Zhao Zisheng from Haiyang contracted malaria in the summer of Xinmao year and urgently invited Meng Ying for diagnosis. Meng Ying said, "This is caused by summer-heat pathogen; one cannot rigidly adhere to Xiao Chai Hu Tang (Minor Bupleurum Decoction)." He prescribed Bai Hu Tang (White Tiger Decoction), and the illness resolved after a single dose.
In the autumn of Jiawu year, Fan Limen suffered from warm malaria (Wen Nue). Meng Ying cured him using Bai Hu Jia Gui Zhi Tang (White Tiger Decoction with Cinnamon Twig).
In the summer of Bing Shen year, Sheng Shaoyun suffered from damp-heat malaria (Shi Re Nue). Meng Ying stabilized his condition with Bai Hu Jia Cang Zhu Tang (White Tiger Decoction with Atractylodes Rhizome).
In the summer of Jihai year, my aunt contracted malaria. After taking two or three doses of Xiao Chai Hu Tang (Minor Bupleurum Decoction), she sweated, fainted, spoke deliriously, and lost bladder control. Some attributed this to her constitutionally weak state and feared a collapse syndrome (Tuo Bian), advising Dushen Tang (Single Ginseng Decoction). Fortunately, my cousin-in-law Shou did not dare administer it hastily but invited Meng Ying to consult. Upon palpating the pulse, which was surging, large, slippery, and rapid, Meng Ying stated, "This is Yangming summer-heat malaria, consistent with the pattern of combined disease of the three yang meridians in Cold Damage." He prescribed Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction), and she recovered after two doses.
In the summer of Gengzi year, Huang Xiaonong from Yunnan traveled from Fuqing to the capital via Wulin and contracted summer-heat malaria. Meng Ying administered Bai Hu Tang (White Tiger Decoction) with added Xi Yang Shen (American Ginseng), and he healed after several doses. In the autumn of Xinchou year, a family member of Gu Weiwu suffered from dan malaria (Dan Nue), and Meng Ying also used this formula with success.
Zhang Anren, wife of Zhonghan Zhuang Zhijie, was over sixty years old and suffered from intense malaria fever. After careful examination, Meng Ying prescribed Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction), stabilizing her condition. Those who heard of this were amazed. I remarked that in these cases, constitutions vary by north and south, and strength differs; without clear differentiation of the syndrome, how could the medicine match the disease and be appropriate?
In the autumn of Guisi year, I contracted malaria in Wuzhou and was misdiagnosed by physicians. Initially treated with exterior-releasing methods, then with tonification. After more than a month, my muscles wasted away, chills and fever persisted, vomiting occurred frequently, appetite was poor, urine was red, and I felt cold. I returned to Hangzhou by boat and entrusted myself to Meng Ying. He diagnosed "Taiyin damp malaria" (Zu Taiyin Shi Nue) and prescribed Bu Huan Jin Zheng Qi San (Unchanging Metal Regulate Qi Powder), resolving the issue after three doses.
Shi Fusheng came to Zhejiang from Sichuan with his father; both contracted malaria simultaneously. Physicians prescribed Xiao Chai Hu Tang (Minor Bupleurum Decoction) with Ginger and Cinnamon Twig, which failed. Switching to "Four Beasts" or "Stop Malaria" methods worsened their chills, prevented food intake, and led them to drink hot ginger wine and sit near fires wrapped in heavy furs. They experienced chest fullness, preferred heat application, yet felt rising cold air, and vomited thick sticky phlegm. By early winter, they were exhausted. Recalling the Bing Shen case, they visited Meng Ying. His pulse was deep, slippery, and rapid; tongue coating was yellow and greasy without thirst; stool was loose, urine red. Meng Ying explained, "This is summer-dampness acquired during travel, neglected by clearing and resolving, then aggravated by warming and tonifying herbs, creating phlegm-fluid retention stagnating in the Triple Burner, blocking qi mechanism. Hence the preference for heat feels like ice internally. Without analyzing the cause, relying only on listening and questioning before pulse-taking leads to misdiagnosis of true cold. Now, combining pulse and symptoms reveals true heat with false cold." I prescribed strong bitter-cold herbs decocted with Radish (Lu Fu), gradually reducing chills, decreasing phlegm, and increasing appetite. Follow-up with sweet-cool methods ensured full recovery for father and son.
Wang Jizai suffered from chronic malaria unresponsive to treatment. Physicians claimed deficiency of primordial qi and used various warming tonics. This led to emaciation, internal burning, dry mouth, cough, night sweats, red urine, and loss of taste. Meng Ying diagnosed "Heat pathogen lingering in the blood level" and prescribed Qin Jiao Bie Jia San (Gentian and Turtle Shell Powder), leading to recovery.
