The *Jiyan Lun* (Collected Proven Experiences) records "Huan Ju" (Slow Carbuncle), which initially presents as a swelling resembling a carbuncle, without distinct head or tail, and the color is unchanged. However, the pain is deep with a rooted hard nodule, adhering closely to the skin and flesh, leading to simultaneous ulceration.
Fan Wang's Feihuang San (Flying Yellow Powder) treats Huan Ju and malignant sores, eliminating necrotic tissue.
Take Cinnabar (Dan Sha) in a clay pot, Southern Realgar (Nan Cihuang), Magnetite (Ci Shi) in the center, Azurite (Zeng Qing) in the north, White Quartz (Bai Yingshi) in the east, [Missing Character] Stone in the west, Gypsum (Shi Gao) on top, followed by Stalactite (Zhong Ru), and Orpiment (Xiong Huang) below, covered by Mica (Yun Mu), with thin cloth underneath. Each ingredient should be two liang. First, pound and sieve them in the clay pot. Cover one pot with another, seal thickly with wool mud, construct a three-cornered stove, and burn it to...
Also for treating Huan Ju, use Feihuang San to eliminate necrotic tissue completely, then fumigate using an earthen stove.
Orpiment (Xiong Huang, 1 liang), Chicken White Excrement (Ji Baishi, 1 liang), Veratrum (Lilu, 1 liang), Cinnabar (Dan Sha, 1 liang), Dried Eel (Gan Manli Yu, 1 liang).
Pound these five ingredients into powder, wrap in green cloth, fumigate for three days then stop. After stopping, massage with She Xian Gao (Snake Tongue Ointment) for best results. (The simplified Fan Wang formula lacks She Xian Gao; Cui Shi's formula is attached later, both from Volume 31).
Cui Shi's She Xian Gao treats abscesses, blood stasis, postpartum blood accumulation, dim vision, and "Cow Neck Saddle Sores".
She Xian (Ophiopogon japonicus root / Snake Tongue, 1 liang), Rhubarb (Dahuang), Aconite (Fuzi, peeled of black skin), Peony (Shaoyao), Angelica Sinensis (Danggui), Asarum (Xixin), Scutellaria (Huangqin), Euphorbia (Daji), Sichuan Pepper (Jiao, remove seeds), Polygonum aviculare / Marsdenia (Mangcao).
Chop these twelve ingredients, soak in vinegar overnight. Add two sheng of refined lard (without water) and one liang of Longxian Teng. Simmer to make Longxian Gao (Dragon Tongue Ointment). Currently, there is also a plant called Long Cao, similar to She Xian but with larger leaves; some use its roots. It is also called Longxian Gao. (From Volume 2).
*Duanju* treats Huan Ju: apply small red beans thinly at onset to resolve it. (From Volume 10).
(For back carbuncles caused by mineral pills, see specific formulas in Volume 38).
*Qianjin Lun* states: All back carbuncles result from taking Five Minerals Pills or Hansheng Gengsheng San, or solely from Stalactite. Some develop spontaneously without prior mineral pill intake. This stems from ancient practices. Symptoms usually appear between the shoulder blades. Initially like millet grains, painful or itchy, reddish. Often ignored initially, growing rapidly. Within ten days, it becomes incurable. At critical stage, it spans 3-4 cun in diameter, 1 cun high, with dozens of pores. Pressing causes pus to reverse-flow, followed by aphasia. For health preservation: upon slight pain/itch on the back, mix clean earth with cold water into cakes (1.5 cun diameter, 0.2 cun thick). Apply coarse moxa cones on the cake over the sore, changing the cake per cone. If millet-sized, 7 cakes suffice. If elm-seed sized, 14 cones suffice. If coin-sized, continuous day-and-night moxibustion is needed. Also take Wuxiang Liaoqiao Tang and Tiejiang (Iron Slurry) medicines. Spray cold water and apply cold stone compresses continuously until healed. Avoid wheat products, alcohol, meat, and Five Pungent Spices. Some occur near the kidneys.
Furthermore, those taking mineral pills must engage in heavy labor; limbs must not remain idle. Otherwise, symptoms may flare. Do not seek warmth comfortably; one must endure cold to prevent future flares, despite initial discomfort.
Another formula for Back Carbuncle:
If swelling arises in the shoulder blade region, white-headed like millet, connected around, red-black, causing confusion, it is named Back Carbuncle. Abstinence from sex is required; avoid garlic and wheat. Delayed moxibustion leads to death. Moxa 700-800 cones directly on the sore. Misdiagnosis as common swellings often proves fatal.
