally and causing hair to become brittle and dry. The third is Tiao Gan (Pinworm Caloric Disease), consuming the spine, wandering through the Five Zang, causing heavy body and edema. The fourth is [missing character] Gan, causing pain and itching in the lower body, and stiffness and urgency in the lower back and spine. The fifth is Hei Gan (Black Caloric Disease), consuming the Five Zang, often resulting in black blood in the stool, leading to death within a few days. Among the five, White Gan is the mildest, Red Gan is next, Pinworm Gan is next, the fourth Gan is next, and Black Gan is the most severe. All these Gan diseases ascend from the intestines, causing ulcers in the throat and gums, and descending to damage and rot the anus, resulting in bloody and purulent diarrhea, vomiting, heat in the palms and soles, lower back pain, and sleepiness. The condition is milder in autumn and winter but extremely severe in spring and summer.

It is also stated: A blue complexion with red cheeks, lackluster eyes, dry lips and mouth, abdominal distension with masses, and progressive emaciation indicate Gan disease (Caloric Disease). The parasites consume the Five Zang, and the patient remains unaware until death.

It is also stated: Chronic Gan diseases may transform into the Five Zheng (Five Steaming Disorders). The Five Zheng are: 1. Gu Zheng (Bone Steaming), 2. Mai Zheng (Vessel Steaming), 3. Pi Zheng (Skin Steaming), 4. Rou Zheng (Flesh Steaming), and 5. Xue Zheng (Blood Steaming). Although their origins and initial presentations differ, once the "Steaming" pathology is established, the general nature of the disease is similar. They all cause lower back pain, chest fullness, weakness and fatigue, progressive emaciation, irregular fever or chills, vexing heat in the hands and feet, cold extremities, diarrhea, constipation, or sweating. The Five Zheng have separate discussions and are related to various deficiency-consumption diseases.
