[Case 28] Patient Fan, aged sixteen. Alternating chills and fever (Wanglai Hanre), persisting for a month without resolution; red tongue body (She Chi); hypochondriac pain (Xietong); slight cough (Wei Ke); pulse is deficient and large (Mai Lai Xu Da). The disease resides in the blood level (Xuefen).
Prescription: Xiao Sheng Di (Rehmanniae Radix Preparata, 1.5 qian), Gui Shen (Angelicae Sinensis Radix, 0.8 qian), Chai Hu (Bupleuri Radix, 0.7 qian), Di Gu Pi (Lycii Cortex, 1.5 qian), Huang Qin (Scutellariae Radix, 1.5 qian), Qing Hao (Artemisiae Annuae Herba, 1 qian), Sang Ye (Mori Folium, 1 qian).
Due to night sweats (Daohan), add Jiang (Zingiberis Rhizoma) and Zao (Jujubae Fructus).
[Case 29] Patient Lu, aged twenty. Chills and fever occur several times daily; at night, only fever occurs; bitter taste in mouth (Kouku) and thirst (Fake); headache and eye pain (Toumu Ju Tong). Pulse diagnosis: left wiry (Zuo Xian), right long (You Chang); chest fullness (Xiongmen) and back stiffness (Bei Ban). This is after the Autumn Equinox, with latent pathogen (Fuxie) attempting to vent, but bound by new cold (Xinhan). It resembles malaria but is not malaria, belonging to the combined disease of the three yang meridians (Sanyang Hebing), thus treated comprehensively.
Prescription: Chai Hu (Bupleuri Radix, 1 qian), Ge Gen (Puerariae Lobatae Radix, 1 qian), Zi Su (Perillae Folium, 1 qian), Qiang Huo (Notopterygii Rhizoma et Radix, 1 qian), Huang Qin (Scutellariae Radix, 1.5 qian), Ju Pi (Citri Reticulatae Pericarpium, 1 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma, 0.4 qian), Lao Jiang (Zingiberis Rhizoma Recens, 2 slices), Da Zao (Jujubae Fructus, 2 pieces).
After taking the medicine, sweat poured out like steaming, and all symptoms relaxed; upon re-taking, sweat was similar, and external symptoms completely receded. Slight residual fever remained, with bitter taste and thirst, due to excessive sweating (Hanxie Taiguo).
Prescription: Sheng Di (Rehmanniae Radix, 4 qian), Mai Dong (Ophiopogonis Tuber, 3 qian), Huang Qin (Scutellariae Radix, 1.5 qian), Hua Fen (Trichosanthis Radix, 1.5 qian), Chuan Hu (Dendrobii Caulis, 3 qian), Qing Hao (Artemisiae Annuae Herba, 1 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma, 0.4 qian), Nan Zao (Jujubae Fructus, 2 pieces), Fu Xiao Mai (Tritici Fructus Immaturus, 2 qian).
Cured after two doses.
[Case 30] Monk, aged fifty-eight. Sudden chills and sudden heat (Shu Han Shu Re); coughing and wheezing with shortness of breath (Keqiang Qiji); cold sweat (Lenghan). Previous physicians mistakenly treated it as deficiency, administering Ren Shen (Ginseng Radix) and Shu Di (Rehmanniae Radix Preparata), resulting in coma and delirium (Shenhun Zhanyu).
A visiting monk, Master Jianyuan, knowing my medical principles were clear, invited me by boat. Upon palpating the six positions (Liubu), he wished to determine life or death immediately. The pulse was floating and wiry (Fu Xian), indicating wind pathogen lodging in the lung (Fengxie Ke Fei), causing qi stagnation and inability to vent, leading to shortness of breath and cold sweat. The patient stated that initially there was vomiting, diarrhea, fever, and thirst, with scanty and difficult urination (Xiaobian Duanse). It should have been treated with diuresis and separation (Fenli), why rush to tonify? If the condition were truly deficient, why did Ren Shen and Shu Di exacerbate the pathogen? Light agents should be used to unblock lung qi.
Prescription: Xing Ren (Armeniacae Semen Amarum), Xiang Chi (Sojae Semnis Praeparatum), Bo He (Menthae Haplocalycis Herba), Qian Hu (Peucedani Radix), Lian Qiao (Forsythiae Fructus), Shan Zhi (Gardeniae Fructus), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Tong Cao (Tetrapanacis Medulla).