My elder cousin Wang Yangyu suffered from malaria for months with no effect from multiple treatments. He had emaciation, spontaneous sweating, soreness and weakness of waist and knees, inability to sit, and extreme aversion to cold. Meng Ying asked, "This is a major deficiency syndrome; why not tonify? Using digestion-promoting methods, what is the intention?" He prescribed Ren Shen (Ginseng), Huang Qi (Astragalus), Bai Zhu (Atractylodes Macrocephala), Zhi Cao (Honey-fried Licorice), Shu Di (Prepared Rehmannia), Bai Shao (White Peony Root), Wu Wei Zi (Schisandra), Du Zhong (Eucommia), Shan Yao (Dioscorea), Long Gu (Fossilized Bone), Mu Li (Oyster Shell), Sang Zhi (Mulberry Twig), Da Zao (Jujube), and Mu Gua (Chaenomeles), resulting in recovery after dozens of doses.
Gu Yun, obese and elderly, contracted malaria in autumn. Pulse was hollow (Kou) with occasional pauses. Meng Ying stated, "Hollow indicates summer-heat; pauses indicate phlegm-damp obstructing qi flow." Strictly forbade warming tonics to avoid aiding pathogens. Clearing and resolving with phlegm-dispelling methods yielded little improvement, followed by bloody stool. Meng Ying reasoned, "Summer-dampness is无形 (formless), but chronic phlegm allows pathogens to lodge, resembling a formless disease. Clearing alone fails; qi and blood are disturbed. Must attack the phlegm to remove the pathogen's basis. Do not fear harsh drugs due to age." His nephew Gui Sheng, a skilled physician, agreed. Formula: Tao Ren Cheng Qi Tang (Peach Kernel and Major Order the Qi Decoction) with Xi Yang Shen (American Ginseng), Hua Shi (Talc), Huang Qin (Scutellaria), Huang Lian (Coptis), Ju Hong (Tangerine Peel), Bei Mu (Fritillaria), Shi Hu (Dendrobium), delivered with Meng Shi Gun Tan Wan (Green Stone Rolling Phlegm Pill). After two doses, massive discharge of sticky phlegm and dirty blood occurred; malaria ceased. Follow-up with moistening methods ensured health.
Yu Jieshan, Vice Magistrate of Suian, contracted malaria while traveling to the capital via Wulin. Fan claimed it was spring cold requiring acrid-warm release; Lai attributed it to alcohol-dampness treating with Wu Ling San (Five Ingredient Powder with Poria), adding Ginseng, Angelica, Ginger, and Jujube, worsening the condition. Ten days later: epigastric fullness, abdominal distension, constipation, rebellious qi, irritability, thirst, sweating, coma, and insomnia. Meng Ying diagnosed, "Dampness accumulation exists, but warm wind invaded externally, transforming into heat. Why assume heat malaria only occurs in summer/autumn? Clearing and resolving will work in ten doses." It proved effective, curing him in ten days.
During extreme heat, malaria was prevalent. Chu Libo suffered; Lu relied on heavenly luck theory (Cold Water ruling heaven, Wet Earth ruling earth, wet transformation), prescribing Ping Wei San (Calm the Stomach Powder) and Li Zhong Wan (Regulate the Middle Pill), leading to critical danger. Xu Hepu recommended Meng Ying. Signs: intense heat, coma, high chest, rebellious qi, ginger-yellow tongue coating, jasper-red urine, hidden pulse, thirst, anorexia, constipation. Meng Ying criticized, "Ignoring current summer-heat pathology for theoretical weather patterns is binding dead books to困 (trap) living people." Given his robust constitution and yin sufficiency, he prescribed Bai Hu Tang (White Tiger Decoction) for ten doses. Ingredients: Sheng Shi Gao (Raw Gypsum), Zhi Mu (Anemarrhena), Yin Hua (Honeysuckle), Zhi Shi (Immature Bitter Orange), Bei Mu (Fritillaria), Huang Lian (Coptis), Mu Tong (Akebia), Hua Fen (Trichosanthes Root), Zhu Ru (Bamboo Shavings), Huang Qin (Scutellaria), Xing Ren (Apricot Seed), Shi Hu (Dendrobium), Hai Zao (Sargassum), Zhu Ye (Bamboo Leaf). Alternating formulas cured malaria decisively in ten days.
