Pediatric dysentery is mostly caused by the combination of summerheat-dampness and food stagnation. Treatment generally favors clearing heat and resolving stagnation, with warming and tonifying methods being less common. One must strictly avoid excessive sweating, purging, or drastic downward-draining methods to prevent the onset of complicated syndromes. Dysentery presents with red or white discharge accompanied by tenesmus (urgency and pain in defecation). Red indicates blood-heat; white indicates cold accumulation. For mixed red and white cases, use Huaihua (Flores Sophorae Immaturis) and Ganjiang (Rhizoma Zingiberis Recens), each 2 qian, decocted in water and taken orally. For pure red cases, use Baicaoshuang (Fuligo Silicea) half a teacup and Shu Laomi (Rice, cooked), one cup, ground together into powder, mixed with 2 liang of Hongtang (Brown Sugar), formed into cakes with water, and consumed in two or three doses. For pure white cases, use Pu Cha (Pu'er Tea), Tang (Sugar), and Zhike (Fructus Aurantii), each 2 qian, decocted in water and taken orally. For constipation, take Duhuang Wan. For severe cases, take Shishi Shendan, which treats bloody dysentery and mixed red-white stagnation.
Danggui (Radix Angelicae Sinensis), Huanglian (Rhizoma Coptidis) (each 1 qian), Zhike (Fructus Aurantii), Baishao (Radix Paeoniae Alba) (each 2 qian), Laifuzi (Semen Raphani), 1 qian, Muxiang (Radix Aucklandiae), Gancao (Radix Glycyrrhizae) (each 5 fen), decocted in water and taken orally.
Another prescription: Kushen (Radix Sophorae Flavescentis) 2 liang, Muxiang (Radix Aucklandiae) 5 qian, take 2 qian per dose.
Another prescription: Collect radish greens abundantly on the Winter Solstice day, spread them on thatched roofs, expose them to sun during the day and dew at night for exactly one month (no more, no less). Store them well in bamboo baskets upon collection. When dysentery occurs, decoct in water and take. This also treats epidemic heat causing sore throat and swelling. It can be used by both children and adults.
Another prescription: Hongtang (Brown Sugar) and Baitang (White Sugar) 4 liang each, Sharen (Fructus Amomi), 2 liang, Qibi (Rhizoma Dioscoreae Hypoglaucae), 3 jin, Sangshen (Fructus Mori) as needed. Seal in a jar and soak in 3 jin of Huojiu (Baijiu/High-proof liquor). When dysentery occurs, have the patient face East. Children eat one Qibi, adults eat three, and they will heal spontaneously.
Pediatric abdominal pain is mostly due to irregular diet injuring the Middle Qi, allowing Cold pathogens to enter via deficiency. Yang deficiency can also cause abdominal pain, along with conditions like worm accumulation. One must carefully observe the location of pain, palpate to distinguish deficiency and excess, and pre-examine the complexion and fingerprint. A floating cyan fingerprint indicates Wind-Cold; a deep cyan fingerprint indicates Yin-Cold; a pale red fingerprint indicates Deficiency-Cold; a deep and choppy fingerprint indicates Food Stagnation. White pits or spots inside the lips indicate Cold-Worms, suitable for warming treatment; red granules indicate Heat-Worms, suitable for expelling worms combined with clearing heat.
Abdominal pain at the Zhongwan point belongs to the Taiyin syndrome; take Lizhong Tang (see Shanghan Men).
Abdominal pain around the umbilicus belongs to the Shaoyin syndrome; take Zhenwu Tang: Baizhu (Radix Atractylodis Macrocephalae), Fuzi (Radix Aconiti Lateralis Preparata), Fuling (Poria), Baishao (Radix Paeoniae Alba) (each 2 qian), Gancao (Radix Glycyrrhizae) (8 fen), with Jiang (Zingiberis Rhizoma) and Zao (Fructus Jujubae) as guides, decocted in water and taken orally.
Lower abdominal pain belongs to the Jueyin syndrome; take Danggui Sini Tang plus Wuyu (Fructus Evodiae) (see Shanghan Men).
For lower abdominal pain caused by Cold entering the Yin aspect, urgently take Xiaojianzhong Tang (see Shanghan Men).
For abdominal pain caused by external contraction combined with retained food, take Huoxiang Zhengqi San (see Huoluan Men).
For continuous, mild abdominal pain without fluctuation, relieved by hot pressure and preference for hot drinks, take Lizhong Tang plus Rougui (Cortex Cinnamomi) and Sharen (Fructus Amomi) (see previous text).
For intermittent abdominal pain not relieved by hot pressure, accompanied by hot lips and dry mouth indicating Heat, also take Erchen Tang modified: Faxia (Pinelliae Rhizoma Preparatum), Guangpi (Pericarpium Citri Reticulatae), Fuling (Poria), Gancao (Radix Glycyrrhizae) (each 1 qian), Zipu (Cortex Magnoliae Officinalis), Zhishi (Fructus Aurantii Immaturus), Huanglian (Rhizoma Coptidis), Huangqin (Scutellariae Radix), Zhizi (Fructus Gardeniae) (each 8 fen). No guide herb, decocted in water and taken orally.
For abdominal pain and fullness due to overeating, take Muxiang Binglang Wan: Muxiang (Radix Aucklandiae), Binglang (Semen Arecae), Chenpi (Pericarpium Citri Reticulatae), Xiangfu (Rhizoma Cyperi), Shujun (Rhei Radix et Rhizoma Preparated), Huangbai (Cortex Phellodendri) (each 1 qian), with Dengxin (Medulla Junci) as a guide. Adults add Ezhu (Rhizoma Curcumae) and Qianniu (Semen Pharbitidis); if heat is severe, add Huanglian (Rhizoma Coptidis) 1 qian.
For worm-induced pain, intermittent, with spitting clear saliva, take Lizhong Tang (see previous), plus Feizi (Semen Persicae) and Leiwan (Polyporus Umbellatus).
Jiaomei Wan treats abdominal pain with thirst and vomiting sour fluid after drinking. Huajiao (Pericarpium Zanthoxyli) (1 qian), Wumei (Fructus Mume) (3 pieces), Congnao (Allii Cepae Bulbus juice/paste) (3 units), Binglang (Semen Arecae) (1 qian). Grind into fine powder, form pills with rice paste, take with boiled water.
Gutu Wan treats pediatric abdominal deficiency pain with frequent vomiting, diarrhea, and expulsion of worms. Tianshen (Codonopsis Radix), Jiaozhu (Atractylodis Macrocephalae Rhizoma Fried), Feizi (Semen Persicae), Fuling (Poria), Binglang (Semen Arecae), Shijunzi (Fructus Quisqualis), Heshi (Semen Arisaematis), Wuyi (Anthelminticum), Leiwan (Polyporus Umbellatus) (each 1 qian), Gancao (Radix Glycyrrhizae) (5 fen), decocted in water and taken orally.
Deworming Formula: Shijunzi (Fructus Quisqualis) (3 qian, shelled), Binglang (Semen Arecae), Qianniu (Semen Pharbitidis), Dahuang (Rhei Radix et Rhizoma) (each 2 qian), decocted in water and taken orally. Not suitable for those with weak constitution.
Another prescription: Leiwan (Polyporus Umbellatus) 3 qian, Cangzhu (Rhizoma Atractylodis) 1 qian. Grind into fine powder, divide into three doses. Each time, mix with one egg, fry until slightly yellow, and feed to the child. Highly effective.
