Heat rises like steaming, coughing is frequent. Nasal discharge flows from the nostrils, tears fill the eyes. The face is puffy, eyelids are swollen, both cheeks are red. This is caused by the invasion of epidemic measles toxin (Meidu).
Heat rises like steaming, cough sounds are frequent. Eyes are swollen, face is puffy, Qi ascends upward. Restlessness in sitting and lying, scanty phlegm and saliva. Lung lobes are scorched and elevated, heat pathogens steam and roast.
In measles symptoms, coughing continues for a hundred breaths without stopping. During the initial fever stage, before the rash erupts, this is appropriate. No cough or little cough are both unfavorable. To induce coughing, Chenpi (Exocarpium Citri Reticulatae) must be used.
After the rash has completely receded, there should be no cough. If coughing occurs at this time, early treatment is necessary. Clearing Metal (Lung) and draining Lung fire is the primary task. Regretting it when the condition becomes critical is too late.
Measles toxin lingers, causing Qi-rebellious coughing. Attacks last for a hundred breaths without stopping. Chest is high, shoulders shrug, nasal bleeding flows continuously. Those who wave hands and shake heads will become guests beneath the springs (die).
Epidemic measles resembles Cold Damage (Shanghan). The unique features of measles are sneezing, clear nasal discharge, swollen eyelids, and watery eyes. Seeing these symptoms indicates measles. Measles originates from the Heart; excessive Heart Fire overacts on Lung Metal, causing lung lobes to scorch and elevate, thus resulting in coughing. (Zhu's commentary: Explaining the cough in measles syndrome concisely.) The method should be to clear Metal and drain fire, using Jiawei Renshen Baihu Tang (see Article 29), removing Renshen (Ginseng Radix et Rhizoma), Gancao (Licorice Root), and Nuomii (Glutinous Rice), adding Niubangzi (Arctii Fructus) and Lianqiao (Forsythiae Fructus) as main herbs. Alternatively, Baihu Tang (see Article 26) can be used, removing Gancao and Nuomii, adding Niubangzi and Boheye (Menthae Haplocalycis Herba). Or combine Sanwei Xiaodu San with Xiebai San, removing Gancao from both. Sangbaipi (Mori Cortex) in the formula should be fresh and used in large quantities if possible. For severe heat with irritability and thirst, add Zhimu (Anemarrhenae Rhizoma), Tianhuafen (Trichosanthis Radix), Shigao (Gypsum Fibrosum), and Huangqin (Scutellariae Radix). For severe coughing, use Ermu San (Two Mothers Powder) or Maimendong Tang (Ophiopogon Decoction), removing Shengma (Cimicifugae Rhizoma), Chishao (Paeoniae Radix Rubra), and Gancao; or use Qingfei Tang (Clear the Lung Decoction), removing Jiegeng (Platycodonis Radix) and Gancao, adding Suzi (Perillae Fructus); or use Miben Qingfei Tang, removing Jiegeng and Gancao; or use Nie's Qingfei Yin, removing Chaihu (Bupleuri Radix), Chenpi, Jiegeng, and Gancao, etc., chosen according to symptoms. However, one must indeed clear heat and vent toxin, not forcibly stop coughing. Therefore, ancients often used Qingyang Yinzi, removing Gancao and adding Lianqiao as the main herb. Do not use Wuweizi (Schisandrae Fructus) or astringent medicines, lest they block the lung orifices, causing lifelong coughing. (Zhu's commentary: Astringent medicines block lung orifices, remember never to use them.) One should only clear residual heat and eliminate phlegm congestion, and the cough will stop naturally. Beimu Maimendong Yin (Fritillaria and Ophiopogon Drink), removing Gancao and Jiegeng, adding Lianqiao, Niubangzi, Huanglian (Coptidis Rhizoma), and Huangqin. This can be applied to coughs occurring before or after. Coughing during measles indicates that Lung Qi is unobstructed and hair pores are open, facilitating the eruption of the rash. Therefore, before