The patient suffered from Fei Nue (Malaria affecting the Lung), where chills and fever have subsided, but bowel sounds are audible accompanied by diarrhea. Initially painful, the pain has now ceased. The tongue coating remains yellow today. The pulse has changed from wiry and rapid; the right hand shows a sluggish and large quality, while the left hand exhibits a slippery and free-flowing quality. Generally, it is judged that the summer-heat has cleared but damp-heat remains. Treatment continues to focus on the Lung.

Following Dong Wen (Winter Warmth disease), residual heat causes irritability. The pulse is fine and rapid, with the Right Guan position showing slight sluggishness. The black tongue coating is lifting; the sides have lost their yellow color and turned slightly white and slippery. There is dryness in the throat with a desire to drink. The complexion is red. Pain radiates from the left Taiyang area to the eyes, and the ear canal hurts. Wu Shi advocated treating the Lung for warm pathogens. The residual heat stems from deficiency of Lung Yin. Therefore, Sheng Di Huang is used to clear heat from the blood level. This condition arose because menstruation occurred during the illness, further depleting the Blood level, resulting in an absence of sweating. The method should be to nourish Lung Yin. Once the Yin level is nourished, both exterior and interior symptoms will cease. Regarding the eye traction pain and ear pain, these generally belong to concurrent Liver Wind and Fire. Another prescription is formulated to nourish Lung Yin and extinguish Liver Wind and Fire.

Wind pathogen affected the Liver. Sweating has occurred, including cold sweat. However, all six pulses are wiry and rapid, and the tongue is yellow and dry, indicating that heat pathogens in the Lung division have not been fully cleared. Urine is sometimes yellow, and stool is sometimes loose. The summer-heat this autumn was extremely severe, likely夹杂着 damp-heat.

Rash has appeared for three days. Tongue coating is yellow without cough or diarrhea. Only body fever and profuse sweating remain, visible on the back. Pulse is wiry and sluggish. The method should be to vent Wind-Heat.

In the second and third lunar months, when rashes appear, one should generally use the Wind-Warmth method. Lethargy is caused by Wind. *Neijing* states that Wind-Warmth disease presents with burning heat, spontaneous sweating, heaviness, cough, and lethargy; thus, lethargy indicates Wind. Thirst indicates warmth transforming into fire. Sore throat indicates Lung Fire. Difficulty in urination and defecation with sore throat requires treating the Lung and venting the rash. Red tongue tip with delirium indicates involvement of the Heart. A method for Upper Jiao of Heart and Lung is proposed.

Later, residual cough disappeared, but chronic diarrhea persisted. Chills and fever recurred. Everything ingested was vomited. Tongue coating was thin and yellow. Pulse was wiry and rapid, larger than before. This belongs to Liver Yang invading the Stomach, causing vomiting. It should also be attributed to Wood overacting on Earth (Liver attacking Spleen/Stomach). Both Spleen and Stomach are affected by excessive Wood evil. Vomiting should be eliminated first, combined with methods for throat obstruction and chest pain, alternating approaches.

Intermittent malaria occurs always in Shaoyang. Recurrence after cessation is due to lack of proper recuperation, specifically Lao Fu (relapse due to exertion). Now it has stopped again. Right hand pulse has become smaller; Left hand pulse remains wiry and choppy. The method should be to warm and nourish Liver and Gallbladder.

Later, body temperature cooled initially. Stool was loose. Cough had little phlegm. Tongue coating decreased. Appetite remained poor. Pulse became slow. Treatment continued from the Lung using a Sweet-Cold method.

Body fever lasted four days. Yesterday was the third day. There was sweating. Menstruation provided an outlet, lightening the condition. Today, pulse is still rapid, indicating pathogens are not cleared. Complexion is red. Sore throat. Tongue has no coating. Use Lung Wind-Heat method.

Chronic diarrhea resulted in a clean tongue. Abdominal rumbling occurs, with occasional pain. Pulse is wiry and rapid. This must involve Liver and Spleen. Rumbling is more frequent than pain. Diarrhea is watery, indicating severe Spleen deficiency. Propose soothing Liver and supporting Earth (Spleen) method.

Rash has been present for over a month. New wind contraction occurred. Body fever with sweating. Head distension and dry tongue. Nighttime thirst. Hot urine. Pulse is wiry and rapid for four days. Tongue is slightly yellow and dry. Occasional abdominal pain. Propose Pungent-Cool combined with Sweet-Cold method.

After toothache caused by Liver Wind, Right hand pulse is Kou (hollow), Left hand is Choppy. Replenish Liver Blood; sufficient blood extinguishes wind. Tongue coating is slippery; Stomach has damp-phlegm that must be resolved.

Later, pulse is deficient, tongue still yellow. Chronic diarrhea indicates Kidney deficiency. First, expel external Wind-Heat that has moved downward.

Menstruation stopped for three months. Previously suffered from Summer-heat for fifteen days. Now pathogens are cleared, appetite returned. Six pulses are wiry and large, slightly rapid. Constitution is Yin-deficient with internal heat. If considered pregnancy, pulse lacks tranquility and slowness. Pregnancy pulses are typically quiet and small initially; overly forceful pulses suggest unstable fetus. Not yet confirmed as pregnancy. Use cooling and nourishing Liver method.

Residual summer-dampness, heat gone. Urine volume increased. Appetite gradually returning. Pathogens likely cleared. Description: Left hand deep pulse is forceful and rapid. Left hand corresponds to Blood. This is heat in the Blood level. Initially accompanied by Liver pain, generally due to inherent Liver heat.

Tongue coating yellow, appetite poor, pulse sluggish. Diarrhea triggered by eating noodles. Noodles assist damp-heat and move Liver Qi. Propose resolving accumulation and regulating Stomach.

Right hand shows convergence. Guan pulse is slippery, vigorous, and spirited. Left Guan has shrunk compared to before, resembling the Zhen trigram. Currently, occasional abdominal pain and stomach distension. Propose regulating Liver and protecting Yuan Qi method.

Regarding rashes, Ye Shi said treatment must follow seasons. Spring uses Wind-Warmth method; clearing Wind-Warmth clears Wind. It has been six days. Fever started five days ago, followed by rash, bounded by the back. Diarrhea. Dry white tongue. Wiry rapid pulse. Use Pungent-Cool combined with Sweet-Cold method.

After external contraction, residual pathogens should be cleared coolly. Lung heat fears injury. After cooling, cough with phlegm persists. Left hypochondrium has a chronic mass. Patient feels phlegm originates here. Left hypochondrium is Liver location. Mass is likely due to past labor/posture. No distinct shape, not true blood stasis, but Qi stagnation in collaterals. Ancients say: Clearing Metal (Lung) calms Wood (Liver). Clear Lung, and Liver will settle.
