[Case 1]
Patient Xu (female) from Anchang: Blood deficiency leads to Qi rushing upward, resulting in lower back and abdominal pain, leukorrhea, stiffness of the back, a deep and wiry pulse, and occasional epigastric pain. Treatment should focus on nourishing blood, harmonizing the stomach, and pacifying the liver.
Prescription: Danggui Shen (Radix Angelicae Sinensis Root End, 2 qian), Sheng Muli (Concha Ostreae, 4 qian), Chao Duzhong (Cortex Eucommiae, Stir-fried, 3 qian), Chuan Lianzi (Fructus Toosendan, 2 qian), Fushen (Poria with Hostwood, 4 qian), Cao Kou Ren (Seed Alpinia Katsumadai, 1 qian), Xiao Huma (Semen Sesami Nigrum, 3 qian), Meiguihua (Flos Rosae Rugosae, 5 flowers), Xian Banxia (Rhizoma Pinelliae Preparated, 1.5 liang), Mutuhdie (Membrana Calyptrae Ormiparae, 4 fen), Danshen (Radix et Rhizoma Salviae Miltiorrhizae, 3 qian).
Usage: Decocted in water, four doses.
Follow-up: Leukorrhea persists, pulse is fine, tongue coating is thick and yellow, abdominal pain accompanied by nausea. It is appropriate to modify the previous prescription following the same method.
Prescription: Danggui Shen (Radix Angelicae Sinensis Root End, 2 qian), Xian Banxia (Rhizoma Pinelliae Preparated, 1.5 liang), Fupenzi (Fructus Rubi, 3 qian), Xiao Huma (Semen Sesami Nigrum, 3 qian), Chao Baishao (Radix Paeoniae Alba, Stir-fried, 1.5 qian), Guang Pi (Pericarpium Citri Reticulatae, 1.5 qian), Chao Duzhong (Cortex Eucommiae, Stir-fried, 3 qian), Peilan Ye (Herba Eupatorii, 1.5 qian), Sheng Muli (Concha Ostreae, 4 qian), Yanhu (Rhizoma Corydalis, 1.5 qian), Qingmuxiang (Radix Aucklandiae, 0.5 qian).
Usage: Decocted in water, four doses.
Commentary by Shao Jie: Kidney deficiency causes the Dai Mai (Girdle Vessel) to lose its firmness and binding function, leading to back stiffness, lower back pain, and leukorrhea. Liver Yang counterflow obstructs Qi movement, causing epigastric and abdominal pain. The root cause is blood deficiency leading to liver Qi stagnation. Therefore, the treatment primarily focuses on harmonizing the stomach, pacifying the liver, tonifying the kidney, and nourishing blood. Secondly, for abdominal pain and nausea, Qi-regulating herbs are added.
[Case 2]
Patient Li Wenbin from Anchang: Severe stifling pain in the epigastrium, mouth flooding with clear fluid and desire to vomit, wiry pulse, white tongue coating with slight yellow in the center, limbs slightly alternating cold and warm. Treatment should address both the Jueyin and Yangming meridians simultaneously. (July 24th)
Prescription: Ganjiang (Rhizoma Zingiberis Recens, 2 fen), Cao Kou Ren (Seed Alpinia Katsumadai, 1 qian), Jiangxiang (Lignum Dalbergiae Odoriferae, 8 fen), Walengzi (Concha Arcae, 3 qian, crushed and stir-fried with 3 fen of Wuyu), Huanglian (Rhizoma Coptidis, 8 fen), Guding (Ramulus Cinnamomi, 4 fen), Nongpo (Cortex Magnoliae Officinalis, 1 qian), Xian Banxia (Rhizoma Pinelliae Preparated, 1.5 liang), Guya (Fructus Hordei Germinatus, 4 qian), Tongcao (Medulla Tetrapanacis, 1.5 qian), Meiguihua (Flos Rosae Rugosae, 5 flowers).
Usage: Decocted in water, three doses.
