Qi Stagnation Causing Stabbing Pain, and Various Qi Stagnation Conditions.
To treat Qi stagnation, Zhiqiao (Fructus Aurantii) must be used, specifically for conditions located in the upper body. Zhiqiao depletes the highest Qi in the chest; therefore, it should only be taken for two or three doses. It is also said that Qingpi (Exocarpium Citri Grandis) should be used for Qi stagnation, but not in large amounts, as excessive use will drain True Qi. If there is excess heat internally with Minister Fire rushing upward, presenting symptoms similar to Qi stagnation, Zhimu (Rhizoma Anemarrhenae), Huangbai (Cortex Phellodendri), Huanglian (Rhizoma Coptidis), and Huangqin (Scutellariae Radix) are appropriate. In cases of Yin deficiency causing Qi stagnation, Siwu Tang (Four Substances Decoction) added with Xuanshen (Radix Scrophulariae) and Huangbai (Cortex Phellodendri) should be used to nourish Blood.
For Qi stagnation causing stabbing pain, use Zhiqiao (Fructus Aurantii). Depending on the affected area, guide the medicinal action using channel-guiding herbs to ensure efficacy. If the patient has a robust constitution and suffers from Qi stagnation with stabbing pain, Zhiqiao (Fructus Aurantii) combined with Wuyao (Linderae Radix) is suitable. For obese, white-complexioned individuals with Qi deficiency suffering from Qi stagnation stabbing pain, Renshen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma) added with Muxiang (Vladimiriae Radix) are appropriate. If the Qi stagnation stabbing pain results from emotional depression and lack of smoothness, Muxiang (Vladimiriae Radix) should be used.
[Appendix] Chong notes: Zhu Danxi's original works do not contain a specific section on treating Qi. This was added by later authors. It is retained here for readers' convenience.
Humans rely on Qi as their primary substance. If Qi does not circulate even for one breath, vital functions cease; if Qi is not sustained even slightly, the distinction between Heaven and Earth (Yin and Yang) becomes severed. The reason Yin and Yang can ascend and descend is due to Qi. The reason blood vessels can flow is also due to Qi. The reason Ying (Nutritive) and Wei (Defensive) Qi can circulate is this Qi. The reason the Five Zang and Six Fu organs can nourish and generate each other is also this Qi. When Qi is abundant, the body is full; when deficient, it is empty. When smooth, health prevails; when rebellious, disease occurs. Is Qi not the root of the human body? Humans have Seven Emotions (Joy, Anger, Worry, Thought, Sadness, Fear, Fright), which give rise to "Seven Qi" pathologies. The so-called "Seven Qi" refers variously to Cold, Heat, Anger, Resentment, Joy, Worry, Sorrow; or to Joy, Anger, Worry, Thought, Sadness, Fright, Fear. Both interpretations are accepted. However, harmonizing these Qi is paramount, as this also warms the Middle Jiao. Otherwise, interference among the Seven Emotions leads to phlegm-saliva condensation, resembling cotton wadding or membranes, and in severe cases, like a plum-pit, obstructing the throat. It cannot be coughed out nor swallowed down, causing difficulty in eating or sudden shortness of breath. These conditions are termed Qi Ge (Qi Obstruction), Qi Zhi (Qi Stagnation), Mi (Constipation due to Qi), and Qi Zhong (Qi Stroke), extending to Wu Ji (Five Accumulations), Liu Ju (Six Gatherings), and Jia masses (abdominal masses). Heart and abdominal mass pains, which feel as if death is imminent upon attack, occur everywhere. Anger causes Qi to ascend; Joy causes Qi to relax; Sadness causes Qi to dissipate; Labor causes Qi to consume; Thought causes Qi to knot. These seven states can all cause disease. For pain caused by Cold Qi stagnating in the Middle Jiao, use Qiqi Tang (Seven Qi Decoction), Yanjian San (Salt-Boiled Powder), or Dongyuan Shengyang Shunqi Tang (Dongyuan's Ascending Yang and Smoothing Qi Decoction). For rebellious Qi, suppress it using Muxiang Liqi Yin (Muxiang Flowing Qi Drink) or Jiangqi Tang (Descending Qi Decoction). For those with heat, add cooling agents to suppress it, which is what is meant by "drawing Yang from Yin."