Nephew Zhuang Diqing suffered malaria with extreme thirst preferring hot drinks, epigastric fullness, hidden pulse, greasy coating, and nausea. Meng Ying diagnosed, "Internal dampness excess, external summer-heat invasion. Should clear and resolve, but pulse/symptoms indicate phlegm obstructing airway. Clearing is useless; warming aids malaria. Use Meng Shi Gun Tan Wan (Green Stone Rolling Phlegm Pill) first to guide." After dosing, massive phlegm discharge occurred; pulse became wiry-slippery-rapid; vomiting stopped, chest relaxed; tongue coating turned yellow-dry. Prescribed: Shi Gao (Gypsum), Zhi Mu (Anemarrhena), Huang Lian (Coptis), Xing Ren (Apricot Seed), Nong Po (Magnolia Bark), Ju Pi (Tangerine Peel), Ban Xia (Pinellia), Fu Ling (Poria), Hua Shi (Talc), Shi Hu (Dendrobium), Chang Pu (Acorus), Hua Fen (Trichosanthes Root), resolving the condition.
Zhuang Xiaocun, nephew of Zhijie's sister-in-law, worked as a tutor at Jin Yuangu's residence and contracted malaria. Meng Ying said, "Inhaling summer-heat clears upon purification." Days later, severe malaria with red eyes, delirium, and profuse sweating occurred. The employer panicked, suspecting cooling herbs were wrong. Meng Ying arrived during manic behavior. Pulse was surging-slippery without limit; tongue deep yellow-concentrated-dry. Suspecting other meds, he smelled ginger-jujube aroma and asked, "Did you take Ginger-Jujube Decoction?" Answer: "Drank freely for three days." Meng Ying ordered a watermelon cut open for the patient to eat, plus Bai Hu Tang (White Tiger Decoction) with 1 liang 6 qian of Sheng Shi Gao (Raw Gypsum). Recovery was immediate. He died six years later of another illness.
Chen Yangshan suffered malaria; Meng Ying repeatedly used summer-heat clearing methods with no reduction. Suspecting Ginger-Jujube Decoction again, confirmed it. Advised cessation; cure followed. Other cases like Wang Zikuan, Wei Zhishang, Hu Qiuren, etc., treated for summer-malaria with modified Bai Hu Tang (White Tiger Decoction), are omitted here for readers to infer similar principles.
He Yongchang, a friend of Meng Ying, sought help for his wife's malaria occurring every two days. His mother refused "official prescription medicines," using folk notes instead. Ten days later, condition critical. Yongang begged Meng Ying. Pulse: deep-thin-rapid, worse at Chi position. Thirst, eyes unwilling to open, lower back pain like needle pricks, less chills more heat, palpitations. Meng Ying said, "Strange disease. Summer-heat entering Foot-Shaoyin. Yu's method states sweating, purging, and warming are all contraindicated. If elsewhere, you might doubt me; since your wife seeks me, fate intervenes. Your mother says 'official meds bad for malaria'—from where? If official meds are banned, private ones are even more dangerous, ridiculous. Yet if delayed to others, they would likely use exterior-release or warming-tonic, arguably your mother's unintended gain." Prescription: Yuan Shen (Scrophularia), Gui Ban (Turtle Shell), Shi Hu (Dendrobium) each 1 liang; Di Gu Pi (Lycium Bark) 6 qian; Zhi Mu (Anemarrhena) 5 qian; Sang Ye (Mulberry Leaf), Jin Yin Hua (Honeysuckle) each 4 qian; Hua Fen (Trichosanthes Root) 3 qian; Dan Pi (Moutan) 2 qian. Decoct in large pot, frequent sips, no dose limit. After three days, malaria ceased, symptoms reduced, porridge intake increased, recovered without further meds.
Zhou suffered malaria every two days, less chills more heat. Doctor diagnosed "Elderly Three-Day Malaria" and used warming tonics freely. Gradually stopped eating. Relative Li Shiquan invited Meng Ying. Pulse: thin-hard like string, no stomach qi; Right-Chi surging-rapid; tongue glossy-crimson; extreme thirst, scant urine. Meng Ying said, "Foot-Shaoyin summer-heat malaria." Extensive warming tonics depleted fluids; herbs cannot regenerate life now. World knows three dangers of poor malaria treatment: pathogen staying in Liver collateral becomes Malaria Mass; damaging Spleen origin becomes Malaria Distension; consuming Kidney yin becomes Malaria Consumption. Here, meds aided pathogen, threatening life; turning back is impossible. Heat intensified by tonics, yin scorched to death, yin depletion fuels pathogen—like roasting punishment. How pitiable! Died next day. Recorded as warning.
Sheng Mozhuang contracted intermittent malaria in winter. Due to abdominal distension and cold aversion, self-administered Shen Qu Ginger Soup, worsening condition. Meng Ying diagnosed, "Summer-dampness latent inside." Formula: Huang Lian (Coptis), Zhi Shi (Immature Bitter Orange), Nong Po (Magnolia Bark), Shan Zhi (Gardenia), Huang Qin (Scutellaria), Xing Ren (Apricot Seed), Bei Mu (Fritillaria), Zhi Mu (Anemarrhena), Shi Hu (Dendrobium), Xuan Fu (Inula), Ju Pi (Tangerine Peel), Lan Cao (Orchid Stem). Decocted in Radish soup. Recovered after three doses.