Follow-up: Epigastric pain persists, vomiting has improved, wiry pulse, liver Qi horizontal, tongue coating thick, tender, and yellow. Treatment should focus on dispersing and draining the Jueyin meridian. (July 27th)
Prescription: Chuan Lianzi (Fructus Toosendan, 3 qian), Zhishi (Fructus Aurantii Immaturus, 1.5 qian), Gualou Pi (Pericarpium Trichosanthis, 3 qian), Yuliren (Semen Pruni, 3 qian), Yanhu (Rhizoma Corydalis, 2 qian), Chao Guya (Fructus Hordei Germinatus, Stir-fried, 4 qian), Xiebai (Bulbus Allii Macrostemonis, 1 qian), Meiguihua (Flos Rosae Rugosae, 5 flowers), Cao Kou Ren (Seed Alpinia Katsumadai, 1 qian), Pi (Pericarpium Citri Reticulatae, 1.5 qian), Nongpo (Cortex Magnoliae Officinalis, 1.5 qian).
Usage: Decocted in water, three doses.
Second Follow-up: Epigastric pain is reduced, wiry pulse, belching with upward counterflow, Liver Wood not yet harmonized. Temporarily, settling counterflow and harmonizing the stomach is appropriate. (August 4th)
Prescription: Jinfei Hua (Flos Farfarae, 3 qian, wrapped and decocted), Chuan Lianzi (Fructus Toosendan, 3 qian), Walengzi (Concha Arcae, 4 qian), Chao Guya (Fructus Hordei Germinatus, Stir-fried, 4 qian), Daizheshi (Haematitum, 3 qian), Yanhu (Rhizoma Corydalis, 2 qian), Xiebai (Bulbus Allii Macrostemonis, 1 qian), Jineijin (Endothelium Corneum Gigeriae Galli, 3 qian), Xian Banxia (Rhizoma Pinelliae Preparated, 1.5 liang), Pi (Pericarpium Citri Reticulatae, 1.5 qian), Nongpo (Cortex Magnoliae Officinalis, 1.5 qian).
Usage: Decocted in water, four doses.
Commentary by Shao Jie: Liver Qi counterflow invades the Stomach, causing clear fluid to overflow and induce vomiting, and bi-zu (obstructive) pain in the epigastrium. The initial prescription aimed to unblock Yang and drain turbidity; the second focused on harmonizing the stomach and pacifying the liver; the final included sedatives to settle counterflow. The order of treatment was logical, hence effective.
[Case 3]
Patient He from Toupeng: Continuous pain in the epigastrium and abdomen with zheng jia (masses), wiry and fine pulse, white tongue coating, astringent urination. The condition is severe and dangerous; treatment should aim to prevent transmission to Jueyin and await the arrival of Zheng Qi (Righteous Qi). (June 23rd)
Prescription: Gualou Pi (Pericarpium Trichosanthis, 5 qian), Chuan Lianzi (Fructus Toosendan, 3 qian), Yuliren (Semen Pruni, 3 qian), Jiangxiang (Lignum Dalbergiae Odoriferae, 8 fen), Xiebai (Bulbus Allii Macrostemonis, 1.5 qian), Cao Kou Ren (Seed Alpinia Katsumadai, 1 qian), Guang Yujin (Radix Curcumae Longae, 3 qian), Meiguihua (Flos Rosae Rugosae, 5 flowers), Sheng Xiangfu (Rhizoma Cyperi, 3 qian), Tongcao (Medulla Tetrapanacis, 1.5 qian), Chao Yanhu (Rhizoma Corydalis, Stir-fried, 3 qian).
Usage: Decocted in water, two doses.
Follow-up: Epigastric pain persists, stool has passed, wiry and fine pulse, greasy tongue coating. Still need to prevent Jueyin transmission. If there is vomiting and counterflow, focus on harmonizing Liver and Stomach, awaiting Zheng Qi recovery. (June 25th)
Prescription: Xian Banxia (Rhizoma Pinelliae Preparated, 2 qian), Chuan Lianzi (Fructus Toosendan, 3 qian), Jiuxiangchong (Insectus Aspongopus, 3 qian), Tongcao (Medulla Tetrapanacis, 1.5 qian), Zuojin Wan (Zuojin Pill, 8 fen), Zhi Yanhu (Rhizoma Corydalis Processed, 2 qian), Wugu Chong (Insectus Mylabris, 3 qian, wine-stir-fried), Meiguihua (Flos Rosae Rugosae, 5 flowers), Nongpo (Cortex Magnoliae Officinalis, 1 qian), Caodoukou (Seed Alpinia Galanga, 1 qian), Jiangxiang (Lignum Dalbergiae Odoriferae, 8 fen).
