A woman surnamed Zang originally suffered from phlegm-fire (Phlegm-Fire). Due to excessive intake of fire-descending medications, by autumn, phlegm accumulated and stagnated. This caused the qi mechanism to descend but become obstructed in the large intestine (Large Intestine), resulting in a hard mass around the navel that was extremely painful upon palpation. She had been suffering from dysentery with red and white discharge for eight days, passing only drops each time, defecating more than twenty times day and night. She experienced abdominal pain, tidal fever, thirst, difficult urination, tenesmus (urgent need to defecate with a feeling of incomplete evacuation), loss of appetite, heaviness of the body preventing turning over, and inability to move sides. Upon diagnosis, I noted that her left pulse possessed Shen Qi (Spirit/Essence in the pulse, indicating preserved healthy qi). Thus, I adopted the method of Liu Shouzhen: "Moving blood will heal pus in stool; regulating qi will eliminate tenesmus." The prescription used Bai Shaoyao (White Peony Root), Huashi (Talcum), and Taoren (Peach Kernel) as Jun (Sovereign) herbs; Danggui (Angelica Sinensis) as Chen (Minister) herbs; and Muxiang (Aucklandia Root), Binglang (Areca Seed), Shanzha (Hawthorn Fruit), Jiuhuangqin (Scutellaria Baicalensis prepared with wine), Jiuhuanglian (Coptis Chinensis prepared with wine), and Zhike (Aurantium Fructus) as Zuo (Assistant) herbs. After taking the medicine, bowel movements became slightly smoother, and the abdomen felt somewhat relaxed. On the second day, continuing with the same prescription resulted in significant expulsion of stagnation, reducing the dysentery by more than half. On the third day, Shaoyao, Danggui, Huashi, Taoren, Zhigancao (Licorice Root prepared with honey), Jiuhuanglian, and Muxiang were used, taken together with Baohe Wan (Preserve Harmony Pill). In the afternoon, bowel movement occurred, and the pills taken in the morning were expelled with the feces. It was then known that the stagnation had been completely cleared, and all symptoms subsided, except for the pain around the navel which had not fully ceased. Therefore, Xiaojianzhong Tang (Minor Construct the Middle Decoction) from Zhang Zhongjing was modified with added Danggui and Muxiang, after which she recovered stability.
The mother of Ding Yaochuan, a literary scholar, was forty-four years old and frequently suffered from epigastric pain. Having been a widow for fifteen years, she consumed only vegetarian food daily. In July of that year, triggered by anger, she vomited blood approximately one bowlful, recovering after a few days. In September, another episode of anger caused similar hematemesis accompanied by abdominal pain. By February of the following year, she suddenly developed tenesmus, severe anal pain, scanty and涩 (astringent/difficult) urination with only drops passing, unbearable pain, and heat in the lower back and lower abdomen as if soaked in boiling water. She could only lie supine during the day; turning to either side caused pain in the left hip and leg. She had pre-existing pain in both hips. When urination was painful, anal pain decreased; when anal pain was severe, urinary pain decreased. Due to pain, she could not sit. Fear or fright exacerbated the sensation of sinking and pain. Her menstruation had stopped for two months. Previously, during menstruation, she always experienced waist and abdominal pain with the passage of many dark purple-black blood clots. Now, she had profuse leukorrhea resembling a flow, thirst, insomnia throughout the night, anorexia, irritability, puffy face and extremities, a sensation of phlegm obstruction in the throat, and half-body muscle wasting. Her pulse was slow (four beats per breath), weak and soft at both Cun (Inch) positions, slippery at the Right Guan (Bar) position, wiry at the Left Guan position, and choppy at both Chi (Cubit) positions. Based on the pulse, there was deficiency of qi and blood in the Upper Jiao (Upper Burner), phlegm in the Middle Jiao (Middle Burner), and qi stagnation with blood stasis in the Lower Jiao (Lower Burner), transforming into fire. Generally, diseases of the Lower Jiao are governed by the Liver and Kidney; the waist and hips are traversed by the Liver meridian, while urination and defecation are controlled by the Kidney. Regarding symptoms, puffiness of the face and hands indicates very weak Spleen Qi; lack of appetite indicates insufficient Stomach Qi; insomnia results from excessive sorrow and worry leading to Heart Blood deficiency. Overall, this is due to injury from the Seven Emotions. The *Neijing* states: "Diseases of the Two Yangs originate from the Heart and Spleen; women suffering from this do not menstruate." This condition is close to that description. Moreover, it occurs when the Fire phase holds authority, which is indeed concerning. Fortunately, the pulse is not rapid, and the voice is clear and bright, making treatment feasible. Therefore, initially, I prescribed Longdan Xiegan Tang principles (specifically Danggui Longhui Wan - Angelica Sinensis and Gentiopsis Drain Liver Pill) to clear heat from the lower part, followed by Siwu Tang (Four Substances Decoction) combined with Longdancao (Gentiana Scabra), Huangbai (Phellodendron Bark), Zhimu (Anemarrhena Rhizome), Chaihu (Bupleurum Root), and Zelan Ye (Lycopus Herb), decocted and taken with Zishen Wan (Nourish Kidney Pill) for four consecutive days, after which the heat in the waist and lower abdomen began to recede. Later, Xiangru (Elsholtzia Herb), Shiwei (Pyrrosia Leaf), Longdancao, Taoren, Huashi, Duniuqi (Cyathula Root), Gancao Shao (Licorice Tip), and Ruan Chaihu (Soft Bupleurum) were decocted and taken with Zishen Wan, resulting in the complete reduction of pain in both urination and defecation.