Cured after two doses.
[Case 31] Patient Sheng, aged thirty-one. Toothache and neck swelling (Chitong Jingshong); crimson tongue (She Jiang); alternating chills and fever. Belongs to wind pathogen and blood heat (Fengxie Xure).
Prescription: Chai Hu (Bupleuri Radix), Bo He (Menthae Haplocalycis Herba), Gan Ju (Chrysanthemi Flos), Man Jing Zi (Viticis Semen), Gui Shen (Angelicae Sinensis Radix), Xiao Sheng Di (Rehmanniae Radix Preparata), Shao Yao (Paeoniae Radix), Dan Pi (Moutan Cortex), Xiang Fu (Cyperi Rhizoma), Gan Cao (Glycyrrhizae Radix et Rhizoma).
Cured after three doses.
[Case 32] Patient Zhang, aged fifteen. In mid-summer, sudden chills and heat; pulse floating and large (Mai Fuda); spontaneous sweating (Zihan); white tongue coating (She Bai); reddish stool (Bian Chi); heavy head and weak limbs (Touzhong ZhiRuan). Disease is in the upper jiao (Shangjiao), suitable for clearing and dispersing (Qingshu).
Prescription: Xing Ren (Armeniacae Semen Amarum), Dan Dou Chi (Sojae Semnis Preparatus), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Tong Cao (Tetrapanacis Medulla), Lian Qiao (Forsythiae Fructus), Huang Qin (Scutellariae Radix), Bo He (Menthae Haplocalycis Herba), Gan Cao (Glycyrrhizae Radix et Rhizoma).
After one dose, cough started; after two doses, red spots appeared on the body; cured after continuous administration. The previous condition was warm pathogen lodging in the lung, so when the pathogen attempted to vent, it manifested as cough and rash.
[Case 33] Patient Lu, aged thirty-six. Poor appetite and emaciation (Na Shao Xing Xiao); irregular chills and fever (Hanre Wu Shi); rapid and soft pulse (Mai Shu Er Ruan); white tongue coating (She Bai); reddish stool (Bian Chi); insomnia and bad breath (Shaomei Kouhao). This is latent fire syndrome (Fuhuo Zheng). It should be treated after Li Chun (Start of Spring) and before Jingzhe (Awakening of Insects), when yang qi rises and latent pathogen attempts to vent, suitable for clearing and dispersing.
Prescription: Chuan Lian (Coptidis Rhizoma), Tiao Qin (Scutellariae Radix), Chuan Bai (Phellodendri Cortex), Xing Ren (Armeniacae Semen Amarum), Xiang Chi (Sojae Semnis Praeparatum), Zi Su (Perillae Folium), Fang Feng (Saposhnikoviae Radix).
[Case 34] Female Patient Wang, aged six. Daily chills and fever for two months without cure. During attacks, severe abdominal pain (Futong Nanjin); gums and nails pale and lusterless (Yarou Yu Zhijia Candan Wuhua); spirit listless (Shenqi Liaodao). This is commonly known as fetal malaria (Taimie). Previous doctors alternated between tonifying and clearing, lacking consistent judgment. In my view, ancient sages mostly treated malaria from the Shaoyang perspective, but this case involves pathogen entangling the Mu Yuan (Sea of Origin) and Taiyin suffering, evidenced by abdominal pain. Some say: Shaoyang also has abdominal pain. I say: Shaoyang abdominal pain has clear discussions in Zhang Zhongjing's works, but this pulse is small and soft, lacking any wiry quality, so medical skill requires flexibility. Please let me prescribe, and it will prove true.
Prescription: Ren Shen (Ginseng Radix, 0.6 qian), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 1 qian), Gui Shen (Angelicae Sinensis Radix, 1 qian), Cao Guo (Tsaoko Fructus, 0.5 qian), Bai Shao (Paeoniae Radix Alba, 1 qian), Chai Hu (Bupleuri Radix, 0.4 qian), Ban Xia Qu (Pinelliae Rhizoma Fermentatum, 1.5 qian), Ju Pi (Citri Reticulatae Pericarpium, 0.8 qian), Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma, 0.4 qian), Wei Jiang (Prepared Zingiberis Rhizoma, 3 slices), Da Zao (Jujubae Fructus, 2 pieces).