Usage: Decocted in water, two doses.
Commentary by Shao Jie: Liver Yang insults the Stomach, Qi gathers to form masses, causing continuous pain in the epigastrium and abdomen. This is due to emotional depression and inability of Liver Qi to vent. Both prescriptions drain Jueyin to soothe its function and harmonize Yangming to benefit the fu organs. The herbs utilize the descending nature of bitter flavors and the dispersing-unblocking nature of pungent aromas.
[Case 4]
Patient Huang from Anchang: Alcoholism leads to excessive dampness, wiry pulse, liver Qi horizontal, epigastric and abdominal pain. Treatment should resolve alcohol toxicity and primarily use methods to separate, eliminate, and move Qi. (March 3rd)
Prescription: Chuan Lianzi (Fructus Toosendan, 3 qian), Walengzi (Concha Arcae, 4 qian), Jineijin (Endothelium Corneum Gigeriae Galli, 3 qian), Jizi (Herba Siegesbeckiae / or possibly Jishiteng, context suggests damp-draining herb, here translated as Herba Siegesbeckiae based on common pairing, though text says "Ji Zi", likely typo for Jishiteng or similar, literal translation: Chicken Egg/Vine - assuming Jishiteng Herba Siegesbeckiae for dampness), Yanhu (Rhizoma Corydalis, 3 qian), Baikou Ren (Semen Amomi Rotundus, 8 fen, taken with liquid), Nongpo (Cortex Magnoliae Officinalis, 1 qian), Meiguihua (Flos Rosae Rugosae, 5 flowers), Xiao Qingpi (Pericarpium Citri Reticulatae Viride, 8 fen), Wuyao (Radix Linderae, 3 qian), Jiangxiang (Lignum Dalbergiae Odoriferae, 8 fen).
Usage: Decocted in water, three doses.
Commentary by Shao Jie: Dampness from food and drink lodges internally, Spleen Yang fails to transport silently, and the Stomach cannot disseminate. Consequently, Liver Qi takes advantage to insult, causing continuous pain in the epigastrium and abdomen. Treatment involves soothing the Spleen and descending the Stomach to pacify the Liver, allowing Qi mechanism to circulate and gradually improve.
[Case 5]
Patient Yu from Anchang: Continuous pain in the epigastrium and abdomen is reduced, wiry and fine pulse, sensation of heaviness/dropping in waist and hips, damp-heat remains. Continue with modified previous prescription. (First month, 7th day)
Prescription: Chuan Lianzi (Fructus Toosendan, 3 qian), Cao Kou Ren (Seed Alpinia Katsumadai, 1 qian), Jineijin (Endothelium Corneum Gigeriae Galli, 3 qian), Jiuxiangchong (Insectus Aspongopus, 1.5 qian), Yanhu (Rhizoma Corydalis, 3 qian), Fuling (Poria, 4 qian), Mutuhdie (Membrana Calyptrae Ormiparae, 4 fen), Meiguihua (Flos Rosae Rugosae, 5 flowers), Sheng Muli (Concha Ostreae, 4 qian), Cao Guo (Fructus Tsaoko, 3 qian), Tongcao (Medulla Tetrapanacis, 1.5 qian). (Guiding herb) Lulutong (Fructus Liquidambaris, 7 pieces).
Four doses.
Follow-up: Abdominal pain has eased, waist and hips still feel heavy/dropping, back feels pulled/tight, pulse is soggy and fine, sweet taste in mouth. Focus on harmonizing Liver and Stomach. (First month, 29th day)
Prescription: Xian Banxia (Rhizoma Pinelliae Preparated, 1.5 qian), Cao Guo (Fructus Tsaoko, 3 qian), Mutuhdie (Membrana Calyptrae Ormiparae, 4 fen), Du Huo (Radix Angelicae Pubescentis, 1.5 qian), Zuojin Wan (Zuojin Pill, 8 fen), Sigualuo (Retinervus Luffae Fructus, 3 qian), Guang Yujin (Radix Curcumae Longae, 3 qian), Meiguihua (Flos Rosae Rugosae, 5 flowers), Fuling (Poria, 4 qian), Chenxiang Qu (Massa Medicata Fermentata Cum Bile, 1.5 qian), Tongcao (Medulla Tetrapanacis, 1.5 qian). (Guiding herb) Lulutong (Fructus Liquidambaris, 7 pieces).
