If Shanghan is purged too early, qi swells upward in the chest, called Jie Xiong (Chest Binding). More often, Badou (Croton Seed), Fenshuang (Calomel), and Nifen (Mercurous Chloride) are used, resulting in seven or eight deaths out of ten. This is because purging the lower jiao causes lower jiao deficiency, making qi attack the chest and diaphragm even more, often leading to untreatable conditions. For chest and diaphragm diseases, the diaphragm can be purged. If pressing reveals hardness and pain, this is Jie Xiong. If there is water in the chest, Da Huang (Rhubarb Root) and Gansui (Kansui Root) must be used to purge it, such as in Xianxiong Wan (Trap the Chest Pill). If pressing is not very hard and not very painful, this is called Pi (Fullness). It is upper qi deficiency with heat expansion; only Huangqin (Scutellaria Root), Da Huang (Rhubarb Root), and Huanglian (Coptis Rhizome) can resolve it. I once saw a patient with critical chest distension due to Jie Xiong Pi Qi, all treatments failed. Physician Shi gave him this tea, and after passing about one sheng of yellow water, he recovered. I obtained this method and use it effectively. For Pi Qi, it dissolves upon contact. For any Shanghan chest distension, regardless of Jie Xiong or Pi Qi, first take this medicine. If not cured, then use other purgatives. Record this tea to move qi and descend diaphragm, with no other harm. Additionally, Cui Xun's formula from Western Jin treats Shanghan where purging did or did not occur, resulting in knotted fullness in the heart, cold chest and diaphragm,急促 breathing, syncope nearly ceasing, high chest, and touch sensitivity, dying in two or three days. Xiexin Tang and large/small Xianxiong Tang were ineffective. This is likely due to post-purgation deficiency and rebellious qi being chaotic, with toxin attacking upwards again, qi and toxin fighting and knotting in the chest. To treat this, use Jiajian Lizhong Wan (Modified Regulate the Middle Pill). First regulate the qi, then treat other diseases.
Jiajian Lizhong Wan (Modified Regulate the Middle Pill) (From *Huo Ren Shu*): Treats Cold Excess Jie Xiong (Chest Binding) and post-Shanghan vomiting/diarrhea with near-death epigastric fullness, high and painful diaphragm, and touch sensitivity.
Renshen (Ginseng Root), Baizhu (Atractylodes Rhizome), Gancao (Licorice Root) (each 2 liang), Ganjiang (Dried Ginger) (1.5 liang), Zhishi (Immature Bitter Orange Fruit) (roasted 1 liang), Fuling (Poria) (2 liang): Grind the above herbs into powder, mix with honey to make pills the size of bullet seeds. If one pill is ineffective, take another. I usually use this formula with swift results. Upon swallowing, qi connects immediately. Later, giving no more than five or six bullet seeds clears the chest豁然ly. The speed of medication effect has never been seen. If thirsty, add Gualou (Trichosanthes Fruit) 2 liang; if diarrhea, add Muli (Oyster Shell) 2 liang, stir-fried. This is my frequently tested effective formula, with swift merit. Now, taking this pill with Zhishi Tang (Immature Bitter Orange Decoction) together yields no failure. What is called Zhishi (Immature Bitter Orange Fruit) is just Zhike (Bitter Orange Fruit). Ancients called it Zhishi, later separated Zhike. Mr. Dongpo widely disseminated this secret formula and provided discussions to teach people to save lives, showing benevolent intent.