Cured after one dose.
[Case 35] Monk Dachan. Fever, chest irritability and vomiting (Xiongfan Outu), back pain without aversion to wind (Beitong Bu E Feng), pulse wiry and forceful (Mai Lai Xian Er Youli). Symptoms all belong to warm malaria (Wen Nue). Used Xiao Chai Hu Tang removing Ren Shen, adding Zhu Ru (Bambusae Sap) and Gua Lou Pi (Trichosanthis Pericarpium) to remove chest irritability and heat. One dose ineffective, vomiting worsened, added Shi Gao (Gypsum Fibrosum, 5 qian). One dose no progress, palpitations (Zhengchong) recurred, heart shaking incessantly. Immediately conceived a formula, treating palpitations as the primary symptom, causing mental confusion. Due to lack of protective function of the Pericardium (Baoluo), pathogenic heat invaded the citadel (Heart) via deficiency, disturbing the Sovereign. Tonification is contraindicated; only clearing Ding Fire (Heart Fire) is appropriate.
Prescription: Chuan Lian (Coptidis Rhizoma, 0.8 qian), Xiao Sheng Di (Rehmanniae Radix Preparata, 3 qian), Mu Tong (Akebiae Caulis, 1 qian), Gan Cao Shao (Glycyrrhizae Radix et Rhizoma Tip, 0.4 qian), Lian Qiao (Forsythiae Fructus, 1.5 qian), Shan Zhi (Gardeniae Fructus, 1.5 qian).
Heart condition gradually stabilized after taking.
Next day examination, pulse still wiry and large, disease worsening, coma and delirium. Carefully asked about discomfort, gently pressed abdomen, reaching left side of umbilicus, body showed resistance. This indicates pain and fear of pressure, clearly pathogen has entered interior (Ru Li). Realized that absence of headache and aversion to wind, yet presence of vomiting, irritability, and anorexia, also belongs to Shaoyang marginal symptoms. Abandoning Da Chai Hu Tang method, no other prescription available.
Prescription: Da Huang (Rhei Radix et Rhizoma, 4 qian, added late), Shi Gao (Gypsum Fibrosum, 5 qian), Mang Xiao (Natrii Sulfas, 1.5 qian), Chai Hu (Bupleuri Radix, 1 qian), Huang Qin (Scutellariae Radix, 1.5 qian), Ban Xia (Pinelliae Rhizoma, 1.5 qian), Ju Pi (Citri Reticulatae Pericarpium, 1 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma, 0.4 qian).
Monk's relative serving medicine saw the harsh prescription and dared not take it lightly. Living in Fengxi, where a spirit-writing medium was very effective, they consulted. The medium produced a formula matching mine in herbs and dosages, except adding Hou Po (Magnoliae Officinalis Cortex, 0.8 fen) and Zhu Ye (Bambusae Folium, 10 leaves). Returned home, decocted and took, cured after two doses.
[Case 36] Yu, aged forty-four. Both pulses wiry and large (Liang Mai Xian Da), aversion to wind (E Feng), chills and fever, yellow tongue and dry teeth (She Huang Chi Zao), thirst. Severe summerheat malaria (Shu Nue). Xiao Chai Hu Tang removing Ren Shen, adding Shi Gao (Gypsum Fibrosum), Xing Ren (Armeniacae Semen Amarum), Hua Shi (Talcum), cured in two patches.
[Case 37] Wang, aged twenty-two. Early malaria should harmonize and clear Shaoyang. Why give Zhu Sha (Cinnabar), Hu Po (Amber), Long Chi (Dragon's Teeth) for gallbladder fear and heart palpitations? What benefit to the disease! Does one not know Shaoyang meridian belongs to Gallbladder? For a month, not only pathogen not vented, but rich food taken as tonification, causing phlegm-saliva solidifying in stomach, chills and fever worse, coma and delirium, pathogen entering Yangming. Proposed dual harmonization method.