Yao Xiaoheng Ling suffered malaria, mild chills severe heat, twice daily. Wang gave Chai Hu Tang (Bupleurum Decoction) for two doses, worsening condition. Tongue crimson, extreme thirst, no urination. Meng Ying said, "Fluids about to dry up." Prescribed: Xi Yang Shen (American Ginseng), Sheng Di (Rehmannia), Zhi Mu (Anemarrhena), Hua Fen (Trichosanthes Root), Shi Hu (Dendrobium), Mai Dong (Ophiopogon), Zhi Zi (Gardenia), Bai He (Lily Bulb), Zhu Ye (Bamboo Leaf). Five doses stopped malaria.

One dose cured. Many such cases; recorded briefly.
Qiao Younan, 39 years old, suffered Pin Malaria (Cold-predominant) for 20 days, ineffective treatment. Relative Xu Hepu suspected latent summer-heat, invited Meng Ying. Pulse faint-no spirit, fatigue lying down, constipation half month, red urine, anorexia, excessive phlegm, dry mouth, needs chest/back rubbing to swallow rice drink. Tongue black-greasy with mossy appearance. Meng Ying said, "Essence, Qi, Spirit all severely deficient. Cannot compare with seasonal latent summer-heat late onset. Previous doctor's method focused on moving middle, causing no great harm." Formula: Ren Shen (Ginseng), Bai Zhu (Atractylodes), Rou Gui (Cinnamon Bark), Fu Pian (Aconite Slice), Chen Xiang (Aquilaria), Stir-fried Shu Di (Prepared Rehmannia), Lu Jiao Jiao (Deer Antler Glue), Shi Ying (Quartz), Rou Cong Rong (Cistanche), Gou Qi (Goji), Dang Gui (Angelica), Fu Ling (Poria), Du Zhong (Eucommia), Zao Ren (Jujube Seed), Tu Si Zi (Dodder Seed), Shan Zhu Yu (Cornus), Ju Pi (Tangerine Peel), Xia Tian Qu (Fermented Wheat), Hu Tao Rou (Walnut Meat). Seven more doses passed black-dry stool. Not defecating for 40 days. Chills/fever ceased, ate well, urine clear, fully recovered.
Someone asked why Meng Ying criticizes warming tonics yet uses them heavily. Meng Ying replied, "Warming tonics are a valid method; cannot abolish. Just used less. Doctors who don't recognize disease use this to flatter the rich, killing countless people. Turning life-saving ancient prescriptions into world-harming drugs—how hateful!"
In September, Zhang Chunqiao suffered malaria, less chills more heat, every two days. After two attacks, emaciated. Meng Ying diagnosed: Wiry-thin pulse, very rapid at Chi, onset at midnight, dry mouth craving drink. "Foot-Shaoyin Heat Malaria." Rapid emaciation shouldn't be ignored. "I offer excellent medicine; expect immediate stop. Take immediately, do not consult others to create doubt." Formula: Yuan Shen (Scrophularia), Sheng Di (Rehmannia), Zhi Mu (Anemarrhena), Dan Pi (Moutan), Di Gu Pi (Lycium Bark), Tian Dong (Asparagus Root), Gui Ban (Turtle Shell), Fu Ling (Poria), Shi Hu (Dendrobium), Sang Ye (Mulberry Leaf). Chunqiao trusted him, took it. One dose stopped malaria; follow-up with nourishing yin cured. I say one-dose cure seems easy, but requires true talent. Others would use warming tonic or lifting exterior, causing disaster. Regardless of severity, initial correct treatment is key. Why let mild become severe?世俗 sentiment requires severity before appreciating medical merit—sigh. People know it as Three-Yin Malaria, treat generally with warming tonics; rarely differentiate by meridian. Now specifying Shaoyin, creating cool formula, precise medication, quick effect, novel method, complete logic. Living person treating living disease, operating flexibly. Inspires wisdom, sets example.

Diagnosed "Summer-heat Malaria." Gui Zhi Bai Hu Tang is good at onset, but now warming/drying meds aided pathogen and scorched fluids. Pulse extremely rapid, sweating/thirst unceasing. Although Bai Hu Tang (White Tiger Decoction) is indicated, adding Gui Zhi (Cinnamon Twig) aids heat and consumes fluids, hindering oneself. Prescribed: Large dose Bai Hu Tang (White Tiger Decoction) with Hua Fen (Trichosanthes Root), Zhu Ye (Bamboo Leaf), Xi Yang Shen (American Ginseng), Yuan Shen (Scrophularia), Shi Hu (Dendrobium). Immediate relief. After ten doses, malaria ceased, but tongue lacked coating, thirst continued. Sweet-cool moistening for thirty doses finally cured.