Four doses.
Commentary by Shao Jie: Liver Yang insults the Stomach; Stomach deficiency fails to control tendons and bones. Additionally, damp-heat stagnates in the Spleen and Kidney meridians, blocking the pathways of Qi and Blood circulation, causing dropping pain in the waist and hips. The back belongs to the Yangming fu organs; disturbed by Wind-Yang, it cannot bind tendons/bones or facilitate joints, leading to back pulling. Thus, treatment uses formulas to pacify the Liver, harmonize the Stomach, seep out dampness, and unblock collaterals.
[Case 6]
Patient Shen from Yuzhuang: Epigastric pain reduced, wiry pulse, yellow tongue coating thick at root, alternating chills and fever. Modify previous prescription appropriately. (April 29th)
Prescription: Chuan Lianzi (Fructus Toosendan, 3 qian), Za'er Binglang (Fructus Arecae Cum Zizipho Jujubae, 3 qian), Sheng Xiangfu (Rhizoma Cyperi, 3 qian), Zuojin Wan (Zuojin Pill, 8 fen), Yanhu (Rhizoma Corydalis, 2 qian), Guang Yujin (Radix Curcumae Longae, 3 qian), Chuan Po (Cortex Magnoliae Officinalis, 1 qian), Chao Guya (Fructus Hordei Germinatus, Stir-fried, 4 qian), Jiangxiang (Lignum Dalbergiae Odoriferae, 8 fen), Tongcao (Medulla Tetrapanacis, 1.5 qian), Ke Qiao (Pericarpium Fructus Amomi Rotundus, 1.5 qian). (Guiding herb) Lulutong (Fructus Liquidambaris, 7 pieces).
Four doses.
Commentary by Shao Jie: Spleen-Stomach damp-heat is not cleared; Liver Yang takes the opportunity to invade and insult, causing epigastric pain with chills and fever. Treatment focuses on pacifying the Liver and seeping out dampness.
[Case 7]
Patient Wang from Dayi: Epigastric pain has improved, bowel movements are not smooth, wiry pulse, thick yellow coating at tongue root, red urine. Treatment should open the diaphragm, harmonize the middle, and soothe the Liver. (November 3rd / September 20th)
Prescription: Gualou Pi (Pericarpium Trichosanthis, 3 qian), Chuan Lianzi (Fructus Toosendan, 3 qian), Chao Guya (Fructus Hordei Germinatus, Stir-fried, 4 qian), Walengzi (Concha Arcae, 4 qian), Xiebai (Bulbus Allii Macrostemonis, 1 qian), Yanhu (Rhizoma Corydalis, 2 qian), Yuliren (Semen Pruni, 3 qian), Meiguihua (Flos Rosae Rugosae, 5 flowers), Nongpo (Cortex Magnoliae Officinalis, 1 qian), Tongcao (Medulla Tetrapanacis, 1.5 qian), Jineijin (Endothelium Corneum Gigeriae Galli, 3 qian), Lulutong (Fructus Liquidambaris, 7 pieces).
Commentary by Shao Jie: Damp-heat obstructs Clear Yang, Liver Yang counters upward, Stomach Qi does not descend. This causes the Large Intestine to lose conduction and the Small Intestine to lose transformation, resulting in difficult urination and defecation. However, the pulse remains wiry, so treatment drains the Liver, harmonizes the Stomach, opens the diaphragm, and unblocks Yang.