Prescription: Ban Xia (Pinelliae Rhizoma), Ju Pi (Citri Reticulatae Pericarpium), Hou Po (Magnoliae Officinalis Cortex), Gua Lou Pi (Trichosanthis Pericarpium), Fu Ling (Poria), Chai Hu (Bupleuri Radix), Huang Qin (Scutellariae Radix), Gan Cao (Glycyrrhizae Radix et Rhizoma).
Cured in two doses.
[Case 38] Wu, aged thirty-two. Fever for ten days, dry cough incessant, white and rotten tongue coating (She Tai Bai Fu), frequent loose stools, pulse neither floating nor sinking, wiry and forceful. This is Lung Malaria (Fei Nue).
Prescription: Xing Ren (Armeniacae Semen Amarum, 3 qian), Ju Pi (Citri Reticulatae Pericarpium, 1.5 qian), Jie Geng (Platycodonis Radix, 0.5 qian), Qing Hao (Artemisiae Annuae Herba, 1 qian), Lian Qiao (Forsythiae Fructus, 1.5 qian), Gua Lou Ren (Trichosanthis Semen, 2 qian), Zhu Ling (Polyporus, 1 qian), Fu Ling (Poria, 2 qian).
Two doses caused sweating, fever subsided, cough stopped. Although the two seeds are slippery, "when there is disease, the disease bears it."
[Case 39] Zhang, aged twenty-eight. Fever and cough, lighter during day, heavier at night, pulse floating, wiry, and rapid (Mai Fu Xian Er Shu). This is Hand-Taiyin Malaria (Shou Taiyin Nue), should treat from Lung.
Prescription: Xing Ren (Armeniacae Semen Amarum, 3 qian), Qian Hu (Peucedani Radix, 1.5 qian), Ju Hong (Citri Reticulatae Pericarpium Rubrum, 1 qian), Huang Qin (Scutellariae Radix, 1.5 qian), Chai Hu (Bupleuri Radix, 0.8 qian), Ban Xia (Pinelliae Rhizoma, 1.5 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma, 0.4 qian).
[Case 40] Fang, aged twenty-four. Triadic malaria for over a year, cold binds, heat scorches muscles, should harmonize Ying and Wei (Yingwei).
Prescription: Chai Hu (Bupleuri Radix, 1 qian), Gui Zhi (Cinnamomi Ramulus, 0.4 qian), Huang Qin (Scutellariae Radix, 1.5 qian), Ban Xia (Pinelliae Rhizoma, 1.5 qian), Ju Pi (Citri Reticulatae Pericarpium, 1 qian), Gui Shen (Angelicae Sinensis Radix, 1.5 qian), Hou Po (Magnoliae Officinalis Cortex, 1 qian), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 1.5 qian), Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma, 0.4 qian), Lao Jiang (Zingiberis Rhizoma Recens, 3 slices), Da Zao (Jujubae Fructus, 2 pieces).
Three doses reduced chills and fever, appetite improved. Should tonify Ying and Wei, use Bu Zhong Yi Qi Tang modified, following Lizhai's method.
Prescription: Dang Shen (Codonopsis Radix, 2 qian), Zhi Bai Zhu (Prepared Atractylodis Macrocephalae Rhizoma, 2 qian), Huang Qi (Astragali Radix, 2 qian), Gui Shen (Angelicae Sinensis Radix, 1.5 qian), Ju Pi (Citri Reticulatae Pericarpium, 1 qian), Fu Ling (Poria, 1.5 qian), Ban Xia (Pinelliae Rhizoma, 1.3 qian), Chai Hu (Bupleuri Radix, 0.4 qian), Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma, 0.5 qian), Lao Jiang (Zingiberis Rhizoma Recens, 3 slices), Da Zao (Jujubae Fructus, 2 pieces).
Four doses, disease disappeared unknowingly.
[Case 41] Wang, aged fifty-six. Triadic malaria for years, headache before onset, sweating during resolution, abdominal distension after eating, occasional abdominal pain and cough. Long illness causes Qi and Blood deficiency, Ying-Wei disharmony. Without multiple tonics, the disease demon is hard to dispel. Superstitious belief in shamans, disbelief in medicine.