the initial fever and eruption of measles, coughing is most desirable. (Zhu's commentary: The reason for coughing in measles must be known.) Frequent coughing is a favorable sign, commonly referred to as "Pu Ma" (smashing measles). Once the measles have erupted fully, Heart Fire subsides, the Lung is no longer oppressed, and coughing stops naturally. (Zhu's commentary: After eruption, there is naturally no cough because Heart Fire subsides and the Lung suffers no punishment.) During the active eruption phase, if there is no continuous coughing, or little/no coughing, the interstices (Couli) are closed, making it difficult for the rash to emerge through the surface. Urgently add Liubai Chenpi (aged Tangerine Peel) to diaphoretic/herb-raising medicines to induce coughing, thereby opening hair pores and smoothing interstices. (Zhu's commentary: Adding aged Chenpi to induce coughing is equivalent to smoothing the interstices.) Then the rash will easily emerge fully. After the rash has emerged, and during/after recession, the absence of coughing is ideal. If the rash has emerged but coughing persists, accompanied by fever, thirst, irritability, anorexia, and a surging pulse, use the ancient formula Huanglian Jiedu Tang (see Article 35), adding Shengdihuang (Rehmanniae Radix) and Xiangu Pigu (fresh Cortex Lycii). For Lung heat manifesting as cough and wheezing, use Shengdihuang San, removing Chenpi, adding Huangqin, Tingli (Descurainiae Semen seu Fructus), and Sangbaipi. For severe wheezing, add Gualou Shuang (Benincasae Semen), Baijiezi (Sinapis Semen), Suzi, and Laifuzi (Raphani Semen). Slight coughing after the rash has fully emerged and receded indicates incomplete elimination of residual pathogens and unsettled Lung Qi. Medication is unnecessary. If treatment is desired, use Xiebai San combined with Sanwei Xiaodu San, removing Gancao. For cough due to Lung dryness and obstruction, use Lijin Tang, removing Chenpi, Jiegeng, Gancao, and Shengjiang (Zingiberis Rhizoma Recens), adding Maimendong (Ophiopogonis Radix), Niubangzi, Huangqin, and Sangbaipi. For full eruption with high fever, cough, and constipation, use Chu Re Qingfei Tang, removing Chishao and Gancao, adding Huanglian, Huangqin, Digupi (Cortex Lycii), Huomaren (Cannabis Semen), and Zhike (Aurantii Fructus) to clear and promote urination/bowel movement. Or use Dahuang (Rhei Radix et Rhizoma) to purge. If coughing is severe and persistent after full emergence and recession, it belongs to Stagnant Heat; treatment should clear heat and descend fire, assisting with phlegm-resolving agents. Use Qingyan Zifei Tang, removing Yuzhu (Polygonati Rhizoma), Jiegeng, and Gancao, adding Huanglian and Ku Huangqin (scorched Scutellariae Radix); or Nie's Qingfei Yin, removing Chenpi, Chaihu, Jiegeng, and Gancao, adding Beimu (Fritillariae Bulbus) and Huanglian. For persistent coughing with abundant phlegm after recession, use Qingyan Zifei Tang, removing Yuzhu, Maimendong, Jiegeng, and Gancao, adding Zhike, Fuling (Poria), and Chenpi. Or Nie's Qingfei Yin, removing Chaihu, Jiegeng, and Maimendong, adding Zhike. If on the fifth or sixth day of measles, coughing remains coarse with external heat persisting, residual toxin remains in the Lung and Stomach. Use Meng Jieshi's Qingfei Yin, removing Jiangcan (Bombyx Batryticatus), Chaihu, Chenpi, Jiegeng, and Gancao, adding Digupi, Sangbaipi, and Tingli. Or Jingyuan Zhuren's Qingfei Yin, removing Jiegeng and Gancao, adding Shigao and Huangqin. For post-measles cough with unclear voice, anorexia, unclear vision, and dry lips/mouth, use Qingfei Xiaodu Huatan Tang, removing Jiegeng and Gancao. If post-measles wheezing and incessant coughing occur, toxin has flowed into the Lung Meridian, latent fire exists in the Lung, Metal is deficient and leaves