[Case 8]
Anonymous Patient: Water deficiency with hyperactive Wood, wiry pulse in both hands, food regurgitation/stagnation in epigastrium after eating, distension and stuffiness below the umbilicus, heat in palms and soles at night. Temporarily focus on nourishing the Stomach, harmonizing the middle, and clearing the Liver. (March 12th)
Prescription: Chaihu Shi (Herba Dendrobii Cum Caulibus, 3 qian), Jineijin (Endothelium Corneum Gigeriae Galli, 3 qian), Guya (Fructus Hordei Germinatus, 4 qian), Chao Qingpi (Pericarpium Citri Reticulatae Viride, Stir-fried, 7 fen), Shengtou Cao (Herba Patriniae, 1.5 qian), Shijueming (Concha Halioetidae, 6 qian), Xiangfu (Rhizoma Cyperi, 1.5 qian), Lvemei (Flos Rosae Chinensis 'Viridicalyx', 1.5 qian), Zuojin Wan (Zuojin Pill, 8 fen), Chuan Lianzi (Fructus Toosendan, 1.5 qian), Hehuan Pi (Cortex Albiziae, 2 qian).
Usage: Decocted in water, three doses.
Commentary by Shao Jie: Stomach fluids are depleted, Spleen loses healthy transportation, and Liver Yang becomes more horizontal. Treatment raises the Spleen, nourishes the Stomach, and softens the Liver.
[Case 9]
Patient Sun from Shufu: Abdominal pain extending to the epigastrium, wiry pulse, liver Qi horizontal, belching with upward counterflow. Temporarily focus on soothing the Liver and harmonizing the middle.
Prescription: Chuan Lianzi (Fructus Toosendan, 3 qian), Jineijin (Endothelium Corneum Gigeriae Galli, 3 qian), Sheng Xiangfu (Rhizoma Cyperi, 3 qian), Zuojin Wan (Zuojin Pill, 8 fen), Yanhu (Rhizoma Corydalis, 2 qian), Gong Chenxiang (Lignum Aquilariae Resinatum, 5 fen), Guang Yujin (Radix Curcumae Longae, 3 qian), Foshouhua (Flos Citri Sarcodactylis, 8 fen), Chao Qingpi (Pericarpium Citri Reticulatae Viride, Stir-fried, 8 fen), Chao Guya (Fructus Hordei Germinatus, Stir-fried, 4 qian), Zhike (Fructus Aurantii, 1.5 qian), Lulutong (Fructus Liquidambaris, 7 pieces).
Four doses.
Commentary by Shao Jie: Horizontal Liver Qi stagnation and weak, disharmonious Stomach cause abdominal pain extending to the epigastrium and belching with counterflow. This is a syndrome suitable for Xuanshen Daizhe Shi Tang (Inula and Hematite Decoction). Using soothing Liver and harmonizing Middle methods here is innovative.
[Case 10]
Patient Pang from Yifeng: Ying (Nutritive) deficiency causing stomach pain, deficient pulse, palpitations. Treatment should use Pungent-Sweet method. (Another month, 3rd day)
Prescription: Danshen (Radix et Rhizoma Salviae Miltiorrhizae, 3 qian), Chenxiang Qu (Massa Medicata Fermentata Cum Bile, 1.5 qian), Jiuxiangchong (Insectus Aspongopus, 1.5 qian), Sheng Muli (Concha Ostreae, 4 qian).
Usage: Decocted in water, four doses.
Follow-up: Stomach pain persists, pulse is deficient and wiry on the left, palpitations feel like hanging/suspension. Continue nourishing blood and pacifying the Liver. (June 8th)
Prescription: Quan Danggui (Radix Angelicae Sinensis, 1.5 qian), Chuan Lianzi (Fructus Toosendan, 3 qian), Fushen (Poria with Hostwood, 4 qian), Wuyao (Radix Linderae, 1.5 qian), Jiuxiangchong (Insectus Aspongopus, 1.5 qian), Chao Yanhu (Rhizoma Corydalis, Stir-fried, 1.5 qian), Chao Guya (Fructus Hordei Germinatus, Stir-fried, 4 qian), Meiguihua (Flos Rosae Rugosae, 5 flowers), Sheng Muli (Concha Ostreae, 4 qian), Cao Kou Ren (Seed Alpinia Katsumadai, 1 qian), Danshen (Radix et Rhizoma Salviae Miltiorrhizae, 3 qian).
Usage: Decocted in water, four doses.
Commentary by Shao Jie: The fluids of the five zang organs fail to ascend to the Heart; Heart Yang moves excessively, consuming Ying Yin further, resulting in palpitations and stomach pain. Nourishing blood and pacifying the Liver is indeed the key strategy for treating this syndrome.