Prescription: Ren Shen (or intermittent Dang Shen), Yu Zhu (Atractylodis Rhizoma), Huang Qi (Astragali Radix), Gui Shen (Angelicae Sinensis Radix), Ju Pi (Citri Reticulatae Pericarpium), Chai Hu (Bupleuri Radix), Ge Gen (Puerariae Lobatae Radix), Hong Hua (Carthami Flos), Ban Xia (Pinelliae Rhizoma), Shen Qu (Massa Medicata Fermentata), Yi Zhi Ren (Alpiniae Oxyphyllae Fructus), Bai Shao (Paeoniae Radix Alba), Sha Ren Ke (Amomi Fructus Shell), Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma), Jiang (Zingiberis Rhizoma), Zao (Jujubae Fructus).

[Case 43] Monk, aged forty-four. Malaria changed to single fever, insomnia at night, bitter taste and vomiting, lasting a month, no appetite. Condition is heavy, but pulse has root, no worry. Treat as malaria pathogen entering blood level.
Prescription: Xiao Sheng Di (Rehmanniae Radix Preparata), Gui Shen (Angelicae Sinensis Radix), Hua Shi (Talcum), Chai Hu (Bupleuri Radix), Huang Qin (Scutellariae Radix), Ban Xia (Pinelliae Rhizoma), Ju Pi (Citri Reticulatae Pericarpium), Gan Cao (Glycyrrhizae Radix et Rhizoma), Jiang Pi (Zingiberis Pericarpium).
One dose fever subsided, vomiting continued. Due to stomach deficiency, Mi Yin (Rice Decoction) mixed with Ren Ru (Human Milk) half cup each, taken and settled. This is Hua Xian's formula.
[Case 44] Shen, aged nineteen. Malaria (i.e., San Yin Nue/Triad Malaria). Onset, pulse deep and choppy (Chen Se), severe chills and fever. This condition is most common among farmers, mostly starting in late autumn. Due to latent summerheat inside, exterior wind-cold, food intake causing spleen-stomach dysfunction, generating phlegm-saliva, forming this condition. Treatment must distinguish predominant pathogen, body constitution thickness, to weigh herb strength. Observe depth of pathogen and deficiency/excess of Yuan Qi to weigh thorough clearing and tonification. Long-term recurrence transmits to five zang organs, requiring careful pulse and history. Follow ancient sages, treat separately, why suffer lingering disease? Secular view calls it incurable for two or three years.
Prescription: Shi Gao (Gypsum Fibrosum, 3 qian), Zhi Mu (Anemarrhenae Rhizoma, 1.5 qian), Huang Qin (Scutellariae Radix, 1.5 qian. These three clear internal summerheat), Chai Hu (Bupleuri Radix, 1 qian), Su Ye (Perillae Folium, 1.5 qian), Fang Feng (Saposhnikoviae Radix, 1.5 qian. These four cleanse stomach phlegm), Gan Cao (Glycyrrhizae Radix et Rhizoma, 0.4 qian), Jiang Pi (Zingiberis Pericarpium, 0.4 qian), Da Zao (Jujubae Fructus, 2 pieces, to harmonize Ying-Wei).
Cured in four doses. This formula treats excess pathogen in strong constitution, showing precise formulation avoids empty prescriptions.
[Case 45] Wu, aged twenty-two. Daily malaria lingering, fifty days uncured, much phlegm, bitter taste, glossy tongue and thirst, fine and rapid pulse. Caused by latent summerheat, stomach loss of transport. Normally yin-deficient constitution, plus long fever. Gui and Di nourishing blood seems correct, but path is divergent. Summerheat enters via nose, is formless; tonifying/nourishing brings no benefit, only harm.
Prescription: Xing Ren (Armeniacae Semen Amarum), Xiang Chi (Sojae Semnis Praeparatum), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Tong Cao (Tetrapanacis Medulla), Lian Qiao (Forsythiae Fructus), Hua Shi (Talcum), Qing Hao (Artemisiae Annuae Herba), Gan Cao (Glycyrrhizae Radix et Rhizoma), Xi Gua Cui (Watermelon Rind).

Method: raise Yang and scatter cold, clear summerheat and remove heat, aromatic-dry regulate stomach. Use Zhu Ling (Polyporus) intent to prevent Yin-Yang intermixing. Seek source of disease: emission due to Water-Fire non-interaction. Astringents close pathogen, worsening disease. Harmonize Yin-Yang, all symptoms heal.