are scorched. Use Miben Mendong Qingfei Tang or Gupi Qingge San, removing Jiegeng, Gancao, and Nuomii. If body heat is present, also use Miben Mendong Qingfei Tang, removing Gancao, Jiegeng, and Nuomii, adding Huanglian, Huangqin, and Niubangzi. For severe coughing with incessant wheezing, even vomiting water upon eating, this is heat-toxin attacking the Lung. Also use Miben Mendong Qingfei Tang, removing Gancao, Jiegeng, and Nuomii, adding Mi Zhi Pipaye (Honey-fried Loquat Leaf). If blood is seen, add Mao Gen (Imperatae Rhizoma) and Ejiao (Colla Corii Asini). For coughing with hemoptysis, use Maimendong Qingfei Tang, removing Zharon (Crataegi Fructus) and Jiegeng, adding Shengdihuang as the main herb. If accompanied by vomiting water, use this formula removing Zharon and Jiegeng. For post-measles cough with shortness of breath, use Zhike Qianhu Tang, removing Gancao and Jiegeng, replacing Sugeng (Perillae Caulis) with Suzi, adding Jiezi, Laifuzi, Tingli, and Gualou Shuang. Chenxiang (Aquilariae Lignum Resinatum) may also be added, which seems harmless post-measles. For post-measles merely coarse breathing, use Nie's Qingfei Yin, still removing Chaihu, Jiegeng, and Gancao, adding Jiezi, Suzi, Laifuzi, and Tingli. For persistent post-measles cough, unresolved internal heat, restlessness, and sleep disturbance, use Tianzhen Gao, removing Huangqi (Astragali Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), and Yiyiren (Coicis Semen), adding Jiu Chao Huanglian (Wine-fried Coptidis Rhizoma), Jiu Chao Huangqin, and Chen Sha (Cinnabar). For post-measles cough with loss of voice and fever, use Tingli Wan, adding Digupi, Tianhuafen, Jiu Chao Huanglian, and Huangqin, decocted in water. For post-measles cough with abdominal distension, wheezing, irritability, diarrhea, hoarseness, blue lips/mouth, use Jia Lanfeng's Prescribed Formula, removing Mahuang (Ephedrae Herba), Chenpi, Jiegeng, Gancao, and Baishao (Paeoniae Radix Alba), adding Maimendong, Gupi, and Jiezi. Baishao may remain, and Chaihu may be added. Treatment varies by individual. However, do not hastily use Baizhu. For long-term post-measles cough, use Shengdihuang, Dangguishen (Angelicae Sinensis Radix), Baifuling (Poria), Chuan Beimu (Fritillariae Cirrhosae Bulbus), Sangbaipi, Xingren (Prunus Semen), and Shishuang (Diospyros Frost) to nourish blood, cool blood, and clear Metal, stopping the cough. Some use Siwu Tang (Four Substances Decoction) combined with Zhiju Erchen Tang, adding Gualou Shuang and Xingren. While harmless, it is inferior to the previous seven-herb formula, which is simpler and more effective. For accompanying thirst, add Maimendong and Zhike. For accompanying wheezing, add Suzi and Jiezi. Tingli and Laifuzi may also be added. For those contracting Wind-Cold with cough, irritability, vomiting clear water, red eyes, sore throat, and mouth/tongue ulcers, use Fangfeng Baidu San (see Article 5), adding Sheng Huangqin. For cough, wheezing, burnt lips, internal heat accumulation, irritability, or epistaxis/hematemesis regardless of timing, use Xijiao Jiedu Huatan Tang, removing Chishao and Gancao, adding Maimendong, Xingren, and Tingli. Or Nie's Qingfei Yin, removing Chenpi, Chaihu, Danzhuye (Lophatheri Herba), Gancao, and Jiegeng, adding Huanglian and Dangguiwei (Angelicae Sinensis Radix). If these signs appear after recession, use Jiawei Renshen Baihu Tang, removing Renshen, Gancao, and Nuomii, adding Shengdihuang, Digupi, and Huangqin to moisten. If long-term cough persists with facial puffiness, eye swelling, turtle-back chest, shoulder shrugging, wheezing, head shaking, epistaxis, complexion varying from cyanotic to red or pale/withered, dry nose, and drowsiness, these are untreatable syndromes. (Zhu's commentary: Descriptions of untreatable syndromes added.)
